A repeatable mortgage quality control (QC) checklist is an operating instrument inside a written QC plan—not a generic list of boxes. Build it by defining risk standards and ownership first, then translating them into prefunding and post-closing review tasks with required evidence, documented sampling, exception handling, management reporting, retention, and independent testing.
The framework below follows Fannie Mae Selling Guide guidance current April 1, 2026, plus a June 29, 2026 Fannie Mae defect-trend article. Treat those items as Fannie Mae examples and requirements; confirm the rules for your investor, products, jurisdictions, and delivery channels.

What a mortgage QC checklist must accomplish
The checklist should let a different trained reviewer reach a consistent conclusion from the same loan file, show exactly which evidence was examined, and allow management or an auditor to reconstruct what happened. Fannie Mae describes an effective QC program as part of the lender’s overall control environment because quality risk can create financial impacts and affect a borrower’s sustainability.
“An effective QC program is a key component of the lender’s overall control environment as quality risk can create financial impacts for the lender and may also affect a borrower’s sustainability.” — Fannie Mae, Selling Guide, “Lender Quality Control Programs, Plans, and Processes,” updated April 1, 2026.

A checklist is therefore only one layer of control. The written plan supplies the standards, risk assessment, governance, independence, procedures, sampling method, reporting cadence, retention rules, and audit approach. The checklist turns those decisions into repeatable work at loan level.
Start with the written QC plan
Define the objective and measurable standards
Write a short objective that states what the program must detect and prevent: fraud, negligence, errors, omissions, eligibility failures, inaccurate data, unsupported underwriting decisions, closing defects, and failures to meet applicable law or program requirements. Convert each risk into a testable standard. “Income is accurate” is too vague; “income used for qualification is supported by the required documentation, calculations are reproducible, and any required reverification is reconciled to the origination file” can be tested.
Define defect-severity levels before reviewing files. Your plan should explain what constitutes a critical, significant, or minor issue, how severity affects escalation, and when a corrected document still counts as a process defect. Use the same definitions in every channel so trend reports are comparable.

Pick the symptom - the matching free tool is one click away.
Set scope across products, channels, and parties
List every population the program covers: retail, broker, correspondent, delegated and non-delegated channels; each mortgage product; automated and manual underwriting; and loans handled by processors, appraisers, brokers, document custodians, or other third parties. Sampling and procedures should make it possible to identify which population a review represents.
Map the laws, investor guides, agency requirements, product overlays, and state or local rules that apply to your footprint. Fannie Mae review guidance includes compliance with laws, but a Fannie Mae checklist cannot substitute for the requirements of another investor or jurisdiction. Record the rule source and effective date for each test so a future reviewer can tell which version applied.
Assign accountable roles and independence
Name the executive accountable for the QC program, the QC manager, reviewers, rebuttal or appeal owners, business-area owners for remediation, and the person who approves checklist changes. Document reporting lines and conflict controls. Prefunding QC should operate independently of production where practical; the same principle should guide post-closing work. A production employee should not be able to suppress a finding or approve their own rebuttal without an independent control.
Version-control the instrument
Give the plan and checklist a version number, effective date, approval record, and change log. Keep the prior version with the files reviewed under it. When a rule, form, underwriting system, or product changes, identify which checklist rows changed, train reviewers, and record the first review date under the new version.

Separate prefunding and post-closing reviews
These stages answer different questions and require different evidence. Keep separate queues, deadlines, and reports even when one team performs both reviews.
| Dimension | Prefunding review | Post-closing review |
|---|---|---|
| Timing | Before closing or acquisition, as applicable | After closing or acquisition |
| Primary purpose | Prevent defects before the transaction is funded or acquired | Measure delivered-loan quality, detect recurring problems, and drive remediation |
| Selection record | Document timing, population, selection method, and verification procedures | Document the population, monthly selection, method, sample size, and any targeted rationale |
| Evidence available | Current underwriting file and outstanding conditions; obtain missing evidence before closing where possible | Final closing package, post-closing documents, reverifications, servicing or delivery data, and any rebuttal material |
| Escalation | Route unresolved defects to underwriting, closing, or management before funding or acquisition | Record loan-level findings, identify trends, assign corrective action, and report results to management |
| Fannie Mae timing point | Review timing must be documented and performed before closing or acquisition, as applicable | Complete the post-closing QC cycle within 90 days from the month of disbursement for originated loans or acquisition for acquired loans |
Do not treat a prefunding pass as proof that post-closing controls are unnecessary. Some defects become visible only after final documents, delivery data, reverifications, or later-discovered liabilities are available.
Design a defensible loan-selection process
Define the population before drawing a sample
For each review period, freeze a population report containing loan identifier, disbursement or acquisition month, channel, product, underwriting method, property type, geography, and reviewer status. Record the date the population was extracted and reconcile it to production or delivery totals. A reviewer should be able to explain which loans were eligible for selection and which were excluded under a documented rule.
Use representative random selection
Random review provides coverage that can support a representative view of delivered-loan quality when the population, method, and sample size are documented. Your written process should state the randomization method, seed or system output retained, replacement rules, and how incomplete files are handled. Sample size should reflect the lender’s risk profile and production characteristics; do not copy a percentage from another lender or present a locally chosen percentage as a universal requirement.
Add discretionary targeted selection
Targeted review complements random selection. Preserve the trigger, the risk signal, the date identified, and the reason the loan was selected. Examples include a new product, a new originator or third party, an unusual occupancy pattern, a high-risk documentation type, a prior defect category, a complaint, a system change, or a concentration in one channel. Label targeted results separately: they are useful for finding risk, but they cannot be used as a population-wide defect-rate estimate.
Calendar the full cycle
Schedule selection, assignment, file access, review, rebuttal, final disposition, management reporting, and corrective-action follow-up. For Fannie Mae post-closing reviews, monitor each monthly cohort against the 90-day completion window from the relevant disbursement or acquisition month. Escalate a projected miss before the deadline rather than waiting for the period to expire.
Build each checklist row around evidence
Every row should contain the requirement or risk, the source document or data field, the reviewer action, the expected result, an evidence reference, a finding code, and follow-up status. Avoid yes/no questions that do not identify what was checked.

Eligibility and underwriting decision
- Identify the investor, product, occupancy, transaction type, loan purpose, and applicable eligibility rules.
- Reperform or independently verify the key underwriting decision using the information available at approval.
- Confirm that every approval condition is satisfied, dated, and supported in the file; distinguish an approved exception from an unmet condition.
- Check that the final terms delivered match the terms approved under the applicable program.
Credit, income, assets, liabilities, employment, and occupancy
- Trace qualifying income, employment history, assets, liabilities, and occupancy from the application through underwriting findings and source documents.
- Check calculations, dates, signatures, and consistency across the application, verification reports, automated underwriting output, and closing documents.
- Determine whether required reverifications were obtained. Reconcile each reverification to the original documentation and explain any change, gap, or unresolved discrepancy.
- Test for undisclosed liabilities, mortgages, debts, or changes in employment and occupancy when the applicable rules require those checks.
Collateral and property data
- Confirm that the property supports the loan and that the appraisal or permitted property-data alternative was completed under the applicable rules.
- Review appraisal quality indicators, comparable selection and adjustments, property eligibility, and any condition or marketability issue relevant to the underwriting decision.
- Document how a collateral concern was resolved, escalated, or accepted under a documented authority.
Data integrity and closing
- Compare application, credit report, income and asset documents, underwriting system data, closing disclosure or settlement data, note, security instrument, and delivery records.
- Check borrower names, property address, loan amount, rate, term, payment, credits, cash to close, subordinate financing, and other material transaction terms for alignment.
- Record the exact document or system field supporting each value that was tested.
Legal, regulatory, and program compliance
- Identify the laws and program requirements applicable to the loan’s product, state, transaction type, and channel.
- Verify required disclosures, timing, tolerances, notices, signatures, and correction procedures under the lender’s compliance matrix.
- Route legal questions to the designated compliance authority rather than allowing a reviewer to invent an interpretation.
Third-party work
- Record which broker, appraiser, verification provider, processor, closing agent, or other party contributed to the file.
- Check whether the third party met the lender’s approval, documentation, service-level, and quality-monitoring requirements.
- Capture evidence of oversight, not merely the vendor’s assertion that work was completed.
Run the review in a consistent sequence
- Confirm scope. Verify the loan belongs to the selected population, identify the applicable checklist version, and record reviewer and review dates.
- Secure the file. Inventory the documents and systems reviewed. Note missing, unreadable, or inaccessible evidence before reaching a conclusion.
- Reconstruct the decision. Read the application, underwriting findings, conditions, approvals, and final terms in sequence so the reviewer understands why the loan was approved.
- Test source data. Trace material credit, income, assets, liabilities, employment, occupancy, collateral, and transaction terms to primary evidence.
- Reconcile reverifications. Compare required reverifications with the original file and explain every material difference.
- Review closing and delivery. Compare final documents and delivery data with the approved decision and applicable eligibility rules.
- Classify each exception. Use the predefined category and severity, cite the evidence, and state the requirement that was not met.
- Complete quality assurance. Have a second reviewer or QC lead check a defined portion of reviews for consistent application of the checklist and severity rules.
- Issue the result. Send the finalized report to the accountable owner, open rebuttal or correction tasks, and update the review log.
Handle findings as controlled corrective action
Record a complete finding
For every issue, capture the loan identifier, checklist row, evidence reference, defect description, category, severity, date found, reviewer, business owner, due date, and whether the issue is loan-level or indicates a broader trend. State the condition, the applicable requirement, and the impact separately. “Income problem” is not an auditable finding; “the overtime income used in qualifying lacks the required history and the calculation cannot be reproduced from the file” tells the owner what must be corrected.

Provide a controlled rebuttal path
Allow the responsible business area to submit evidence or an explanation by a defined date. A rebuttal should identify the specific checklist row, document, or rule supporting its position. The QC owner or an independent authority should decide whether to uphold, modify, or close the finding. Preserve the original finding, rebuttal, decision, and date; do not overwrite history.
Distinguish correction from prevention
A corrected document may resolve a loan-level condition without proving that the process worked at origination. Track both the immediate correction and the preventive action: training, form change, system validation, underwriting guidance, vendor control, or targeted sampling. Assign an owner and expected completion date for each action.
Escalate trends in writing
When the same category appears across loans, issue written corrective action that names the action, expected resolution, responsible area, implementation date, and completion evidence. Trend ownership should sit with the process owner, while QC verifies that the action was implemented and effective.
Report, retain, and audit the program
Make monthly reports useful at two levels
Fannie Mae guidance calls for prefunding and post-closing results to be reported to management at least monthly. A useful report separates random and targeted samples, shows counts and rates by channel, product, underwriting method, defect category, and severity, and identifies open, rebutted, corrected, and overdue items. Include recurring procedural or operational problems, the responsible business area, corrective-action status, and any sampling limitation that affects interpretation.
Do not combine a targeted review’s findings with random results in a way that implies a representative defect rate. Explain the selection basis and population for every metric.
Retain the evidence package
Retain QC reports, the loan files reviewed, findings, reverification documents, rebuttals, decisions, and corrective-action records for at least three years under Fannie Mae requirements. Keep them searchable, access-controlled, and tied to the checklist and plan versions used. If another investor or jurisdiction requires longer retention, apply the longer documented period.

Test the testers
Independently audit whether staff followed procedures and applied assessments consistently. The audit should sample completed reviews, compare conclusions with the evidence, test severity and escalation decisions, and verify that management received reports. Distribute audit results to management and track remediation to completion.
Oversee outsourced QC
Outsourcing review work does not transfer the lender’s responsibility for the QC program. Document the vendor’s scope, sampling method, reviewer qualifications, security controls, turnaround times, defect taxonomy, escalation process, and output validation. Under Fannie Mae guidance, a lender using a QC vendor must review at least 10% of the post-closing QC sample reviewed by that vendor each month. Preserve the lender’s validation results and act on differences between vendor and lender conclusions.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsUse current defect information without misusing it
Fannie Mae’s June 29, 2026 Quality Insider article describes initial significant defects in a discretionary sample of reviews completed from October 2025 through March 2026. The named examples include undisclosed liabilities, borrower employment, interested-party contributions, undisclosed mortgages, occupancy, monthly-payment calculations, debt documentation, and comparable-property adjustments.
Use those observations to ask whether your own risk assessment, training, and targeted checks cover the same areas. Do not present them as a random-sample prevalence estimate, a universal ranking for all lenders, or proof that one category is more common in your portfolio. Compare them with your own random-sample results and documented production characteristics before changing sample allocation.

Common failure modes and fixes
A checklist copied from another lender
Why it fails: It may omit your products, channels, overlays, jurisdictions, or third-party risks, and its sample percentage may not fit your production.
Fix: Start with your written plan, map every applicable requirement, and document the risk basis for scope and sample design.

A repeatable program produces the same chain of evidence every cycle: a defined population, a documented selection decision, a versioned checklist, a reviewer trail, source-linked conclusions, controlled rebuttals, management reporting, retained records, and verified corrective action. If a new reviewer can follow that chain without relying on informal knowledge, and an independent auditor can test it months later, the checklist is functioning as a control rather than a form.
