Suspected plague is a medical emergency. If a person develops a rapidly painful, swollen lymph node or severe pneumonia, sepsis-like illness, or fever after possible contact with infected fleas, rodents, wildlife, or a person with pneumonic plague, they need immediate medical evaluation. Plague is caused by Yersinia pestis; prompt antibiotics can cure it, but treatment should begin when clinicians suspect plague rather than waiting for a laboratory confirmation.
Diagnosis combines symptoms, exposure and geography with laboratory testing. Clinicians may collect blood, a lymph-node aspirate, sputum or tissue, then start an appropriate antimicrobial regimen. The treatment details below summarize current U.S. Centers for Disease Control and Prevention (CDC) guidance; regimens in other countries may differ.

What plague is and when clinicians suspect it
Plague is an infection caused by the bacterium Yersinia pestis. In the United States, naturally occurring cases are concentrated in parts of the West. People most often become infected after a bite from an infected rodent flea, and less often after handling an infected animal. The CDC describes plague as potentially life-threatening and emphasizes that antibiotics must be given promptly. See the CDC overview, updated August 24, 2026: About Plague.
Clinicians do not diagnose plague from one symptom alone. They combine the illness pattern with recent travel or residence, animal and flea exposure, and the timing of symptoms. A painful lymph node that enlarges quickly is particularly suggestive of bubonic plague, but other forms may not produce an obvious “bubo.”

| Form | Typical clues | Why it can be missed |
|---|---|---|
| Bubonic | A rapidly developing, painful swollen lymph node, often in the groin, armpit or neck; fever and systemic illness may accompany it. | The initial flea bite may not be noticed, and the swollen node can be mistaken for another infection. |
| Pneumonic | Plague involving the lungs, with a severe respiratory presentation. | It may lack a characteristic bubo. Because person-to-person spread is possible, suspected cases require isolation and droplet precautions. |
| Septicemic | Infection in the bloodstream, which can cause a rapidly serious systemic illness. | There may be no localized sign that points specifically to plague, so exposure history and clinical judgment are critical. |
The CDC’s diagnostic guidance explains how these forms are evaluated: Diagnosing Plague.
What to do if plague is possible
Seek emergency medical care immediately rather than waiting to see whether symptoms improve. Tell the clinician about recent travel or residence in an area where plague occurs, flea bites, contact with rodents or a sick or dead animal, and close contact with anyone who had a serious pneumonia-like illness. That information can change which specimens are collected and which infection-control measures are used.
Do not try to confirm plague with an over-the-counter test or use leftover antibiotics. Prescription selection, route and duration depend on the form and severity of disease, age, pregnancy status, allergies, kidney and other medical conditions, and local public-health advice. If respiratory symptoms are present, alert the facility before arrival when possible so staff can prepare appropriate precautions.

How clinicians diagnose plague
1. Clinical assessment comes first
Doctors first assess the pattern and speed of illness, examine lymph nodes and the lungs, and ask about exposures and geography. A bubo strongly supports consideration of bubonic plague, while severe pneumonia or bloodstream infection after a plausible exposure can raise concern for pneumonic or septicemic disease even when no bubo is visible. Because these presentations overlap with other serious infections, the initial diagnosis is usually a clinical suspicion that must be tested urgently.

2. Specimens are collected for laboratory testing
Clinicians should obtain pretreatment specimens when this can be done without delaying care. The specimen depends on the presentation:
| Specimen | When it is useful | What the laboratory may do |
|---|---|---|
| Lymph-node aspirate | A painful, enlarged bubo is present. | Microscopy and culture can look for Y. pestis; buboes may contain many organisms. |
| Blood | Especially important when bloodstream infection is suspected; blood culture is a sensitive way to isolate the organism. | Culture and other laboratory work can support confirmation. |
| Sputum or another respiratory specimen | Severe respiratory illness suggests pneumonic plague. | Respiratory culture may be possible in very ill patients. |
| Tissue or other material | Used when the clinical situation makes another sample more appropriate. | Testing is selected in consultation with the laboratory and public-health authorities. |
No single specimen is mandatory for every patient. The clinical form, what can be collected safely and the laboratory’s capabilities determine the combination.
3. Microscopy can provide an early clue
Stained smears of blood, lymph-node material or sputum may show small Gram-negative rods with bipolar staining, sometimes described as having a “safety pin” appearance. This is presumptive evidence, not a reason to wait for final identification. The finding should trigger urgent communication with the treating team, laboratory and public-health authorities.

Culture can isolate Y. pestis from an appropriate specimen. If cultures are negative but suspicion remains, serology may help. CDC guidance calls for an early serum sample followed by a convalescent sample four to six weeks or more after disease onset. The later sample is important because antibody levels may not yet be informative at the beginning of illness.

5. Automated identification can be misleading
Some automated biochemical systems and MALDI-TOF mass-spectrometry databases may not include Y. pestis. An isolate can therefore be misidentified as Yersinia pseudotuberculosis if the laboratory does not recognize the clinical context. When plague is suspected, the laboratory should be told immediately and should coordinate confirmatory work with public-health laboratories. CDC’s current technical guidance is available at Clinical Testing and Diagnosis for Plague (updated April 6, 2026).
6. Treatment must not wait for the final report
CDC’s instruction is explicit: “Never delay or withhold treatment pending the receipt of laboratory test results.” Specimens should be collected first when feasible, but collection, transport, culture, specialized identification or serology must not postpone antibiotics. This sequence matters because plague can worsen quickly and the benefit of early treatment is greatest when therapy starts promptly. The full clinical-care guidance is at Clinical Care of Plague.
How plague is treated today
First-line medicines in U.S. CDC guidance
For the United States, CDC identifies gentamicin and fluoroquinolones as first-line treatment options. Clinicians choose among appropriate agents based on the patient’s illness, organ function, drug interactions, allergies, pregnancy, age and other medical factors. A person with suspected plague should be treated by a medical team; these medicines are not suitable for self-prescribing.

That evidence context is one reason infectious-disease consultation and public-health coordination may be appropriate, particularly for unusual presentations or patients who cannot receive a standard option.

Intravenous versus oral therapy
The route depends on severity and the patient’s ability to absorb and take medicine. Intravenous therapy is generally used when illness is severe, the patient is unstable, or oral absorption is unreliable. Oral therapy may be appropriate in less severe situations or when a clinician transitions a recovering patient, provided the chosen drug is suitable. The decision is individualized rather than determined by the disease name alone.
Typical duration
CDC’s clinical-care page lists a generally recommended treatment duration of 10 to 14 days. Clinicians may extend therapy when fever or other concerning signs persist. The duration is a clinical recommendation, not a fixed self-treatment schedule; stopping early, changing drugs or switching routes without medical direction can leave infection inadequately treated.
Children, pregnancy and underlying conditions
Age, pregnancy, kidney or liver disease, immune status, allergies and other medicines can change the safest regimen. Pediatric and pregnant patients need clinician-directed selection and dosing. A treating team may consult infectious-disease specialists, pharmacists and public-health officials when the presentation is severe or the standard choices are unsuitable.
Isolation, infection control and reporting
Suspected pneumonic plague
Suspected pneumonic plague requires isolation and droplet precautions to reduce the risk of person-to-person transmission. Healthcare workers use the facility’s infection-control procedures while diagnostic specimens are collected and treatment begins. The need for isolation is tied to the suspected respiratory form; it is not a reason to delay emergency evaluation.
Best Value
Immediate public-health notification
Suspected plague should be reported immediately to local and state health departments. Public-health involvement helps arrange specialized testing, interpret possible laboratory misidentification, identify people who may have been exposed and coordinate infection-control decisions. CDC emergency guidance is available at Guidance for Responding to a Plague Bioterrorism Event (August 23, 2024).
Who may need preventive antibiotics after exposure?
Post-exposure prophylaxis (PEP) is different from treating active suspected plague. CDC recommends PEP for defined, significant exposures, including close, sustained contact within six feet of a person or animal with pneumonic plague, or direct contact with infected body fluids or tissues. The CDC clinical guidance specifies a seven-day course for indicated PEP.
Not everyone who was nearby automatically needs antibiotics. Health departments and clinicians assess the type, duration and intensity of contact, the source’s form of plague and whether precautions were used. People should not begin prophylaxis on their own or share someone else’s prescription.
For healthcare workers and first responders who can maintain standard and droplet precautions, routine pre-exposure prophylaxis is generally not considered necessary. Unusual circumstances, such as poor ventilation, overcrowding or an inability to maintain precautions, can change that judgment. The CDC’s detailed recommendations are in Antimicrobial Treatment and Prophylaxis of Plague (July 16, 2021).
Can plague be cured?
Yes. Plague can be cured when effective antibiotics are started promptly, but it can become life-threatening when treatment is delayed. The CDC states: “The earlier a patient seeks medical care and receives treatment that is appropriate for plague, the better their chances are of a full recovery.” Recovery still requires completing the prescribed course and following medical instructions about follow-up, because clinicians may adjust treatment if fever or other concerning signs continue.
The practical rule is simple: compatible symptoms plus a plausible exposure warrant urgent medical assessment; clinicians collect suitable specimens when possible, begin treatment without waiting for final laboratory results, apply isolation for suspected pneumonic disease and involve public-health authorities immediately.
