How to Assess and Reduce Off-Target Toxicity in Targeted Protein Degraders

Assessing a targeted protein degrader (TPD) requires more than showing that its binding ligand is selective. The linked molecule can create new protein and pathway effects, while event-driven degradation can produce delayed, persistent or non-linear toxicity. A defensible workflow maps intended and unintended degradation in relevant cells, relates those changes to functional injury and exposure, checks species and tissue biology, then iterates chemistry or dosing without losing the desired pharmacology.

This is a drug-development framework, not a universal regulatory checklist or a clinical prescribing guide. The exact assays, models and decision thresholds must follow the candidate’s target, recruited E3 ligase, tissue distribution and development stage.

What off-target toxicity means for a degrader

PROTACs join a target-binding ligand to an E3-ligase recruiter through a linker. Molecular glue degraders work differently: they alter an E3 ligase’s substrate recognition so that a target or other protein is recruited. Both mechanisms can trigger ubiquitination and proteasomal removal. The pharmacology is therefore event-driven: one degrader molecule can turn over multiple target molecules, so continuous target occupancy is not the only determinant of biological response. The European Medicines Agency’s 2026 horizon-scanning report identifies this feature as a source of non-linear exposure-response behavior and a development challenge.

For safety, the relevant question is not simply whether the warhead binds the intended protein. It is what the complete treated system does: which proteins fall or rise, in which cells and tissues, at what concentration, for how long, and with what functional consequences.

  • Intended degradation: depletion of the disease target to the depth and duration needed for efficacy.
  • Unintended degradation: loss of other proteins, including E3-ligase neosubstrates or proteins engaged by the bivalent molecule.
  • Non-degradative effects: pathway perturbation, altered protein interactions, stress responses or toxicity caused by the degrader’s chemistry even when a protein is not measurably degraded.
  • Excessive intended degradation: the desired target may be necessary in normal tissues, making on-target activity toxic at high or prolonged exposure.
  • Context effects: tissue-specific E3 expression, substrate abundance, proteasome capacity and species differences can change the result.

Testing the two component ligands separately can be useful as a control, but it cannot establish the safety of the linked molecule. Ternary-complex formation and the resulting degradation event can create de novo effects that neither component produces alone.

Why binding selectivity and component testing are insufficient

Binding assays answer whether a ligand recognizes a protein under defined conditions. They do not reveal the full degradome in a living cell. A PROTAC’s linker can change permeability, distribution, ternary-complex geometry and E3 recruitment. A molecular glue can create a new protein-protein interface. At high concentrations, productive ternary-complex formation can even decline through the hook effect, making a simple concentration-versus-effect curve misleading.

Safety decisions should therefore combine four evidence layers:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Molecular layer: target depletion and broad protein changes measured in relevant cells.
  2. Mechanistic layer: evidence that a protein change depends on the recruited E3 ligase, ubiquitination or proteasomal machinery.
  3. Functional layer: viability, cellular function and organ-relevant injury endpoints at matched exposures.
  4. Translational layer: pharmacokinetics, tissue exposure, recovery, species relevance and efficacy retained after mitigation.

A proteomic hit is a toxicity hypothesis, not a toxicity conclusion. Conversely, an apparently clean component panel is not evidence that the complete degrader is safe.

A practical workflow for assessing toxicity

1. Define the candidate and its risk hypotheses

Before running large safety studies, record the variables that determine what “off-target” means for this molecule:

  • Target protein, disease context and the intended depth of depletion.
  • E3 ligase and recruiter chemistry, including known substrate or neosubstrate biology.
  • Warhead, linker series, stereochemistry, physicochemical properties and likely metabolites.
  • Intended tissues, expected human exposure range, dosing interval and whether activity should persist after compound removal.
  • Evidence for high-risk normal tissues, pathways or cell states based on target and E3 expression.

For cereblon (CRBN)-recruiting compounds, explicitly include unintended neosubstrate degradation and developmental or reproductive risk in the hypotheses. A 2026 review in Nature Reviews Drug Discovery, “Safety considerations for cereblon-recruiting targeted protein degraders”, relates this concern to the developmental toxicity history of thalidomide-related immunomodulatory drugs (IMiDs). That history justifies rigorous assessment; it does not mean every CRBN degrader has the same neosubstrate profile or will cause a particular adverse effect.

2. Profile intended and broader proteome responses

Use quantitative proteomics in cellular contexts that reflect the intended target tissue and relevant normal tissues. Measure the intended target and the rest of the proteome across multiple concentrations and time points. The design should distinguish:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Onset of degradation and the concentration producing it.
  • Delayed protein changes that appear after the primary target falls.
  • Changes seen only at supratherapeutic concentrations.
  • Persistence or recovery after compound washout.
  • Proteins that change in parallel with functional injury.

Data-independent acquisition (DIA) mass spectrometry can provide broad, quantitative coverage. In a HepG2 workflow, the 2026 Nature Chemical Biology study “Proteome-guided drug discovery maps and mitigates therapeutic degrader toxicity” consistently quantified 3,331 proteins under its described acquisition conditions. That is a study-specific performance result, not a guaranteed count for every instrument, tissue or sample preparation.

The study combined high-throughput proteomics with machine learning to connect protein-level changes to toxicity mechanisms. Its authors describe the goal as: “Here, we developed a drug discovery framework that exploits high-throughput proteomics to map and mitigate toxicity mechanisms of emerging drug modalities.” Use the approach as a model for integrating data, not as a substitute for candidate-specific confirmation.

3. Confirm priority changes with orthogonal experiments

Rank proteomic findings by magnitude, reproducibility, concentration relationship, time course, tissue relevance and known biological function. Confirm high-priority proteins with an independent assay such as targeted mass spectrometry, immunoblotting, immunoassay or imaging. Orthogonal confirmation reduces the chance that an analytical artifact becomes a development decision.

Then test mechanism. Useful controls can include:

  • A chemically related inactive degrader or a linker-matched control that cannot recruit the E3 ligase.
  • Recruiter-only and target-ligand-only conditions when they answer the candidate’s specific risk question.
  • E3-ligase knockdown, knockout or pharmacologic disruption, provided the manipulation is interpretable in that cell system.
  • Proteasome or ubiquitination-pathway perturbation to determine whether the change is degradation-dependent.
  • Rescue or add-back experiments for a suspected causal protein or pathway.

Controls should be chosen to separate whole-degrader effects from component effects, not collected as a ritual panel. A negative result in one control does not rule out a liability produced by a different mechanism.

4. Connect molecular changes to functional toxicity and exposure

Run functional assays over an exposure range that includes the concentration required for intended degradation, the projected clinical range and a justified supratherapeutic margin. Match molecular and functional samples to the same concentration, time and washout conditions.

Depending on the target and tissue, endpoints may include:

  • Cell viability, apoptosis, proliferation and clonogenic recovery.
  • Mitochondrial respiration, membrane potential and cellular energy balance.
  • Hepatocellular injury, bile-transport function or other liver-relevant measures.
  • Cardiac electrophysiology or contractility where cardiac biology is plausible.
  • Neuronal, immune, hematopoietic or reproductive-cell functions when those tissues express the target, E3 ligase or suspected substrate.

A hepatic cellular model can identify a liver risk mechanism, but it is not equivalent to whole-organism or clinical safety evidence. For every signal, ask whether the protein change precedes functional injury, occurs at a lower exposure than injury, and recovers after removal. Include pharmacokinetic measurements and, where possible, tissue concentrations rather than relying only on nominal media concentration.

Event-driven pharmacology changes study timing. A short exposure may produce target depletion that persists after the compound disappears; a delayed pathway effect may be missed if samples stop at the first target measurement. Conversely, a transient change may recover before a later safety endpoint. Build a time course that covers exposure, target loss, downstream effect, injury and recovery.

5. Select biologically relevant nonclinical models

Species selection must be based on mechanism, not convenience. Determine whether each species expresses the recruited E3 ligase, intended target and relevant substrates at comparable levels and in comparable tissues. Confirm that the species forms the relevant ternary complex and can produce the suspected degradation event.

A negative animal result can be uninformative if the animal lacks a human substrate or expresses the E3 ligase differently. The 2023 industry perspective on nonclinical safety assessment of heterobifunctional degraders and its associated discussion emphasize adapting assay conditions, endpoints, timing, proteomic assessment and study duration to degrader pharmacology rather than applying a single universal battery.

For CRBN-recruiting agents, examine species expression and substrate biology before interpreting developmental or reproductive findings. Tailor those studies to the compound’s mechanism and program context. The available sources do not establish one mandatory species, schedule or test package for every CRBN candidate.

6. Mitigate the signal and repeat the same evidence chain

Once a reproducible liability is linked to a mechanism, choose a mitigation that addresses that mechanism:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Change the linker, warhead or recruiter to reduce unwanted ternary-complex formation or engagement.
  • Change the recruited E3 ligase if the liability tracks with its tissue biology or neosubstrate profile.
  • Narrow systemic exposure, reduce peak concentration or alter dose frequency when efficacy permits.
  • Use intermittent dosing when target depletion persists but toxicity depends on continuous exposure.
  • Modify the molecule’s distribution or clearance if a normal tissue receives disproportionate exposure.

Repeat target degradation, broad proteomics, functional toxicity and exposure measurements with the revised candidate. A mitigation is successful only if the safety signal improves while intended degradation and efficacy-relevant activity remain adequate.

The 2026 proteomics study provides a concrete, preclinical example. In one androgen-receptor degrader series, modest linker changes reduced engagement associated with electron-transport-chain complex I and lowered hepatotoxicity while preserving androgen-receptor degradation. The optimized analogues also improved antitumor activity in a castration-resistant prostate-cancer xenograft model. This is evidence for iterative optimization in that series, not proof that linker changes generally remove toxicity.

Designing the assay matrix

A written matrix prevents a promising molecular result from being separated from the exposure and function needed to interpret it.

Question Recommended design Decision enabled
Does the degrader remove its intended target? Quantitative target protein measurement across concentration and time, including washout Define potency, maximal depletion, persistence and recovery
What else changes? Unbiased proteomics in relevant disease and normal-cell contexts Identify unexpected degradation or pathway perturbation
Is a hit caused by the recruited E3? E3-dependence, proteasome or ubiquitination controls, plus orthogonal protein assays Separate mechanism-specific liabilities from secondary stress responses
Does the molecular change injure cells? Viability and organ-relevant functional assays at matched exposures Prioritize biologically meaningful signals
How long does the effect last? Early, delayed and post-washout sampling Set study duration and dosing interval
Will the result translate? Pharmacokinetics, tissue exposure and species-specific E3/substrate characterization Interpret animal findings and human risk

CRBN-specific questions

CRBN recruitment deserves a dedicated risk section because the recruiter can alter cereblon’s substrate recognition. IMiD-related chemistry has a documented developmental toxicity history, and unintended neosubstrate degradation is therefore a central hypothesis for CRBN-based TPDs. Evaluate known and plausible neosubstrates in the relevant cells, then determine whether changes occur at exposures near intended target degradation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not convert that class concern into an assumption that all CRBN degraders share one toxicity profile. Recruiter structure, linker geometry, target ligand, ternary-complex architecture, tissue distribution and exposure can change the degradome. A candidate-specific profile is required, including developmental and reproductive considerations when the mechanism and program warrant them.

How to compare degrader candidates

Use the same conditions and decision rules when comparing analogues, recruiters or schedules. The following axes keep efficacy and safety in one view:

Comparison axis What to measure Why it matters
Intended versus unintended protein effects Target depletion alongside proteome-wide changes A potent degrader may also have a broad or mechanistically distinct degradome
Concentration and duration Potency, maximal effect, delayed changes and recovery Persistent depletion can make peak exposure or dosing interval more important than average occupancy
E3 and tissue biology Recruiter dependence, E3 expression and substrate distribution The same warhead can behave differently across tissues or recruiters
Functional toxicity Organ-relevant injury at matched free or intracellular exposure A protein hit matters most when it predicts a functional consequence
Species relevance Cross-species target, E3 and substrate comparability A negative animal result may not test a human-specific liability
Efficacy after mitigation Target degradation and efficacy-relevant activity after chemistry or schedule changes Lower toxicity is not useful if the desired pharmacology disappears

These axes are a development decision framework, not a validated numerical scoring instrument. Keep raw concentration, time, exposure and recovery data visible rather than collapsing them into a single selectivity number.

Common failure modes and corrective actions

Relying on ligand panels alone

Failure: The warhead and recruiter look selective in isolation, so the linked molecule is advanced without broad profiling.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Correction: Profile the complete degrader in relevant cells and include component controls only to answer specific mechanistic questions.

Sampling only one concentration or early time point

Failure: Delayed toxicity, recovery or a high-exposure hook effect is missed.

Correction: Use several concentrations, early and late time points, washout and pharmacokinetic measurements.

Treating every proteomic change as toxic

Failure: Normal adaptive responses are confused with injury, leading to unnecessary molecule termination.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Correction: Confirm priority proteins orthogonally and connect them to viability, organ function, mechanism and exposure.

Ignoring species biology

Failure: A nonresponsive animal species is interpreted as evidence of safety.

Correction: Verify E3, target and substrate comparability before assigning meaning to a negative result.

Reducing exposure without checking efficacy

Failure: A safer schedule simply under-degrades the disease target.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Correction: Re-measure target depletion, persistence and efficacy-relevant biomarkers after every dose or schedule change.

Assuming one successful linker strategy generalizes

Failure: A linker change that helped one androgen-receptor series is treated as a universal solution.

Correction: Re-run the complete evidence chain for each scaffold, target and E3 recruiter.

Interpreting current evidence and dates

The EMA’s 2026 report estimated more than 40 TPD candidates in clinical development. That is a time-bound landscape estimate, not a live count. The field is expanding, but the number does not establish a class-wide toxicity rate or a universal safety standard.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The strongest recent mitigation example described here is a preclinical 2026 study of experimental CRBN-recruiting androgen-receptor degraders. Its complex I and hepatotoxicity findings should not be generalized to every target, recruiter or degrader scaffold. Likewise, the EMA document is horizon scanning rather than binding guidance, and the 2023 industry perspective supports adapting nonclinical studies without prescribing one regulatory protocol.

Decision rule for advancing a candidate

Advance a degrader when its intended degradation is reproducible at a defined exposure, unexpected protein changes have been mapped and mechanistically investigated, functional toxicity is characterized over the relevant time course, the nonclinical model is biologically informative, and any mitigation preserves efficacy-relevant activity. If a signal remains unexplained, treat the uncertainty as a development risk rather than inferring safety from clean component assays or a single negative species.
