How to Audit and Monitor AI Agent Actions Across Tools

To audit AI agent actions across tools, connect every step in a run with a stable trace ID, record both each tool-call request and its outcome, and check authorization at the execution boundary—not in the agent’s own reasoning. Monitor service health and security behavior together, and limit sensitive trace data to what your debugging, incident-response, and audit needs require.

What should an AI agent audit trail capture?

Design the trail around a complete run, not a collection of disconnected tool logs. Give each run or conversation a stable identifier and propagate it through the agent, model, retrieval, tool, and subagent steps. OpenTelemetry can provide a common telemetry layer; the OWASP Agent Observability Standard describes agent-specific extensions to OpenTelemetry and OCSF concepts. See the OWASP Agent Observability Standard trace overview and supported events.

For each tool interaction, create a record before execution and another after it. The OWASP event model calls these toolCallRequest and toolCallResult, linked by an execution identifier. The request shows what the system attempted; the result shows whether it was allowed, denied, failed, or completed. Keeping both prevents a successful action from being confused with an attempted one, or a denial from disappearing among successful calls.

  • Correlation and identity: run or trace ID, execution ID, agent and tool identities, and the initiating user, service, or trigger.
  • Action details: tool name, action, target, timestamp, and arguments—or a redacted or otherwise privacy-safe representation.
  • Decision evidence: risk classification, permission context, authorization result, approval identifier when required, and the policy version used.
  • Outcome: success, denial, failure, or completion; relevant result or error details; and execution time.
  • Context boundaries: retrieval source and provenance, material memory reads or writes, and agent-to-agent or MCP interactions where those boundaries exist.

Microsoft’s observability guidance calls out execution details such as tool names, arguments, permissions, and outputs. OWASP’s event specification also describes knowledge retrieval, memory storage and retrieval, and A2A/MCP protocol events. Treat these as useful boundaries to instrument, not a direction to indiscriminately store every payload: Microsoft Learn: Observability for Generative AI and agentic AI systems and OWASP supported events.

How can you trace tool calls across a run?

  1. Start at the trigger. Create the run ID when a user request, scheduled job, or service event starts the work. Record the initiating identity or trigger and pass the ID into each component that handles the run.
  2. Instrument the handoffs. Record model and agent steps, retrieval, tool execution, and subagent calls as related spans or events. Include an execution ID for each tool interaction so its request and outcome can be paired.
  3. Emit the request before invoking the tool. Record the intended tool, target, arguments or safe representation, the actor’s permission context, and the authorization decision. A denial should be recorded as a decision; do not invoke the tool after it.
  4. Emit the outcome after the call. Record whether execution completed, failed, or was denied, along with relevant result or error details and a timestamp. Preserve failed attempts as well as successful actions.
  5. Verify trace coverage. Follow a test run end to end in your query or tracing system. Check that IDs survive service boundaries and that the trace includes downstream services that can actually perform the action—not only the agent framework.

These steps describe a useful event design, not a requirement to capture full prompts or outputs. Define what each event contains according to the investigation purpose and data contract.

How should consequential actions be authorized?

Observability and enforcement solve different problems. A trace explains what the instrumented system recorded; an authorization boundary determines whether an action can execute. An agent’s proposed tool call is not evidence that the actor is permitted to make it. Put the permission and approval check in the execution component or a policy service that stands between the agent and the tool.

For destructive, financial, administrative, or externally visible actions, use risk-based authorization and approval at that boundary. Bind an approval to the exact actor, tool, target, normalized parameters, time, and expiry. A broad, session-wide approval does not show that the user approved a particular consequential operation. Use short-lived authorization artifacts and replay protection for irreversible actions, and make operations idempotent where possible.

Do not let an unknown or unmapped action silently inherit a low-risk classification. The execution component should still verify authorization and any required approval even if the action has already been classified. OWASP’s AI Agent Security Cheat Sheet recommends failing closed when risk classification, approval validation, policy lookup, or audit logging fails. In practical terms, if the system cannot verify a required control or record the required event, it should not carry out the protected action.

What should you monitor and alert on?

Combine operational health with policy and behavior signals. Establish a baseline for your own system, then alert on meaningful deviations rather than treating any example threshold as a universal default.

  • Service health: latency, errors, token use, request volume, and tool-call volume.
  • Action outcomes: tool failures and changes in the mix of successful, denied, and failed calls.
  • Security behavior: elevated privilege use, unusual invocation frequency, repeated attempts to bypass approval, approval drift, and increases in high-risk actions.
  • Task quality: evaluation results for safety, groundedness, quality, and tool-use correctness where your system supports those checks.

Route alerts to an owner who can investigate the linked run and its decision evidence. Tune alerts against observed normal behavior for that agent, tool, and workload; example thresholds in OWASP sample code are illustrative, not validated benchmarks for every system. Microsoft also recommends monitoring usage and performance alongside security-relevant activity in its observability guidance.

How do you protect sensitive telemetry?

Tool arguments and outputs, retrieved documents, memory, prompts, and messages between agents may contain sensitive information. More detail can help an investigation, but it also increases exposure. Write a data contract that specifies what telemetry is collected, who can access it, why it is retained, and when it is deleted.

  • Collect only the fields needed for operational monitoring, investigation, and audit.
  • Use redaction, hashes, identifiers, or other privacy-safe representations when full arguments or outputs are unnecessary.
  • Restrict access and encrypt telemetry in transit and at rest; apply relevant residency, retention, and legal requirements.
  • Separate access to sensitive payloads from access to routine service metrics where practical.
  • Protect and monitor the telemetry pipeline itself so that logging failures or unauthorized access do not go unnoticed.

Microsoft’s guidance emphasizes balancing forensic needs with privacy, minimization, data residency, retention, and legal obligations. The reviewed documentation does not establish one retention duration or a universal tamper-evidence standard for all deployments; set those requirements for your organization and applicable obligations rather than assuming a trace is immutable or sufficient proof on its own.

How should you choose tracing and observability tools?

Start with the event and control requirements, then assess whether a platform can support them within your existing identity, policy, security, and compliance workflows. Compare candidates on these practical dimensions:

  • Coverage of model, tool, retrieval, and subagent spans, and the ability to correlate a full run.
  • Recording of both pre-execution requests and post-execution results.
  • Export or query support for telemetry standards and integration with your SIEM and other workflows.
  • Privacy controls, access management, data location, and retention configuration.
  • Instrumentation effort and ongoing operating cost.

OpenAI documents agent, generation, and tool spans, with optional OTLP JSON trace export subject to configuration and permissions in its Agents API tracing documentation. Amazon OpenSearch Service documents hierarchical agent traces, GenAI semantic conventions, and OpenTelemetry integration in its AI observability documentation. These are examples of documented capabilities, not a comparative performance test or endorsement; verify current documentation for the version and deployment you plan to use.

As a deployment example rather than a universal logging requirement, OpenAI’s May 8, 2026 article Running Codex safely at OpenAI describes Codex telemetry that can include prompts, tool approval decisions, execution results, MCP server usage, and network proxy allow/deny events, with logs centralized in SIEM and compliance systems. Choose the scope of your own telemetry according to your threat model and data contract.
