Researchers are working to make the drugs more selective, deliver light deeper and more precisely, and reduce side effects—but investigational approaches are not established improvements in patient outcomes.
What is photodynamic therapy?
A photosensitizer is given by mouth, applied to the skin, or administered into a vein, depending on the tumor and treatment plan. Typically, light is applied 24 to 72 hours later, after more of the drug has left normal cells while remaining in cancer or precancer cells. Light of a specific wavelength activates the medicine, producing reactive oxygen that damages cancer cells. PDT may also damage blood vessels supplying a tumor and activate the immune system. The National Cancer Institute (NCI) explains the treatment and its uses in patient information reviewed May 15, 2025.

How light reaches the treatment area
For skin lesions, a clinician can direct light onto the skin. For tumors in the throat, airways, or lungs, light may be delivered through an endoscope and a fiber-optic cable. Extracorporeal photopheresis is a related light-based procedure used to treat abnormal white blood cells in cutaneous T-cell lymphoma.

Where PDT is used—and where it is limited
In the United States, NCI lists FDA-approved PDT uses for actinic keratosis, advanced cutaneous T-cell lymphoma, Barrett esophagus, basal cell skin cancer, esophageal cancer, non-small cell lung cancer, and stage 0 squamous cell skin cancer. Approval and availability can differ by country. PDT can also be used to relieve symptoms when some esophageal cancers block the throat or non-small cell lung cancers obstruct airways.

PDT is a local treatment, not a whole-body cancer therapy. NCI says its light cannot pass through more than about 1 centimeter of tissue, making it better suited to tumors on or just beneath the skin, or on the lining of internal organs or cavities, than to large or deep tumors. The illuminated area can include healthy tissue, so local injury is possible.
What researchers are trying to improve
NCI describes several goals: develop more powerful photosensitizers, target tumors more precisely, get light farther into tissue, reduce side effects, and improve treatment equipment and light delivery. These goals address distinct constraints: a more selective drug could spare healthy tissue, while improved light penetration or delivery could make otherwise hard-to-reach areas more accessible. They are research aims, not guarantees that PDT can treat deep or widespread cancers.

Photoimmunotherapy
Photoimmunotherapy is a developing approach that combines a photosensitizer with an immune protein intended to direct treatment to cancer cells. NCI reports studies in people with head and neck cancers. Its research status should not be confused with established PDT uses or proof that it improves outcomes.
Fluorescence-guided surgery followed by PDT for colorectal cancer
An active phase I/II trial listing accessed October 7, 2026, describes fluorescence-guided tumor assessment followed by intraoperative PDT after surgery. Its objectives include evaluating safety and potential efficacy; those are questions the trial is designed to investigate, not demonstrated benefits. The listing names Roswell Park Comprehensive Cancer Center as the lead organization and Anthony S. Dakwar as principal investigator. See the NCI clinical-trial listing.

Verteporfin and an optical-fiber system for localized prostate cancer
A separate active phase I/II trial listing accessed October 7, 2026, studies verteporfin with the SpectraCure P18 system for localized prostate cancer. Optical fibers are inserted into the prostate. The stated aims include collecting safety data, establishing dose parameters, and evaluating efficacy; the approach remains investigational in the listing. See the NCI clinical-trial listing. Trial status and recruitment can change, and the listing does not establish that this treatment is effective.
How to assess a proposed PDT improvement
Different PDT strategies solve different delivery problems, so a fair comparison starts with the tumor and the treatment setup—not a claim that one investigational technique is better overall.

- Location and light access: Is the target on the skin, on an accessible internal lining, or deeper in tissue?
- Photosensitizer and route: Which medicine is used, and is it given orally, applied locally, or administered intravenously?
- Light delivery: Is light directed at the skin, passed through an endoscope, or delivered by fibers placed in tissue?
- Status and evidence: Is the use an established clinical application or a trial approach? A trial’s phase and objectives describe what researchers are assessing, not whether the treatment works.
- Risks and precautions: What local effects and photosensitivity precautions apply to that specific drug and treatment site?
The colorectal and prostate trial listings cannot be used as a head-to-head efficacy comparison: they concern different cancers, treatment setups, and research questions.

PDT may cause burns, swelling, pain, or scarring in the treated area; other effects depend on the site. NCI also notes that photosensitizers tend to accumulate in abnormal cells and that light can be focused on a target, limiting damage to healthy cells. It lists no scarring among PDT’s benefits, but local scarring is also a possible side effect. The actual risk depends on the treatment context.

Porfimer sodium can make skin and eyes sensitive to light for about six weeks, according to NCI. During that period, it advises avoiding direct sunlight and bright indoor light. This duration applies to porfimer sodium, not automatically to every photosensitizer. Follow the light-protection instructions from the treating team for the specific medicine used.
