How Robotic Exoskeletons Help People With Paralysis Stand and Walk

Powered overground exoskeletons can help some people with spinal cord injury (SCI) rise from a chair, stand, and take steps. A wearable frame supports the pelvis and legs while motors at the hips, knees, and sometimes ankles provide movement. The person usually operates controls, shifts weight, contracts available muscles, and uses a walker or crutches while a trained helper remains close by.

This is assisted mobility and exercise—not a cure for paralysis or a dependable return to ordinary, independent walking. The strongest evidence concerns carefully selected people with SCI who train under supervision. Eligibility, expected benefit, and safe use must be decided through an assessment by a rehabilitation team and against the current labeling for the specific device.

What a robotic exoskeleton is

An overground exoskeleton is a wearable medical device with rigid supports around the legs and pelvis. Battery-powered motors at one or more joints help create the movement needed for sit-to-stand, standing, and stepping. Sensors and software coordinate the motors with the user’s posture and commands.

“Overground” means the device moves across a normal floor rather than along a treadmill or inside a body-weight-support harness. The user may start a step by shifting weight, pressing a button on a controller, or contributing a voluntary muscle contraction, depending on the model. The system then guides the legs through a programmed pattern.

Most devices are designed to be used with a walker or forearm crutches. A trained person stays nearby to help with setup, transfers, balance, or an unexpected stop. The Model Systems Knowledge Translation Center (MSKTC) describes the category as “a device that people wear to help them stand and walk.”

How standing and stepping happen

Putting the frame on

A session begins with checking skin, range of motion, joint alignment, footwear, batteries, and harnesses or straps. The user is transferred into the frame, and the supports are adjusted so the hip, knee, and ankle joints line up with the person’s joints. Incorrect alignment can cause pain, pressure, or unsafe movement.

Moving from sitting to standing

The clinician or trainer positions the feet and activates the stand function. Depending on the design, the user leans forward and shifts weight while the motors extend the hips and knees, or the device performs more of the lift after a button command. The helper watches blood pressure, posture, foot placement, and any signs of dizziness.

Taking steps

Once upright, the controller initiates a step sequence. The exoskeleton shifts the user’s weight to one side, advances the opposite leg, places the foot, and repeats on the other side. Some systems require the user to trigger each step; others detect a weight shift or use a preset cadence. A walker or crutches provide additional stability while the trainer monitors the surface and the person’s fatigue.

Sitting and stopping

To sit, the user approaches a stable chair, aligns the legs, and selects sit-to-stand or stand-to-sit controls as directed. The device bends the hips and knees in a controlled way. Emergency-stop procedures and battery limits are reviewed before walking begins, because the frame cannot simply be removed while the user is standing.

Who may be eligible

“Paralysis” includes many conditions, but the cited patient guidance and most research concern SCI. A diagnosis alone does not determine suitability. The rehabilitation team evaluates motor function, joint flexibility, bone and skin health, cardiovascular responses, cognition, and the ability to follow commands.

The July 2025 MSKTC factsheet reports these broad ranges and clearances, while stressing that they vary by device and must be checked against current labeling:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Assessment item Information reported by MSKTC
FDA-cleared injury level for personal home use T3 and below
FDA-cleared injury level for clinical rehabilitation training C7 and below
Typical height range About 5 ft 3 in to 6 ft 2 in (160–190 cm)
Typical body-weight limit Under 220 lb (100 kg)
Leg proportions Legs generally need to be close to equal length

These figures are not a universal prescription. A particular model may use different limits, and a current device label can change. Clinicians also check hip, knee, and ankle range of motion; the ability to use mobility aids; standing blood-pressure responses; breathing; spasticity; fracture risk; pain; skin integrity; and other medical conditions.

Questions the assessment must answer

  • Can the joints reach the positions required by the frame without painful restriction?
  • Can the user tolerate upright posture without dangerous blood-pressure changes or breathing problems?
  • Can the user understand instructions and operate the model’s controller?
  • Can the person safely hold a walker or crutches and recover from small balance changes?
  • Is the skin protected from pressure and friction during the entire session?
  • Are bones, joints, and hardware strong enough for repeated loading?

What training normally involves

Training is progressive. Early sessions concentrate on transfers, standing tolerance, weight shifting, and a few controlled steps. Later sessions may add longer walks, turns, or changes in direction if the person can perform them safely. The trainer records fatigue, blood-pressure symptoms, pain, skin changes, and the amount of physical help needed.

In the 2016 systematic review and meta-analysis, programs generally met three times per week for 60–120 minutes per session and lasted from one to 24 weeks. Those schedules describe the included studies, not a mandatory prescription. A clinician may shorten sessions, add rest, or stop training when symptoms or skin findings warrant it.

Practice usually occurs on flat, wide, unobstructed flooring. Turning, backing toward a chair, crossing a threshold, and stopping require more control than straight-line walking. Stairs, steep slopes, loose surfaces, and crowded spaces are generally unsuitable for routine training unless a program has specifically assessed them.

What the evidence shows—and what it does not

The main quantitative evidence is limited by small, selected cohorts and supervised protocols. Miller, Zimmermann, and Herbert’s 2016 review included 14 studies and 111 participants: eight studies used ReWalk, three used Ekso, two used Indego, and one did not specify a device. The pooled findings describe what happened in those studies, not the probability for every person with paralysis.

Outcome reported in the 2016 review Result How to interpret it
Participants able to ambulate without physical assistance after training 76% Study outcome after supervised protocols; it does not mean independent community walking
Weighted mean six-minute walk distance 98 m Average across the included cohorts and testing conditions
Participants with reported improvement in spasticity 38% Reported association in the review, not a guaranteed treatment effect
Participants with reported improvement in bowel regularity 61% Reported association in the review, not an assured individual benefit
Falls during training 4.4% Pooled result from the older included studies, including a first-generation device study
Bone fracture during training 3.4% Pooled result from those studies, not a current device-specific rate

The falls reported in that review did not result in injury, but the age and mix of devices mean the figures should not be used as a current risk estimate for a particular product. A later MSKTC patient factsheet says regular training has been associated in small studies with possible bowel changes, less chronic pain or spasticity, exercise, greater self-confidence, and cardiovascular or body-composition changes. It also says more research is needed on the amount of training and the size of the benefit.

A comparative systematic review found no studies directly comparing exoskeletons used as assistive devices with conventional knee-ankle-foot orthoses (KAFOs). For rehabilitation in chronic SCI, results across exoskeleton studies were mixed, with no consistent advantage over varied conventional rehabilitation methods. That leaves the choice dependent on the person’s goals, available expertise, and risk profile rather than a universal “best” technology.

Assisted stepping is not the same as independent walking

A person may complete steps in a clinic while still needing another person, a walker, a flat surface, and a carefully prepared route. “Without physical assistance” in a study can mean the trainer did not hold the person’s body; it does not necessarily mean the user can put on the device, stand from a household chair, turn in a crowded room, climb stairs, carry objects, or walk outdoors alone.

Exoskeleton walking is also much slower than ordinary walking. The frame is heavy and bulky, donning and doffing can require help, and turns may be difficult. These practical distinctions explain why a device can be valuable for exercise or the experience of standing upright without replacing a wheelchair or other everyday mobility method.

Safety risks and how programs manage them

Falls, fractures, skin injury, pain, fatigue, and abnormal blood-pressure responses are the main reasons for clinical screening and close supervision. A trained companion who knows the device remains with the user; the MSKTC factsheet puts it plainly: “Someone needs to be with you who knows how to use the device in case something goes wrong.”

Surface and environment

Flat, wide floors reduce hazards. Slopes, uneven ground, wet areas, door thresholds, clutter, and tight turns increase the chance of a loss of balance. Stairs are a particular barrier because most systems are not designed to negotiate them.

Skin, joints, and bones

Straps and braces can rub or concentrate pressure. The user and trainer inspect skin before and after each session and stop if redness persists, numbness appears, or pain develops. Limited joint motion, osteoporosis, or a previous fracture may make loading unsafe even when the person meets height and weight limits.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fatigue and medical symptoms

Long sessions can exhaust the arms used on the walker and the muscles contributing to posture. Light-headedness, headache, chest symptoms, unusual shortness of breath, severe spasticity, or a sudden change in control requires stopping and medical review. A program should define who responds to an emergency and how the device is safely powered down.

Why daily use can be difficult

In qualitative interviews, users commonly valued therapy, the chance to stand eye-to-eye with other people, and psychological or social benefits. They also described barriers that are easy to miss in a demonstration: concern about falling, inability to operate the system independently, difficulty with stairs and uneven surfaces, awkward transfers, inability to carry objects, discomfort while sitting, fatigue, and skin irritation. These are participant experiences rather than population-wide percentages, but they illustrate why a successful clinic session does not automatically translate into household independence.

A wheelchair or other mobility aid may remain the faster and safer option for long distances, crowded buildings, emergencies, and surfaces the exoskeleton cannot handle. The exoskeleton can instead serve a specific role, such as supervised stepping practice, supported exercise, or standing during social interaction.

Access, coverage, and choosing a program

Access includes more than the frame itself. A person may need assessment visits, repeated training, a suitable walking aid, transportation to a center, a trained helper, maintenance, charging facilities, and a safe indoor route. The MSKTC factsheet identifies expense and limited insurance payment as barriers.

People can ask a rehabilitation service about Medicare, Medicaid, private insurance, or Veterans Affairs programs, as well as fundraising. Coverage and eligibility are individual; the device provider and payer should confirm them in writing before a purchase or long training commitment.

Questions to ask a rehabilitation team

  • Which injury levels and medical conditions does the current device labeling cover?
  • Will the goal be standing, exercise, therapy, or a specific walking task?
  • How many sessions are needed before progress can be judged?
  • Who will provide hands-on assistance, and what happens if the user becomes fatigued or dizzy?
  • What floors, ramps, thresholds, or chairs can be used safely?
  • What are the total costs for assessment, training, equipment, maintenance, and replacement parts?
  • What alternative treatments or mobility aids provide the same goal with less risk or effort?

Making a realistic decision

Start with a measurable goal rather than the promise of “walking again.” Examples include tolerating 20 minutes of upright exercise, practicing transfers, reaching eye level during conversations, or taking supervised steps in a therapy gym. The clinician can then match the goal to a device’s control method, fit range, walking-aid requirement, and environment limits.

Ask for a supervised demonstration and record how much help is needed for every stage: transferring into the frame, standing, initiating steps, turning, stopping, sitting, and removing the device. Compare that complete task—not just the most impressive straight-line walk—with the person’s current wheelchair or orthosis routine.

For selected people with SCI, an exoskeleton can add supported standing and stepping to rehabilitation. The evidence supports potential benefits but remains too limited to promise independent everyday walking or reliable prevention of secondary conditions. A careful medical assessment, current model labeling, trained supervision, and a clearly defined goal are the basis for deciding whether the technology is appropriate.
