How the Mathematics of Alzheimer’s Plaques Could Inform Future Treatment Strategies

Mathematical models and advanced imaging may help researchers understand why Alzheimer’s amyloid plaques differ and how amyloid changes across the brain. A 2026 model reports that personalized treatment schedules reduced simulated plaque concentration more effectively than constant schedules—but that is a computational finding, not evidence that a new schedule benefits patients. Separate imaging research has identified different plaque signatures in human brain samples, while approved medicines such as lecanemab and donanemab remain the clinical treatments discussed in current NIH guidance.

What researchers mean by the “hidden mathematics” of plaques

The phrase points to two complementary lines of work, rather than one confirmed discovery. One examines the structural signatures of amyloid plaques in brain tissue. The other uses equations and patient imaging data to model how amyloid changes through brain space and time. Neither establishes that a mathematical technique has produced a proven new Alzheimer’s treatment.

Imaging plaque structure

A National Institute on Aging (NIA) research highlight describes work led by Mathias Jucker at the German Center for Neurodegenerative Diseases in Tübingen. The researchers used luminescent conjugated oligothiophenes (LCOs), dyes that bind beta-amyloid, and analyzed the dyes’ spectral signatures. The NIA account says those signatures revealed three-dimensional aspects of protein aggregates and differed among brain samples from people with distinct Alzheimer’s types. This is evidence that plaques in the studied samples are not necessarily structurally uniform. Whether plaque shape reliably tracks dementia severity, or can identify a useful treatment target, remains a question for future research in the NIA account—not an established clinical conclusion.

Modeling amyloid across the brain

A separate 2026 PubMed-indexed paper proposes a spatial reaction-diffusion model of amyloid-beta plaque dynamics. The authors formulate an optimal-control problem intended to reduce plaque concentration while balancing treatment benefit and risk. They use a finite-element method to calculate solutions and calibrate patient-specific parameters with longitudinal amyloid PET data from the Alzheimer’s Disease Neuroimaging Initiative (ADNI). The paper’s abstract reports that optimized schedules outperform constant schedules in simulations across patient groups. That result describes model performance under simulated conditions. It does not show that the schedules have been tested as a prescribing protocol, improve symptoms, or are safe and effective in patients.

How the two research approaches differ

What this could mean for Alzheimer’s treatment

The potential value is better-informed research: structural differences might help scientists investigate whether plaque types behave differently, while spatial models might help generate hypotheses about treatment timing or schedules. To become a clinical strategy, such hypotheses would need testing in people, including whether they improve outcomes and how their risks compare with their benefits.

NIH’s 2026 progress report identifies lecanemab (Leqembi) and donanemab (Kisunla) as FDA-approved treatments for early Alzheimer’s, with continuing research into efficacy, clinical use, access, and use across stages and populations. NIH public-health information says these medicines can slow symptom worsening in some people with early Alzheimer’s and require careful monitoring for side effects. They provide clinical context for why dose, timing, benefit, and risk matter; they are not products of the spatial-model study. NIH’s 2026 Alzheimer’s research progress report and NIA’s information on Alzheimer’s treatment describe the current context.

Earlier antibody research also cautions against equating plaque reduction with meaningful cognitive improvement. NIA’s drug-development report documents mixed outcomes from earlier monoclonal-antibody trials and the risk of certain brain abnormalities. Monoclonal antibodies can bind beta-amyloid and are one strategy for reducing plaques, but removing or reducing plaques alone does not guarantee improved thinking or daily function. What the findings do—and do not—establish

  • Established in the cited tissue work: spectral dye analysis identified differing plaque signatures in the brain samples studied.
  • Reported by the 2026 model paper: optimized schedules performed better than constant schedules in simulations.
  • Not established by either line of work: that plaque shape predicts an individual’s dementia severity, that a model-derived schedule is clinically effective, or that either finding provides a treatment recommendation.
  • Current clinical context: NIH describes lecanemab and donanemab as approved for early Alzheimer’s; they can slow worsening in some people and need careful side-effect monitoring.
