The most defensible approach is to assess teamwork and clinical reasoning as related but separate outcomes. Use structured observation for behaviors that can be seen or heard, a scenario-specific rubric for reasoning, and a focused debrief to explain events and set next steps. Teamwork ratings cannot substitute for evidence that a learner recognized cues, prioritized problems, chose appropriate actions, or reassessed a patient.

Start by defining the two constructs

Assessment becomes inconsistent when “good performance” is left undefined. Before the simulation, write what you expect participants to do and what evidence will show that they did it. The TeamSTEPPS 3.0 curriculum organizes teamwork around communication, team leadership, situation monitoring, and mutual support, and includes simulation-training units. Those domains provide a practical starting structure, but they still need behaviors tailored to the case.

Teamwork: observable actions

Describe teamwork in terms an observer can identify in the room or on a recording. Examples include:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Communication: the participant states a clear message, directs it to the right person, uses a check-back or read-back when appropriate, and shares a change in the patient’s condition.
  • Team leadership: someone establishes priorities, allocates tasks, invites input, and keeps the group oriented to the immediate objective.
  • Situation monitoring: members watch the patient, equipment, workload, and one another, then voice concerns when the situation changes.
  • Mutual support: a participant offers assistance, redistributes work when a colleague is overloaded, or uses an appropriately worded assertion to protect the patient.

Do not score a domain merely because a participant has a desirable attitude toward teamwork. A rating should point to a behavior, its timing, and its effect on the team’s work.

Clinical reasoning: case-linked decisions

Clinical reasoning requires its own criteria. A practical local blueprint can ask whether learners:

  1. Recognized important cues in the history, examination, monitoring, or environment.
  2. Interpreted those cues in relation to the patient’s condition.
  3. Prioritized the most urgent or likely problems.
  4. Selected and justified an action or escalation.
  5. Reassessed the patient and updated the plan after new information or an intervention.

These categories are useful rubric-writing suggestions, not a named validated instrument identified in the official materials cited here. State that limitation whenever the rubric is used. The AHRQ teamwork tools describe teamwork constructs; they do not establish a measure of clinical reasoning.

Build an assessment blueprint before the scenario

Convert objectives into visible evidence

Write one or more objectives for each construct, then list the behavior that would demonstrate the objective. For example, an objective to “coordinate initial management of deterioration” might produce teamwork indicators such as assigning roles, using a closed-loop communication exchange, and announcing a change in priorities. The corresponding reasoning indicators might be identifying the deterioration cue, ranking immediate threats, selecting an intervention, and checking the response.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep indicators specific enough that two trained observers could decide whether the behavior occurred. “Communicates well” is too broad. “States the concern, names the patient, and requests a repeat blood pressure from a named teammate” is observable. Include the context or trigger when timing matters, such as “after the monitor shows a falling oxygen saturation.”

Choose a rating scale deliberately

A simple formative scale can distinguish not observed, needs development, effective, and exemplary. Define each level with behavioral anchors in your own program. Do not imply that this locally chosen scale is a validated pass/fail standard. If a score will affect progression, credentialing, or employment, obtain instrument-specific validity evidence and governance approval instead of treating an AHRQ worksheet or an internally created rubric as proof of competence.

Plan who will collect each kind of evidence

Assign an observer to record actions during the exercise, while the facilitator protects the learning environment and manages the scenario. Decide in advance whether the learner will complete a perception or attitude questionnaire, whether the debrief is recorded, and which evidence will be used for feedback. Document that each source answers a different question:

Select a tool that matches the evidence you need

The AHRQ TeamSTEPPS Measurement Tools page describes instruments with different purposes. They should not be treated as interchangeable or combined into a single score without a defensible rationale.

Method Evidence captured Best use Limitation to state
Team Performance Observation Tool Observer ratings of team structure, communication, leadership, and situation monitoring Visible teamwork behaviors during a simulation or practice event The AHRQ description does not say that it assesses clinical reasoning.
Teamwork Perceptions Questionnaire 35-item self-report using a five-point Likert scale; AHRQ describes it as assessing Level II Learning and Level III Behavior Perceived teamwork at the learning and behavior levels described by AHRQ Self-report is not direct observation of what a team did in a particular scenario.
Teamwork Attitudes Questionnaire Attitudes related to team structure and the four essential TeamSTEPPS skills Understanding participants’ teamwork attitudes Attitudes should not be presented as demonstrated performance.
Scenario-specific reasoning rubric Locally defined evidence linked to the case objectives Recognizing cues, prioritizing, acting, and reassessing in a particular exercise No validated instrument or psychometric evidence for a general clinical-reasoning rubric is established by the sources cited here.
Structured debrief Reconstruction of events, interpretation, successful behaviors, and improvement goals Learning from performance and planning a change A debrief complements assessment; discussion alone does not create a validated score.

Use direct observation for behavior

The Team Performance Observation Tool is suited to an observer who needs to mark whether teamwork behaviors occurred. Adapt the observation sheet only with care: preserve the original construct labels, explain any added local indicators, and train observers on what counts as evidence. If a behavior never had a fair opportunity to occur, mark it “not observed” rather than “not performed.”

Use questionnaires for perceptions or attitudes

The Teamwork Perceptions Questionnaire contains 35 items and uses a five-point Likert response format. AHRQ describes it as addressing Level II Learning and Level III Behavior. It can show how participants perceive teamwork, but a high perception score does not demonstrate that a specific team used a check-back, shared a concern, or redistributed workload during the exercise.

The Teamwork Attitudes Questionnaire addresses attitudes associated with team structure and the four essential TeamSTEPPS skills. Use it to understand readiness or beliefs, not as a replacement for an observed-performance rating.

Keep the reasoning rubric local and transparent

Because the cited sources do not identify a validated general clinical-reasoning rubric, publish the logic of your local rubric to learners and raters. Tie each criterion to a scenario objective, specify the information available at that point in the case, and distinguish a missed cue from a cue that was not presented. Record the learner’s stated rationale when possible; an appropriate action reached for an incorrect reason is different from a justified action that produced an unexpected result.

Run the observation without losing the clinical picture

Prepare observers

  • Give observers the scenario objectives, behavior definitions, rating anchors, and a timeline of expected triggers.
  • Use a short calibration exercise or video so raters practice distinguishing “not observed” from “ineffective.”
  • Tell observers to record concrete words, actions, and timing rather than motives or personality judgments.
  • Limit the number of indicators to those that matter for the case; an overloaded checklist causes missed events.

Capture events in sequence

Use time-stamped notes or a simple event log. Record the trigger, the team’s response, and the immediate consequence. For example: “At 04:10, oxygen saturation fell; the team leader named the change and asked for a repeat set of vital signs. The request was acknowledged, but no person was assigned.” This note supports both a teamwork rating and a useful debrief question without claiming more than was observed.

Protect the simulation

Observers should not coach participants during a scored exercise unless the design explicitly calls for formative prompting. If safety requires an intervention, document the interruption and exclude the affected item from an unadjusted comparison. Make clear to learners whether the exercise is practice, evaluation, or a mixture; hidden high-stakes scoring undermines psychological safety and makes feedback less useful.

Score clinical reasoning step by step

A reasoning rubric should follow the information flow of the case rather than simply count correct tasks. One workable sequence is:

  1. Notice: Did the learner identify the cue that should change the working picture?
  2. Interpret: Did the learner connect the cue to a plausible clinical problem, uncertainty, or risk?
  3. Prioritize: Did the learner explain what required attention first and why?
  4. Act: Did the learner choose an action, seek help, or escalate that fit the available information and role?
  5. Reassess: Did the learner check the patient or new data and revise the plan when the response was inadequate?

Use descriptors that reflect the case. For a deterioration scenario, “recognizes the cue” might mean noticing a trend rather than one isolated value. “Reassesses” might require checking a repeat measurement and verbalizing whether the intervention worked. Do not score a learner down for failing to mention information that the scenario never supplied.

Separate reasoning from teamwork when both occur in one sentence

A participant might say, “The blood pressure is falling, so we should treat shock and reassess in five minutes.” The statement contains reasoning evidence (interpreting and prioritizing the cue) and communication evidence (sharing a plan). Mark each construct in its own column. Otherwise, a confident communicator can receive an inflated reasoning score, or a clinically sound but quiet thinker can receive an unfairly low teamwork score.

Use performance levels for feedback, not false precision

Totals can help learners see patterns, but a numerical average can hide a critical omission. Highlight the highest-risk reasoning gap and the teamwork behavior that would make the next attempt safer. If you report a total, show the underlying indicators and explain that the score is local to the scenario and purpose.

Debrief the events and convert them into action

AHRQ’s debrief guidance recommends accurately recounting key events, examining what happened and why, identifying lessons, reinforcing useful behaviors, and setting improvement goals. It says debriefs are most effective when mistakes are treated as learning opportunities. Keep the discussion focused on performance rather than blame.

A practical debrief sequence

  1. Reconstruct: Ask the team to describe what happened in order, using the patient and team timeline rather than assumptions about intent.
  2. Analyze: Explore which cues changed the plan, how information moved through the team, and what made an action easier or harder.
  3. Connect to objectives: Review one or two teamwork behaviors and one or two reasoning decisions that mattered most in this case.
  4. Reinforce: Name effective actions specifically, including the observable behavior and its benefit.
  5. Plan: Agree on one behavior to repeat and one change to test in the next exercise or clinical shift.

AHRQ describes a brief team debrief as typically about three minutes or less. Treat that as a practical example, not a universal duration requirement; a complex scenario may need more time, while a focused event may need less.

Prompts that elicit useful reflection

  • What did the team do well, and what evidence supports that?
  • What did not go well that you can improve?
  • Which event changed your plan or should have changed it?
  • What would you repeat in the next scenario?
  • What will you change, who will do it, and when will you check whether it worked?

Ask open questions first, then provide the observer’s data. If a participant’s account differs from the record, explore the difference without turning the debrief into a prosecution. The aim is a shared, accurate mental model and a specific next step.

Combine evidence without pretending it is one measure

A useful formative package can contain an observation sheet, a scenario-specific reasoning rubric, a learner reflection, and a debrief plan. Present the results side by side:

Evidence source Question it answers Example output
Observer checklist What teamwork behaviors were visible? “Role assignment occurred after prompting; concern was not acknowledged with a check-back.”
Reasoning rubric How did the learner recognize, interpret, prioritize, act, and reassess? “Recognized the trend and acted, but did not reassess after the intervention.”
Learner reflection What did the learner perceive and believe? “The team felt coordinated, but the learner identified uncertainty about escalation.”
Debrief plan What change will be attempted next? “The team leader will assign a named person to repeat critical observations.”

Do not average these outputs into a single “competence” number unless your program has a documented rationale, rater training, and validation evidence for that use. The sources cited here do not provide a head-to-head reliability or validity comparison among the AHRQ tools, nor do they establish a pass/fail threshold.

Make the exercise part of a sustained training effort

The U.S. Defense Health Agency’s TeamSTEPPS overview describes implementation as a phased effort: assess readiness, plan and implement, then sustain the change. It states that training alone typically has a very small impact on success. For an organization, that means an assessment form cannot carry the improvement program by itself.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Readiness

Identify the clinical problem, affected teams, available facilitators, local policies, and barriers to speaking up. Decide whether the simulation is intended to introduce a teamwork skill, diagnose a system problem, or evaluate progress.

Planning and implementation

Align the scenario, observer training, debrief process, and reporting route. Pilot the rubric with a small group, revise ambiguous indicators, and ensure that leaders can act on hazards uncovered during the exercise. Keep patient-safety concerns separate from an individual learner’s developmental score when the system contributed to the event.

Sustainment

Repeat practice at appropriate intervals, review whether agreed behaviors appear in later exercises or clinical work, refresh facilitators, and share improvements with the people who raised the issue. Sustainment is also where you decide whether the local rubric remains fit for its purpose; a tool built for feedback may not be suitable for credentialing.

Common assessment failures and their fixes

One score for everything

Failure: A single teamwork total is described as clinical competence. Fix: Report teamwork and reasoning separately, with the evidence and limits of each.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Vague behavioral language

Failure: Items say “shows leadership” or “communicates effectively.” Fix: Specify the action, trigger, recipient, and expected effect.

Confusing attitudes with performance

Failure: A favorable questionnaire response is treated as proof of behavior. Fix: Use self-report to understand perception or attitude and direct observation to document what occurred.

Scoring opportunities that never existed

Failure: A participant is penalized because a cue, role, or opportunity was absent. Fix: Mark the item “not observed,” record why, and avoid forcing it into a denominator.

Debriefing as blame allocation

Failure: The facilitator searches for the person who caused the error. Fix: Reconstruct the event, examine contributing conditions, reinforce effective actions, and agree on a change to test.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Overclaiming validation

Failure: A locally written reasoning checklist is presented as a validated universal scale. Fix: Label it scenario-specific, document its design, and seek appropriate validation before consequential use.

Resources for implementation

The official AHRQ TeamSTEPPS Pocket Guide is available as a practical reference, alongside the measurement tools and debrief guidance. The TeamSTEPPS 3.0 materials provide the framework’s communication, leadership, situation-monitoring, and mutual-support structure. Use these resources to define teamwork behaviors, then add a clearly labeled, case-specific reasoning rubric for the clinical decisions your simulation is designed to teach.
