A vulnerability remediation service-level agreement (SLA) works when it turns risk into a specific owner, a defined clock, a feasible deadline, documented exceptions and verified closure. The most reliable design is not a copied table of “critical” and “high” due dates. It combines technical severity with exploitation evidence, internet exposure, asset criticality, business impact, available mitigations and the team’s actual change capacity.
This guide shows how to design that SLA, choose reasonable starting targets, handle vulnerabilities that cannot be patched on time and measure whether the program is reducing exposure without hiding overdue high-risk work.

What a vulnerability remediation SLA should accomplish
An SLA is an operating agreement for vulnerability work, not merely a policy statement. For every finding it should answer six questions:
- What event starts the clock?
- Which risk tier does the finding belong to, and what evidence supports that tier?
- Who is accountable for remediation?
- What counts as an acceptable temporary mitigation?
- What evidence proves the issue is closed?
- Who can approve an exception, and when must that decision be revisited?
The agreement should cover internal teams and, where relevant, service providers or cloud providers. CISA’s implementation guidance for Binding Operational Directive (BOD) 26-04 says appropriate remediation timelines should be included in an SLA or contractual arrangement with a provider. That is guidance in the context of the federal directive, not a universal law requiring every private organization to use one contract format.

CISA issued BOD 26-04 on June 10, 2026. It establishes risk-based security-update prioritization for covered federal agencies. A private organization can use its approach as a reference, but must set its own targets around its assets, obligations, operating model and capacity. NIST’s Secure Software Development Framework (SSDF) likewise describes a vulnerability-response process, not a universal deadline schedule.
Define scope before setting deadlines
A deadline is only meaningful when everyone knows which findings and assets it covers. Write the scope as an explicit inventory rather than assuming that the scanner’s default coverage is the program boundary.
Asset classes to consider
- Employee endpoints and mobile devices
- Physical and virtual servers
- Cloud accounts, workloads, storage and managed services
- Web, mobile and internal applications
- Network and security equipment
- Containers, images and orchestration platforms
- Open-source and commercial dependencies
- Externally managed services where the contract gives your organization remediation rights or notification duties
Record an owner and business criticality for each asset. A central asset register should identify the service owner, technical custodian, environment, data sensitivity, internet reachability and any safety or regulatory relevance. Without those fields, a technically severe issue on an unused test host can receive the same treatment as the same issue on a revenue-generating system.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesFindings that enter the process
State which sources create an SLA record. Typical sources include authenticated vulnerability scans, cloud security findings, vendor advisories, penetration tests, bug-bounty reports, incident investigations and vulnerabilities in application dependencies. A source can have a different intake workflow, but it should still produce a normalized record in the system of record.
| Finding source | Minimum intake action | Clock implication |
|---|---|---|
| Scanner or cloud control | Deduplicate observations, identify the asset and validate that the condition is real | Do not start the remediation clock from an unvalidated duplicate unless policy explicitly says so |
| Vendor advisory | Map the affected product and versions to owned assets and check whether exploitation is reported | Use the agreed advisory or validated-finding event, documented in the record |
| Penetration test or bug bounty | Confirm reproducibility, impact and affected components; attach the tester’s evidence | Start when the finding is accepted as valid under the intake rule |
| Dependency or build pipeline | Identify the shipped versions and release paths, not only the developer workstation | Start at the validated production impact event if that is the organization’s stated rule |
Normalize and validate each finding
Before assigning a tier, create one authoritative record for the vulnerability and its affected assets. NIST SSDF recommends documenting vulnerabilities, gathering enough information to plan remediation, estimating potential impact and remediation effort, and considering other relevant planning factors.
Required record fields
- Stable vulnerability identifier, such as a CVE, when one exists
- Affected asset identifiers, versions and environments
- Validation status and evidence, including the date checked
- Severity method and version, such as CVSS version and vector
- Known exploitation status and the source or observation date
- Internet exposure, reachable attack paths and authentication requirements
- Business, data, safety and availability impact
- Available patch, upgrade, configuration change or workaround
- Estimated remediation effort, testing needs and change-window constraints
- Named remediation owner, security contact and risk owner
Deduplicate scanner results before creating tickets. One vulnerability affecting 400 hosts may be one risk record with an asset list and separate deployment tasks, rather than 400 unrelated deadlines. Conversely, the same vulnerability on isolated development systems and a public production service may need separate risk treatment.

Use severity as an input, not the decision
CVSS or another technical score describes characteristics of the vulnerability; it does not know whether your particular asset is exposed, critical or already protected. Build the priority decision from several signals.
- Exploitation evidence: active exploitation, a reliable public exploit or credible threat-intelligence reporting should raise priority.
- Exposure: internet reachability, public endpoints, lateral-movement paths and weak authentication increase urgency.
- Asset criticality: systems supporting identity, payments, safety, production, backups or essential operations deserve stronger treatment.
- Potential impact: consider sensitive data, privilege escalation, service disruption, fraud and safety consequences.
- Exploit practicality: a low-complexity unauthenticated exploit is different from one requiring unusual local access.
- Control coverage: segmentation, application firewalls, restricted access or endpoint controls may reduce residual risk, but only when their effectiveness is demonstrated.
- Fix availability: a vendor patch, safe configuration change or reliable workaround changes what can be delivered by the target date.
Document which facts can override a nominal severity band and who is authorized to make that call. Reclassify a finding when exploitation evidence, asset exposure or business context changes; do not leave it in its original tier merely because the ticket was created earlier.
Create a small set of staffable tiers
Four tiers are usually easier to operate than a dozen severity bands. Each tier needs a triage target, an interim-mitigation target, a final-remediation target, an escalation point and closure evidence. The following is an illustrative starting framework, not a mandated industry standard. Validate it against recent ticket volumes, emergency-change capability, testing time, vendor availability, patch windows and staffing.
| Tier | Typical trigger | Illustrative starting targets | Escalation expectation | Closure evidence |
|---|---|---|---|---|
| Urgent active exploitation | Confirmed exploitation or a credible, practical attack against an exposed or highly critical asset | Triage the same business day; apply a containment or mitigation within 24 hours when a permanent fix is not immediately safe; deploy and verify the fix as an emergency change or within a few days | Escalate immediately to the security and service owner; executive risk notification before the target is missed | Deployment record plus a targeted verification scan, test or equivalent evidence |
| High-risk exposed or critical-system | High technical risk combined with public exposure, sensitive data or a critical service, whether or not exploitation is confirmed | Triage within one business day; mitigate within several business days if needed; remediate in roughly one to two weeks where testing and change control allow | Escalate when a dependency or change window threatens the date | Successful patch or configuration evidence and validation on every affected asset group |
| Routine material | Material weakness without current exploitation or exceptional business impact | Validate within several business days and schedule remediation in the next suitable release or patch cycle, commonly within about 30 days | Team lead escalation before the due date if work is not scheduled | Release, patch or component-removal evidence followed by verification |
| Lower-risk backlog | Limited impact, constrained exposure and no meaningful exploitation signal | Record and prioritize within the normal queue; complete in a planned maintenance cycle, often within 60–90 days according to capacity | Review aging and concentration monthly; re-tier when context changes | Change evidence or an approved risk decision with a review date |
These example windows are planning aids, not promises that every organization can meet them. A target that repeatedly fails because the normal change process takes six weeks is not a useful SLA; either increase delivery capacity, redesign the control or set a transparent target that risk owners have consciously accepted.
Define the clock and every state transition
Most SLA disputes come from an undefined clock. Write the start and stop events in plain language.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Choose the start event
Possible start events include scanner detection, validated finding, vendor disclosure, internal confirmation of a dependency, or entry into the system of record. Choose one for each workflow and record the timestamp, timezone and calendar convention. “Within five days” is incomplete unless the SLA says calendar days or business days, how holidays are treated and which timezone applies.

Define pauses and stops
Decide whether the clock pauses while waiting for a vendor, a maintenance window, a customer change or additional evidence. A pause should require a reason, an owner and a next action; otherwise it becomes a way to hide aging risk. Define the stop event as verified remediation, not merely a ticket status of “resolved.”
Reclassification, discovery of additional assets or new exploitation intelligence should restart or adjust the applicable target according to a documented rule. CISA’s federal directive illustrates why timelines may change as facts change, but its requirements apply to covered agencies rather than serving as a private-sector universal schedule.
Assign ownership and escalation before work starts
Every record needs one accountable remediation owner, even when several teams contribute. Separate that operational owner from the risk owner who can accept residual risk. Name the security triage contact and the person or group that validates closure.
Recommended Free Tools
- Security intake: validates the finding, sets the initial tier and records the evidence.
- Service or asset owner: plans and delivers the patch, upgrade, configuration change, isolation or retirement.
- Change authority: approves normal or emergency implementation under the organization’s change process.
- Risk owner: approves an exception or alternate treatment and accepts the documented residual risk.
- Independent validator: checks that the vulnerable condition is gone or that the approved control is operating.
Escalate before a deadline is missed when a dependency, failed test, vendor delay or change freeze threatens delivery. The escalation record should show the remaining exposure, decisions requested, interim controls and revised date. Waiting until the due date produces a bad metric and leaves leaders with no safe choice.
Design exceptions and temporary mitigation
A team will eventually encounter a vulnerability that cannot be patched on time because no fix exists, the vendor has not supplied one, the change would interrupt a critical service, or testing reveals unacceptable risk. The correct response is a controlled exception, not silent lateness.
Exception record checklist
- Vulnerability identifier, affected assets and current tier
- Reason the permanent fix cannot meet the target
- Residual-risk assessment and assumptions
- Compensating controls, such as isolation, access restriction, disabling a feature or enhanced monitoring
- Named owner for maintaining each control
- Specific next action toward permanent remediation
- Risk owner’s approval and approval date
- Expiry or review date, with a maximum duration defined by policy
- Evidence that the controls remain effective
A temporary mitigation is an outcome with its own owner and verification, not a note saying “workaround applied.” Test whether the control actually blocks the attack path, monitor for drift and remove it when the permanent fix is deployed. NIST SSDF recommends making a risk-based treatment decision, including risk acceptance or other treatment, and planning temporary mitigation when a permanent solution is unavailable.

Exception workflow
- Notify the security and service owners as soon as the target is at risk.
- Describe the blocker and quantify the remaining exposure.
- Apply and test the interim control.
- Obtain approval from the authorized risk owner before the due date expires.
- Set a review or expiry date and a named permanent-remediation action.
- Reassess when exploitation, exposure, asset criticality or fix availability changes.
Make closure verifiable
Close the SLA only when evidence shows that the vulnerable condition is remediated or the approved alternate treatment is active. Acceptable outcomes can include a deployed vendor patch, removal of the vulnerable component, decommissioning of the asset, a verified configuration change or an approved mitigating action.
Use a verification scan when it can reliably test the condition, but do not rely on a clean scan alone if the scanner cannot see the asset, the version inventory is wrong or the vulnerable code is reachable through another path. Supplement automated evidence with release records, configuration snapshots, test results, asset retirement records or targeted manual validation. Record the verification date, validator and affected asset set. Reopen the item when the vulnerability returns or the control drifts.
Measure both delivery reliability and risk reduction
Report by tier rather than hiding performance in one blended average. Averages can look healthy while a small number of severe findings remain overdue.
| Measure | What it tells leaders | Useful breakdown |
|---|---|---|
| Time from validated identification to verified remediation | How quickly exposure is actually removed | Median and a tail measure such as the 90th percentile by tier |
| Percentage meeting the target | Whether the SLA is operationally achievable | By service team, asset class and finding source |
| Open and overdue exposure | Where current risk remains | Count and age of high-risk or actively exploited findings |
| Exception count and age | Whether exceptions are controlled or becoming permanent backlog | By risk owner, reason and expiry window |
| Reopen rate | Whether closure evidence is reliable | By control type, scanner and remediation team |
| Dependency and change-window delay | Why targets are missed | Vendor, testing, staffing, freeze or ownership cause |
These are implementation choices, not a prescribed NIST scorecard. NIST identifies measuring the time between identification and remediation as a useful vulnerability-management activity, while SSDF emphasizes issue tracking, remediation planning and learning from root causes. Review trends on a defined cadence and after material incidents, major exposure changes or repeated missed targets. BOD 26-04 includes formal annual reassessment for agencies within its scope; that federal requirement is not a universal private-sector cadence.
Connect the SLA to contracts and obligations
Review regulatory, customer, cyber-insurance and contractual requirements before approving internal targets. A customer contract may require notice of a vulnerability or a particular response period; a regulator may define reporting rather than patching; a provider agreement may need a service-level commitment and evidence rights. Map each obligation to the internal tier and identify which party owns the action.

Best Value
Roll out the SLA without breaking operations
- Baseline the current state: sample recent findings, measure actual identification-to-verified-remediation time and list recurring blockers.
- Draft the data model: add fields for asset owner, criticality, exposure, exploitation, tier, clock events, mitigation, exception approval and verification evidence.
- Run a calibration workshop: have security, infrastructure, application, cloud, legal, procurement and business owners classify real findings together.
- Test the targets: replay the proposed tiers against recent ticket volumes, emergency changes, release schedules and vendor response times.
- Pilot one service group: use a limited environment to test notifications, escalation, pauses, exception approvals and closure validation.
- Publish the operating procedure: include definitions, roles, examples, evidence requirements and the escalation path, not only a deadline table.
- Review and tune: inspect overdue exposure, exceptions and reopenings; change targets or capacity based on evidence rather than quietly waiving misses.
How to compare two SLA designs
If you are choosing between an existing severity-based schedule and a risk-adjusted design, compare them on operational behavior rather than on the number of tiers.
| Comparison question | Stronger design |
|---|---|
| Can exploitation and asset context change priority? | Yes; the rule names the signals and the authorized reclassification decision |
| Is the clock unambiguous? | Yes; start, pause, stop, timezone and business-day rules are written in the workflow |
| Are owners and exception authority explicit? | Yes; remediation, validation and risk acceptance are assigned to different accountable roles where appropriate |
| Do targets fit change windows and staffing? | Yes; targets were calibrated against actual delivery data and dependencies |
| Is overdue high-risk exposure visible? | Yes; reports show counts and age by tier instead of only a blended average |
| Does the policy map to obligations? | Yes; contractual, regulatory and internal objectives are clearly distinguished |
Common failure modes and fixes
Copying a generic “critical in seven days” table
Why it fails: it ignores exploitation, exposure, asset criticality and the organization’s ability to change safely. Fix: treat published schedules as references, then calibrate targets with risk owners and capacity data.
Starting the clock at an unvalidated scan result
Why it fails: duplicates and false positives consume the same deadline as confirmed exposure. Fix: define validation and deduplication, record the event timestamp and keep the intake target separate from the remediation target.
Calling a ticket “closed” after a change request is approved
Why it fails: approval does not prove deployment or removal of the vulnerable condition. Fix: require post-change evidence and independent verification.
Letting exceptions expire without a decision
Why it fails: the organization accumulates invisible residual risk. Fix: require an authorized decision, compensating controls, an expiry or review date and a permanent-remediation action.
Reporting only average remediation time
Why it fails: a few urgent, overdue findings can disappear inside a favorable average. Fix: show counts and age by tier, plus median and tail measures when data quality supports them.
The governing principle
“Have a policy that addresses vulnerability disclosure and remediation, and implement the processes needed to support that policy.”

That statement is task RV.1.3 in NIST’s 2020 Secure Software Development Framework. A team-meet-able SLA is the practical implementation: risk is classified with context, work has a named owner, the clock is explicit, exceptions are controlled and closure is proved. Targets should be demanding enough to reduce exposure and realistic enough that teams can meet them consistently.
