Your eligibility, health and family history, other medicines, pregnancy plans, and treatment goals all matter; a drug’s general approval does not determine whether it is suitable for you.
Start with the exact medicine and its approved use
“GLP-1 drug” can refer to different products with different active ingredients, indications, and label warnings. Semaglutide is a GLP-1 receptor agonist. Wegovy is the semaglutide product labeled for weight management; other semaglutide brands have different indications and should not be treated as interchangeable with it.

Zepbound contains tirzepatide, which acts at both GIP and GLP-1 receptors. Its FDA labeling specifies use alongside reduced-calorie eating and increased physical activity for long-term reduction and maintenance of body weight in adults with obesity, or adults with overweight and at least one weight-related condition. Check the current label for the particular product being considered rather than relying on a class-wide description.
| Product | Active ingredient and action | Weight-management indication described in the label | Important product-specific checks |
|---|---|---|---|
| Wegovy | Semaglutide; GLP-1 receptor agonist | FDA-labeled for weight management. Confirm the current indication and eligibility criteria in the Wegovy prescribing information. | The label lists contraindications involving personal or family history of medullary thyroid carcinoma (MTC), multiple endocrine neoplasia syndrome type 2 (MEN 2), and serious hypersensitivity to the product. (FDA, Wegovy Prescribing Information, revised March 2026.) |
| Zepbound | Tirzepatide; GIP and GLP-1 receptor agonist | For long-term weight reduction and maintenance with reduced-calorie eating and increased physical activity in adults with obesity, or adults with overweight plus at least one weight-related condition. | Review the Zepbound label’s warnings and precautions, including severe gastrointestinal reactions, pancreatitis, gallbladder disease, hypoglycemia, acute kidney injury, and hypersensitivity. (DailyMed/NLM, Zepbound Prescribing Information, January 2026 label listing.) |
The FDA’s approved prescribing information reflects its finding about safety and effectiveness under the labeled conditions of use; it is not a guarantee that a medicine is safe for every person or every use.

Check whether your situation fits the indication
The American College of Physicians’ April 2026 living guideline suggests semaglutide or tirzepatide with lifestyle modifications for nonpregnant adults with obesity and for certain adults with overweight plus a specified weight-related condition. That guidance does not replace the product label or a clinician’s assessment of your diagnosis and circumstances.
At an appointment, ask the prescriber to confirm:
- Which diagnosis and weight-related conditions make you eligible under the exact product’s current indication.
- Whether the expected benefits fit your health priorities and weight goals.
- How your other health conditions, life expectancy, and treatment preferences affect the balance of likely benefits and harms.
- What alternatives are available and how cost, insurance coverage, and product availability affect the choice.
The ACP recommends weighing expected benefits, harms, costs, access and availability, relevant comorbidities, weight goals, life expectancy, values and preferences, and contraindications or warnings such as pregnancy. That framework is more useful than asking which drug is universally “best.”

Review your history, medicines, and pregnancy plans
Bring a current medication list and be ready to discuss your medical and family history. A condition or medicine does not automatically rule treatment in or out unless the specific product’s information says so; your prescriber can check for relevant risks and interactions.
Contraindications and thyroid history
For Wegovy, the FDA label contraindicates use with a personal or family history of MTC, MEN 2, or serious hypersensitivity to Wegovy or its components. The label says it is unknown whether the thyroid C-cell tumors observed in rodents apply to humans. Do not transfer this exact contraindication wording to another product without checking its own prescribing information.

Other medicines and conditions that need monitoring
The Wegovy label warns about pancreatitis, acute gallbladder disease, hypoglycemia when used with insulin or an insulin secretagogue, and acute kidney injury related to volume depletion. Zepbound’s label includes warnings about severe gastrointestinal reactions, pancreatitis, gallbladder disease, hypoglycemia, acute kidney injury, and hypersensitivity. These are issues to discuss and monitor—not predictions that you will experience them.
Wegovy can delay gastric emptying and may affect absorption of oral medicines. Tell the prescriber about all prescription drugs, over-the-counter medicines, and supplements you take so they can decide whether monitoring or another adjustment is needed.

Pregnancy and planned procedures
Wegovy’s current label says to discontinue it for weight reduction when pregnancy is recognized and to stop at least two months before a planned pregnancy. If you are pregnant, planning pregnancy, or could become pregnant, raise that with the prescriber before starting or continuing treatment; confirm the instructions for the particular product you use.
Tell your clinician and procedural team if you use one of these medicines and have surgery or another procedure planned. Product labels address the risk of pulmonary aspiration during general anesthesia or deep sedation. Ask the team for product-specific instructions about the procedure; do not change or stop a dose on your own.

Verify what product you are actually receiving
FDA says compounded drugs do not undergo its premarket review for safety, quality, or effectiveness. The agency has also described dosing errors and other concerns involving unapproved GLP-1 products. Do not assume a compounded product is interchangeable with an FDA-approved medicine.
Before using a prescription, ask your prescriber or pharmacist to identify the exact product and active ingredient, confirm that it is the product they intended, and explain its dosing instructions. Do not use an unapproved source, share an injection device, or make unsupervised dose changes.
Prepare for a clinician-led decision
Use this checklist to make the appointment specific and productive:
- Write down your weight-related health goals and any conditions you hope treatment may help address.
- Bring your medication list, relevant medical records, and details of personal or family history that may matter for the chosen product.
- Tell the clinician about pregnancy plans, upcoming surgery or sedation, and any previous problems with similar medicines.
- Ask which current FDA label applies, what benefits and harms are realistic for your circumstances, what monitoring is planned, and when to seek medical advice.
- Discuss cost, coverage, availability, and what to do if the prescribed product cannot be obtained.
This is general information, not a personal risk calculation or a prescription recommendation. The decision depends on the exact medicine and an individualized review with a licensed clinician.
