Get emergency help now for trouble breathing, swelling of the face, lips, tongue or throat, fainting, a seizure, severe confusion, or any symptom that is rapidly worsening or could be life-threatening. For severe, persistent, worsening or unusual symptoms, contact the prescribing clinician promptly. Mild symptoms that are improving can usually be monitored with the medicine’s Medication Guide, but the exact drug and formulation always matter.
This guide reflects U.S. Food and Drug Administration (FDA) safety information checked October 13, 2026. It cannot diagnose pancreatitis, gallbladder disease, kidney injury, low blood sugar or an allergic reaction, and it cannot make an individual triage decision without the person’s medicine, dose, medical history and symptoms.

Use this three-level check first
When a side effect starts, classify the situation by danger, not by whether the symptom is listed as “common.” A common reaction can still require medical care if it is intense, prolonged, worsening or preventing hydration.
| Situation | What to do now | Examples |
|---|---|---|
| Emergency | Call local emergency services or go to an emergency department. Do not delay to search for an explanation or submit a side-effect report. | Difficulty breathing; swelling of the face, lips, tongue or throat; fainting; seizure; severe confusion; inability to swallow safely; or another rapidly worsening, potentially life-threatening symptom. |
| Prompt clinician contact | Call the prescriber, on-call clinician or pharmacist as soon as possible for individualized instructions. Use urgent care if you cannot reach them and the symptom is severe or worsening. | Severe or persistent abdominal pain; repeated vomiting or diarrhea; inability to keep fluids down; markedly less urination; faintness; a new unusual symptom; a suspected dosing error; or symptoms that interfere with normal activity. |
| Mild and improving | Follow the product’s Medication Guide, record what happened and when, and contact a clinician if it persists, worsens or worries you. Do not independently double, stop, restart or change a prescription dose. | Short-lived nausea, constipation, diarrhea or mild abdominal discomfort that is improving and does not prevent drinking or normal functioning. |
This is a practical framework, not a validated scoring tool. If you are unsure whether a serious symptom can wait, seek urgent medical advice.

Call emergency services for signs of immediate danger
Suspected severe allergic reaction
Breathing difficulty or swelling involving the face, lips, tongue or throat can signal a severe allergic reaction. FDA labeling says to stop Wegovy (semaglutide) immediately and seek medical help if a severe allergic reaction is suspected. FDA’s Zepbound (tirzepatide) approval announcement similarly states that patients should stop Zepbound immediately and seek medical help when a severe allergic reaction is suspected. Do not take another dose while waiting for instructions.

Fainting, seizure or dangerous altered consciousness
Call emergency services for fainting, a seizure, severe confusion, inability to stay awake, or inability to swallow safely. These symptoms can make a person unsafe and can be especially urgent for someone taking insulin or another medicine that stimulates insulin release, because GLP-1 medicines can increase the risk of low blood sugar when combined with those treatments. Do not give food, drink or oral medicine to someone who is not fully alert or cannot swallow safely.

Any rapidly worsening or life-threatening symptom
The emergency threshold is not limited to the examples above. Severe chest symptoms, collapse, signs of shock or a symptom that is escalating quickly should be treated as an emergency. Use local emergency services rather than waiting for a prescriber to return a call.
Contact the prescriber promptly for serious or persistent symptoms
Severe or persistent abdominal pain
Severe abdominal pain, particularly pain that persists or occurs with vomiting, needs clinical assessment. FDA labeling for Wegovy warns about pancreatitis and gallbladder disease and directs patients to discuss symptoms that could indicate either problem with a health care provider. Pain alone does not prove pancreatitis or gallstones, so do not self-diagnose or assume that a familiar “stomach side effect” is harmless.

Repeated vomiting or diarrhea
Contact a clinician if vomiting or diarrhea keeps recurring, you cannot keep fluids down, you feel faint, or urination becomes markedly less frequent. The Wegovy label warns that volume depletion from gastrointestinal reactions can cause acute kidney injury and says clinicians should monitor renal function when adverse reactions could lead to volume depletion. The label does not establish a universal number of episodes or a fixed time threshold; severity, duration, fluid intake, urine output and medical conditions determine urgency.
Symptoms that are new, unusually severe or worsening
Nausea, diarrhea, vomiting, constipation and abdominal pain are listed among common Wegovy reactions, and similar gastrointestinal effects occur with other medicines. “Common” describes how often an effect was observed in a studied population; it does not mean every episode is safe to ignore. Call when the symptom prevents drinking, sleeping, working, taking essential medicines or carrying out normal activities, or when it is getting worse instead of better.
Possible low blood sugar
People using insulin or a medicine that increases insulin release should ask their clinician in advance how to respond to low-blood-sugar symptoms while taking a GLP-1 medicine. Severe confusion, seizure, loss of consciousness or inability to swallow safely is an emergency. For a person who is awake and able to swallow, follow the individualized diabetes plan rather than guessing at a dose change.
Drug-specific warning: over-the-counter alli (orlistat)
FDA added kidney-injury and kidney-stone warnings to the labeling for OTC alli (orlistat) in June 2026. Stop alli and consult a health care provider if you develop any of these symptoms:
- Back or groin pain
- Painful urination
- Blood in the urine
- Swelling of the feet or legs
- Less frequent urination
These instructions are specific to orlistat and should not be generalized to every weight-loss medicine. FDA’s review described 12 postmarketing cases: eight acute kidney injuries, two acute kidney injuries with oxalate nephropathy, and two cases of hyperoxaluria with calcium oxalate stones. Eight people were hospitalized and five required dialysis. The cases are not an incidence estimate; FDA notes that postmarketing reports can be incomplete.

How to judge vomiting, diarrhea and dehydration
Fluid loss becomes more concerning when it is ongoing or combined with risk factors such as kidney disease, older age, pregnancy, diuretic use or inability to drink. Warning patterns include:
- Repeated vomiting or diarrhea that does not settle
- Inability to keep fluids down
- Markedly reduced urination
- Faintness, weakness or worsening dizziness
- Increasing abdominal pain or confusion
Call the prescriber promptly for these patterns. Seek emergency care for collapse, severe confusion, inability to stay awake or another rapidly deteriorating condition. Do not wait for a specific episode count: FDA’s Wegovy label gives no universal number, and an individual’s health status changes the risk.
What to do while waiting for advice
- Keep the medicine package, injector or bottle nearby so the exact product can be identified.
- Record the time of the last dose and when vomiting, diarrhea or pain began.
- Unless emergency personnel tell you otherwise, follow the product instructions and your clinician’s hydration advice; do not take extra doses to compensate for vomiting.
- Do not restart a held dose or alter the schedule until the prescriber or pharmacist gives medicine-specific instructions.
What to do after a dosing mistake or with a compounded injection
A suspected measurement or dosing error deserves prompt professional advice, especially with compounded semaglutide or tirzepatide. FDA has described dosing mistakes, serious adverse events and some hospitalizations involving compounded GLP-1 products. Contact the prescriber, dispensing pharmacist, compounder or poison control for immediate, individualized instructions. If symptoms are severe, call emergency services first.

FDA says compounded products are not FDA approved and do not undergo FDA review for safety, effectiveness and quality before marketing; it also warns about dosing errors and fraudulent products.
Do not attempt to convert “units,” milligrams and milliliters yourself. Have the vial label, syringe markings, concentration, amount drawn, intended dose and time of administration available. FDA specifically advises patients using compounded semaglutide or tirzepatide to ask their provider or compounder about measuring and administering the intended dose.

Common questions about specific symptoms
“Is this side effect normal?”
Some nausea, constipation, diarrhea, vomiting and abdominal discomfort are listed as common with GLP-1 medicines, but a label category cannot determine your personal urgency. Severity, persistence, progression, hydration and other medicines matter. A symptom that is severe, prolonged or disabling warrants a clinician call even if it appears on the common-effects list.
“When should I call my doctor about a weight-loss injection?”
Call promptly for severe or persistent abdominal pain, repeated vomiting or diarrhea, inability to keep fluids down, much less urination, faintness, a new or worsening symptom, or a suspected dosing error. Call emergency services for breathing difficulty, facial or throat swelling, fainting, seizure, severe confusion or rapid deterioration.
“Should I stop my medicine if I am vomiting?”
Do not make a general stop-or-continue decision from an article. Repeated vomiting, inability to hydrate or worsening symptoms require prompt medical advice. For suspected severe allergic reaction with Wegovy or Zepbound, FDA specifically instructs patients to stop the medicine and seek immediate help. For other situations, the prescriber or pharmacist should give the exact hold, restart and dose-adjustment instructions.

“What if I took too much semaglutide?”
Treat a suspected overdose or measurement error as a prompt call to the prescriber, pharmacist, poison control or compounder. Provide the exact concentration, amount, syringe markings and time. Seek emergency care for severe symptoms, altered consciousness, a seizure, breathing trouble, collapse or rapid worsening.
How to use the Medication Guide and label
Find the current Medication Guide and prescribing information for the exact product, not just the drug class. Look for sections titled “Warnings and Precautions,” “Adverse Reactions,” “Patient Counseling Information” and instructions for missed or changed doses. Branded semaglutide, tirzepatide, orlistat and compounded preparations can have different concentrations, devices, contraindications and instructions. FDA-approved labels can change, so use the current version supplied with the medicine or posted by FDA.
Never substitute another product’s instructions. For example, the kidney-symptom stop-and-call advice for alli is not a universal rule for injectable medicines, while the immediate-stop language for suspected severe allergy is specifically stated for Wegovy and Zepbound.
How serious-event numbers should be interpreted
The 2025 Wegovy prescribing-information revision reported severe gastrointestinal adverse reactions in 4.1% of adults in weight-reduction trials receiving Wegovy versus 0.9% receiving placebo. This is a trial finding for the studied product and population, not a prediction of an individual reader’s risk and not a threshold for deciding whether to seek care.
FDA had received 990 reports involving compounded semaglutide and more than 730 reports involving compounded tirzepatide as of May 31, 2026. These are adverse-event reports, not incidence rates; they may be underreported and cannot prove that a product caused a particular person’s event. The numbers should reinforce the need to disclose a compounded product or dosing error, not be used to calculate personal odds.
Report a suspected adverse reaction after getting care
Suspected reactions to FDA-regulated medicines can be reported through FDA MedWatch. Reporting helps regulators identify safety signals, but it does not replace emergency treatment, a prescriber’s evaluation or poison-control advice. Ask the clinician or pharmacist to help document the exact product, lot or batch information when available, dose, timing and outcome.
