Retrieval-augmented generation (RAG) answers a question by searching an external document library at request time, selecting useful passages, and giving those passages to a language model as context. The model then writes an answer from the question plus that evidence. RAG changes what the model can consult; it does not retrain or modify the model’s underlying weights.
A dependable system therefore has two distinct jobs: build a searchable representation of the library, then retrieve and assemble the right evidence for each question. Better indexing can improve relevance, but retrieval is not a guarantee of completeness or truth. Poor parsing, unsuitable chunks, missing documents, contradictory passages, or an overconfident model can still produce a wrong answer.

What happens when a RAG system searches a research library?
A typical request follows this sequence:
- Documents are collected and converted into searchable text or other structured data.
- The material is divided into passages small enough to retrieve independently.
- Each passage is represented for search, usually with an embedding and often with a keyword index.
- The user’s question is normalized, optionally rewritten, and represented using a compatible query method.
- The search engine returns candidate passages, which may be filtered or reranked.
- The application packs the best evidence, source information, and the original question into the model’s context window.
- The language model generates a response using that supplied context.
The model does not read the entire research library for every request. It sees only the retrieved subset, so decisions made before generation—parsing, chunking, indexing, ranking, and context selection—largely determine what the model has a chance to use.

Parse each source into usable content
Research collections rarely arrive as clean plain text. A pipeline may need to extract text from PDFs, preserve headings and tables, transcribe audio, run optical character recognition on scanned pages, or decode spreadsheets and web pages. The UK Government’s overview of RAG systems gives audio transcription and PDF or image processing as examples of preprocessing work.

Parsing errors become retrieval errors. If a scanned paper produces no text, its findings cannot be found by a text or vector query. If a table’s columns are merged, a retrieved passage may appear plausible while carrying the wrong value. Keep the original file and a traceable document identifier alongside the extracted representation so an answer can point back to the source.
Attach metadata before indexing
Useful metadata can include title, authors, publication date, section, page number, document type, language, subject, access level, and version. Metadata enables filters such as “only peer-reviewed papers after 2022” or “exclude superseded policy.” It also lets the application display source identity and detect when two passages came from the same document.
Metadata is separate from the passage text, but it affects what can be found. A technically relevant chunk hidden behind an incorrect access label or missing date may be excluded before ranking. Treat metadata validation as part of ingestion rather than as a later user-interface task.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesStep 2: Split documents into retrievable chunks
Why chunking is necessary
Long papers are usually divided into smaller units so a search can return the section that answers a question instead of an entire book. A chunk may be a few paragraphs, a section, a table plus its caption, or another structure that preserves meaning. Chunking lets the system compare many focused passages within the same document.
How to choose chunk boundaries
Chunk size and overlap should follow the material and the questions users ask. A methods question may need a complete procedure and its conditions; a definition question may need only a paragraph; a table lookup may require the table heading, units, and footnotes together. Splitting in the middle of a sentence, citation, equation, or table row can remove the context that makes a passage interpretable.

Document structure, data type, domain vocabulary, and expected question types all matter. When chunking rules or embedding choices change, existing indexes may need to be rebuilt because the searchable units and their representations have changed. There is no chunk size that is universally best across a mixed research library.
Step 3: Build representations and indexes
Dense vector representations
An embedding model maps each chunk to a numerical representation in which related meanings are close together. A question such as “evidence that a treatment lowers inflammation” can retrieve a passage that uses different wording, such as “reduced inflammatory markers.” This semantic matching is the main strength of dense vector search.

Keyword or sparse indexes
A full-text or sparse index is better at literal matching. Exact paper titles, gene names, product codes, legal terms, acronyms, and unusual technical strings can be damaged by a purely semantic search. BM25-style methods score terms using their occurrence and rarity, making them useful when the user’s wording must appear in the source.
Use compatible query and document representations
Official Google Cloud and AWS reference architectures embed both documents and user queries with the same embedding model and parameters before searching a vector index. Mixing incompatible models or preprocessing settings can make distances meaningless even when the index itself is healthy. Store the model name, version, dimensionality, and preprocessing configuration with the index so a later update can be audited.
Step 4: Interpret and search the user’s question
Preserve conversational context
In a multi-turn conversation, a short follow-up such as “What was its sample size?” may be ambiguous on its own. A query-rewriting stage can add the relevant earlier subject before searching. The rewritten query should remain traceable to the user’s original wording; otherwise, an unnoticed rewrite can retrieve a different topic.

Choose semantic, lexical, or hybrid retrieval
Semantic search handles paraphrases and conceptual similarity. Keyword search handles exact terminology. Hybrid retrieval runs both and combines their results, allowing a passage to rank well because it is conceptually related, literally matches a rare term, or both.
| Retrieval approach | Best at | Typical weakness | Useful controls |
|---|---|---|---|
| Dense vector | Paraphrases, related concepts, varied wording | May miss exact names, codes, or rare jargon | Embedding model, distance metric, top-k, metadata filters |
| Keyword or sparse | Exact phrases, identifiers, domain terms | May miss a relevant passage that uses different wording | Analyzers, stemming, field boosts, BM25 parameters |
| Hybrid | Queries needing both meaning and exact terminology | More moving parts and ranking calibration | Score fusion, per-search weighting, separate top-k values |
NVIDIA describes hybrid search as combining sparse keyword methods with dense embeddings, while Microsoft describes parallel keyword and vector queries whose matches are unified. Those descriptions explain the design pattern, not a universal benchmark winner. Evaluate each approach against the questions and vocabulary of the target library.
Step 5: Select, filter, and rerank evidence
Retrieve a candidate set
The first search usually returns more candidates than the application will show the model. A larger candidate set can improve recall—the chance that a useful passage appears somewhere in the results—but it also increases noise and context cost. Metadata filters should be applied when the question specifies a date, document type, language, or permission boundary.
Rerank for question-specific relevance
A reranker examines the question and candidate passages together and reorders them using a more expensive relevance model or additional rules. It can prefer a passage that directly answers the question over one that merely shares vocabulary. Deduplicating near-identical chunks and limiting repeated passages from one document can make the final context more diverse.
Assemble context within model limits
The application must fit the question, instructions, retrieved passages, and any required output format inside the model’s context limit. Packing every result is rarely optimal. Keep the passage text, title, page or section, and stable source identifier together. If a passage is truncated, preserve the citation metadata and make the truncation explicit rather than implying that the model saw the complete source.

Step 6: Generate an answer from the evidence
The final prompt normally contains the user’s question plus selected passages. Instructions can tell the model to distinguish evidence from inference, cite source identifiers, say when the supplied material is insufficient, and avoid inventing details. The language model still performs generation: it does not mechanically copy the retrieved text and can combine, misread, or contradict it.
Applications that need verifiability should return source identity and citations with the answer. Keeping citations attached to each retrieved passage helps a reader inspect the underlying paper instead of treating a fluent paragraph as proof. Retrieval improves the information available to the model; it does not remove the need to check claims.
Why retrieval can still fail
| Failure mode | What the user may see | Likely remedy |
|---|---|---|
| Unparsed or poorly OCR’d document | The system says no evidence exists although the paper is present | Fix extraction, OCR, language detection, or table handling and reindex the document |
| Bad chunk boundary | An answer quotes a result without its conditions, units, or exception | Chunk by document structure and keep headings, captions, and footnotes attached |
| Embedding mismatch | Semantically related questions return unrelated passages | Embed queries with the same model and parameters used for indexed documents |
| Exact-term miss | A search for a rare acronym or identifier returns broad conceptual matches | Add lexical retrieval or use hybrid search |
| Overly broad query | Top results are individually relevant but collectively noisy | Rewrite the query, apply metadata filters, and rerank a focused candidate set |
| Incomplete or contradictory sources | The model presents one side as settled or fills a gap with a guess | Expose competing passages, require uncertainty language, and inspect the cited sources |
| Context overflow | Important passages are omitted or the model ignores late context | Reduce candidates, deduplicate, summarize carefully, and reserve space for instructions |
The UK Government notes that retrieval can fail on poorly indexed or noisy documents and that incomplete or contradictory results can leave hallucinations unresolved. Its concise warning is worth retaining: “Hallucinations remain a concern even in RAG (Retrieval-augmented generation)-enhanced models.”

Measure retrieval separately from writing
Test whether the correct document and passage appear in the candidate results before judging the generated prose. Useful measures include recall at a chosen top-k, ranking quality, exact-term coverage, citation correctness, latency, and cost. Build a question set that represents real users: broad literature questions, exact terminology, follow-ups, date filters, table lookups, and questions whose answer is absent.

For each response, reviewers can ask whether every material claim is supported by a retrieved passage, whether the citation points to that passage, and whether the answer distinguishes a source’s result from an inference. Test contradictory papers deliberately; a system that silently selects one result is less reliable than one that reports the disagreement.
Best Value
Interpret published numbers narrowly
A 2025 HySemRAG arXiv preprint by Alejandro Godinez reported semantic similarity scores 35.1% higher than its PDF-chunking approaches: 0.655 ± 0.178 versus 0.485 ± 0.204, with p < 0.000001. The evaluation covered 643 observations from 60 testing sessions and reported 68.3% single-pass success and 99.0% citation accuracy for that system’s validated responses. These are study-specific results, not guarantees for another library, model, parser, or query set.
Reference architecture and deployment choices
Google Cloud’s reference architecture separates ingestion from serving: parse and chunk data, create embeddings, build or update a vector index, embed each incoming query with the same model and parameters, retrieve grounding data, and add it to the prompt. AWS presents a similar document-to-query vector-search flow. These architectures illustrate the common sequence rather than endorsing one vendor or deployment.
For a small, stable collection, a single service can combine parsing, indexing, retrieval, and generation. Larger or frequently changing libraries usually benefit from separate ingestion jobs and an online serving path. Separate components make it possible to reprocess failed files, roll out a new embedding model, enforce document permissions, and monitor retrieval without redeploying the answer generator.
Choosing an approach for your library
- Start with corpus characteristics: estimate document count, languages, scan quality, update frequency, tables, figures, and access restrictions.
- Map question types: record how often users ask conceptual, exact-term, comparative, date-bounded, or conversational questions.
- Prototype two retrieval modes: compare dense and keyword search on the same labeled questions before deciding whether hybrid fusion is worth the added complexity.
- Define evidence policy: decide whether answers may use one passage, must cite multiple sources, or must decline when no passage meets a relevance threshold.
- Budget operational limits: measure indexing time, query latency, storage, model calls, and context usage under the expected workload.
- Plan for change: version parsers, chunking rules, embedding models, metadata schemas, and indexes so a result can be reproduced after an update.
Microsoft presents classic RAG as attractive when simplicity, speed, or fine-grained pipeline control is the priority, and agentic retrieval as an option for complex or conversational queries and structured responses. That is product guidance, not a universal rule. A controlled evaluation on the intended library and questions is the safer basis for selection.
Practical checklist before launch
- Open representative files and verify that extracted text, tables, headings, and page references are correct.
- Inspect random chunks for complete context, sensible boundaries, and preserved metadata.
- Confirm that document and query embeddings use the same model and preprocessing configuration.
- Run exact-term, paraphrase, date-filtered, and no-answer queries through both retrieval and generation tests.
- Check that permission filters are applied before passages reach the model.
- Log query text, rewritten query, retrieved identifiers, scores, reranking decisions, prompt version, and answer citations without exposing restricted content.
- Review contradictory-source cases and require the answer to state uncertainty rather than silently choosing a result.
- Reindex deliberately after changing chunking, embedding models, analyzers, or metadata fields.
The central design principle is simple: RAG searches an external library at answer time, but the quality of that search depends on the entire chain from source preparation to evidence selection. Treat retrieval as a measurable information-access system and generation as a separate, fallible interpretation step.
