Assess execution risk by testing whether the project’s scope, design, estimate, schedule, contracts and controls are credible—and then keeping the assessment active from scoping through closeout. A sound review identifies uncertain events, estimates their cost and time effects as ranges, assigns each response to an accountable party, and sets triggers for escalation. A one-time risk register is not an execution-risk management system.

What execution risk covers

Execution risk is the possibility that a project cannot deliver its approved scope, cost, schedule, technical performance, quality, safety or public commitments as planned. In a highway or civil project, exposure begins before a contractor mobilizes. Incomplete design, unavailable right-of-way, utility conflicts, environmental commitments, unrealistic production rates or an unpriced traffic-control requirement can make a construction plan infeasible even when the visible work appears straightforward.

Define the review around the full delivery chain that applies to your project:

  • Design completion and technical approvals.
  • Procurement, bidding, contracting and long-lead materials.
  • Right-of-way acquisition, utility and railroad coordination, permits and environmental mitigation.
  • Construction access, staging, traffic management, safety, quality and commissioning or opening.
  • Change control, claims, records, handover and closeout.

Risk assessment should start during scoping and continue through construction management. Revisit it whenever the project’s assumptions change; do not treat the initial workshop as the finished product.

1. Set a clear assessment boundary

Write a short assessment charter before reviewing numbers. It should identify what is being assessed, the information date and the decision the review must support.

Boundary item What to record Why it matters
Project phase Scoping, preliminary design, final design, procurement, active construction, commissioning or closeout The unknowns and available evidence differ by phase.
Geography and jurisdiction Country, state or province, local agencies and project site Permits, labor conditions, standards and oversight are location-specific.
Delivery and contract method Design-bid-build, design-build, construction manager arrangements, public-private partnership or another model Responsibility, design maturity and risk pricing depend on the contract structure.
Funding and oversight Federal assistance, state or local funding, private financing and applicable agency requirements Funding conditions can change reviews, approvals and reporting obligations.
Design and estimate status Design percentage or issue, estimate date, escalation basis, contingency basis and schedule data date A forecast cannot be judged without knowing what it represents.
Execution definition Which interfaces, third-party work and post-construction activities are included Excluding a dependency can make a baseline look complete when it is not.

For federally assisted U.S. major projects, the Federal Highway Administration (FHWA) describes a Cost and Schedule Risk Assessment (CSRA) as an independent, collaborative, probabilistic review. Its current CSRA page, updated August 4, 2026, says at least one CSRA is required over the life of an applicable Major Project and describes a preconstruction CSRA before completion of the Initial Financial Plan. Those statements apply to FHWA’s major-project context, not automatically to every highway or civil project. Confirm current federal, state, local and contract requirements before relying on them.

FHWA’s 2007 major-project guidance defined a project for that guidance as one receiving federal financial assistance with an estimated total program cost greater than $500 million in year-of-expenditure dollars, or one otherwise identified by FHWA as a major project. That is a dated policy-context threshold, not a universal definition or a project-performance statistic.

2. Challenge whether the baseline is deliverable

Check that the scope includes the complete program

Start with a scope map, not the construction bid schedule. Reconcile the authorization, requirements, drawings, specifications, estimate, schedule, agreements and operating commitments. Ask what must be completed for the facility to open safely and legally, even if that work is outside the main construction package.

FHWA estimating guidance identifies the following cost and scope areas as minimum subjects to examine:

  • Roadway and structures.
  • Utilities and utility adjustments.
  • Right-of-way acquisition.
  • Environmental mitigation and commitments.
  • Construction engineering and contract administration.
  • Project management, public outreach and stakeholder coordination.
  • Railroad relocations and other third-party work.
  • Inflation or escalation and an explicitly reasoned contingency.

Use an interface list to expose work between packages: drainage that must be completed before pavement, a railroad agreement that controls access, a utility relocation required before bridge foundations, or an operations-system installation needed before opening. For each interface, name the predecessor, owner, required evidence and latest acceptable date.

Test design maturity and site information

Do not call a schedule “construction-ready” while critical design decisions remain unresolved. Review design status by discipline and identify assumptions that affect quantities, methods, access, temporary works and approvals. Search specifically for:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Unverified geotechnical, groundwater, drainage or existing-condition information.
  • Unknown condition of existing pavements, bridges, culverts, retaining systems or buried assets.
  • Design interfaces between roadway, structures, utilities, drainage, intelligent transportation systems and maintenance facilities.
  • Standards, permits or owner decisions that could change after bidding.
  • Temporary works, traffic stages and means-and-methods assumptions not reflected in the drawings or specifications.

Early information may not support a single defensible value. FHWA advises considering a carefully reasoned worst case when information is insufficient and using a range when uncertainty remains. Apply that principle to the actual site; geotechnical conditions, weather, existing facilities, phased contractors and compressed schedules are prompts for investigation, not automatic findings.

Test schedule logic rather than the finish date

Trace the schedule from notice to proceed through opening and closeout. Verify that it contains realistic relationships, review durations, procurement lead times, access constraints, work windows, seasonal conditions, third-party interfaces and commissioning activities.

  • Ask whether every major activity has a predecessor and a measurable deliverable.
  • Check that permits, utility relocations, railroad work and right-of-way possession are logic activities, not notes outside the network.
  • Compare assumed production rates with quantities, crew access, traffic restrictions, shifts and site conditions.
  • Identify the critical and near-critical paths and the constraints that could move onto them.
  • Test recovery options: resequencing, additional crews, alternate access, longer shifts or revised work windows.
  • Separate contractual milestones from internal targets and state the evidence supporting each date.

FHWA project-management guidance says the schedule should represent implementation and that the sponsor should define how deviations will be monitored and addressed. A schedule that has no documented variance thresholds or recovery process is not a complete execution control.

Test the estimate and contingency

Reconcile quantities, unit rates, labor and equipment assumptions, subcontractor and supplier inputs, escalation, indirect costs, temporary works, traffic control, construction engineering, owner costs and third-party payments. Confirm the estimate date and the price basis. A low number may simply omit scope or assume conditions that the schedule cannot support.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Contingency should evolve as risks become defined costs through design and investigation. Do not substitute an arbitrary percentage for identifying the events that contingency is intended to cover. Consider whether longer duration, winter or rainy-season work, dependent phases and a compressed schedule could increase exposure. Show the assumptions and a range where evidence does not justify a point value.

3. Identify risks across the delivery system

Use a facilitated workshop or structured review with the people who can see different failure modes: the owner or sponsor, designer, estimator, scheduler, contractor or construction manager where appropriate, geotechnical and other technical specialists, utility and railroad representatives, environmental staff, operations personnel, safety and traffic specialists, and affected partners. FHWA’s CSRA approach is collaborative, involving the sponsor, subject-matter experts, FHWA and consultants in its applicable context.

Risk family Questions to ask
Scope and design Are requirements incomplete? Which interfaces, design decisions, standards or quantities could change? Are constructability reviews closed?
Site and technical conditions What is unverified below ground or inside existing assets? Could drainage, structures, environmental constraints or condition surveys alter the method?
Right-of-way and third parties Are parcels, access rights, utility relocations, railroad agreements, permits and mitigation commitments on the schedule’s critical path?
Cost and market Which labor, material, equipment, supplier, escalation or contractor-capacity assumptions are supported by current project evidence?
Schedule and construction What could disrupt sequence, productivity, work windows, procurement, staging or recovery? How will changes and claims be controlled?
Safety, traffic and public effects Can workers and road users move safely through each stage? Is emergency access maintained? Are public communications and incident responses ready?
Governance and external context Could funding, legal or regulatory decisions, stakeholder opposition, political changes, security concerns or public-relations issues affect delivery?
Quality and contract administration Are testing, inspection, acceptance, records, payment, change orders, claims and responsibility boundaries unambiguous?

FHWA materials group examples into technical, nontechnical and political risks. Treat those as prompts and document the project-specific evidence rather than labeling a generic category as a risk without a cause and consequence.

4. Build a risk record that supports action

Write each material risk as a chain: uncertain condition or cause, possible event, and effect on an objective. “Utilities are a risk” is not actionable. “The unverified fiber route may conflict with pier excavation, delaying the westbound stage and requiring redesign” identifies what must be investigated and who needs to act.

Field What a useful entry contains
Risk statement Cause, event and consequence in one sentence.
Evidence and assumptions Survey, design issue, agreement, production basis, estimate input or explicit information gap.
Impact Potential cost, time, scope, quality, safety, environmental, traffic or reputation effect.
Likelihood or range A defined qualitative scale or a probability and range supported by evidence.
Response Specific investigation, design change, procurement action, contract treatment or contingency.
Accountable owner A named role or organization with authority to act, not an unassigned team.
Trigger and due date Observable condition that requires escalation and the date by which a decision is needed.
Residual exposure and status What remains after the response, current condition, next review and closed/open state.

This format synthesizes FHWA expectations to identify, analyze, document, track and respond to risk; it is not a single FHWA-mandated template. Tailor fields to the project’s governance and contract.

5. Analyze and prioritize uncertainty

Use qualitative ranking carefully

A likelihood-versus-consequence matrix helps a workshop focus on material exposures. Define the scales in words and state the decision rule. For example, “high” might mean an event could threaten a contractual milestone or require owner approval beyond delegated authority; it should not mean a mathematically precise probability. Record the basis for the rating and the person who made it.

Rank risks by potential effect on objectives, urgency, detectability, reversibility and the time available to respond. A low-likelihood event with a long procurement lead time may deserve earlier action than a more likely issue that can be corrected quickly.

Use probabilistic cost and schedule analysis when justified

When the project is mature enough and the decision warrants it, model uncertain quantities, durations and discrete events rather than presenting one certain outcome. FHWA describes CSRA as using Monte Carlo simulation to produce an array or distribution of possible cost and schedule outcomes. The output should show a range, confidence information selected by the project team and the assumptions driving the tails—not a guarantee.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Invite estimators, schedulers and subject-matter experts to agree on input ranges and correlations. Avoid double-counting the same exposure in both a risk event and a blanket contingency. Explain whether a result is a base estimate, a risk-adjusted forecast, a funding target or a contractual limit; these are different decisions.

Make contingency traceable

Link contingency to identified risks and information maturity. As design, investigations, bids and agreements resolve uncertainty, move an exposure from an allowance into a defined cost or schedule activity, or remove it with evidence. If the project still has substantial unknowns, retain a range and state what work will narrow it. False precision can hide an untested assumption.

6. Select responses and allocate ownership

For every priority risk, choose a response and document the action, authority, budget, date and trigger.

  • Avoid: Change the scope, method, alignment or sequence so the exposure no longer exists.
  • Reduce: Investigate, redesign, protect, test, phase or otherwise lower likelihood or consequence.
  • Transfer or share: Allocate a manageable portion through a contract, insurance, warranty, agreement or partner arrangement.
  • Accept and monitor: Keep the exposure when further treatment is disproportionate, with an owner and trigger.
  • Prepare a contingency: Define the fallback method, time, funding and decision authority before the event occurs.

Allocate a risk to the party best able to control its cause or consequence, and make the allocation transparent and workable. Contractual wording does not eliminate the underlying exposure. A clause assigning unknown conditions to a party that cannot investigate or manage them may raise bids, encourage disputes or make delivery harder; treat that as a design concern to test, not as a guaranteed outcome.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Review whether the delivery method, payment approach, incentives, warranties, information supplied at bid, change provisions and construction-management process create aligned behavior. Ask each party: What can you actually control? What information do you receive? What will the risk cost if priced? What happens when the assumption is wrong? FHWA international construction-management guidance emphasizes transparent, equitable allocation and warns that onerous clauses can increase project costs or contribute to failure.

7. Test the controls that make the plan executable

A risk plan is credible only when it connects to daily project controls. Verify that the project management plan explains how the team will:

  • Control scope and approve changes against the authorized baseline.
  • Forecast cost, commitments, contingency drawdown and estimate-at-completion.
  • Monitor the critical path, milestones, trends, recovery actions and schedule variance.
  • Manage design reviews, submittals, requests for information, nonconformance and acceptance testing.
  • Administer the contract, payment, claims, notices, change orders and responsibility boundaries.
  • Protect safety, traffic operations, emergency access and environmental commitments in every work stage.
  • Maintain communication, records, decisions, approvals, lessons learned and closeout evidence.

Set thresholds that trigger action: a forecast milestone slip, a contingency draw above delegated authority, an unresolved utility conflict, a quality trend, a safety event, a permit condition or a market change. The threshold, recipient and required decision should be written before the event, not improvised afterward.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

8. Include work-zone, safety and public impacts

Constructability review must cover how the public and workers experience the project, not only whether the permanent design fits. For each stage, test lane closures, detours, queue management, sight distance, barrier and access arrangements, pedestrian and cyclist routes, emergency response, incident management, night work, weather protection and communication with road users.

FHWA work-zone material treats worker safety, road-user safety, traffic operations and staging as execution concerns. A plan that meets a production target only by assuming unsafe access or unmanageable traffic is not a credible plan. Require a named owner for traffic-control revisions, incident decisions and public notices, and connect those actions to schedule and cost forecasts.

9. Compare delivery alternatives consistently

When choosing between phasing plans, bids, delivery methods or risk responses, compare the same evidence for each option. There is no universal FHWA scoring weight or cutoff; make the criteria and decision authority explicit.

Comparison axis Evidence to compare
Scope and design maturity Unresolved interfaces, assumptions, design deliverables and owner decisions.
Cost basis Quantities, escalation, contingency logic, uncertainty range and third-party exposure.
Schedule confidence Network logic, critical constraints, work windows, completion range and recovery options.
Technical and site uncertainty Geotechnical, structural, environmental, right-of-way and utility evidence.
Safety and constructability Worker and road-user protection, staging, access, traffic effects and public communication.
Risk allocation Clarity, controllability, pricing consequences and incentives created by contract terms.
Response readiness Named owner, action, trigger, authority, funding and residual exposure.
Control strength Quality, reporting, change management, records, oversight and claims processes.

10. Keep the assessment current during execution

Put risk review on the normal reporting cadence. At each review, reconcile the current scope, forecast cost, critical-path status, quality and contract actions with the risk register. Close risks only when evidence supports closure; otherwise reduce, re-rate or reassign them.

Reassess after material events such as:

  • A significant design revision or changed standard.
  • New geotechnical, utility, environmental or existing-condition information.
  • A right-of-way, permit, railroad or third-party agreement development.
  • A bid result, supplier failure, long-lead procurement problem or major market movement.
  • Contractor mobilization, changed means and methods or a staging revision.
  • Severe weather, an incident, a quality failure or an unexpected site condition.
  • A material cost or schedule variance, claim, change order or funding decision.

Update the project management plan when controls, responsibilities or reporting routes change. Preserve the decision record: the assumption, evidence, approval, response, forecast effect and residual exposure. This history supports claims administration, oversight, lessons learned and closeout.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Warning signs that the execution plan is not ready

  • The estimate and schedule use different scope, quantities, price dates or completion assumptions.
  • Right-of-way, utilities, permits or third-party work appear in narrative notes but not in the logic network and cost plan.
  • Contingency is a fixed percentage with no linked risk events or drawdown rules.
  • “Contractor to coordinate” is used where an owner decision, agreement or access right is still missing.
  • Critical design decisions are deferred without a decision date, responsible authority or time allowance.
  • The risk register has categories but no triggers, response budgets, accountable owners or residual ratings.
  • Traffic stages, emergency access, quality testing or commissioning are absent from the schedule.
  • The contract transfers unknown conditions without giving the assigned party adequate information or control.
  • Reports show variance but do not specify who must act, by when and with what authority.

Each sign points to a corrective action: reconcile the baseline, investigate the condition, add the missing activity, revise the allocation, quantify a range or establish a trigger before proceeding.

FHWA guidance: useful context, not a universal rulebook

FHWA’s current CSRA material is the strongest process reference in this set for applicable federally assisted U.S. major projects. It describes collaborative probabilistic cost and schedule analysis and says a preconstruction CSRA is completed before the Initial Financial Plan. The same FHWA process description places a Cost Estimate Review at least 90 days before NEPA decision approval. These are agency process timings, not performance statistics and not automatic requirements for every project.

Other useful FHWA material has different dates and purposes: detailed major-project estimating guidance from January 2007, project-management-plan guidance from approximately 2017, and highway risk-allocation and construction-management guidance from 2006 and related international practice chapters. Verify current statutes, regulations, funding conditions, state department of transportation procedures, contract provisions and agency templates for the project in front of you.

Two historical statements capture the practical aim. Tom Sorel, FHWA Minnesota Division Administrator, said in January/February 2006, “Risk analysis determines where the high and low risk areas of a project are.” Mark Gabel, Program Manager for WSDOT’s Cost Risk Assessment Office, said in the same FHWA issue, “Using the CEVP, our goal is to deliver projects within the target cost range and schedule range.” They are useful descriptions of intent, not current policy or evidence of a particular project’s outcome.

A concise assessment sequence

  1. Set the boundary: document phase, location, funding, delivery method, design maturity, estimate date, schedule data date and what execution includes.
  2. Challenge the baseline: reconcile complete scope, design, site evidence, third-party dependencies, estimate assumptions, escalation, contingency and schedule logic.
  3. Identify: conduct a multidisciplinary review across technical, commercial, safety, environmental, stakeholder and governance categories.
  4. Analyze: write cause-event-effect statements, rank them transparently and use probabilistic cost or schedule analysis where evidence and decision value justify it.
  5. Respond and allocate: choose avoidance, reduction, sharing, acceptance or contingency; name the owner, action, funding, authority, trigger and due date.
  6. Monitor: integrate the register with scope, cost, schedule, quality, contract, change, communication, records and closeout controls, and refresh it after material change.

The result should be a decision-ready picture of what is known, what is uncertain, who can influence it and how much time remains to act—not a single optimistic number presented as certainty.

Frequently Asked Questions

Is a formal FHWA Cost and Schedule Risk Assessment required for every civil construction project?

No. FHWA describes CSRA requirements in the context of applicable federally assisted U.S. Major Projects. Requirements for other projects depend on geography, funding, agency rules, contract, project size and delivery method. Confirm the current requirements that govern your project.

What should be in a highway-project risk register?

Record each cause, possible event, consequence, evidence, assumptions, likelihood or range, cost and schedule effect, response, accountable owner, trigger, due date, residual exposure and status. Link entries to the project’s scope, estimate, schedule and control systems.

How can I tell whether a construction schedule is realistic?

Check its logic, quantities, production assumptions, access and staging, procurement lead times, permits, right-of-way, utilities, third-party interfaces, review durations, seasonal conditions, commissioning and recovery options. Then compare the completion date with a documented uncertainty range rather than accepting a single point forecast.
