Which Karlach and Astarion content is this?
Patch 6 added epilogue reactivity for Avatar Karlach and Avatar Astarion during dialogue with God Gale. Larian’s Patch 6 notes describe these two avatars asking Gale for a cure for their conditions. The note does not say that the specific added lines are available when Karlach or Astarion is instead a companion.

That distinction matters: the characters’ names alone do not identify the required route. For this particular dialogue, the player character must be Karlach or Astarion as an avatar.
How to reach the dialogue
- Play as Avatar Karlach or Avatar Astarion. The Patch 6 addition names these player-character routes, not every playthrough involving either character.
- Complete the main story and defeat the Netherbrain. The epilogue follows the Netherbrain’s defeat; see Larian’s Patch 5 notes, which introduced this post-ending gathering with your allies.
- Continue into the epilogue and speak with God Gale. The cure request is part of that conversation. Patch 6 identifies God Gale’s dialogue as the place where the two added avatar reactions occur.
Do you need DLC or another purchase?
The cited Patch 6 notes describe this as game-update dialogue and do not identify a separate DLC purchase or add-on needed to unlock it. Larian’s official Baldur’s Gate 3 page lists a physical Deluxe edition, but that is an edition of the game—not a separate unlock for these lines.

These scenes were added in Patch 6, so they are update content rather than a newly released 2026 scene. In its April 15, 2025 final-patch announcement, Larian said, “Outside of minor bug fixing, Patch 8 will be the last game patch to introduce new content.”
