To let visitors choose an answer by clicking an image, use a WordPress form plugin that supports image choices. Add the relevant choice field, assign an image and a descriptive label to each option, then preview the published form to check how it looks and behaves. The exact field and image settings depend on your plugin.

Before you start: decide how choices should work

Image choices are provided by form plugins; the sources below do not establish an equivalent setting in WordPress core. First check the documentation for the form plugin already installed on your site. Then decide whether visitors should select one option, several options, or an exact number or range. That choice determines which field and selection rules to use.

  • One answer: use a single-choice field, commonly presented as radio-button choices.
  • More than one answer: use a checkbox or multi-select field.
  • A required number or range: choose a field that supports selection limits, if your plugin offers one.

Keep a meaningful text label associated with every image. Labels identify choices to visitors and assistive technologies, and they can also be important to the form’s submitted values and logic.

How the image-choice options compare

Gravity Forms Use the dedicated Image Choice field in Standard Fields. A separate third-party Image Choices add-on is another route for radio and checkbox fields; it is not the same field. The dedicated field supports single or multiple selection, an exact number, or a range. Images uploaded through its field interface are cropped to a 1:1 ratio. Retain descriptive labels; hiding them triggers an accessibility warning. The add-on is supported by its developer and is obtained from that developer, so check its availability and terms separately.
Formidable Forms In a Radio Button or Checkbox field, select Images as the Display Format. Choose a square size: Small 150 px (default), Medium 250 px, Large 320 px, or Extra large 400 px. Styling includes border, selected or hover color, border width, and corner radius. Image-enhanced options are a Premium Feature. Each option needs a text label, even if labels are hidden from visitors.

Add image choices in WPForms

WPForms’ documentation was updated August 20, 2026. Its editor’s placeholder images are for the editor and do not appear on the published form.

  1. Install and activate WPForms, then verify your license.
  2. Create a form or edit an existing one in the WPForms form builder.
  3. Add a supported field: Multiple Choice, Checkboxes, Multiple Items, Checkbox Items, or Ranking.
  4. Open the field options and enable image choices.
  5. For each option, use Upload Image to select an image from the WordPress Media Library or upload one. Add or keep a descriptive text label for the option.
  6. Select a layout. WPForms offers One Column, Two Columns, Three Columns, and Inline; Inline is the default when image choices are enabled, and WPForms recommends it in most cases because it adapts to different screen sizes.
  7. Preview the published form on desktop and mobile, checking that images and labels are clear and options are easy to select.

WPForms says it does not crop or resize choice images when they are added. Prepare consistently sized images; its guide recommends 250 by 250 pixels or smaller. If using a Multiple Items field to take payments, configure a payment integration such as Stripe, PayPal, Authorize.Net, or Square as well: selecting an image does not itself process a payment.

Add image choices in Gravity Forms

The dedicated Image Choice field is in Gravity Forms’ Standard Fields. It lets you configure whether respondents may select one choice, multiple choices, an exact number, or a selection within a range.

  1. Add an Image Choice field to the form.
  2. Open Edit Choices, then add the choice labels and corresponding images.
  3. Set the selection behavior to single choice, multiple choices, an exact number, or a range, according to the form’s requirements.
  4. Keep descriptive labels visible where possible. The field documentation warns that hiding choice labels produces an accessibility warning.
  5. Preview the form and verify the displayed images, labels, selection rules, and submitted values.

Images uploaded through the Image Choice field’s own interface are cropped to a 1:1 aspect ratio. For images already in the Media Library—or images not uploaded through that interface—the documentation says a WordPress plugin may be needed to resize them. Do not assume every image source follows the same crop behavior.

If you need images on conventional radio or checkbox fields rather than the dedicated field, Gravity Forms also lists a third-party Image Choices add-on. It is a distinct route, described for Survey, Quiz, Poll, Product, and Options contexts. Its developer supports it, and it is purchased or downloaded from that developer; check its current availability and licensing rather than assuming it is included with every Gravity Forms license.

When labels contain HTML or special characters, Gravity Forms warns they can cause problems with calculations, conditional logic, dynamic population, or validation unless Show Values is enabled and each choice has a simple, unique value. If your form uses those features, configure and test the values deliberately.

Add image choices in Formidable Forms

Formidable Forms documents image-enhanced radio and checkbox choices as a Premium Feature.

  1. Add or edit a Radio Button or Checkbox field.
  2. Set Display Format to Images.
  3. Choose an image size and upload or select an image for each option.
  4. Give every option a text label. Formidable requires labels even when they are hidden from visitors.
  5. Adjust the available styling—border, selected or hover color, border width, or corner radius—if needed.
  6. Preview the form and test the choices, including any confirmation or email output you rely on.

The documented square image sizes are Small 150 px (the default), Medium 250 px, Large 320 px, and Extra large 400 px. The feature does not support image options in Lookup and Dynamic fields, the bulk editor, or Add Other options. In plain-text emails, Formidable shows the labels without images; selected images can be displayed in confirmations, emails, or Views where supported.

Make the choices accessible and easy to use

  • Use descriptive labels. A label should convey the choice, not just say “Option 1.” Do not remove visible text without deliberately addressing how users of assistive technology will identify and select each choice.
  • Use images that remain clear at the displayed size. Crop behavior differs: WPForms says it does not crop or resize added choice images, while Gravity Forms crops images uploaded through its Image Choice interface to a square. Formidable offers specified square sizes.
  • Keep the set visually consistent. Similar dimensions, framing, and backgrounds make options easier to compare. Preview actual published output rather than relying only on the builder.
  • Test selection and submission. Confirm single versus multiple selection, any required count or range, conditional logic, validation, and the values received by the site.
  • Check plan and feature availability. Product documentation establishes how these features work, not which plan a particular account includes. Confirm current licensing before building around a paid feature or add-on.

Choose the right implementation

Use the feature in the plugin you already have if it supports the needed selection behavior, image handling, labels, and layout. WPForms adds images to several existing field types; Gravity Forms offers a dedicated Image Choice field as well as a distinct add-on route; Formidable Forms changes a radio or checkbox field’s display format. The product documentation does not establish an overall best plugin, so choose based on the field behavior and constraints your form requires.
