- Access to the second island, also called Forest Island or Island 2 in guides.
- Three leeches to exchange with the woman by the lake.
- A ranged weapon and healing or food, prepared before you cast the bait.
The bait is described as “Modified Leech” in some guides and “Leech Bait” in others. Use the item name displayed in your inventory. Casting it begins the encounter, so finish preparing first.

How to beat the Giant Piranha
Choose a spot and loadout before casting
Start from open ground where you have room to move. Bring healing or food and equip a ranged weapon before triggering the fight. Guides mention both a pistol and a shotgun; they do not establish one as the universally best choice. Pick a weapon you can use reliably at your preferred range and with the resources you have.
Keep moving and manage the smaller piranhas
Do not stand still to attack. Keep moving so you have room to react, and focus damage on the Giant Piranha whenever you have a clear opportunity. Smaller piranhas can disrupt movement or make aiming difficult; deal with them when they block your route or interfere with your shot, then return your attention to the boss.

Weapon choice is a practical trade-off, not a proven ranking: consider whether you can keep moving while dealing damage, whether the weapon works at your range, its ammunition or resource cost, and how easily you can manage smaller piranhas. No controlled comparison establishes a winning loadout.
What to do after the kill
- Pick up the Giant Piranha Skeleton.
- Return it to the woman beside the lake.
- Complete the hand-in to receive the coordinates or access needed to continue toward Island 3.
The boss’s defeat alone does not complete this progression step; recover and hand in the skeleton.

A guide updated September 8, 2026 says it reviewed material through patch 1.0.12, but also discloses that it was not independently playtested.
