A trustworthy AI-agent audit is an attribution chain, not a transcript of the agent’s final answer. You should be able to connect the initiating person or workload, tenant, agent identity, authorization decision, tool call, target resource, approval, and result across every system involved. That requires inventory, explicit authorization outside the model, consistent event fields, correlation IDs, protected retention, and a tested response process.
No single vendor log automatically provides this view across all company accounts. Coverage depends on the audit events each connected service exposes and on the instrumentation you add around the agent and its tools.

What an AI-agent audit must prove
An agent can read a customer record, create a draft, call an external API, send a message, or change permissions before producing one harmless-looking chat response. The response describes the end state; it does not prove which identity acted, which policy allowed the action, what data was accessed, or whether a human approved it.
Define an auditable chain with these links:
- Initiator: the human, scheduled job, application, or workload that started the request, plus the tenant or customer context.
- Agent: the specific agent instance, owner, version or blueprint, and delegated or app-only authority it used.
- Decision: the policy evaluation, consent, approval, elevation, or block that occurred before execution.
- Tool and target: the connector, operation, API, account, record, file, message, or permission object affected.
- Outcome: start and end times, status, returned result, error, and any follow-on action.
If one link is missing, investigators may know that an agent ran without being able to establish what it changed or whether the action was authorized.

1. Discover every place agents can act
Build one inventory
Create a maintained inventory covering development, test, and production agents. Record the agent’s owner or sponsor, business purpose, runtime, model workflow, connected APIs, plugins and tools, tenants, data resources, credentials, and downstream services. Include scheduled agents, embedded assistants, automation scripts with model calls, and temporary experiments.
Microsoft’s least-privilege guidance for AI agents recommends discovery, named ownership, documentation of purpose and dependencies, and review of effective permissions. Apply the same process to guest and cross-tenant connections.
Find shadow agents
Sanctioned systems are only part of the surface. Developers or business teams may connect a model to a shared mailbox, database, ticketing system, or personal automation without registering it. An unregistered agent bypasses normal access review and monitoring. Microsoft’s secure-agents guidance recommends a single inventory and discovery process and warns that broad pre-existing data access becomes more visible when an agent retrieves and summarizes content.

Calculate effective access
Do not assess each grant in isolation. Combine role assignments, group membership, delegated consent, API scopes, tool permissions, tenant relationships, and downstream service rights. Several narrow grants can produce broad effective access when an agent chains tools. Recalculate after a tool, model workflow, purpose, or data scope changes.
2. Make identity and ownership unambiguous
Give each agent a lifecycle-managed identity
Where the platform supports it, assign a distinct identity to each agent rather than sharing a human account or one credential across multiple agents. Record the accountable owner, sponsor, approver, environment, creation date, and retirement path. Microsoft’s recommended wording is: “Use a unique, dedicated agent identity with a named owner/sponsor and approver.” See the Microsoft least-privilege pattern.
Keep human, application or workload, and agent principals distinguishable in logs. For delegated operations, preserve both the initiating user and the agent that exercised the delegated authority. For app-only operations, record the application identity and the policy that allowed it. AWS describes signed user-context claims as one way to pass user context without giving an agent the user’s credentials, and recommends short-lived credentials and permission boundaries in its agent identity and permission guidance.

Review access as a changing graph
Access review should include every tool and downstream system, not just the agent’s home platform. Remove stale credentials, grants, consent, and service connections. Revoke an identity when its owner leaves, its purpose ends, or its workflow changes. Keep a record of each review and its decision so an investigator can distinguish an intentional permission from drift.

A model can propose an action, but application or policy code must decide whether it may execute. Before each tool call, evaluate:
- the initiating user or workload and tenant;
- the agent identity and tool;
- the operation’s risk class;
- the target resource and data scope;
- the role, consent, or delegated permission supporting the request;
- whether human approval or just-in-time elevation is required; and
- whether to allow, block, log, or escalate the call.
Microsoft’s access-pattern guidance says authorization should be enforced outside the model and separates read and write actions. Treat the following classes differently:
| Action class | Typical examples | Control to record |
|---|---|---|
| Read | Search, retrieve, summarize, or inspect a record | Allow only the required resource scope; log the resource and result status. |
| Draft | Prepare an email, ticket, code change, or purchase request | Keep execution separate from drafting; record who reviewed or released it. |
| Write or send | Update a record, send a message, publish content, or create a ticket | Require an explicit policy decision and, where risk warrants, human approval. |
| Irreversible or privileged | Delete data, provision infrastructure, change permissions, add credentials, or transfer funds | Use narrow scopes, time-bound elevation, approval, and a durable outcome record. |
Allowlist tools and operations instead of granting a general-purpose connector. Bind credentials to the smallest resource scope and shortest useful lifetime. A compromised prompt, model, or tool should not automatically inherit every permission available to the user.
4. Capture the complete execution path
Define a common event record
Field names differ by product, but a useful application-side event contains, when available:
| Field group | Minimum useful data |
|---|---|
| Time and result | Event timestamp, duration or start/end, status, error class, and outcome. |
| Initiator | User or workload identity, tenant or customer, authentication context, and session or request ID. |
| Agent | Agent identity, owner, environment, version or blueprint identifier, and delegated versus app-only authority. |
| Action | Tool name, connector, operation, risk class, target resource identifier, and requested data scope. |
| Governance | Policy name and result, consent or role used, approval identity, approval decision, and elevation interval. |
| Correlation | Native provider event, session, and request IDs plus distributed trace_id, span_id, and parent relationship when supported. |
Store identifiers and metadata needed for investigation, but do not put passwords, access tokens, secrets, full sensitive documents, or unnecessary personal data into telemetry. Redact before export and restrict who can query the resulting records.

Understand provider-specific representations
Microsoft Entra’s agent sign-in and audit documentation notes that agent-related activity may appear as application, service-principal, or user events. An agentType field can identify agent involvement, and delegated and app-only sign-ins can appear in different log types. Searching for one generic “agent action” event will therefore miss activity.
Instrument the orchestrator, policy service, tool adapters, and your own business APIs. Preserve native IDs from external SaaS audit logs even when you cannot change their schema. A native event ID is often the only stable handle for a provider-side investigation.
5. Correlate records across accounts and tools
Propagate trace context where you control the path
Pass a distributed trace context from the incoming request through planning, policy evaluation, tool execution, downstream API calls, and the final response. The OpenTelemetry logging specification explains that TraceId and SpanId let investigators correlate logs and traces from the same execution context. Add resource attributes such as tenant, service, environment, and agent version.
Recommended Free Tools
Use child spans for policy checks, approvals, each tool call, retrieval operation, and external request. Record start and end events so a denied or timed-out call is visible, not just a successful one.
Join systems that cannot carry trace context
Legacy and third-party logs may not accept a trace header or may omit request context. Correlate them with native event, session, request, resource, and timestamp fields. Keep clock synchronization under operational control and record the source timezone. A time-and-resource match is weaker than a propagated trace, so mark the confidence of such joins in investigative tooling.
AWS OpenSearch’s AI observability documentation shows hierarchical traces spanning orchestration, model calls, tools, and retrieval. Google Cloud Logging’s correlation guidance describes grouping entries that share a trace field. These are implementation examples; they do not automatically capture events from every unrelated SaaS account.

6. Protect, retain, and govern the audit trail
Separate audit data from mutable application logs
Restrict alteration and query permissions, separate administrator duties where practical, and export high-volume events to a secured archive. Keep an audit record of exports, retention-policy changes, and access to the audit store. Centralization and correlation improve investigation, but they do not by themselves make records tamper-proof or non-repudiable.
Set retention for your obligations
Choose retention based on contractual, legal, regulatory, security-investigation, and regional requirements. Microsoft’s Entra Agent ID best practices call for covering the applicable compliance duration; they do not establish one universal period. Document the chosen period, deletion process, legal holds, and the systems whose native logs expire sooner.
Minimize sensitive telemetry
Log a resource identifier and operation rather than copying an entire document or message. Hash or tokenize values when investigators can work without the original. Keep secrets, tokens, authentication assertions, and unnecessary personal data out of traces. Ensure that redaction occurs before data reaches a broad observability workspace.
7. Test whether the chain is actually complete
Use a controlled test plan in each environment. For every workflow, ask an investigator to identify the initiator, tenant, agent, tool, target, policy result, approval, and outcome from the records that are really available.
- Read test: retrieve one known record and verify the target identifier, scope, user context, agent identity, and successful result.
- Draft test: generate a message or ticket without sending it; verify that drafting and release are separate events.
- Permitted write: perform a low-risk update and confirm the before/after outcome and policy decision.
- Denied action: attempt a prohibited operation and confirm a denial event, reason, initiator, and absence of downstream execution.
- Approval-gated action: request a high-impact operation, approve it with a test identity, and verify approval, execution, and final result are linked.
- Cross-system action: traverse two connected services and test whether trace or native IDs join the records; document any gap where context is lost.
Repeat after adding a connector, changing a model workflow, moving tenants, or altering policy. Treat an unjoinable event as a coverage defect, not as proof that the action did not occur.
8. Monitor, investigate, and contain
Alert on identity and permission anomalies
Monitor unexpected sign-ins, excessive failed token requests, new credentials, permission or role changes, unusual tool access, access to unfamiliar APIs or resources, and sudden changes in agent behavior. Baselines should be tied to the agent’s documented purpose and normal tenant scope.
Best Value
Use the chain during incidents
Start with the suspicious native event or resource, pivot through request and trace IDs, identify the agent and initiator, inspect the policy and approval records, then enumerate downstream calls. Review effective access across all connected systems; a single revoked permission may not stop a second path to the same data.
Maintain a tested stop path
Keep a procedure to pause the agent, disable its workload identity, revoke delegated consent and credentials, and block downstream tokens. Test that containment reaches each connected service. AWS guidance describes permission-drift detection, access reviews, and revocation as maturity practices; use those reviews to verify that emergency controls still work.
What documented platforms show—and what they do not
Product documentation demonstrates useful patterns, not universal coverage. Microsoft’s Azure SRE Agent audit documentation lists separate events for model generation, tool execution, agent handoffs, and approval decisions with shared correlation fields. That schema is an example of one product’s telemetry design.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Similarly, an identity provider can show a sign-in while the target SaaS service holds the decisive record of which file, ticket, or message was touched. A tracing system can link components that emit context but cannot reconstruct a request context omitted by a legacy connector. Validate every integration’s event types, API permissions, export path, retention, and regional availability before declaring coverage.
The NIST NCCoE 2026 concept paper treats agent identity metadata, authentication and key revocation, least privilege, delegated authority, human-in-the-loop authorization, tamper-resistant logging, and non-repudiation as open questions for a research agenda. It is not a finalized universal standard, so describe your controls precisely rather than claiming that correlation alone proves non-repudiation.
How to compare audit implementations
Choose an architecture by the evidence it can produce, not by the presence of a chat history or a dashboard. Compare alternatives on these axes:
| Decision axis | Questions to ask | Evidence of adequate coverage |
|---|---|---|
| Identity attribution | Can records distinguish each agent, its owner, the initiator, tenant, and delegated versus app-only authority? | Stable identities and linked user, workload, and agent fields. |
| Event depth | Are policy checks, approvals, tool start/end, target resources, errors, and outcomes captured? | Events exist for allowed, denied, failed, and approved paths. |
| Cross-system correlation | Can context move from orchestrator to identity provider, SaaS tool, and downstream API? | Trace/span propagation where possible, native IDs and timestamp/resource joins elsewhere. |
| Coverage and gaps | Which services expose audit events, which require custom instrumentation, and which omit request context? | A documented integration matrix with known limitations and owners. |
| Review and containment | Can owners review effective access, detect drift, revoke credentials, and disable an agent quickly? | Scheduled reviews and a tested pause-and-revoke runbook. |
| Data handling | Can telemetry be minimized, redacted, access-controlled, exported, and retained for the required period? | Protected storage, documented retention, and audit-store access logs. |
| Operational fit | What permissions, licensing, deployment model, geography, and export capability are required? | Validated requirements for your actual accounts and jurisdiction; no universal cost or licensing assumption. |
Common audit failures and fixes
| Failure | Why it fails | Fix |
|---|---|---|
| Keeping only the final chat transcript | It omits hidden retrieval, tool calls, policy decisions, and target resources. | Log the execution chain and retain native downstream event IDs. |
| One shared service account | Investigators cannot distinguish agents or humans, and revocation affects unrelated work. | Use dedicated, lifecycle-managed agent identities with named owners. |
| Trusting model instructions as authorization | Prompt injection or a faulty plan can cause an unapproved write. | Enforce deterministic policy outside the model before every tool call. |
| Logging only successful calls | Denied, blocked, and failed attempts disappear from the incident timeline. | Emit start, decision, end, error, and denial events. |
| Assuming one vendor covers every account | External services may have separate schemas, retention, or no request context. | Check each integration’s native audit events and add instrumentation you control. |
| Putting secrets or full documents in traces | Telemetry becomes a second sensitive data store. | Minimize fields, redact before export, and restrict audit-data access. |
| No tested shutdown path | Revoking one token may leave another connector active. | Pause the agent, revoke all relevant credentials and consent, and verify downstream containment. |
An operating rhythm for governance
Assign an owner for each agent and an operator for the audit pipeline. Review inventory, effective permissions, unusual events, and retention exceptions on a schedule appropriate to the risk. Trigger an out-of-cycle review when an agent gains a tool, changes its purpose or model workflow, crosses a tenant boundary, or handles a new data class.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsKeep an integration matrix that names the system of record for identity, policy, tool execution, target-resource changes, approvals, and retention. During an incident, investigators should know which log to query first and how to pivot to the next system. This operational map is as important as the telemetry itself.
The practical standard
You can call an agent activity auditable only when an investigator can reconstruct who initiated a request, which agent acted under what authority, which policy and approval allowed or blocked it, what tool and resource were involved, and what happened afterward. Build that chain deliberately, test it with allowed and denied workflows, and state every integration gap. The result is a defensible cross-account audit even though no single platform provides the whole picture.
