If ransomware or a data breach is already unfolding, activate a written incident response plan, isolate affected systems without destroying evidence, establish one decision-making team, and start a separate privacy-and-notification workstream. If you are preparing in advance, write those actions down, keep an offline copy, obtain executive approval, and rehearse them under realistic conditions.
This guide shows how to build a plan that still works when email, identity systems, shared drives, or normal phone contacts are unavailable. It covers ransomware, data extortion, theft of personal information, and incidents that involve more than one of those conditions.

What an incident response plan must accomplish
An incident response plan (IRP) is the organization’s approved playbook for detecting, containing, investigating, communicating about, and recovering from a cyber incident. It is not a list of security tools. It defines who can make high-impact decisions, which systems are dealt with first, how evidence is protected, and how the organization communicates when facts are incomplete.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesRansomware and data breaches often overlap. An attacker may steal credentials, move through the network, copy data, and encrypt systems later. Treating “encrypted files” as the entire incident can leave an earlier compromise active. CISA’s #StopRansomware Guide calls for a cyber incident response plan and a coordinated communications plan covering ransomware, data extortion, breach response, and notification procedures.

Keep the technical IRP and the communications plan as separate documents with shared triggers and contacts. The IRP controls investigation and recovery; the communications plan controls approved facts, audiences, spokespersons, and updates. Both need an offline, quickly accessible copy.
“Create, maintain, and regularly exercise a basic cyber incident response plan (IRP) and associated communications plan that includes response and notification procedures for ransomware and data extortion/breach incidents.” — CISA, #StopRansomware Guide
Write the plan’s scope, activation rules, and authority
Define what is covered
At the front of the plan, list the legal entities, locations, cloud environments, business units, operational technology, third-party services, and information types in scope. State whether the plan applies to suspected events as well as confirmed incidents. Include ransomware, suspected data theft or extortion, loss of personal information, destructive malware, compromised accounts, major service outages, and incidents at critical suppliers.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Record dependencies that can change the response: identity providers, remote-access systems, backup platforms, managed security services, payment systems, production systems, safety systems, and customer-facing applications. Name the business owner for each critical service and the technical and vendor contacts needed to restore it.
Set activation thresholds
Use observable triggers rather than vague language. Examples include a ransom note, mass file encryption, confirmed unauthorized access, credible evidence of data exfiltration, compromise of an administrator account, or an outage affecting a critical service. Allow the security lead or an on-call delegate to activate the plan on suspicion; waiting for certainty can give an intruder more time.
Give people explicit decision rights
Name the incident commander and at least one deputy. Document who may approve network isolation, disable accounts, block remote access, suspend a service, invoke business-continuity procedures, hire forensic specialists, contact law enforcement, and issue an external statement. Include an escalation rule for disagreements and a time at which the commander must brief senior leadership.
Require written executive approval of the plan and make the chain of command understandable to every participant. CISA’s leadership guidance emphasizes senior involvement in preparedness and response; see CISA’s Shields Up guidance for corporate leaders and CEOs.

Assign roles and build resilient contact paths
Assign a person and a deputy for every critical function. A job title alone is not enough: people change roles, may be unavailable, and may not have authority during an emergency.
| Function | Responsibilities to assign in advance |
|---|---|
| Incident commander | Declares the incident, sets priorities, approves major containment and recovery decisions, and maintains the decision log. |
| Security and IT response | Identifies affected assets, contains access, reviews security telemetry, preserves technical evidence, and coordinates rebuilding. |
| Business-service owners | Identify safety, revenue, customer, and operational consequences; set service priorities and acceptable temporary workarounds. |
| Legal and privacy counsel | Directs legal analysis, privilege-sensitive investigation, contractual duties, regulator and law-enforcement coordination, and notification decisions. |
| Communications | Maintains approved facts, drafts audience-specific messages, manages the spokesperson, and sets the update rhythm. |
| Executive leadership | Resolves business trade-offs, authorizes extraordinary spending or service disruption, and communicates organizational priorities. |
| Forensic support | Collects images, memory, logs, malware samples, and indicators of compromise using documented handling procedures. |
| Human resources | Supports employee communications, insider-risk concerns, staffing, and workplace procedures when relevant. |
| Insurer, vendors, and external responders | Provide contracted response, infrastructure, notification, recovery, or specialist capabilities under the terms and contacts recorded in the plan. |
Provide at least two independent ways to reach the response team. Store emergency numbers and escalation instructions on paper or in an offline encrypted file. Define a fallback channel for the loss of corporate email, single sign-on, collaboration tools, or the phone system. Do not make a compromised identity platform the only way to authenticate responders.
Keep a contact owner responsible for quarterly verification and for updating the list after personnel, supplier, or technology changes. Record time-zone coverage and who can make decisions outside business hours.
Prepare the technical facts responders will need
Maintain an asset and dependency view
Keep current inventories of endpoints, servers, cloud accounts, privileged identities, network segments, critical applications, data stores, backup locations, and external connections. Mark systems that support health and safety, essential operations, revenue, legal obligations, or customer access. Include restoration dependencies so that the recovery team does not bring up an application before its identity, database, or network prerequisites are trustworthy.

Name who can collect system images, memory captures, logs, malware samples, and indicators of compromise. Specify the approved tools, storage location, access list, retention period, and chain-of-custody fields. Preserve short-retention and volatile information promptly: memory, active sessions, network connections, endpoint telemetry, authentication logs, and cloud audit records can disappear during routine rotation or a reboot.
Coordinate collection with qualified forensic responders and counsel when appropriate. The Federal Trade Commission’s Data Breach Response: A Guide for Business gives a direct instruction: “Do not destroy evidence.” Avoid wiping or reimaging a device until the incident commander and the evidence lead agree that required information has been captured or that safety and containment require the action.
Define the minimum incident record
Use a single controlled timeline. For every observation or decision, record the time zone, source, person responsible, affected asset, confidence level, action taken, approval, and next review time. Separate confirmed facts from working hypotheses. This prevents an early assumption about the attacker or scope from becoming an uncorrected “fact” in executive briefings or notices.

What should we do first after a ransomware attack?
Use the following sequence as the operational outline in your approved plan. Adapt individual actions to safety, availability, evidence, and legal requirements; a checklist cannot replace an incident commander’s judgment.
- Activate the plan. Declare the incident under the stated threshold and contact the incident commander through an independent channel. Start the timeline and decision log. Identify the next briefing time.
- Establish what is known and uncertain. Record the ransom note or alert, first observed time, affected users and systems, current outages, suspected access paths, and immediate safety concerns. Do not claim a complete scope before investigation supports it.
- Isolate affected systems carefully. Disconnect impacted devices or network segments, disable known malicious connections, and restrict remote access as appropriate. Coordinate the action with the service owner so that isolation does not create an avoidable safety hazard or destroy volatile evidence.
- Protect critical services. Identify systems supporting safety, essential operations, revenue, and legally required functions. Use approved continuity workarounds while technical responders contain the incident.
- Preserve evidence and telemetry. Capture volatile data and retain relevant endpoint, antivirus, intrusion-detection, identity, firewall, cloud, and application logs. Bring in qualified forensic assistance when internal capability is insufficient.
- Investigate for an earlier compromise. Review authentication, privilege changes, remote-access activity, persistence mechanisms, lateral movement, and signs of data staging or exfiltration. Assume the encryption event may be a later phase of an intrusion until evidence shows otherwise.
- Secure identities and access paths. Contain compromised accounts, revoke unauthorized sessions and tokens, rotate credentials according to the response plan, and protect administrator and backup accounts. Sequence these actions so responders do not lock themselves out of evidence or essential systems.
- Coordinate outside parties. Notify leadership, counsel, communications, the insurer, relevant service providers, law enforcement, and government assistance contacts according to the plan and applicable contracts. Record who was contacted, when, and what information was shared.
- Start the breach assessment. Determine whether personal, confidential, regulated, or customer data was accessed or removed. Route the legal and contractual analysis to counsel while technical containment continues.
- Recover in a clean environment. Restore prioritized services from protected backups, rebuild or reimage systems as necessary, validate that the recovery environment is clean, and verify each service before reconnecting it to trusted networks.
- Close deliberately. Continue the timeline through restoration, document unresolved risks, hold a lessons-learned review, assign corrective-action owners and due dates, and update the plan and exercises.
Ransomware containment decisions that belong in the plan
Isolation versus continuity
Pre-write authority for disconnecting a workstation, server, cloud account, or network segment. Include a rapid decision path for systems where an abrupt shutdown could affect safety or essential operations. The plan should tell responders what to isolate immediately, what requires service-owner approval, and who can authorize a temporary exception.
Extortion and ransom communications
Preserve the ransom note, wallet or payment details, chat transcripts, and timestamps as evidence. Route any contact with an extortionist through the incident commander and counsel; do not let an individual employee negotiate independently. The plan should identify who assesses sanctions, legal, insurance, operational, and recovery implications, without promising that payment restores systems or prevents publication.
Credential and remote-access compromise
List privileged accounts, service accounts, remote-management tools, VPN or zero-trust access, and emergency “break-glass” credentials. Define how to disable or rotate them without losing the ability to investigate. Require a review of newly created accounts, altered multifactor settings, suspicious forwarding rules, and changes to backup administration.
Backup integrity
Separate backup administration from ordinary domain administration, protect offline or otherwise isolated copies, and record the order in which backups are validated. A backup that has not been tested or may have been altered during the intrusion is not a clean recovery point. Include a decision for using alternate operations while backup validation continues.
Run data-breach response as a parallel workstream
Identify whose information may be involved
Create an evidence-backed inventory of potentially affected data, individuals, business customers, employees, service providers, and jurisdictions. Distinguish data that was merely present on an affected system from data for which access or exfiltration is supported by logs or forensic findings. Track uncertainty instead of converting it into a definitive statement.
Route legal and contractual analysis to counsel
Notification duties depend on the applicable federal and state laws, sector rules, contractual commitments, the type of information, and the facts established during the investigation. Build a jurisdiction and sector matrix before an incident that lists regulators, affected-person requirements, business-customer clauses, law-enforcement coordination, and internal approval owners. Do not hard-code one universal deadline into a plan for every organization.

The FTC’s August 2023 business guide explains that legal requirements vary and discusses state and federal obligations, as well as separate considerations for health records. Counsel should determine which rules apply, what facts must be established, and when notices can be accurate enough to send.
Prepare useful notices
Draft templates for employees, customers, business partners, regulators, and the public, but leave factual fields controlled by the incident team. Each notice should explain what is known, what is not yet known, what the organization is doing, how people can obtain help, and practical protective steps such as monitoring accounts or changing credentials when appropriate. Use plain language, a monitored contact channel, and a process for correcting facts in later updates.

Build a communications plan that survives uncertainty
Designate one source of approved facts, one approval path, and one spokesperson for each audience. The communications lead should receive scheduled updates from the incident commander and counsel rather than relying on rumor or unverified technical reports.
- Employees: explain how to report symptoms, which systems or devices to stop using, where workarounds are posted, and how to avoid spreading unverified information.
- Customers and partners: state service effects, known protective actions, support channels, and the time of the next update.
- Regulators and law enforcement: provide facts through the approved legal and reporting process, preserving consistency with the forensic record.
- Public audiences: publish only verified information, avoid exploitable technical detail, and do not speculate about attribution, motive, or scope.
Prepare holding statements for a suspected incident, confirmed encryption, suspected data theft, service restoration, and correction of an earlier statement. Set an update rhythm even when there is no material change. A short statement that explains what is being investigated and when the next update will arrive is more useful than silence or unsupported certainty.
Recover from a clean, prioritized state
Order restoration by business and safety impact
Create a restoration list before an incident, with service owners, dependencies, backup locations, validation tests, and acceptable temporary operation for each service. Restore the functions that protect health and safety and sustain essential operations before lower-priority convenience systems, while preserving the evidence needed for the investigation.
Use a clean recovery environment
Keep systems and accounts suspected of compromise out of trusted networks until they have been validated. Rebuild or reimage where appropriate, apply known-good configurations, rotate compromised credentials, and verify administrative access. Restore data only after checking the backup’s provenance and integrity.
Validate before reconnecting
Test authentication, logging, endpoint protection, network controls, backups, application behavior, and data integrity. Confirm that persistence, unauthorized accounts, malicious scheduled tasks, and unexpected remote-access paths are absent. Have the service owner and incident commander approve reconnection, and monitor closely for recurrence.
Best Value
Document alternate operating procedures for the period when restoration is incomplete. Include manual processing, priority customers, safety checks, capacity limits, and the point at which a workaround must be replaced or stopped.
Exercise, review, and improve the plan
Run realistic scenarios
Exercise a ransomware-and-data-theft scenario that includes unavailable email or collaboration tools, uncertain scope, a critical-service outage, a compromised administrator account, and possible notification obligations. Use a tabletop discussion for decisions and a technical exercise where feasible. CISA provides cyber-exercise resources that organizations can use to evaluate or develop their plans.
Measure operational friction
After each exercise or incident, record contact failures, delayed decisions, unclear authority, missing logs, untested backups, vendor escalation problems, and dependencies that were not documented. Assign one owner and one due date to every corrective action. Keep unresolved risks visible to leadership rather than burying them in meeting notes.
Set review triggers
Review the plan after every exercise and incident, after major system, identity, cloud, supplier, or organizational changes, and when applicable laws or contractual requirements change. Verify offline copies, contact paths, restoration instructions, and notification templates as part of the review.
Use current guidance without treating it as legal advice
NIST SP 800-61 Rev. 3 was published on April 3, 2025, and supersedes Rev. 2. It explains how to incorporate incident-response recommendations throughout cybersecurity risk management using the NIST Cybersecurity Framework 2.0, reduce incident impact, and improve detection, response, and recovery.
CISA’s ransomware guide is the ransomware-specific operational reference used for the checklist above; its page metadata identifies a September 2023 revision. Check the live CISA guide page and the CISA resource page for the current document version when maintaining your plan.
The FTC guide is dated August 2023 and is focused on business response to personal-information breaches. It is useful for evidence preservation, communications, and notification planning, but it does not replace advice from qualified counsel familiar with the organization’s jurisdictions, sector, contracts, and facts.

An organization without in-house incident-response capacity should identify optional support before an emergency. Evaluate providers and advisers against the plan’s actual gaps rather than buying a generic promise of “ransomware protection.” Ask whether support includes technical containment, forensic evidence handling, legal and insurer coordination, recovery engineering, communications support, sector experience, 24-hour escalation, and clear contract terms.
A paid tabletop facilitator can make an exercise easier to run, but facilitation is not a prerequisite; CISA also offers no-cost exercise resources. Backup or recovery services can help with protected storage and restoration, but no single product makes an organization resilient if credentials, isolation, testing, and restoration priorities are missing. Specialized privacy or data-breach counsel can help interpret notification duties, while the organization remains responsible for supplying accurate facts and making business decisions.
Quick Recap
Plan completion checklist
- Scope, activation triggers, incident commander, deputies, and decision authority are written down.
- Roles cover security, IT, business services, legal and privacy, communications, leadership, forensics, HR where needed, and key suppliers.
- Offline contacts and fallback communication channels work without corporate email or single sign-on.
- Asset, identity, dependency, backup, and vendor inventories have named owners.
- Evidence collection, chain of custody, log retention, and “do not destroy evidence” instructions are explicit.
- Ransomware containment includes isolation, credential security, remote-access review, and earlier-compromise investigation.
- Data-breach assessment identifies affected data and people, routes legal analysis to counsel, and uses a jurisdiction matrix.
- Communication templates identify approved facts, audiences, spokespersons, update timing, and practical help for affected people.
- Recovery procedures validate backups, use a clean environment, rotate compromised credentials, and test before reconnection.
- Exercises produce assigned corrective actions, and the plan has review triggers for incidents, changes, and legal updates.
