Arrange a veterinary assessment, too: disorientation, sleep changes, house soiling and anxiety can fit canine cognitive dysfunction, but other health problems can cause similar signs.
Start with a veterinary assessment, not an assumption
Canine cognitive dysfunction (CCD), sometimes called canine dementia, can involve disorientation, getting stuck in familiar places, altered social interaction, disrupted sleep, house soiling, anxiety, changes in activity and loss of previously learned behaviors. Signs may develop gradually, but they should not automatically be dismissed as normal aging.

Pain, arthritis, sensory loss, seizures and other medical or neurological conditions can produce similar changes. Cornell University’s overview of cognitive dysfunction syndrome and the AAHA 2023 Senior Care Guidelines describe diagnosis as a process of evaluating signs and checking for other causes. A veterinarian may take a history, examine your dog, assess neurological function, and recommend blood and urine tests; imaging such as MRI may be appropriate in some cases.

Before the visit, keep a dated log of behavior, sleep, appetite, elimination, mobility and possible triggers. A short video can help show an intermittent behavior. A behavior questionnaire such as DISHAA can support assessment or monitoring, but it does not diagnose CCD on its own.

Keep the day predictable while allowing comfortable choices
Use dependable times and familiar locations for meals, walks, rest, and food and water. Avoid moving beds, bowls or furniture when possible. Predictable anchors can make daily life easier to navigate; keep flexibility for the dog’s comfort rather than insisting on a rigid schedule.
- Keep walk times and distances roughly consistent, while varying the route if your dog enjoys new smells.
- Provide a calm, familiar resting place and let your dog choose company or quiet.
- Watch for fatigue and make sure there is a comfortable place to retreat.
- Use reinforcement and calm guidance. Punishment for confusion, pacing or accidents can increase anxiety.
For disrupted nights, try a consistent sleep place and bedtime. Daytime interaction and light, a darker room at night, and a nightlight may help some dogs; a final walk before bed is another option. Adapt these measures to your dog’s vision, mobility and response.

Make house-soiling easier to manage
Ask the veterinarian to check for other causes of accidents, such as a urinary infection, before attributing them to cognitive change. More frequent chances to go outside may help. If your dog already uses an indoor potty area, keep it easy to reach and conveniently located; introducing new training can be difficult at this stage.

Understand what home care can—and cannot—do
There is no single cure for CCD. Home routines, environmental changes and appropriate enrichment can support quality of life, while diet, medication and concurrent health problems require an individualized veterinary plan. The AAHA guideline describes selegiline as the only drug labeled for canine cognitive dysfunction in its guideline context; product labeling and availability depend on jurisdiction. Ask the veterinarian whether any treatment is suitable for your dog.
Do not start human medicines, melatonin, supplements or a therapeutic diet based on an article alone. Cornell advises discussing melatonin and supplements with a veterinarian, and AAHA notes limitations in the quality and rigor of evidence for various supplements, nutraceuticals, acupuncture and diets.
For context, the Merck Veterinary Manual reports estimates from different kinds of studies: one cited study found signs consistent with cognitive dysfunction in 28% of dogs aged 11–12 and 68% of dogs aged 15–16; a separate internet survey estimated 14.2% prevalence among dogs older than 10, with more than 85% of identified cases undiagnosed. These figures use different methods and case identification, so they should not be combined or treated as a universal rate.
A 2025 survey of 318 US veterinarians described reported diagnosis and management practices, as well as uncertainty about best practices and treatment efficacy. It reports clinicians’ experience; it does not establish that a particular treatment works.
