Choose an armor piece, select a look you have unlocked, and apply it; the look changes while the equipped gear’s gameplay build remains in use. Since Patch 8, this option is available from the beginning on PlayStation and PC, so you no longer need Level 9 gear.
Change an armor piece’s appearance

Open the equipment or armor menu and select the piece of gear whose look you want to change.

Choose the on-screen Edit Appearance prompt. In the cited PlayStation control guide, press Square to open it.

Select an appearance you have unlocked. On PlayStation, the guide says to hold Square to apply the chosen look.

Do you need Level 9 gear?
No. Santa Monica Studio’s January 30, 2025 Patch 8 notes say Edit Appearance no longer requires equipment to be fully upgraded to Level 9. The feature is available from the beginning for all PlayStation and PC players.

Older guides may say otherwise because the original rule was different. Santa Monica Studio’s November 2022 progression guide said, “Once your equipment reaches Level 9, you’ll be able to apply any appearance currently in your inventory.” That was the launch-era requirement; Patch 8 superseded it.

What the appearance option changes
Edit Appearance changes the visual style of equipment. It is not a general character creator or an unrestricted way to change Kratos’s face or body model. The appearance you select must be unlocked or owned, and using it changes the gear’s look rather than the gameplay build of the equipped piece.

New Game+ armor looks
New Game+ is not required for the general Edit Appearance feature. It does, however, include specific extra armor looks. In its April 5, 2023 New Game+ announcement, Santa Monica Studio said 13 existing armor appearances received new color combinations and styles:

Twelve of the added looks could be purchased at the Huldra Brothers’ Shop and applied to existing Level 9-or-higher armor in the transmog menu.

The Berserker Armor appearance required defeating the Berserker King on a New Game+ save.

Those are unlock conditions for the added New Game+ looks, not prerequisites for changing appearances generally.
