To archive an access review as defensible PDF evidence, run the decision report with the correct filters, export every page (not just its summary), verify that the tables and decision-maker fields survived export, and store the file with context, review attribution, integrity controls, and the governing retention rule. The report’s documented archival option is PDF export because the result set can be large.
A trustworthy package lets an independent reviewer reconstruct what happened: which system and review period were covered, who made each decision, who checked the report, when those actions occurred, and whether the preserved content is complete and unchanged.

What a complete access-review evidence package contains
An access-review decision report normally has a summary followed by detailed pages for each review, certification entity, item, decision, and decision maker. The summary is an index, not the complete record. Preserve the full page set in the original order so a reviewer can see both the totals and the underlying decisions.
The U.S. Department of Justice/DIAMD Access Review Decision Report documentation states that, because of the volume of data, “the best option is to export the report results to a PDF file.” Treat that export as the primary rendered record, then retain links or references to the underlying review system and related evidence.

Minimum contents
- The report summary and every detail page.
- The report name, system or application, and review period.
- The creation, signed-off, and due-date filters used for the run.
- Manager, application, tag, and other filters that affected the result set.
- Run date and time, operator, approver or reviewer, and a unique record identifier.
- Visible decision-maker and decision fields for each reviewed item.
- References to source records such as approval logs, emails, confirmations, tickets, or digital signatures.
- Evidence of quality control, including who checked the export and the date of that check.
Run the report with reproducible filters
Start in the access-review system, not in a browser print dialog. Record the intended scope before running the report. A later reviewer should be able to reproduce the same population or understand why it changed.
Set the decision and date filters
- Select the access-review decision report.
- Enter the creation-date range, signed-off-date range, and due-date range that define the review period. If a range is intentionally open-ended, record that fact.
- Choose the application or system under review.
- Set the manager, certification entity, tag, status, and decision filters used by your procedure.
- Run the report and note the system’s run time, result count, and any warnings about incomplete or still-running data.
Do not silently change filters between the report run and the PDF export. If the report must be rerun, assign a new run identifier and explain the difference in the evidence manifest.
Capture the sign-off state
“Completed” can mean different things in different systems. Preserve the signed-off status, the person or role that signed off, and the timestamp shown by the application. If the report displays a due date but not a completion date, do not infer completion; retain the displayed value and document the gap.
Export the entire report to PDF
Use the report’s built-in PDF export when available. It is preferable to printing individual browser screens because it is designed for the report’s multi-page structure. If the system only offers a web view, use the browser procedure below and treat the result as a controlled rendition that must be checked page by page.
Built-in export procedure
- Open the completed report result, with the final filters still visible.
- Choose the report’s PDF or export command.
- Wait for generation to finish; do not close the tab while a large report is being assembled.
- Save the file using a stable identifier, such as
access-review-[system]-[period]-[run-id].pdf. Keep the identifier consistent with your evidence index. - Record the export time, operator, and any export options shown by the application.
Browser print-to-PDF procedure
- Open the report at its first page and confirm that the summary and filter context are visible.
- Use the browser’s print command (for example, Print or Ctrl+P/Cmd+P).
- Select Save as PDF or the equivalent PDF printer.
- Enable background graphics only if they carry meaning, such as status colors or table headings. Do not use a setting that clips columns.
- Set orientation and paper size so every table column is readable. Use landscape for wide decision tables when necessary.
- Choose all pages, not the current page or a selected range.
- Disable browser headers and footers if they add misleading URLs or timestamps; retain them only when they are part of the system’s evidence.
- Save the PDF, then close and reopen it in a separate viewer for validation.
Validate completeness, legibility, and accuracy
Export success is not evidence quality. Perform a documented inspection before the file enters the archive.
Page-set checks
- Compare the PDF page count with the report’s displayed page count or export summary.
- Confirm the first page is the expected summary and that detail pages follow in order.
- Look for blank, duplicated, or truncated pages.
- Check that page numbers, headers, filters, dates, and report identifiers are present where the source displays them.
Content checks
- Open several records from the beginning, middle, and end of the report.
- Verify that application, manager, certification entity, item, decision, and decision-maker fields are legible.
- Check wide tables for clipped columns, wrapped values that became unreadable, and missing continuation pages.
- Search the PDF for a known user, item, or decision to confirm text is selectable when the source was text-based.
- If pages are images, zoom in and verify that names, dates, and decision values can still be read.
Accuracy and alteration checks
The National Archives and Records Administration describes web-record integrity as content being complete and unaltered. Compare the PDF against the live report or an approved export during quality control, then restrict the archived copy from casual editing. If a correction is needed, preserve the original, create a new version, and record why the replacement was made.

Create an evidence manifest alongside the PDF
A PDF without context can be difficult to authenticate months later. Store a small manifest in the same controlled repository or in the record’s metadata.

Keep the manifest factual. If a value is not displayed by the system, mark it as unavailable rather than guessing.
Store the PDF in a controlled repository
Choose a records-management archive or document repository that can protect the file for its entire lifecycle. Government records guidance identifies capture, indexing, output, retention, disposal, backup, migration, maintenance, security, workflow, date and time accuracy, quality control, and version control as procedural concerns.
Controls to configure
- Access: grant read access only to people with an explicit business or audit need.
- Access logging: log views, downloads, permission changes, replacements, and deletions; review those logs.
- Immutability: use a write-protected or tamper-evident state where policy requires it.
- Versioning: never overwrite an original export; preserve superseded versions and the reason for each change.
- Backup: maintain recoverable copies and test restoration.
- Migration: document format or platform migrations and verify that files remain readable.
- Time controls: keep system clocks and displayed time zones accurate and record the time zone in the manifest.
- Legal hold: suspend disposal when litigation, investigation, or regulatory hold applies.
Access to audit trails should itself be restricted and audited. Cryptographic controls can add tamper evidence for critical records, but a hash is useful only when the trusted value is protected separately from the file.
Preserve the audit trail, not just the rendered report
The Canada Revenue Agency defines an audit trail as information needed to recreate a sequence of events related to a business transaction. For an access review, that sequence can include the report run, each item-level decision, the reviewer’s sign-off, and subsequent changes.

Document performance and review
PCAOB AS 1215 requires audit documentation to show that work was performed, who performed it, who reviewed it, and the date of review. Apply that principle to the export process:
- The operator records the report run and PDF export.
- A second person, where required by policy, checks page completeness, legibility, filters, and decision fields.
- The reviewer records the review date, name or authenticated identity, and any exceptions.
- Exceptions are resolved or accepted in writing before the record is marked final.
Do not treat a file’s creation timestamp as proof that someone reviewed its contents. Review attribution belongs in the manifest or repository workflow.
Apply the correct retention schedule
Retention is jurisdiction- and policy-specific. Do not adopt a published period without confirming that it governs your organization, record class, and legal obligations.

Before destruction, check for legal holds, open investigations, contractual requirements, and a permanent-preservation decision. Record the approval and date of disposal or transfer.
Choose an archive by evidence controls
Compare repositories against the controls that determine whether an independent reviewer can trust and retrieve the record.

Troubleshooting export and archive failures
The PDF contains only the summary
Cause: the export was run from a summary view or the print dialog was set to the current page. Fix: reopen the complete report, select all pages, and compare the resulting page count with the report’s own count.
Columns or names are cut off
Cause: portrait orientation, an unsuitable paper size, or browser scaling. Fix: switch to landscape, choose a larger paper size, adjust scale conservatively, and inspect wide tables at high zoom.

Cause: lazy-loaded content, a timed-out report, or a print operation started before rendering finished. Fix: wait for the report to finish, reload once, confirm the data on screen, and rerun the export. Preserve the failed attempt only if your incident procedure requires it.
The report changed between runs
Cause: live data, changed filters, or a late decision. Fix: assign separate run IDs, retain both files, record the differences, and identify which run was used for sign-off.
The archive cannot prove who reviewed the file
Cause: a file timestamp was mistaken for a review event. Fix: use an authenticated review workflow or manifest entry that records the reviewer and review date, as required by policy.
A reviewer cannot open the related source
Cause: an expired link or permission restricted to the original operator. Fix: store a durable record ID and retrieval instructions, grant controlled reviewer access, or preserve an approved export of the related evidence.

A file was replaced without explanation
Cause: repository versioning or write protection was not enabled. Fix: restore the original if available, preserve the replacement as a new version, and document the reason, person, and date.
Final pre-archive checklist
- All intended filters and the report run time are recorded.
- The summary and every detail page are present and in order.
- Tables, dates, filters, decisions, and decision makers are legible.
- The PDF has a unique identifier and a companion manifest.
- The operator and independent reviewer, with dates, are recorded.
- Source links or record IDs allow the transaction sequence to be reconstructed.
- Access, audit logging, backup, migration, version, and integrity controls are active.
- Retention, legal-hold, and disposal rules are identified for the applicable jurisdiction.
Frequently Asked Questions
Can a screenshot replace the original access-review record?
No. Use the PDF as a stable rendered record and retain the underlying review, logs, and related evidence needed to reconstruct the decision sequence.
What should happen when a signed-off review is corrected later?
Keep the original export, create a separately identified corrected version, record the reason and approval, and preserve the relationship between both versions.
Is seven-year retention mandatory for every access review?
No. Seven years is the PCAOB AS 1215 example for covered audit documentation unless law requires longer. Other schedules, including UK internal-audit examples, specify different ranges.
