An AI audit is a bounded assurance exercise: you test defined claims about a named model or system, under stated conditions, using evidence another qualified reviewer can inspect. It is not a universal certification that the model is safe. Start by fixing the audit object, version, deployment configuration, intended uses, affected populations, risk claims and exclusions. Then map each claim to complementary tests, preserve traceable records, examine independence and access, and report conclusions with their uncertainty and limits.
This guide follows that workflow for frontier models, from the first scope sentence to a defensible final report. It distinguishes a base model from a deployed system, a product and an organisational control programme, because evidence about one does not automatically establish properties of the others.

What a frontier-model audit actually covers
“Frontier model” describes a class of highly capable general-purpose systems, not a single audit object. Your conclusion is only as broad as the thing you examined. A test of raw model weights cannot establish how a retrieval layer, system prompt, tool permission, rate limit, human reviewer or incident process behaves in production.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minute| Audit object | Evidence that belongs in scope | What a conclusion can address |
|---|---|---|
| Base model | Exact model identifier and build, inference API or local runtime, prompting protocol, sampling settings, benchmark or task data, and evaluator rubric | Capabilities, failure patterns and safety behaviour of that model under the tested interface and settings |
| Model plus serving stack and safeguards | Base model, system instructions, moderation or policy layers, retrieval sources, tools, memory, routing, monitoring, fallback paths and configuration changes | Behaviour of the configured system, including interactions among safeguards and model outputs |
| Particular product or feature | The deployed workflow, user interface, account permissions, geography, user population, release version, support process and operational data | Whether that product meets specified requirements for its intended use and users |
| Organisational risk-control programme | Governance roles, risk registers, approval gates, training, incident response, supplier controls, monitoring and remediation records | Whether the organisation operates the described controls during the audited period |
State exclusions as prominently as inclusions. An audit might exclude training data, undisclosed model components, non-production interfaces, languages outside the sample, or post-release updates. An exclusion is not a finding of safety; it marks where the evidence stops.

The NIST AI Risk Management Framework (AI RMF) 1.0 is voluntary risk-management guidance, not a certification requirement. Its Core organises work through Govern, Map, Measure and Manage. Use it as a structure for an audit, while keeping any contractual, regulatory or internal criteria separate.
Step 1: Turn the assignment into testable claims
Begin with an audit question that could be answered by observable evidence. Avoid a question such as “Is the model safe?” Replace it with bounded claims:
- Does model version X meet a stated accuracy and calibration threshold on tasks representing the intended decision?
- Does the configured assistant refuse a defined class of disallowed assistance across specified languages and attack strategies?
- Does monitoring detect a defined event within a stated time, and does the response process contain it?
- Does the release process prevent deployment when a specified risk criterion is not met?
For every question, create a scope record before testing. Include:

- Subject: model name, exact version or build identifier, provider and interface.
- System configuration: system prompt or policy version, retrieval corpus, tools, plug-ins, memory, filters, routing, temperature or other sampling controls, context limits and fallback models.
- Conditions: geography, language, date range, account tier, device or network assumptions, user group and expected workload.
- Intended use: the task, decision or assistance the system is supposed to provide, including prohibited uses.
- Claims and criteria: the threshold, pass condition, tolerance or control outcome that would count as evidence.
- Lifecycle stage: design, development, pre-release, production, post-incident or periodic review.
- Boundaries: components, populations, languages, versions and risks not examined, with why each omission matters.
- Roles and access: commissioning party, audit team, system owner, data custodians, permissions granted and confidentiality constraints.
Write the boundary in one sentence that a reader can quote accurately. For example: “This review assesses the production configuration of version 4.2 used by customer-support agents in the European Union between 1 July and 31 August 2026; it excludes training data provenance, consumer use and future updates.” That sentence prevents a narrow result from being marketed as a property of every deployment.
Step 2: Map risks to evidence
Create a risk register that connects each claim to a possible harm, affected people, controls and the evidence needed to evaluate it. The register should preserve qualitative risks that cannot be reduced to a reliable number. The NIST Generative AI Profile (AI 600-1, 26 July 2024) advises recording risks that cannot be measured quantitatively and explaining why quantification is not appropriate.

| Risk-register field | What to record |
|---|---|
| Claim or hazard | The behaviour or failure that matters, written so a test can observe it |
| Affected people and assets | Users, non-users, workers, confidential information, infrastructure or public services exposed to harm |
| Likelihood and severity rationale | Available incident data, expert judgment, exposure assumptions and why the rating was chosen |
| Existing controls | Model training, policies, filters, permissions, human review, logging, rate limits and response procedures |
| Evidence plan | Test set, red-team exercise, user study, field logs, interviews, documentation review or a combination |
| Residual uncertainty | Sampling error, untested languages or contexts, missing records, evaluator disagreement and possible distribution shift |
| Owner and action | The person or team responsible for mitigation, acceptance, monitoring or retesting |
Separate risk identification from measurement. A low score on one benchmark does not erase a severe but rare misuse pathway. Conversely, a vivid red-team example does not establish prevalence. Record both quantitative results and reasoned qualitative judgments, with the assumptions behind each.
Step 3: Design complementary tests before running them
Choose methods that probe different failure modes. The NIST ARIA Evaluation Planning Manual, published 18 September 2026, describes holistic evaluations that combine model testing, red teaming and user testing. The ARIA programme also describes model testing, red teaming and field testing. Select the mix that matches the claim rather than defaulting to the easiest benchmark.

| Method | Risk question addressed | Deployment relevance | Reproducibility | Independence | Known limitations |
|---|---|---|---|---|---|
| Model testing | Does the model meet a defined capability, robustness, bias, calibration or refusal criterion on controlled tasks? | High when tasks and interface mirror the intended workflow; lower when tools, users or safeguards change behaviour | Usually strongest: prompts, labels, metrics and seeds can be versioned | Can be run internally or by an external evaluator; disclose who selected the data and metric | Benchmarks can be narrow, contaminated, gamed or unrepresentative of real use |
| Adversarial red teaming | Can skilled testers elicit dangerous, misleading, privacy-invasive or policy-evading behaviour? | Strong for abuse paths when testers use the production configuration and realistic attacker access | Moderate: preserve prompts, tools, tester instructions and reproduction rate, but attacks evolve | External or cross-functional testers can reduce blind spots; disclose incentives and expertise | Findings show exploitable possibilities, not their population frequency or absence elsewhere |
| User or human assessment | Can representative people complete the task, understand uncertainty and avoid harmful reliance? | High for usability, accessibility, trust calibration and workflow effects | Requires a documented protocol, sampling frame, training and inter-rater analysis | Recruitment and compensation can bias results; protect participants and disclose conflicts | Small or artificial samples may not predict behaviour across populations |
| Field testing | What happens in operational use, including drift, workarounds, incidents and control performance? | Highest for the actual service, data, latency, monitoring and escalation paths | Depends on complete logs, stable definitions and privacy-preserving data access | Independent access to production telemetry may be limited | Rare harms can be missed, and observed correlations may not identify causes |
Specify the test protocol
For each risk, document the sample or test-set construction, prompt or task format, labels, scoring rubric, baseline comparison, number and type of trials, tool and evaluator versions, environment, sampling procedure, stopping rule and analysis plan. Define the success threshold before looking at results. A baseline can be a prior model, a human workflow or a policy target, but explain why it is a meaningful comparison.
Include ordinary, edge and adversarial cases. Stratify results by language, demographic or user group, task difficulty, tool path and relevant geography when those dimensions affect harm. If human participants are involved, obtain the approvals and informed consent required for the setting, minimise collection of sensitive data, and assess whether the sample represents the people who will rely on the system or be affected by it.
Step 4: Run evaluations with an evidence trail
Use a run log that lets a qualified reviewer reconstruct what happened. Capture the following for every test batch:

- Uncertainty estimates, confidence or credible intervals where appropriate, sensitivity analyses and comparison with the baseline.
- Access controls, hashes or immutable storage references, retention period and redactions needed for privacy or security.
Do not report a single run or score as definitive evidence of broad behaviour. Re-run stochastic tests with a predeclared sampling plan, distinguish independent trials from retries, and explain how evaluator changes affect comparability. Preserve failures as well as successful demonstrations; deleting inconvenient outputs destroys the audit trail.

It also expects documentation of limitations on generalisability. A result without its conditions is not reproducible evidence.
Step 5: Test the configuration people will actually use
Base-model behaviour is only one input to a deployed system. If the intended service uses retrieval, tool execution, guardrails, human approval, rate limits or a particular interface, test that configuration. A safeguard can fail because a tool grants excessive permissions, retrieval injects hostile instructions, a fallback model behaves differently or a user interface hides uncertainty.
Mirror production conditions as closely as safety and confidentiality allow. Use representative context lengths, latency budgets, account permissions, traffic patterns, content filters, language settings and escalation routes. Record configuration changes during the evaluation and freeze the tested version long enough to interpret results.
State generalisation limits explicitly. If testing occurred in a staging environment, say which production properties were absent. If only English prompts were assessed, do not extend the conclusion to other languages. If logs omit deleted conversations, state how that affects incident estimates. The NIST AI RMF requires criteria to be demonstrated under conditions similar to deployment and limitations beyond development conditions to be documented.

Step 6: Examine independence, conflicts and evidence access
Identify who commissioned the work, who designed and ran each test, who built or operated the system, and who approved the report. Explain financial, employment, research or advocacy interests that could influence choices. NIST states: “Processes for independent review can improve the effectiveness of testing and can mitigate internal biases and potential conflicts of interest.” Link that principle to a practical control, such as an external reviewer, separation from the release team or an independent sign-off.
Describe access in concrete terms. List whether the auditors received model outputs only, API access, weights, training and validation data, safety cases, incident reports, source code, system logs, vendor documentation or customer feedback. Note sampling restrictions, nondisclosure limits, unavailable records and redactions. If the team could not inspect training data or production telemetry, narrow the assurance statement accordingly.
Independence does not mean every test must be outsourced. Internal experts may know the system best, while external reviewers can challenge assumptions. Use more than one perspective where stakes justify it, and preserve the provenance of each observation.
Step 7: Convert observations into bounded findings
Write each finding so a reader can follow the chain from criterion to evidence to action. A useful structure is:
- Criterion: the requirement, threshold or control being assessed.
- Condition: the exact model, configuration, population and period observed.
- Evidence: test identifiers, sample size, logs, interviews or documents, with uncertainty and limitations.
- Result: pass, fail, partial performance or an unresolved question, using the predeclared rule.
- Risk rationale: affected people, plausible harm, severity and exposure assumptions.
- Priority: why remediation is urgent or can be scheduled, including dependencies.
- Action and owner: specific mitigation, accountable team, due date or acceptance decision.
- Retest or monitoring: the evidence that will show whether the action worked and how drift will be detected.
Distinguish “no problem was found” from “evidence shows no problem exists.” The former means the tested procedures did not detect an issue within their limits. Report residual risks and unresolved uncertainty even when a criterion passed. If a result is inconclusive because access or sample quality was inadequate, say so instead of forcing a pass or fail.
Best Value
Keep observed findings, developer assertions and auditor interpretation in separate fields. A provider’s safety claim is evidence to examine, not an observation by the audit team. Include a versioned executive summary, detailed methods, data dictionary, results, deviations, limitations, management responses and an appendix of test cases or secure references.
Step 8: Check legal scope separately from technical assurance
Technical evidence and legal conclusions answer different questions. The NIST AI RMF remains voluntary guidance, and its overview notes that the framework is being revised; confirm the current version when you plan the audit.
Article 55 of Regulation (EU) 2024/1689 creates duties for providers of general-purpose AI models with systemic risk. Among them are evaluation using state-of-the-art standardised protocols and tools, documented adversarial testing to identify and mitigate systemic risks, systemic-risk assessment and mitigation at Union level, serious-incident reporting and adequate cybersecurity protection. These duties do not automatically apply to every model, deployer or auditor.

A practical report structure
A publication-ready report should let a technically informed reader reproduce the reasoning without exposing sensitive prompts or personal data. Organise it as follows:
- Purpose, commissioning party, audit team and independence statement.
- Scope sentence, system diagram, versions, dates, intended uses and explicit exclusions.
- Risk register linking claims, affected populations, controls and residual uncertainty.
- Test plan, sampling frame, methods, baselines, thresholds, tools and human-participant protections.
- Execution record, configuration changes, deviations, raw-evidence custody and quality checks.
- Results with subgroup breakdowns, uncertainty, benchmark comparisons and reproducibility notes.
- Findings ranked by risk, with owners, corrective actions, deadlines and retest criteria.
- Limitations, access constraints, unresolved questions and conditions under which conclusions no longer apply.
- Monitoring plan for incidents, drift, new tools, model updates and periodic reassessment.
- Management response and version history showing what changed after review.
Provide secure appendices or controlled-access repositories for sensitive test cases. Public summaries should retain enough methodological detail to prevent a reassuring headline from outliving the conditions that produced it.
Failure modes that undermine an AI audit
- Auditing the wrong object: a benchmark on base weights is presented as assurance for a product with tools and human workflows.
- Scope drift: the model, policy, interface or user population changes while the report still describes the original test.
- Benchmark tunnel vision: one public score substitutes for misuse testing, user assessment and operational evidence.
- Untraceable prompts: evaluators publish averages without preserving inputs, seeds, versions, rubrics or failed runs.
- Hidden exclusions: languages, accessibility needs, non-production paths or affected non-users disappear from the risk register.
- Conflicted review: the team that built the safeguard is the only party deciding whether it worked.
- Overclaiming absence: a clean sample is described as proof that no harmful behaviour exists.
- Legal overreach: voluntary guidance is called a certification, or Article 55 duties are assumed to apply without checking the model category and provider role.
- No retest trigger: the report has a verdict but no owner, monitoring signal or condition that would require reassessment.
A defensible audit is therefore a chain of bounded claims, risk-linked methods, traceable evidence and explicitly limited conclusions. The stronger the model and the higher the stakes, the more important it is to combine controlled tests with adversarial, human and field evidence rather than treating any single result as a safety guarantee.
