How to Build a Medicare Advantage Star Ratings Improvement Plan

A workable Medicare Advantage (MA) Star Ratings improvement plan starts with the contract’s current, measure-level baseline for the target rating year. Validate the data and specifications, identify the operational causes of each gap, then assign interventions, accountable owners, milestones and monitoring. Treat CMS’s annual technical notes and cut points as moving inputs—not fixed targets—and connect the work to care coordination and evidence-based clinical protocols.

What a Star Ratings improvement plan should contain

Your plan is an operating document for changing care and service processes, not a form prescribed by CMS. It should let an executive see which measures matter, why performance is off track, who owns the response, what will happen by when, and how the team will know whether implementation is working.

As an Amazon Associate I earn from qualifying purchases.

At minimum, include:

  • the contract identifier, contract type and rating year;
  • the measurement period and the date on which ratings will be displayed or used for quality bonus purposes;
  • a validated baseline for every in-scope measure, including score, star, direction, denominator, exclusions, data source and specification version;
  • a ranked list of improvement opportunities with an explicit rationale;
  • cause analysis tied to the member journey and operating workflow;
  • interventions with an accountable executive, operational owner, milestones and leading indicators;
  • an outcome-monitoring and escalation process; and
  • a decision log for data, intervention and CMS-specification changes.

This framework is recommended operating practice. CMS does, however, require MA organizations to maintain a quality-improvement program based on care coordination. CMS describes the foundation of that strategy as “improving care coordination and providing health care using evidence-based clinical protocols” in its Medicare Advantage Quality Strategy. Use that requirement to anchor governance, clinical review and cross-functional ownership.

1. Set the rating-year scope before choosing a target

Write the scope on the first page of the plan. Specify the CMS rating year, the underlying measurement period, the contract number, whether the contract is MA-PD or MA-only, and whether Part D measures are included. Do not use “2027 performance” as shorthand for several different dates.

  1. Name the rating year. Use the year whose technical notes, measure file and cut points govern your decisions.
  2. Record the measurement period. A measure may use claims, encounter, pharmacy, survey or clinical data from a period that precedes publication of the ratings.
  3. Separate publication from payment timing. CMS’s 2027 fact sheet says ratings published in October 2026 appear in Medicare Plan Finder for the 2027 open-enrollment cycle and affect 2028 MA quality bonus payments. The rating year, enrollment year and payment year are therefore not interchangeable.
  4. Define the contract population. State whether the analysis is for one contract, a sponsor’s group of contracts, or a benchmark view. CMS reports Star Ratings measure data at the contract or sponsor level; a sponsor average cannot substitute for a contract baseline.
  5. Freeze the version used for planning. Save the technical notes, measure specifications, data tables and display-measure files used to set the baseline. Reopen the scope whenever CMS publishes an applicable update.

2. Understand how measures roll up to ratings

Prioritization fails when teams treat every measure as an independent five-star score. CMS aggregates measures into domains and then into summary and overall ratings, with different averaging approaches at each level. For MA-PD contracts, the overall rating combines unique Part C and Part D measures. The exact contribution, weighting and eligibility rules must come from the technical notes for the rating year you are targeting.

Rating level What it represents Planning use
Measure A contract-level result for a defined numerator, denominator, exclusions and direction of “better.” Diagnose the process and select an intervention; verify the current specification and cut-point context.
Domain A group of measures combined under the applicable CMS domain method. Find clusters of related weakness and assess whether a measure change can affect the domain result.
Summary rating A higher-level rating calculated using the CMS method for the applicable contract type. Model the effect of improving one or more domains rather than assuming a one-measure change moves the summary rating.
Overall rating For MA-PD, a combination of unique Part C and Part D measures; MA-only contracts follow the applicable Part C method. Keep Part C and Part D owners aligned and use the current technical notes for the final aggregation.

Annual cut points are not permanent thresholds. CMS recalculates them each year from performance during the relevant measurement period, so a score that earned four stars previously may fall into a different star range later. Set an internal operating target above the expected cut-point risk when possible, but label it as an internal target rather than a CMS promise.

3. Build a validated baseline

Start with the current files on CMS’s Part C and D Performance Data page and the applicable technical notes. That page links data tables, measure files, technical notes and display-measure materials and is regularly updated.

Create one inventory row per measure. The following fields prevent a plausible-looking score from becoming a bad decision:

Baseline field What to capture Validation question
Measure identity CMS measure name, identifier, domain and whether it is Part C, Part D or a combined-rating input. Is this measure in the target rating year, or is it display-only or retired?
Current result Score, star, numerator, denominator and any reported confidence or suppression status. Can the result be reproduced from the source data?
Direction Whether a higher or lower value is better. Could a dashboard’s color coding or “variance” label reverse the interpretation?
Time period Measurement period, run-out date and date the data were extracted. Are claims, encounters, pharmacy or survey files complete enough for a decision?
Specification Current numerator logic, denominator, exclusions, eligible population and data-source rules. Did CMS respecify the measure or change the source in this rating year?
Weight and roll-up Applicable measure weight and the domain, summary or overall ratings to which it contributes. Are you using the method for this contract type and year?
Trend Prior-year results using comparable specifications, plus the direction and size of change. Is the trend real, or does a specification change make the comparison invalid?
Data confidence Completeness, timeliness, encounter acceptance, coding quality, survey response and known integrity issues. Could a data defect explain the gap more plausibly than a care-delivery problem?

Keep an evidence note beside every baseline value. If the specification changed, show the old and new definitions and mark the trend as non-comparable rather than forcing a year-over-year conclusion.

4. Prioritize opportunities with explicit criteria

Do not rank measures by the lowest star alone. Use a transparent review that balances rating leverage with beneficiary impact and the organization’s ability to change the underlying process. A practical internal screen considers:

  • Current gap and trend: distance from the current cut-point context and whether performance is improving, flat or deteriorating.
  • Rating leverage: measure weight and potential contribution to a domain, summary or overall rating under the current method.
  • Data confidence: whether the result is complete, timely and specification-consistent.
  • Operational influence: control over provider workflows, outreach, access, documentation and submission.
  • Beneficiary impact: consequences for outcomes, safety, access, experience and continuity of care.
  • Equity and access signals: disparities that are validly measured and large enough to warrant action without making unsupported causal claims.
  • Time to effect: whether a process indicator can move in weeks while the rated outcome may require months.
  • Feasibility and risk: staffing, clinical review, compliance, member communication and the possibility of unintended effects.

Use a simple 0–3 score for each criterion if a common language is needed, but keep the scoring rule in your governance document and do not represent it as a CMS formula. Separate priorities into three portfolios:

Portfolio Typical characteristics Management approach
Stabilize now Large gap, high confidence, strong operational control and a short feedback loop. Launch a defined intervention, review leading indicators frequently and escalate execution barriers quickly.
Build capability Important measure with access, provider, data or workflow constraints that require preparation. Fix infrastructure, test the workflow and set a realistic outcome horizon.
Protect and monitor Currently strong, high-impact measure vulnerable to drift or specification change. Maintain controls and watch for denominator, data or cut-point movement.

5. Diagnose the cause before selecting an intervention

A low result is a signal, not a diagnosis. Map the member journey and the internal workflow for each priority measure. Review contract-level findings alongside valid provider, geography, product, language, age or other stratifications, protecting privacy and avoiding conclusions from unstable small cells.

Identification and eligibility

Determine whether eligible members are being identified on time. Check attribution, enrollment files, exclusions, risk or clinical flags and handoffs from care management. An apparent outreach problem can actually be a denominator or eligibility defect.

Outreach and engagement

Measure reach, contactability, language access, channel mix, refusal rates and the interval from identification to first contact. Confirm that scripts and notices are accurate and that escalation paths exist for members who cannot be reached.

Appointment or service access

For measures requiring a visit, test, medication or follow-up, examine appointment supply, transportation, network adequacy, prior authorization, pharmacy availability and scheduling latency. A reminder cannot fix a service that members cannot obtain.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Completion and clinical delivery

Observe whether clinicians and care teams perform the required action using an evidence-based protocol where appropriate. Look for variation in workflow, standing orders, referral closure, medication reconciliation and follow-up after an abnormal result. Involve clinical leadership before changing care processes.

Documentation and data capture

Trace a completed service into the medical record, encounter or pharmacy file and then into the submitted measure data. If care occurred but is not captured, documentation standards, coding education, interface defects or submission timing may be the real intervention point.

Measurement interpretation

Reconcile your local calculation with CMS’s specification. Confirm numerator logic, exclusions, data lag, survey methodology and any respecification. Do not label a measure a care-quality failure until data and definition checks are complete.

6. Match interventions to the diagnosed mechanism

Write the causal link in one sentence: “Because identified mechanism is limiting measured event, we will change specific process.” That discipline prevents generic reminders or software purchases from becoming unfocused programs.

Diagnosed mechanism Potential intervention design Guardrail and leading indicator
Members are not identified or attributed reliably. Repair eligibility and attribution logic, establish a daily or weekly work queue, and reconcile exceptions with enrollment and clinical teams. Track queue accuracy, aging and false-positive rates before judging the rated outcome.
Members cannot be reached or do not understand the action. Use language-appropriate, accessible outreach; test channel and timing; offer assisted scheduling and a documented escalation route. Monitor successful contact, informed agreement and scheduled-service rates, not calls placed alone.
Access or network capacity is the bottleneck. Reserve appointment capacity, close referral loops, address transportation or pharmacy barriers and escalate network gaps. Track days to service, no-show recovery and unresolved access cases.
Provider workflow is inconsistent. Embed evidence-based protocols, standing orders, decision support or team huddles after clinical review; give providers feedback they can act on. Monitor protocol adoption, appropriate completion and safety exceptions.
Valid care is missing from the measure data. Standardize documentation and coding, repair interfaces, train staff and reconcile source records to submissions. Track data completeness, acceptance rates, error types and lag.
The specification or cut point changed. Rebaseline with the current CMS definition, document comparability limits and reset internal milestones. Track file-version confirmation and the date of each CMS update.

Every intervention should name its target population, inclusion and exclusion rules, responsible teams, start date, expected mechanism, risks and stop or adjust criteria. Do not promise that any intervention will raise a star rating; annual cut points and the performance distribution can move even when execution improves.

7. Assign accountability, milestones and measures

Use one accountable executive for the result and one operational owner for day-to-day delivery. Add named data, clinical, compliance, provider-network and member-experience partners when the intervention crosses their authority. A useful intervention charter contains:

Charter element Required decision
Objective Which measure and population will change, under which rating-year specification?
Accountable executive Who resolves priority conflicts and accepts residual risk?
Operational owner Who runs the workflow and reports status?
Milestones What must be true at design, pilot, scale and sustainment checkpoints?
Leading indicators Which reach, access, completion, documentation or data-quality signals should move first?
Outcome measure What rated result will be reviewed, and when can it reasonably change?
Data owner Who certifies definitions, refresh dates, completeness and defects?
Review cadence Which forum reviews weekly execution, monthly performance and quarterly re-prioritization?
Escalation rule What threshold or missed milestone triggers executive action, redesign or pause?
Controls What clinical, compliance, privacy, accessibility and member-communication checks are required?

Keep the charter versioned. Record why a target, denominator, owner or intervention changed, who approved it and which CMS document or internal data justified the change.

8. Monitor execution, outcomes and unintended effects

Use different cadences for different signals

  • Weekly or biweekly: workflow completion, queue aging, contactability, appointment capacity, referral closure, data-feed failures and urgent safety issues.
  • Monthly: stratified process indicators, provider variation, member complaints, access barriers, documentation completeness and intervention reach.
  • Quarterly: measure results when available, domain and rating-model implications, equity patterns, cost and feasibility, and whether priorities still fit the current CMS specification.

Plot results over time instead of reacting to one data point. Compare intervention and non-intervention populations only when the design supports a fair comparison; otherwise describe movement as an association, not proof that the intervention caused it. Watch for unintended effects such as inappropriate testing, unnecessary visits, member fatigue, provider burden, inequitable reach or documentation that does not reflect actual care.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When CMS releases new technical notes, measure files or data tables, run a specification-impact review. Reconfirm measure status, weights, cut points, denominator rules and display or retirement changes before carrying targets into the next cycle.

2027 Star Ratings context to include in your plan

CMS published the 2027 Medicare Advantage and Part D Star Ratings on October 8, 2026. For those ratings, MA-PD contracts are rated on up to 43 unique measures and MA-only contracts on up to 32. The fact sheet describes two newly added measures, respecifications and a measure returning from the display page, increased weights for Improving or Maintaining Physical Health and Improving or Maintaining Mental Health, and measures moved to display or retired. These details apply to that rating year; verify later years separately.

National results are useful context but are not a contract target: 37% of MA-PD contracts—188 contracts—earned four or more stars for 2027, and approximately 71% of MA-PD enrollees were in contracts rated four stars or higher. A contract’s mix, data, provider network and measure performance determine its own result.

The Medicare 2026 Part C & D Star Ratings Technical Notes explain aggregation and technical definitions for that rating year. Use the notes that match your target year rather than carrying forward a familiar formula. For policy context, the Contract Year 2026 Policy and Technical Changes final-rule fact sheet, issued April 4, 2025, is a reminder to rely on finalized current rules and technical materials rather than an earlier proposal.

A practical 90-day launch sequence

  1. Days 1–15 — establish scope and data control. Name the rating year, measurement period, contract and contract type; download the applicable CMS files; assign a data steward; and document known lags or integrity concerns.
  2. Days 16–30 — certify the baseline. Reconcile each measure’s score, star, denominator, exclusions, direction, specification, weight, trend and source. Obtain clinical and compliance sign-off on interpretation.
  3. Days 31–45 — rank and diagnose. Apply the agreed prioritization criteria, select a manageable portfolio, map the member journey and validate causes with operational, provider and member-facing teams.
  4. Days 46–60 — design and approve interventions. Complete charters, define leading and outcome measures, test risks, assign owners and approve pilots.
  5. Days 61–90 — pilot, review and scale. Launch in a defined population, review leading indicators at the planned cadence, correct execution barriers, and scale only when the workflow is reliable and safe.

Failure modes that undermine otherwise strong plans

  • Using last year’s cut point as a promise: annual recalculation can move the threshold.
  • Mixing rating and measurement years: publication and payment timing do not define the data period.
  • Optimizing a low score without checking leverage: a large gap may have little effect on the applicable roll-up.
  • Diagnosing from a contract average alone: averages hide provider, access, population and data-capture differences.
  • Calling a data problem a care problem: incomplete encounters or changed specifications can create an artificial gap.
  • Launching generic outreach: reminders are ineffective when the real constraint is access, workflow or documentation.
  • Counting activity instead of outcomes: calls, training sessions or alerts do not prove completion or better care.
  • Ignoring display and retired measures: resources can be wasted on measures that no longer contribute as assumed.
  • Leaving accountability diffuse: a committee can review a measure, but one owner must be responsible for changing the process.

The Bottom Line

Build the plan as a living contract-level operating system: current specifications and data first, explicit cause analysis second, targeted interventions with named owners third, and disciplined monitoring throughout. Revalidate the plan whenever CMS changes the measure set, weights, specifications or cut points.
