How to Build an Incident-Response Playbook for State-Sponsored Attack Campaigns

A state-sponsored intrusion must be managed as a campaign, not a single alert. Build a practiced playbook around five sections: preparation and governance; detection and analysis; containment, eradication and recovery; communications and reporting; and exercises and improvement. Base the structure on NIST SP 800-61 Rev. 3 (published April 2025 and aligned with CSF 2.0), then add the identity, cloud, operational-technology, legal and continuity decisions that a long-dwell espionage operation demands.

What makes a nation-state incident different

Criminal incidents often have a visible point of entry and a short time horizon. A state-sponsored campaign may use stolen credentials, trusted relationships, cloud identities, legitimate administration tools and quiet persistence for months. The first alert can be one symptom of activity that began long before detection and may continue through several tenants, suppliers or business units.

Your playbook should therefore answer two questions at once: what is happening right now? and what related access has the actor established elsewhere? Assume the investigation will expand until evidence proves otherwise. CISA, the FBI and the NSA advise organizations to create, maintain and exercise incident-response, resilience and continuity plans, while reducing personnel coverage gaps that an attacker could exploit.

  • Campaign scope: track identities, endpoints, cloud tenants, SaaS applications, network segments, data stores and third parties as one case.
  • Long dwell: search both before and after the first alert; the earliest confirmed event is not necessarily the initial compromise.
  • Trusted access: investigate valid accounts, OAuth grants, federation, remote administration and supplier connections instead of looking only for malware.
  • Mission impact: preserve the ability to operate safely if IT systems are unavailable, especially where OT or physical safety is involved.
  • Strategic reporting: preserve evidence and coordinate legal, regulatory, sector and law-enforcement decisions while technical work proceeds.

Design the playbook and decision authority first

Use a five-part lifecycle

Organize the document so responders can move from preparation through improvement without opening several disconnected procedures:

  1. Preparation and governance establishes people, authority, telemetry, assets, contacts and continuity measures.
  2. Detection and analysis turns alerts and threat intelligence into a scoped campaign timeline with confidence levels.
  3. Containment, eradication and recovery removes access while preserving evidence and keeping essential services safe.
  4. Communications and reporting maintains a fact-checked flow of information and meets applicable obligations.
  5. Exercises and improvement tests assumptions, records gaps and assigns corrective work.

NIST SP 800-61 Rev. 3, published in April 2025, is the current standards anchor and integrates incident response with the NIST Cybersecurity Framework 2.0. Use it as the governing model, then write operational checklists that match your technology and jurisdiction.

Name the incident command team

Assign an incident commander and deputies before an incident. The core roster should include security operations, incident response or DFIR, identity and cloud engineering, endpoint and network teams, IT and OT owners, legal, privacy, communications, executive leadership, business continuity, insurance, procurement and vendor management. Record primary and alternate contacts, time zones, emergency channels and authority limits.

Define who can declare a nation-state incident, isolate a production segment, suspend an account, revoke tokens, disable a cloud application, contact law enforcement, notify a regulator and communicate with customers or employees. Put thresholds and escalation times beside each decision. During a tabletop, require management to make the decision with incomplete information; a playbook that only describes technical tasks is not an authority model.

Set surge coverage and external support

List the people who can work nights, weekends and holidays, and specify how a deputy takes over when the primary responder is unavailable. Maintain pre-approved contacts for CISA, sector coordinators, law enforcement, outside counsel, DFIR retainers, cloud providers, critical suppliers, cyber insurance and communications support. Test every number and secure channel during exercises. CISA specifically warns that coverage gaps are exploitable.

Preparation: make the organization observable and resilient

Inventory the crown jewels and trust paths

Keep an owner, business purpose, dependency map and recovery priority for each item below:

  • Identity providers, privileged and service accounts, federation and break-glass accounts.
  • Cloud tenants, management groups, subscriptions, control-plane roles and administrative workstations.
  • Email, collaboration, SaaS applications, OAuth applications and external sharing.
  • Remote-access services, VPNs, jump hosts, third-party support paths and supplier trust relationships.
  • Endpoints, servers, domain controllers, network devices, backups and key data repositories.
  • OT networks, safety systems, industrial controllers, manual controls and IT/OT dependencies.

Document which services can be degraded, disconnected or operated manually, who approves that action and how long the business can tolerate it.

Collect the telemetry needed for a campaign investigation

Require time-synchronized, searchable logs for identity authentication and risk events, endpoint processes and command lines, cloud control-plane actions, email and mailbox access, OAuth consent, network flows, DNS, VPN, firewall, administrator changes and security-tool alerts. Record retention periods, legal holds, access permissions and export procedures. A log that exists but cannot be retrieved during an emergency is not dependable evidence.

Prepare evidence and recovery assets

Define how responders capture volatile memory when appropriate, preserve disk and cloud records, hash exports, record time zones and maintain chain of custody. Keep known-good images, configuration baselines, infrastructure-as-code, offline or immutable backups, emergency accounts and clean administration workstations. Validate that backups are restorable and that recovery credentials are not dependent on the potentially compromised identity system.

Write continuity and safety procedures

For each critical service, specify a manual workaround, minimum safe capacity, decision owner, communications method and return-to-service test. CISA recommends ensuring OT can operate at necessary capacity even when IT is compromised. Include plant, facility and safety personnel in exercises; an IT-only recovery plan can create physical risk.

Detection and analysis: investigate the campaign, not just the alert

Start with high-value signals

Prioritize alerts involving password spraying, token or session abuse, anomalous OAuth or application consent, unusual mailbox access, privilege escalation, new persistence, unfamiliar administrative tooling and suspected exfiltration. Treat a successful login from a familiar device as suspicious when its timing, token, application, privilege or data access is inconsistent with the user’s role.

Run a cross-domain hunt

Search identity, endpoint, cloud, network and SaaS records for related activity before and after the triggering event. Pivot on account names, device identifiers, IP addresses, domains, application IDs, token issuance, command lines, mailbox rules, newly created credentials and administrative changes. Map observations to MITRE ATT&CK techniques to expose blind spots and to communicate consistently across technical teams.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Preserve evidence before destructive changes

Capture volatile evidence, timestamps, identity context, process trees, command lines, cloud audit records, network-flow data, mailbox metadata and relevant configurations before reimaging, disabling accounts or deleting applications. Note the collector, source, time zone, export method and hash where practical. Coordinate urgent containment with the forensic lead so an emergency action does not erase the only proof of access.

Build a confidence-rated campaign timeline

Maintain one timeline that separates known, suspected and unknown facts. For each event, record the source, UTC time, affected identity or asset, observed technique, evidence location, confidence and next test. Track potential access to data, tenants, suppliers and privileged roles, not only confirmed malware infections.

CISA’s September 3, 2025 revision of advisory AA25-239A describes PRC state-sponsored actors targeting global networks, including telecommunications and government infrastructure, and recommends threat hunting and incident response when malicious activity is suspected or confirmed. Use that posture to justify proactive searches when an alert is credible but incomplete.

Declare the incident when thresholds are met

Examples of declaration triggers include confirmed nation-state tradecraft, compromise of a privileged or cloud-control-plane identity, evidence of persistence across systems, access to regulated or mission-critical data, unexplained activity spanning business units or suppliers, or an inability to bound the intrusion. The incident commander can downgrade later; delaying declaration while waiting for perfect attribution increases exposure.

Containment, eradication and recovery

Apply graduated containment

Choose the least disruptive action that blocks the actor, then escalate as evidence requires. Record the expected effect, owner, approval and rollback for every change.

  1. Revoke active sessions, refresh tokens and suspicious credentials.
  2. Reset passwords and enforce phishing-resistant MFA for affected and privileged identities.
  3. Disable compromised accounts, malicious applications, OAuth grants, forwarding rules and persistence mechanisms.
  4. Isolate affected hosts or network segments while preserving forensic images and required safety functions.
  5. Block confirmed command-and-control infrastructure, domains, hashes and unauthorized egress paths.
  6. Restrict privileged access to clean administration workstations and named emergency accounts.

Do not treat a password reset as complete containment if stolen tokens, certificates, service principals, API keys or third-party trust remain valid.

Prioritize identity and cloud control planes

A single compromised account can expose many services. Review privileged-role assignments, consented applications, federation settings, conditional-access changes, mailbox delegates, service principals, access keys, automation identities and audit-log retention. CISA Emergency Directive 24-02, issued April 11, 2024 after the Midnight Blizzard campaign, required analysis of exfiltrated email, credential resets and stronger protection for privileged Azure accounts. Use the same logic for every cloud provider: assume control-plane access can outlive the endpoint where it was first observed.

Eradicate persistence and rebuild trust

After evidence collection, remove scheduled tasks, services, startup items, rogue accounts, malicious applications, mailbox rules, unauthorized certificates, backdoors and altered policies. Reimage or rebuild from known-good sources when integrity cannot be demonstrated. Rotate secrets, keys and certificates; review domain, federation, supplier and remote-support trust; and validate that backups predate the intrusion or are otherwise clean.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Recover in controlled stages

Restore the most critical functions first, with explicit owner sign-off. Validate clean accounts, least privilege, MFA, logging, endpoint controls, network boundaries, backup integrity and business transactions before reconnecting each system. Maintain heightened monitoring for recurrence and keep the campaign case open until the commander and system owners agree that persistence, unauthorized access and residual risk are understood.

Communications and reporting

Operate one fact-checked information flow

Use a single incident information line or controlled collaboration space, with access logging and a named editor. Publish situation reports on a fixed cadence even when there is no major change. Each report should state the reporting period, current severity, confirmed impact, suspected scope, actions completed, actions next, decisions needed, evidence confidence, business or safety effect and owner for each open issue.

Maintain an obligations matrix

Legal counsel should maintain a jurisdiction- and sector-specific matrix covering regulator, customer, partner, insurer, law-enforcement and sector-coordinator notifications. For every obligation, record the trigger, deadline, required content, approving officer, submission channel and proof of delivery. Requirements vary by geography, industry and affected data; there is no universal deadline that can safely replace this matrix.

CISA guidance calls for reporting confirmed incidents to appropriate external entities and considering additional voluntary reporting. CISA AA25-239A also advises organizations to consider all mandatory reporting requirements to relevant agencies and regulators under applicable laws and regulations when malicious activity is suspected or confirmed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Prepare holding statements

A holding statement should accurately describe service impact, protective actions and where customers can obtain updates without disclosing investigative details, indicators or operational weaknesses. Route every external statement through legal, security and communications review. Give employees a separate channel with practical instructions such as password-reset procedures, suspicious-message reporting and how to avoid discussing the investigation publicly.

Exercises that prove the playbook works

Exercise distinct campaign scenarios

  • Credential compromise with mailbox access and token theft.
  • Cloud control-plane compromise involving a privileged administrator or application consent.
  • Espionage with data staging and exfiltration but no destructive malware.
  • Destructive action after a long dwell period.
  • Simultaneous IT and OT disruption requiring manual operation.
  • Supplier or trusted-relationship compromise that crosses organizational boundaries.

Test the uncomfortable dependencies

During each tabletop or technical exercise, verify access to logs, emergency accounts, backups, clean administration devices, contact lists, secure communications, manual workarounds and external responders. Inject unavailable personnel, conflicting business priorities, incomplete attribution and a regulator or journalist inquiry. Require executives to decide when to isolate, notify, shut down or continue operating with risk.

Measure and improve

Record decision delays, unavailable data, false assumptions, missed detections, failed contacts, unsafe dependencies and actions that would have destroyed evidence. Assign every corrective action an owner, due date, verification method and priority. Update detections, checklists, contacts, training, retention settings and continuity procedures after each exercise and real incident. NIST’s preparation resources include tabletop and after-action materials that can support this cycle.

Do not substitute a generic mean-time-to-contain target for evidence. The official sources used for this guidance do not establish a broadly applicable statistic for containing state-sponsored campaigns; campaign complexity, dwell time, sector and authority differ too widely. Track your own baselines, such as time to declare, time to revoke privileged sessions, percentage of critical logs retrievable and percentage of corrective actions verified.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Choose an operating model that matches your risk

Decision axis Questions your playbook must answer Evidence of readiness
Technology coverage Can the team investigate identity, cloud, endpoint, network, SaaS and OT together? Named owners, searchable telemetry and tested cross-domain queries.
Containment authority Who can revoke access or isolate a system, and how quickly can approval be obtained? Written thresholds, alternates and exercise decisions.
Forensic depth Can volatile data, cloud records and configurations be preserved before remediation? Collection procedures, chain-of-custody records and retention coverage.
Continuity and safety Can critical operations continue if IT is unavailable or segmented? Manual workarounds, minimum safe capacity and owner sign-off tests.
Reporting and communications Can the organization meet varied legal and sector obligations without contradictory statements? Maintained obligations matrix, approval chain and situation-report cadence.
Exercises and improvement Are gaps converted into verified corrective actions? Scenario schedule, action register and retest evidence.
External capacity Can the organization obtain DFIR, threat intelligence and surge staffing at any hour? Current retainers, contact tests and documented hand-off procedures.

A small organization may combine roles, but it should not omit responsibilities. A large enterprise may distribute them across regions or business units, but the incident commander still needs one campaign view and authority to resolve conflicts.

Printable first-hour checklist

  1. Open the incident record, assign the commander and start a UTC timeline.
  2. Classify facts as known, suspected or unknown; preserve the triggering alert and related evidence.
  3. Identify affected identities, privileged roles, hosts, tenants, data and third parties.
  4. Activate legal, privacy, communications, continuity, OT and executive contacts as the threshold requires.
  5. Search for related identity, cloud, endpoint, mailbox, network and persistence activity before the first alert.
  6. Protect clean administration paths and emergency accounts; avoid using suspected systems for response.
  7. Choose graduated containment, documenting impact, approval, evidence risk and rollback.
  8. Start the situation-report cadence and update the obligations matrix with counsel.
  9. Record every decision, delay, assumption and unassigned action for the after-action review.

Final standard for readiness

A credible state-sponsored incident playbook is not a binder of generic indicators. It is an authority map, evidence plan, campaign-hunting workflow, continuity design and communications system that people have practiced under pressure. If responders can identify the scope, protect identity and cloud control planes, preserve proof, keep essential operations safe, meet reporting duties and recover in verified stages, the playbook is doing its job.
