Treat an AI-generated vulnerability report as a hypothesis, not a conclusion. Reconstruct exactly what it claims, verify that the code path and attacker conditions exist, reproduce the effect independently in an authorized environment when safe, and assign severity only to the impact you can demonstrate. A model’s confidence, a plausible explanation, a matching CWE, or a proof-of-concept printed by the same model is not proof.

What a “real” finding must establish

A useful security finding connects five things: a specific artifact, a reachable path, an attacker-controlled condition, an observable security effect, and a realistic impact. If any link is missing, the report may still deserve investigation, but it is not verified.

As an Amazon Associate I earn from qualifying purchases.

  • Artifact: the repository commit or package version, file, function, line or configuration item, and relevant dependency versions.
  • Path: the entry point and data flow from input to the operation the model calls dangerous.
  • Capability: what the alleged attacker can submit or control, including authentication, role, network position and other prerequisites.
  • Effect: a target-side state change, unauthorized read or write, bypass, code execution, data exposure or other observable result.
  • Impact: the assets and consequences affected under the stated threat model.

OWASP’s Secure Code Review guidance treats severity, CWE, location, description, impact, reproduction, recommendation, references and status as parts of a complete finding. Those fields organize an investigation; none substitutes for evidence.

1. Freeze the claim before you test it

Save the model’s complete output, including prompts, tool calls, generated code and timestamps. Record the exact source revision or released package, build mode, runtime, operating-system image, configuration, feature flags and dependency lockfile. AI analyses often silently assume a different version or deployment than the one you operate.

Build a claim record

Field What to capture
Location Repository, commit or package version, file path, function and line range
Classification Vulnerability class or CWE, stated attack path and affected component
Entry point URL, API, message, job, command, parser or other route into the code
Attacker model Unauthenticated or authenticated user, role, network position, supplied input and required privileges
Preconditions Configuration, feature flag, data state, dependency, account, timing or deployment assumption
Expected effect The exact unauthorized behavior the report says should occur
Evidence supplied Trace, request, response, log, state change, callback, screenshot or generated proof-of-concept

Rewrite the report as a falsifiable sentence: “An actor with these capabilities can send this input through that entry point and cause this observable result in version this.” If the statement cannot be made that precisely, ask the model for missing assumptions, but do not treat its answer as confirmation.

2. Check whether the suspicious pattern is reachable

A dangerous-looking line is not automatically a vulnerability. Read surrounding code and trace source-to-sink flow. Determine whether the alleged input reaches the operation, whether transformations change its meaning, and whether a safe API or validation step intervenes.

Trace the complete data flow

  1. Locate the claimed sink, such as a database query, template renderer, shell invocation, deserializer, file operation, authorization decision or cryptographic operation.
  2. Work backward to every source that can supply the value. Include HTTP parameters, headers, cookies, files, queues, environment variables, database records and internal service responses.
  3. Inspect parsing, canonicalization, encoding, validation, allowlists, escaping and type conversion at each boundary.
  4. Follow control flow through wrappers, middleware, framework defaults, feature flags, error handlers and asynchronous jobs.
  5. Check whether the route, function or job is actually enabled and reachable in the affected deployment.

Review authentication and authorization separately. A value may be user-controlled but available only to a trusted administrator; conversely, an apparently internal route may be exposed through a proxy or alternate entry point. Examine object-level authorization, tenant boundaries, business rules and state transitions, not just syntactic input checks.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Inspect the surrounding system

  • Configuration: production settings, default values, secrets handling, debug modes, network bindings and security headers.
  • Dependencies: exact versions, patched forks, transitive packages and whether the suspected behavior is intentional in the installed release.
  • Architecture: trust boundaries, service-to-service credentials, queues, caches, storage permissions and egress controls.
  • Business logic: rate limits, workflow order, ownership checks, approval states and replay protections.
  • Cryptography: key generation, storage, algorithm choice, nonce handling, verification order and failure behavior.

Manual contextual review is particularly important for authorization and business-logic claims. A CWE label can help you search and communicate, but a label is a classification, not validation.

3. Reproduce the claimed effect independently

For a runtime claim, build a minimal test in an isolated, authorized environment that matches the reported version and preconditions. The discovering model’s transcript is not an oracle. Re-run the interaction with a harness, test client or script that was not generated as a self-confirming part of the same analysis.

Use an independent oracle

Confirm behavior through evidence outside the model’s narrative: an HTTP response plus a server-side state change, a new record visible to an authorized observer, a controlled file created in a test directory, a log entry, a permission change, or an out-of-band callback to infrastructure you control. A response that merely echoes the payload, a canned “success” string, or output produced without contacting the target does not demonstrate exploitation.

  1. Clone or deploy the exact affected revision in a disposable environment.
  2. Apply the stated configuration, roles, data fixtures and network conditions; record anything you cannot reproduce.
  3. Prepare the smallest payload that tests the alleged condition. Avoid destructive actions, persistence, credential theft and production traffic.
  4. Run the request or interaction from a separate harness. Capture request, response, timestamps, logs and relevant state before and after.
  5. Verify the effect through an independent channel, such as a target-side record or controlled callback.
  6. Repeat enough times to distinguish a deterministic result from a race, timeout or environmental accident.
  7. Destroy test data and preserve the environment description and artifacts for another reviewer.

OWASP’s APTS advisory requirements describe deterministic evidence replay as the primary authenticity check and recommend a separate verifier. If safe reproduction is impossible, document why—production-only state, destructive impact, unavailable hardware or legal restriction—and continue with static and artifact review without calling the claim verified.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Interpret a failed or partial replay correctly

A failed reproduction is evidence, but not always disproof. A mismatch in package version, configuration, credentials, role, feature flag, timing or prerequisite can block an otherwise valid path. Preserve the exact environment and compare every precondition with the report.

Classify the outcome

Status Use it when Required record
Verified The stated path and security effect were independently reproduced under the defined conditions. Exact revision, environment, steps, evidence, scope and demonstrated impact
Needs review Evidence is incomplete, conditions differ, behavior is nondeterministic, or static artifacts indicate a plausible issue that replay cannot settle. Unresolved assumptions, attempted conditions, artifacts and a concrete next check
Rejected The path is unreachable, a documented control prevents the effect, the artifact is mismatched, or the alleged behavior is not a security issue. Contradicting code/configuration, test output and reason for rejection

OWASP APTS uses the labels VERIFIED, FLAGGED and REJECTED for integrity status. A “needs review” or “flagged” result is preferable to silently dropping an uncertain report or inflating it into a confirmed vulnerability.

5. Test for alternative explanations and fabricated proof

AI-generated reports can combine a genuine code smell with an incorrect exploit story. Deliberately look for explanations that do not require a vulnerability.

  • The proof-of-concept never sends a request to the target, or its output is hard-coded.
  • The source location in the narrative does not match the checked-out revision.
  • The reported vulnerability class is unsupported by the actual artifact; for example, a string is called a command injection while it reaches only a fixed-argument API.
  • A framework, middleware layer or library performs validation or authorization that the model omitted.
  • The behavior is documented and intentional, such as an administrative export available only to an authorized role.
  • A required precondition—account ownership, enabled feature, writable directory, network route or specific database state—is absent.
  • The demonstrated effect is real but narrower than the report’s claim, such as reading one tenant’s record rather than taking over the service.
  • The model has mistaken a test fixture, mock, example configuration or dead code for production behavior.

OWASP APTS recommends seeded verification cases containing genuine findings, canned output, unsupported claims and inflated severity, plus a separate finding judge for issues that replay cannot settle. Use the same discipline on your own reports.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

6. Match severity to demonstrated impact

Do not inherit a severity score from the model. Establish who can reach the behavior, what prerequisites they need, which assets are affected and what consequence follows. A valid weakness can be real while its generated severity is overstated.

Questions for a defensible severity decision

  • Is the attacker unauthenticated, a basic user, a tenant administrator, an internal service or a host-level operator?
  • Can the attacker reach the entry point directly, or must another user or service perform an action?
  • Does exploitation require a special configuration, local access, a race, a known secret or prior compromise?
  • Does the effect compromise confidentiality, integrity, availability, authentication, authorization or only diagnostic information?
  • How many assets, tenants or environments are in scope?
  • Is the observed result the maximum demonstrated effect, or merely a stepping stone that has not been shown?

State the tested scope beside the severity: version, deployment mode, role and prerequisites. A successful demonstration supports the effect in that environment; it does not prove exposure in every deployment or justify a broader consequence that was not shown. If the evidence cannot support the assigned severity, flag the mismatch for human review.

7. Combine verification methods instead of seeking one magic test

No universal test proves every vulnerability class. NIST IR 8397 lists eleven broadly applicable software-verification recommendations, including threat modeling, automated testing, static scanning, secret heuristics, black-box and structural tests, historical tests, fuzzing, web-application scanners and checks of included software. The list is a practical baseline, not a complete verification program.

Method Best question it answers Important limitation
Manual code and architecture review Is the path reachable, and do design, authorization or business rules make it unsafe? Requires context and can miss behavior that appears only at runtime
Static analysis Do source patterns and data flows indicate a weakness across code paths? Can produce false positives and false negatives; effectiveness varies by case and bug class
Dynamic tests Does the running service exhibit the claimed behavior under controlled input? Coverage depends on reachable states, credentials and test data
Fuzzing Do broad or malformed input spaces trigger crashes, corruption or unexpected states? Needs suitable harnesses, oracles and time; not every logic flaw has a useful fuzz signal
Web-application scanning Can common network-facing weaknesses be detected from an external perspective? May miss authorization context, internal routes and business rules
Dependency and included-software checks Is a vulnerable library or service version actually present and reachable? A database match does not establish exploitability in your configuration
Threat modeling Does the claim fit real trust boundaries, assets and attacker capabilities? It identifies assumptions; it does not by itself demonstrate runtime impact

NIST’s SATE VI report, covering evaluations from 2018 through 2023, found static-analysis performance varied with test case, bug class and complexity, with higher-complexity cases harder for tools. Its practical advice is to test a tool or set of tools on your own codebase before using it in production. Treat both an alert and silence as inputs to review, not final judgment.

8. Keep AI’s knowledge and tests within their limits

OWASP’s Secure Coding with AI guidance warns that a model may not know CVEs published after its training cutoff or after a tool’s security-index update. Audit dependencies and compare installed versions with current vulnerability databases; do not accept an AI statement that a package is safe without checking the version and advisory status yourself.

The same guidance states: “A passing test suite generated by the same agent that produced the code provides no independent assurance.” Have a human review AI-generated security tests, add adversarial and negative cases the model did not write, and independently verify tests that protect security-critical behavior. A second model agreeing with the first is still not independent runtime evidence.

A 2024 preprint, Automated Software Vulnerability Static Code Analysis Using Generative Pre-Trained Transformer Models, evaluated five GPT models on 36 NIST SARD source-code examples. The study used ten inference temperatures and 100 repetitions at each setting—5,000 queries per vulnerable source example—and selected examples without natural-language cues revealing vulnerability status. Its authors concluded that the evaluated models were not suitable for fully automated vulnerability scanning because false-positive and false-negative rates were too high for likely practical use. Those results apply to the models, examples and setup in that paper; they do not provide a universal accuracy rate for every current model or codebase.

9. Write an auditable finding another reviewer can repeat

Your final record should let a reviewer reproduce the reasoning without access to the model’s hidden context. Separate observed facts from assumptions and mark every unresolved condition.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Minimum report contents

  • Summary: concise claim and current status.
  • Scope: repository commit or package version, deployment, files, dependencies and affected assets.
  • Threat model: attacker role, network position, privileges and required preconditions.
  • Technical analysis: source-to-sink path, controls encountered, relevant configuration and intended behavior.
  • Reproduction: isolated setup, exact steps or test command, payload, expected result and actual result.
  • Independent evidence: request/response, logs, state diff, callback or other target-side observation.
  • Impact and severity: demonstrated consequence, scope, rationale and limits of extrapolation.
  • Uncertainty: failed conditions, nondeterminism, unavailable environments and assumptions still needing confirmation.
  • Recommendation: a specific code, configuration, dependency or monitoring change, with regression tests.
  • References and status history: relevant standards, advisories, reviewers, dates and transitions between flagged, verified or rejected.

Do not paste a model-generated proof-of-concept without labeling what it actually did. Preserve raw artifacts separately from your interpretation so a later reviewer can distinguish an observed result from generated commentary.

10. A fast decision path for triage

  1. Can you identify the exact version and location? If not, mark the report for review and obtain the missing artifact.
  2. Is the alleged entry point reachable by the stated actor? If no, reject the claim for that deployment; if uncertain, inspect routing, roles and configuration.
  3. Does data reach the claimed sink without an effective control? Trace transformations and framework behavior before deciding.
  4. Can you reproduce a safe, observable effect independently? If yes, record it and assess scope. If no, preserve the attempted environment and continue with static, configuration and threat-model evidence.
  5. Does the impact match the score? Reduce or flag severity when prerequisites or consequences are narrower than claimed.
  6. Can another reviewer repeat the assessment? If not, the record is incomplete even when the issue appears plausible.

Common mistakes to avoid

  • Calling a report verified because the model sounds certain or cites a familiar CWE.
  • Running the model’s own proof-of-concept and treating its printed output as the oracle.
  • Testing a different commit, package version, role or configuration from the one described.
  • Using only a static scanner for a business-logic or authorization claim.
  • Using only a dynamic test for a dormant, configuration-dependent or unreachable path.
  • Assuming a dependency name match means the vulnerable code is present and exploitable.
  • Promoting a successful exploit in a lab to a claim about every production deployment.
  • Discarding a failed replay without recording version, credentials, configuration and preconditions.
  • Letting a second AI opinion stand in for independent evidence.
  • Allowing an AI-generated test suite to validate AI-generated code without adversarial human review.

The practical standard

Report an AI-generated finding as verified only when a precise claim survives contextual code and configuration review and, where a runtime effect is claimed, independent reproduction or equivalent evidence. Otherwise use a review status that exposes the uncertainty, preserve the conditions that prevented a conclusion, and keep severity proportional to what was actually demonstrated. This approach turns an AI alert into an auditable security decision rather than an untested assertion.
