Space radiation can damage DNA and other cellular systems, and faulty repair may leave mutations that contribute to cancer over time. That is a plausible biological pathway, not a guaranteed outcome: the effect on any astronaut depends on the radiation exposure, the body’s response and repair, and other factors. NASA says important mechanisms and individual risk estimates remain uncertain.
Can space radiation cause cancer?
It can contribute to cancer risk. Beyond the protection of Earth’s atmosphere, astronauts encounter energetic particles from the Sun and galactic cosmic rays. When ionizing radiation passes through tissue, it deposits energy that can damage DNA and alter cellular processes. If damage is repaired inaccurately, mutations may result; if mutations accumulate, they can potentially help drive cancer.

This is a possible chain of events, not a prediction that every exposed astronaut will develop cancer. Cancer is a multistep process, and exposure is only one influence on whether it occurs. NASA says scientists do not fully understand all the ways ionizing radiation affects the human body.
How can radiation damage DNA?
Radiation can disrupt DNA strands or knock out DNA bases—the chemical units that encode genetic information. It can also affect other cellular processes. In a 2017 NASA article, Peter Guida, Ph.D., a liaison biologist for the NASA Space Radiation Laboratory, described the cell’s response: “The cell will make an attempt to repair these damages. Sometimes it’s effective and sometimes it’s not, and sometimes it can be misrepaired.”

- Energy enters tissue. An energetic particle travels through a cell and deposits energy along its path.
- Cellular structures are damaged. DNA strands or bases can be affected, alongside other cellular processes.
- The cell attempts repair. Repair can be accurate, incomplete, or erroneous.
- Some errors may persist. A misrepaired gene can become a mutation; mutations that accumulate over time can potentially contribute to cancer.
Guida summarized the last step this way: “Genes that have been misrepaired can become mutations, and the accumulation of these mutations over time can potentially lead to cancer.” The word “potentially” matters: a mutation does not by itself establish that a person will develop cancer.
Why is space radiation different from an X-ray?
The word “radiation” covers different kinds of exposure. A medical X-ray uses photons; space radiation includes energetic particles, and the mix can include protons, heavy ions, and particles created when radiation interacts with shielding or tissue. The particle type, energy, track through tissue, exposure duration, and shielding all affect the biological picture. A diagnostic X-ray is therefore not a direct stand-in for the space-radiation environment.

Heavy ions can deposit energy densely along a particle track, producing patterns of DNA and oxidative damage that differ from those associated with X-rays or gamma rays. NASA Space Radiation Element Scientist Lisa C. Simonsen described these differences in an April 7, 2015 briefing. In that briefing, “high LET” means linear energy transfer greater than 10 keV/µm in tissue. Its example of a 1 GeV/u iron-56 nucleus at approximately 150 keV/µm is an illustration for that particle, not a value for all space radiation.
Space exposure also includes secondary particles. Simonsen’s briefing describes interplanetary exposure as involving high-energy protons and heavy ions, as well as secondary protons, neutrons, and fragments produced in shielding and tissue. Shielding changes the radiation environment; it does not mean every particle simply disappears.

Are astronauts beyond Earth’s magnetic field more at risk?
Astronauts traveling beyond Earth’s protective environment face a different and more challenging radiation exposure than people on the ground. Earth’s atmosphere provides substantial protection, and Earth’s magnetic field also affects charged particles. The ISS remains within Earth’s protective environment but is not radiation-free; NASA says instruments measure radiation exposure aboard the station.
The National Research Council’s 2012 report identifies solar particle events, galactic cosmic rays, secondary radiation from surface impacts, and transported nuclear-isotope power sources among mission-relevant sources. The mix and resulting exposure depend on the mission and its environment, so “outside the magnetic field” alone is not enough to calculate an astronaut’s cancer risk.

What is known about astronaut cancer risk—and what is uncertain?
NASA and the National Research Council identify cancer as a possible late effect of space radiation, alongside other potential health effects. The National Research Council’s 2012 consensus report also discusses possible early or late effects such as radiation sickness, central nervous system damage, cataracts, cardiovascular damage, heritable effects, impaired wound healing, and infertility. The report says uncertainty remains about both biological effects and the level and types of risk astronauts face.
Estimating human cancer risk is difficult because results from experiments must be used to model exposures and outcomes in people. NASA’s April 7, 2015 briefing said that no human data existed to estimate risk from the heavy ions found in space, and described reliance on animal and cellular models using simulated space radiation. That statement applies to the evidence described in that 2015 briefing; it is not a claim that there is no human evidence about radiation of any kind.

A NASA Technical Reports Server abstract published January 23, 2017 describes career permissible exposure limits intended to control late effects, including a 3% risk of exposure-induced death (REID) from cancer. This is a risk-management limit in that abstract—not a 3% cancer probability for every astronaut, mission, or individual. The abstract also emphasizes large uncertainties in estimating space-radiation health risks. No current, mission-specific lifetime cancer probability is established by these sources.
How do astronauts protect themselves from radiation?
Risk management combines measuring exposure, designing the mission and spacecraft to reduce it, and studying health effects. NASA describes several approaches:
- Exposure monitoring: Instruments aboard the ISS measure radiation exposure so it can be monitored during missions.
- Shielding and spacecraft design: NASA describes shielding work for Artemis and the planned lunar Gateway. Shielding has to be evaluated for the radiation environment and mission design, including the possibility of secondary particles.
- Health surveillance: Monitoring astronaut health helps track effects over time; it does not remove exposure.
- Research into assessment and countermeasures: NASA describes tissue research aimed at personalized cancer-risk assessment based on radiation dose and genetic factors, and medicines under investigation. These are research efforts, not proof that a medicine prevents space-radiation cancer.
Ground-based facilities help researchers study parts of the problem. NASA describes work at the NASA Space Radiation Laboratory at Brookhaven National Laboratory and the Loma Linda University Proton Treatment Center. At the NASA Space Radiation Laboratory, ion beams are used to simulate aspects of cosmic rays and assess risks relevant to human space travel. These experiments help inform risk models, biological research, and shielding work; they do not eliminate the uncertainty in predicting an individual astronaut’s long-term outcome.

“Cascade” describes a sequence that can happen: particle energy can damage cellular material; repair can be imperfect; mutations may persist and accumulate; and some accumulated mutations may contribute to cancer. Each step is conditional. Space radiation is a recognized hazard for exploration missions, but the biological pathway does not make cancer inevitable or yield a precise personal forecast on its own.
