Researchers measure gene-editing performance in two separate stages: they quantify what happened at the intended (“on-target”) site, then look for unintended changes elsewhere and confirm whether those changes occurred in the relevant edited cells. The result is not one universal percentage. It depends on the editor, guide, delivery method, exposure time, cell type, DNA-repair pathway and the assay’s detection limits.
A credible assessment therefore combines outcome measurements, candidate-site discovery, targeted confirmation and, when the application warrants it, structural and functional testing. A report that says “no off-target effects detected” means only that the chosen methods found none above their limits in the tested samples; it does not prove that no unintended event exists anywhere in the genome.

What “editing efficiency” actually measures
Efficiency is meaningful only after the intended outcome is defined. A nuclease experiment might count insertions and deletions (indels) produced by error-prone repair. A precise-editing experiment might count alleles carrying a specified base change. A donor-mediated experiment might count correct donor incorporation. A reporter system might count cells that activate an engineered signal. Those measurements answer different questions and should not be treated as interchangeable.
For an endogenous target, sequencing can show the exact mixture of alleles: the desired change, unwanted substitutions, small insertions or deletions, unedited sequence and other repair products. A reporter assay is useful for a quick, convenient readout in engineered cells, but activity at the reporter may not predict activity at the natural locus, where chromatin and local sequence differ. Enzyme mismatch assays such as T7E1 or CEL-I can provide rapid screening, yet a methodological review reports lower sensitivity than sequencing and no detailed mutation-spectrum readout.

Researchers should state whether a percentage describes edited cells, edited alleles, or reads carrying a particular sequence. Those denominators can produce very different values in a mixed population. The repair machinery also varies between cell types, so percentages from unrelated cell systems are not like-for-like comparisons.
Step 1: Define the edit and the biological system
Before choosing an assay, document the intervention precisely:
- Intended outcome: gene disruption, a defined sequence correction, a base or prime edit, or insertion of a donor-derived sequence.
- Editing reagent: nuclease or other editor, guide sequence, nuclease variant, and any donor template.
- Delivery: the vehicle, dose, exposure duration and whether cells were edited ex vivo or in a tissue.
- Biological material: species, cell type, passage or differentiation state where relevant, and the reference genome used for alignment.
- Controls: untreated or mock-treated cells, delivery-only controls when appropriate, and a positive control with a known editing outcome.
These details determine what “correct” means and which events an assay can see. Regulatory guidance summarized in a review from the U.S. Food and Drug Administration calls for testing in appropriate models and considering functionality, durability, genetic variation and effects on the edited cells where relevant to the product.

Sequencing the endogenous locus
A common workflow amplifies the target region from genomic DNA and sequences the amplicon. Analysis can report the fraction of reads containing indels, the exact sequence distribution and the proportion carrying the intended precise change. For donor-mediated repair, the analysis must distinguish correct donor incorporation from partial or rearranged incorporation, random background integration and other repair products.
Short amplicon sequencing is powerful for small variants but has a blind spot: a large deletion or insertion can prevent primer binding or produce an amplicon outside the expected size. The affected molecule may then be absent from the result rather than counted as a failed edit. When large events matter, pair short-read or amplicon data with assays designed for long-range or structural changes.

Reporter assays
Reporter cells can convert editing into fluorescence, enzyme activity or another easily counted signal. They are useful for optimization and high-throughput comparisons within the same engineered system. Their result is a proxy, however. Reporter chromatin, copy number and repair environment may differ from the endogenous locus, so a reporter-positive percentage should not be presented as the efficiency of the therapeutic or native target without direct confirmation.
Mismatch-enzyme screening
T7E1 and CEL-I detect mismatched DNA structures created when differently edited alleles are reannealed. They are inexpensive, rapid screens for whether a mixed sequence population exists. The reviewed methods literature describes lower sensitivity than sequencing and no information about the individual mutation spectrum. A positive result indicates heterogeneity, not the exact frequency or clinical suitability of an edit.

How to report an on-target result accurately
A useful result includes more than one aggregate percentage. Report:
- the sample and time point;
- the number of biological and technical replicates;
- the denominator (cells, alleles, molecules or sequencing reads);
- the fraction unedited, correctly edited and carrying each major unintended outcome;
- the assay’s validated or estimated limit of detection;
- how reads or molecules that failed quality filters were handled; and
- whether large rearrangements, donor integration and copy-number changes were assessed separately.
For example, a statement that 60% of reads contain indels does not establish that 60% of cells lost gene function. A cell can carry one edited allele and one unedited allele, and an indel may be in-frame rather than disruptive. Functional assays, protein measurements or phenotype tests may therefore be needed alongside sequence data.
Step 3: Nominate possible off-target sites
Off-target analysis begins by generating a defensible list of locations to test. No single nomination method sees every possible event.

Computational prediction
Sequence-similarity tools flag genomic sites that resemble the guide’s target. Prediction is fast and can prioritize sites for laboratory testing, but it depends on the reference genome and scoring assumptions. Genetic variation, chromatin state and the editor’s biochemical preferences can make a poorly ranked site relevant or a highly ranked site inactive.

Methods such as CIRCLE-seq, CHANGE-seq and Digenome-seq expose DNA to the editor under controlled in-vitro conditions and identify cleavage-compatible sequences. They can nominate sites that a prediction tool misses and offer broad search coverage. Because the DNA is outside living cells, these assays do not reproduce chromatin packaging, nuclear access or the repair pathways of the intended cell type. A cell-free hit is a candidate, not a confirmed cellular off-target.
Cell-based discovery
GUIDE-seq and DISCOVER-seq look for editing-associated signals in cells. Their cellular context can reveal events that depend on chromatin and intracellular processing, but capture efficiency, cell health, assay compatibility and cell type affect sensitivity. Some cell types are difficult to transfect or to process with the required tags or antibodies. Combining complementary nomination approaches reduces dependence on one method’s blind spots.
Step 4: Confirm and quantify nominated sites
Every important candidate should be tested in DNA from cells exposed to the editor under the intended or a clearly relevant condition. Targeted deep sequencing can quantify low-frequency substitutions, insertions and deletions at selected loci. Include mock-treated or otherwise appropriate background controls processed in parallel. A signal that is present at the same level in controls may reflect natural variation, sequencing error, PCR artefact or pre-existing damage rather than editing.
Confirmation requires biological context. A computationally predicted site may not be cut in the chosen cells, while an experimentally nominated site may fail to appear under a different delivery dose or exposure window. Record the editor concentration, guide, delivery method, harvest time and cell state so that the reported frequency is interpretable.

Use replicate DNA preparations when possible and set a detection threshold in advance. “Detected” should mean that the signal exceeds the assay’s validated background, not merely that one read contains a change. If the application requires a very low frequency to be excluded, the amount of input DNA, sequencing depth and error-suppression strategy must support that claim.
Step 5: Look beyond small indels
Small-variant sequencing does not characterize every consequence of a double-strand break or other editing mechanism. Depending on the application, investigators may need to examine:
- large deletions spanning primer sites or multiple exons;
- inversions and translocations between cut sites;
- integration of donor or vector DNA at intended and unintended locations;
- copy-number changes and loss of heterozygosity;
- complex rearrangements that produce multiple breakpoints; and
- the persistence of editing components or their genetic cargo.
Long-range PCR can bridge larger intervals, while long-read sequencing can read through complex or repetitive regions. Whole-genome approaches provide broader scope but still have platform-specific detection limits and analysis requirements. Orthogonal structural assays can test a particular event class without relying on the same amplification chemistry as the primary assay.
For ex-vivo products, the FDA guidance summary also includes characterizing edited-cell counts, residual editing components, off-target frequency, chromosomal rearrangements and the complete on-target outcome spectrum. Viability, growth, differentiation and intended cell function may reveal consequences that sequence data alone cannot establish.
Choosing among assay types
| Method category | Primary question | Context | Main strengths | Important limitations |
|---|---|---|---|---|
| Endogenous-locus sequencing | Which sequence outcomes occurred at the intended site? | Edited cells | Exact small-variant spectrum and quantitative frequencies | Short amplicons can miss events that disrupt amplification; structural events need additional assays |
| Reporter assay | Did the engineered reporter produce a signal? | Engineered cells | Fast, convenient optimization readout | May not transfer to an endogenous locus or therapeutic cell type |
| T7E1/CEL-I mismatch assay | Is there detectable sequence heterogeneity? | Edited DNA | Rapid screening | Lower sensitivity than sequencing and no mutation-spectrum readout |
| Computational prediction | Which genomic sites resemble the guide target? | Reference genome in silico | Quick candidate prioritization | Can miss biology-dependent sites and does not demonstrate editing |
| CIRCLE-seq, CHANGE-seq or Digenome-seq | Which sites can be cleaved under controlled conditions? | Cell-free DNA | Broad biochemical nomination | Does not reproduce chromatin, delivery or repair in target cells |
| GUIDE-seq or DISCOVER-seq | Which editing-associated events are captured in cells? | Cellular | Includes aspects of cellular context | Capture efficiency and cell-type compatibility constrain sensitivity |
| Targeted deep sequencing | What is the frequency of changes at a nominated site? | Edited and control samples | High sensitivity at selected loci | Tests chosen sites rather than the entire genome |
| Long-read, long-range or whole-genome methods | Are structural or widely distributed events present? | Edited cells | Can address larger or complex outcomes | Different event classes, coverage and detection limits require method-specific validation |
Compare methods on seven practical axes: whether they measure an outcome or only nominate a site; cellular versus cell-free context; validated sensitivity; event classes detected; dependence on PCR and the reference genome; throughput and analysis burden; and suitability for the target cell type and intended use. The evidence does not support naming one universally best assay.
Best Value
Common interpretation errors and how to avoid them
Calling a nomination a confirmed off-target
A predicted, cell-free or discovery-assay hit is a location to test. Confirmation requires an empirical signal in editor-exposed cells that exceeds the appropriate background.
Comparing unlike percentages
Do not compare a reporter-positive percentage with endogenous indel frequency, or an allele percentage from one cell type with a cell percentage from another, as though they describe the same outcome. State the denominator and biological context beside every number.
Treating “not detected” as “absent”
Use wording such as “not detected above the assay’s limit in the tested samples,” and identify the loci or genome-wide scope examined. A negative result is bounded by sample size, sequencing depth, assay chemistry and event class.
Relying on one assay
Prediction, biochemical cleavage, cellular discovery, targeted sequencing and structural testing answer different questions. A layered design is more informative than repeating one method that shares the same blind spot.
Ignoring large events
If primer loss, rearrangement or donor integration could alter safety or function, add an assay that can see those outcomes instead of inferring their absence from a short amplicon.
A practical decision path
- Write the intended molecular and functional outcome, including the denominator for the efficiency claim.
- Characterize the endogenous on-target sequence distribution with a sequencing method suited to the expected variant size.
- Use computational prediction plus at least one complementary biochemical or cell-based nomination approach when the risk assessment calls for broader coverage.
- Rank candidates using guide similarity, assay evidence and relevance to the target cell and delivery condition.
- Deep-sequence nominated loci in edited and matched control samples, with a predefined detection threshold.
- Add long-range, long-read, copy-number or genome-wide assays for structural risks that short amplicons cannot address.
- Relate molecular findings to viability, growth, differentiation or intended function when those outcomes matter.
- Report methods, controls, sample timing, replicates, denominators, detection limits and all major outcome classes so another reader can judge what the result does—and does not—show.
This workflow turns “efficiency” and “off-target rate” from isolated numbers into qualified measurements tied to a specific editor, guide, delivery system, cell type and assay scope.
