Psychological horror builds suspense by making players anticipate danger, interpret incomplete clues and question the spaces they occupy. Instead of depending on repeated loud reveals, it can layer visual and audio cues, distort familiar layouts, limit information or control, and let a story’s meaning emerge over time. These methods can include occasional surprises; the difference is that surprise is a punctuation mark, not the whole design.
Suspense starts with what might happen
A dark doorway or a sound from beyond the player’s view can make the imagination do part of the work. The player anticipates a threat before seeing one, and that interval of uncertainty can carry more tension than a sudden reveal.

The developers of The Tartarus Key describe atmosphere as tension built through visuals and sound, including fear of what the player imagines might be present. They also point out that a quick movement or noise can still create a small scare. The distinction is not that psychological horror must avoid every startling moment: it is that repeating the same kind of shock can make it predictable and less effective. The developers’ March 21, 2022 log on atmosphere and jumpscares explains the approach.

A frightening image or sound does not work in isolation from its timing. A cue can be more unsettling when it follows a period of quiet, recurs in a changed context, or arrives after visual details have led the player to expect something else.

Graja, Lopes and Chanel studied an adaptation of Konami’s P.T., using galvanic skin response and participants’ reports of perceived emotion. Their findings describe how the ordering of effects can shape emotional responses and how sound can contribute to a gradual build-up of psychological impact. The study supports treating audiovisual timing as a design choice; it does not establish a single sequence that will work for every player. Read the study’s publication record.
Unreliable spaces create unease without a visible enemy
When a corridor loops back unexpectedly, a transition breaks the player’s sense of direction, or rooms seem to connect in impossible ways, the environment itself becomes a source of uncertainty. The player may not face a direct attacker, yet familiar rules of navigation no longer feel dependable.
Jiawen Zhang’s 2026 Southern Methodist University thesis examines these techniques in a designed Half-Life 2 level, “Echoes of Manor.” It discusses tension-and-release cycles and light puzzle elements as ways to keep players engaged and avoid boredom. This is a focused design study, not proof that every player will respond identically to unusual layouts. See the thesis.

A confined viewpoint, sparse information about the world outside, or restricted control can make a player feel vulnerable. Gaps invite inference: a sound without an explanation or an unseen space can feel threatening precisely because the player cannot fully inspect it.

Mienke Fouché’s analysis of Iron Lung describes how its confined setting, limited control, sparse outside-world information and first-person perspective contribute to immediacy. The analysis of player reviews also notes a trade-off: while missing information could stimulate imagination, some reviewers felt that a lack of threats weakened immersion. Uncertainty is most useful when the game gives it consequence or connects it to a plausible danger; withholding everything can leave the player with little reason to care. Read Fouché’s case study.
Story layers make earlier clues change meaning
Suspense can continue after a frightening moment has passed if later discoveries make players reinterpret what they have already seen or heard. Audio logs, cinematics, voice-over, production design and sound can reveal different parts of a character or situation across repeated play cycles.

In an interview about Returnal, creative director Gregory Louden described the goal as a story that stays with players: “Not haunting like jump scares, but haunting like something that stays with you.” He explains that the game reveals narrative layers through those varied channels over repeated cycles. The effect is not only a question of what happens next, but of what earlier events meant. Read the Unreal Engine interview with Louden.
Why the approach works—and where it can falter
- Anticipation over repetition: An unseen possibility can keep attention focused between events; an identical shock repeated too often can become familiar.
- Implication over constant attack: Distorted spaces and unexplained cues can unsettle without a visible enemy, but the player still needs meaningful stakes.
- Accumulation over isolated effects: The timing and order of audiovisual details can build tension gradually, though no one sequence is established as universally effective.
- Uncertainty with purpose: Limited information or control may heighten vulnerability, but excessive withholding can make the experience feel empty rather than tense.
- Reinterpretation over explanation alone: Narrative layers can deepen unease when new information changes how earlier clues are understood.
These are design possibilities, not guarantees. The supporting examples span creator accounts, a focused university thesis, a study of a P.T. adaptation and an analysis of Iron Lung player reviews; they do not establish that all players or all psychological horror games respond in the same way.
