How Ship Operators Manage Crew Safety and Fatigue in High-Risk Maritime Transit Zones

Ship operators manage safety in elevated-risk transit zones by assessing the specific voyage, applying current route-relevant security guidance, preparing the crew, and ensuring watchkeepers are rested and fit for duty. The plan must address external threats and the crew’s ability to carry it out. The master retains overriding authority to protect the crew, ship and cargo; company and master share responsibility for realistic voyage risk assessment.

What makes a transit zone high risk?

“High-risk maritime transit zone” is not a single universal legal category or a permanent map label. The relevant danger depends on the route, the date, the vessel and the threat being assessed. Piracy and armed robbery are not the only possible hazards: a ship may also face deliberate targeting or collateral danger, and a measure suited to one threat may be ineffective against another.

That is why operators should not treat an old designation, a familiar route label or a generic security checklist as a substitute for current information. The International Maritime Organization (IMO) says the shipping industry’s Indian Ocean High Risk Area designation was rescinded effective 1 January 2023. That change does not mean every voyage is safe, nor does it set a risk rating for every route. It means operators need to assess the voyage using current regional information rather than relying on that former industry designation. See the IMO’s current piracy and armed robbery guidance.

How operators assess a voyage and its threats

Voyage risk management is a continuing planning loop, not a one-time form completed before departure. The company and master consider the proposed route and current official and industry threat information, decide which threats actually apply, select proportionate measures, brief the crew, and monitor for changes while underway. If conditions or information change, the assessment and plan may need revision.

Match measures to the identified threat

A security measure is useful only if it addresses the threat in question and can be carried out aboard that ship. Industry guidance has described the functions of maritime security planning as helping ships plan, detect, avoid, deter, delay and report attacks. But piracy precautions may not address attacks that have different methods or intent. Operators therefore need to identify the threat before deciding which procedures, watch arrangements and protective measures are appropriate.

IMO reports that consolidated BMP Maritime Security (BMP-MS) was issued in April 2025, replacing Global BMP, BMP5 and BMP West Africa. It says shipping should remain vigilant, apply BMP-MS and use the Maritime Security Center Indian Ocean voluntary registration scheme. Operators should check the current guidance, contact details and relevant regional reporting or coordination channels for the voyage rather than treating older publications as current instructions.

Keep the master’s authority clear

Security guidance informs shipboard decisions; it does not remove the master’s authority and responsibility. The older BMP5 publication stated: “Nothing in this BMP detracts from the Master’s overriding authority and responsibility to protect their crew, ship and cargo.” BMP5 has been superseded, so that sentence is useful as a statement of the principle, not as a reason to use its procedures instead of current BMP-MS.

Commercial or operational considerations should not displace a realistic assessment of whether a voyage can be conducted safely. In a statement dated 9 June 2026, IMO Secretary-General Arsenio Dominguez said, “Seafarer safety must remain the overriding priority at all times,” and recalled that the master and company bear ultimate responsibility for voyage planning and thorough, realistic risk assessments. The statement described conditions in the Strait of Hormuz at that time; its characterization of the route as volatile and without reliable security assurances is specific to that route and date, not a live assessment of other regions or a standing advisory for every later voyage. Read the IMO’s 9 June 2026 Strait of Hormuz statement.

How ships prepare the crew and vessel

Once the voyage assessment identifies relevant risks, the operator translates it into shipboard procedures the crew can understand and execute. The preparation should be route- and threat-specific: relevant current guidance and reporting arrangements inform the plan, while the vessel’s own safety-management procedures and operating realities shape how it is carried out.

  • Brief the crew. Explain the assessed threats, the shipboard plan, individual roles and how updates or concerns are communicated. A generic briefing that does not match the voyage leaves crew members without a clear basis for action.
  • Practice the plan. Use drills and preparation suited to the risks identified and the crew’s duties. The aim is for people to understand their tasks and be able to perform them under the actual shipboard arrangements.
  • Maintain observation and communication. Watchkeeping and reporting are part of preparedness, but equipment alone cannot substitute for trained personnel, clear procedures or an updated risk assessment.
  • Revisit the plan when conditions change. Current notices and regional arrangements matter because the risk picture and relevant contacts can change during planning or transit.

Physical observation aids are only one small part of this system. Older BMP5 guidance recommended sufficient binoculars for an enhanced bridge team, preferably anti-glare. Because BMP5 was replaced in 2025, this should not be treated as current purchasing advice without checking whether BMP-MS makes a comparable recommendation. Binoculars do not independently improve security or replace trained lookouts, drills or voyage-specific planning.

How operators manage hours of work, rest and fitness for duty

Fatigue is a safety issue because a tired person may be less able to keep an effective watch or carry out a designated safety, security or pollution-prevention duty. The IMO’s summary of STCW regulation VIII/1 says administrations must establish and enforce rest periods for watchkeepers and personnel with those designated duties. It also says watch systems must be arranged so fatigue does not impair watchkeeping efficiency, and that people assigned to the first or relieving watch must be sufficiently rested and fit for duty. See the IMO overview of seafarers’ hours of work and rest.

These are general international requirements, not a complete legal checklist for every ship. Exact limits and their application depend on the relevant convention text, flag-state implementation and shipboard procedures. Operators should apply the rules governing the vessel rather than assume that one simplified set of hours covers every flag, duty and circumstance.

Make rest arrangements workable in practice

Compliant records matter, but fatigue management cannot be reduced to recording hours. Operators need to consider whether watch arrangements and operational patterns give people a realistic opportunity to rest, whether safety-critical duties are covered, and whether the people starting or relieving a watch are fit to do so. A schedule that looks complete on paper may still leave a watchkeeper too fatigued to perform effectively.

When security preparations add duties, those duties should be considered alongside watchkeeping and rest arrangements. Briefings, drills, enhanced observation or other tasks have to fit within a workable shipboard plan. The relevant operational question is not simply whether each task was assigned, but whether the crew can perform it without undermining the alertness needed for safety and effective watches.

Recognize the limits of company control

IMO’s 2018 Guidelines on Fatigue address several parts of the problem: company practice, seafarer awareness, training, ship design, administrations and port State authorities. They recognize that a company cannot regulate every seafarer’s sleep. Companies can still reduce fatigue risks through operational and manning policies, ship design choices, training and awareness. The distinction matters: an operator can create conditions and systems that support rest, but cannot guarantee an individual’s sleep.

Why safe manning is part of the safety plan

Safe manning is more than a headcount. IMO’s minimum safe-manning principles connect the ship’s safety and security needs with having qualified and experienced seafarers, and link a seafarer’s ability to meet requirements with training, hours of work and rest, and occupational safety, health and hygiene. Avoiding fatigue is an explicit objective.

This makes staffing and watch design part of fatigue control. The operator needs enough appropriately qualified people to meet the ship’s safety and security duties while arranging work and rest in a way that supports effective watchkeeping. There is no single minimum crew number that can be applied to every ship: the required complement depends on the vessel and the applicable flag-state requirements. A security plan that adds responsibilities should be assessed in light of the crew available to perform them.

When armed guards are considered

Carrying privately contracted armed security personnel (PCASP) is not a universal answer to elevated risk and is not a replacement for voyage assessment or other protective measures. IMO says carriage is a matter for flag and coastal states. Its interim guidance says armed personnel should be considered only after a risk assessment and should not substitute for BMP and other protection. Because the cited IMO guidance on PCASP is interim guidance from 2012, operators need to verify current legal rules and later instruments before making operational or legal decisions.

Any consideration of armed personnel therefore has to account for the specific threat, the vessel’s operational arrangements, crew preparedness and the rules of the relevant jurisdictions. The available guidance does not establish one security configuration as best for every voyage. Operators should weigh whether a proposed measure fits the threat, whether it is legally permitted, whether the ship can operate it safely, and whether the crew can support it without compromising readiness and rest.

A practical decision framework for operators

For each voyage, a defensible process ties security decisions to fatigue and safe manning instead of treating them as separate paperwork exercises:

  1. Establish the route and current information. Review current official and industry material relevant to the voyage, and confirm applicable regional reporting or coordination channels.
  2. Identify the actual hazards. Distinguish piracy or armed robbery from other threats; do not assume one category or its countermeasures cover another.
  3. Choose proportionate, lawful measures. Check route relevance, flag- and coastal-state authority, operational feasibility, and whether the measures are consistent with current guidance.
  4. Check crew capability and workload. Confirm roles, qualifications, training, watch arrangements and rest opportunities can support the plan.
  5. Brief, drill and monitor. Make sure the crew knows the plan and its duties, then monitor updates and revisit the assessment when circumstances change.

This framework cannot supply a universal risk rating, route map or ship configuration. Those decisions require current route intelligence, the vessel’s own safety-management procedures, the ship’s characteristics and crew, and rules applicable to that voyage. The planning principle is stable even when the threat picture is not: measures should fit the identified risk and remain workable for a rested, prepared crew.
