Scientists use bacteriophages—viruses that infect bacteria—as highly targeted tools against some antibiotic-resistant infections. Therapeutic work focuses on lytic phages, which enter a susceptible bacterial cell, reproduce, and destroy it. Because a phage usually recognizes only particular bacterial species or strains, treatment depends on finding a safe, effective match for the bacteria in an individual infection. When one phage is not enough, researchers may combine several in a cocktail or investigate phages alongside antibiotics.
Phage therapy is promising but is not a routine replacement for antibiotics. WHO/Europe reports that, in most countries, phages are not approved medicines and are primarily used compassionately in life-threatening cases after other treatments have been exhausted. More clinical evidence, safety monitoring and regulatory approaches are needed before broad human availability.

What a bacteriophage does inside a bacterial cell
A bacteriophage, or phage, is a virus whose host is a bacterium. Human cells are not its target. In therapeutic development, scientists generally focus on lytic phages because their life cycle ends with destruction of the infected bacterial cell.

A phage first recognizes molecular features on the surface of a compatible bacterium. These features can differ between bacterial species and even among strains of the same species. If the matching receptor is absent or altered, the phage may not attach and infection will not proceed.

Replication and lysis
After attachment, the phage delivers its genetic material into the bacterium and redirects the cell’s machinery to make new phage particles. The infected cell eventually breaks open, a process called lysis, releasing progeny phages that can seek other susceptible bacteria. This self-amplifying behavior occurs only where suitable bacterial hosts are present; it does not make phages a virus of human tissue.
Why specificity matters
Antibiotics can affect a relatively broad group of bacteria, depending on the drug. A phage’s narrow host range can spare bacteria that it cannot infect, which is attractive when clinicians want to focus on a pathogen. The same narrowness creates the central practical problem: a phage that works against one isolate may fail against another.
Why antibiotic resistance has renewed interest in phages
Antimicrobial resistance (AMR) makes infections harder to treat when bacteria survive medicines that would previously have controlled them. WHO/Europe’s 2025 fact sheet reports an estimated 133,000 deaths directly attributable to AMR each year in the WHO European Region and 541,000 deaths indirectly linked to AMR. It also estimates about €11.7 billion in annual costs to the European Union and European Economic Area from health expenditure and lost workforce productivity. The fact sheet does not identify the estimation year for those figures, so they should not be read as measurements made in 2025.

In the United States, estimates cited by the U.S. Food and Drug Administration from the Centers for Disease Control and Prevention indicate at least 2.8 million antibiotic-resistant infections and more than 35,000 related deaths annually. The cited passage does not specify the estimate year. These numbers describe the scale of the AMR problem, not a demonstrated reduction produced by phage therapy.
WHO/Europe describes reported case-study successes involving methicillin-resistant Staphylococcus aureus and Pseudomonas aeruginosa. Such reports show why scientists investigate phages; they do not establish that phages work for every resistant infection or that they can replace standard treatment.

How scientists find a phage that fits an infection
Matching is the defining workflow in phage treatment. A clinician must know which bacterium is causing disease, and a laboratory or phage-development group must identify a phage with the necessary host range and properties. A suitable candidate may not exist in an available collection, and a single infection can contain multiple bacterial targets.
- Identify the pathogen. A clinical sample is analyzed to determine the bacterial species and, when relevant, the characteristics of the patient’s isolate. Treatment cannot be selected reliably from symptoms alone because phage activity is strain-specific.
- Screen candidate phages. Scientists test available phages against the patient’s bacterial isolate. The goal is to find particles that can attach to and lyse that particular host, rather than merely a phage known to infect the same species in general.
- Assess suitability. A candidate must have properties appropriate for therapeutic investigation. The sources emphasize that a phage may be unsuitable even when it appears to match, so activity against the isolate is necessary but not the only consideration.
- Address multiple bacteria. Mixed or complex infections may require more than one phage. Researchers can assemble a cocktail containing phages with complementary host ranges, provided the combination meets the intended safety and quality requirements.
- Consider combination treatment. Phages may be investigated with antibiotics. A phage can potentially improve the effectiveness of an existing antibiotic in some settings, but this is an evidence-dependent strategy, not an automatic rule.
- Monitor during treatment. Bacterial susceptibility, possible phage resistance, immune responses and inflammatory risks all need clinical oversight. WHO/Europe stresses continued evidence gathering and monitoring rather than assuming that an initially active phage will remain effective.
This individualized process helps explain why phage therapy is not sold like a universal antimicrobial. The treatment concept is closer to selecting a biological match for a specific bacterial isolate than to choosing a broad medicine from a standard shelf.

Personalized phage treatment
In personalized approaches, a phage or cocktail is selected for one patient’s isolate. The 2022 Annual Review of Medicine review by Graham F. Hatfull, Rebekah M. Dedrick and Robert T. Schooley describes renewed interest following clinical successes in personalized-phage case studies. These reports are important proof-of-concept evidence, but case studies cannot provide the certainty of large, controlled clinical trials.
Phage cocktails
A cocktail combines multiple phages to cover more than one strain or species, or to reduce the chance that a single bacterial mutation will block treatment. Cocktails are especially relevant when an infection is mixed or when the pathogen’s susceptibility is variable. They also make characterization and oversight more complex because each component and the combination must be evaluated.
Phages used with antibiotics
Researchers investigate phage–antibiotic combinations as a way to preserve or enhance current drugs. The rationale is that phage attack and antibiotic action can place different pressures on bacteria. Any benefit depends on the particular phage, bacterium and antibiotic, so combination therapy must be tested rather than presumed to work.

Phage therapy compared with antibiotics
| Question | Phage therapy | Antibiotics |
|---|---|---|
| What is targeted? | A susceptible bacterial host, often a particular species or strain | Bacteria affected by the drug’s spectrum, which may be relatively broad or narrow |
| How is treatment selected? | Requires a laboratory match between the phage and the infection’s bacterial isolate; a match may not be available | Selected according to the suspected or confirmed pathogen, drug susceptibility, infection site and clinical guidance |
| Can it cover several pathogens? | Often requires a cocktail when an infection is mixed or strains differ | A single broad-spectrum drug or a combination may cover multiple organisms, with trade-offs in adverse effects and resistance selection |
| Can bacteria become resistant? | Yes. Phage resistance is a recognized limitation and may require a different phage or cocktail | Yes. Antibiotic resistance is a major global health problem |
| Is it a substitute? | Not established as a universal substitute; it may be investigated alone in selected situations or with antibiotics | Remains the established standard for many bacterial infections when an effective drug is available |
| Availability and evidence | Broad routine human use is limited; approval and access differ by country | Many antibiotics have established approvals and prescribing pathways, although resistance can limit their usefulness |
Can phages be used against all bacterial infections?
No. A phage must recognize the bacterium causing the infection, and scientists may not find one with adequate activity or appropriate properties. Strain variation means that two isolates identified as the same species can respond differently. A mixed infection can require several phages, while a nonbacterial illness is outside the scope of phage treatment altogether.
Host range can also change as bacteria alter or lose the surface structures that phages use for attachment. Consequently, a phage that worked on an isolate in one test may not cover a later isolate or a resistant subpopulation. This matching limitation is a core boundary of the approach, not a minor logistical inconvenience.
How bacteria and the body can limit treatment
Phage resistance
Bacteria can evolve defenses that prevent phage attachment, block genome injection or interfere with phage replication. The Hatfull, Dedrick and Schooley review identifies phage resistance as a major challenge. Scientists may respond by selecting another phage, assembling a cocktail or studying combinations with antibiotics, but no strategy guarantees that resistance will not emerge.
Immune responses
The patient’s immune system can recognize and clear phage particles. The 2022 review identifies host immune responses as a possible limitation, meaning that the same preparation may behave differently in different patients or after repeated exposure. Clinical studies need to evaluate these effects rather than assuming that phages persist unchanged in the body.
Inflammatory effects
When phages destroy bacteria, components of bacterial cell walls can be released. WHO/Europe warns that endotoxins may provoke inflammatory responses, particularly when large numbers of bacteria are involved. The supplied sources do not establish reliable incidence rates for this risk, so it should be treated as a safety concern requiring monitoring rather than assigned a universal probability.

Microbiome considerations
Specificity may reduce unintended effects on unrelated bacteria, but it does not prove that the microbiome is unaffected. A 2023 FDA Science Forum abstract notes that phages typically target a single bacterial species or strain and that potential effects on the host microbial community warrant assessment. Scientists therefore study both the intended pathogen and possible ecological consequences.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What is the clinical and regulatory status?
WHO/Europe’s fact sheet, published 17 February 2025, describes phage therapy as promising but not ready for routine broad human availability. In most countries, phages are not approved medicines. WHO/Europe says they are primarily used on compassionate grounds in life-threatening situations when other treatments have been exhausted.
Access rules are country-specific, and the available sources do not establish a universal pathway for obtaining or prescribing phages. A reader should not assume that a phage can be purchased as an ordinary consumer medicine or that a clinician can use an identical process in every jurisdiction.
The U.S. Food and Drug Administration lists bacteriophage therapy among nontraditional antimicrobial products on its antimicrobial-resistance regulatory-science program. FDA work includes questions about clinical-trial design, combination antimicrobials and resistance mechanisms. That listing demonstrates regulatory-science interest; it does not approve a particular phage product.
WHO/Europe summarizes the evidence boundary plainly: “Further evidence from clinical studies is needed before phages can become available more widely for therapeutic use in humans.” This is why encouraging case reports should be read as signals for further testing, not as a general efficacy claim.

Better matching and adaptable preparations
Scientists are working toward ways to identify active phages more reliably across diverse clinical isolates and to design combinations that retain coverage as bacteria change. The need is both technical and organizational: a useful system must connect pathogen testing, phage characterization, clinical decisions and safety monitoring.
Clinical evidence
The 2022 review reported that clinical trials were in progress in its publication context, but it should not be treated as a current inventory of trial names, phases or results. Current evidence still needs to establish which infections benefit, how to measure outcomes, how combinations should be selected and how resistance develops during treatment.
One Health applications
WHO/Europe’s “Vintage Innovation” initiative uses a One Health perspective spanning human, animal and environmental applications. Phages are also studied in veterinary medicine, agriculture, aquaculture and environmental settings. Evidence, intended use, regulation and acceptable risk differ among these sectors, so success in one setting cannot automatically be transferred to human clinical care.

What patients and clinicians should take from the science
- Phages are targeted biological agents, not broad replacements for antibiotics.
- A confirmed bacterial cause and a susceptible phage are prerequisites; no suitable match may be found.
- Cocktails and antibiotic combinations are possible strategies under investigation, especially for mixed infections or evolving resistance.
- Resistance, immune clearance, endotoxin-related inflammation and microbiome effects require monitoring.
- Human access is limited and depends on local rules; compassionate use in severe, otherwise difficult infections is the principal context described by WHO/Europe.
For now, phage therapy is best understood as a specialized, evidence-building approach that may complement conventional antimicrobials for selected resistant infections. Its promise comes from precise bacterial targeting; its limits come from that same precision, the ability of bacteria to adapt and the still-incomplete clinical and regulatory evidence.
