Osmotic drug delivery uses water moving through a semipermeable membrane to help push medicine out of an oral dosage form. In some designs, that can produce an approximately constant release rate—but only while the formulation’s conditions, including its osmotic gradient, support it. “Steady” describes a design behavior, not a guarantee for every product or patient.
How an osmotic pump releases medicine
A semipermeable membrane lets water pass through while restricting other contents. In an osmotic pump, water enters the tablet and interacts with an osmotically active core. The resulting pressure helps move the drug through a small outlet.

One documented design is the OROS system described in the FDA’s review of Ditropan XL. Its core has a drug layer and a separate osmotic engine, or “push layer,” enclosed by the membrane. A small opening is made on the drug-layer side. As water enters, the push layer swells and pushes the drug layer through the opening. FDA review of Ditropan XL

Zero-order release means that a dosage form releases drug at an approximately constant rate over a period of time. The FDA review states: “Drug delivery is essentially zero-order as long as the osmotic gradient remains constant.” The qualification matters: the release profile depends on the formulation and on maintaining the conditions that drive water entry. It is not a promise of perfectly uniform delivery in every circumstance.

How osmotic drug delivery appears in oral medicines
Ditropan XL
The FDA describes Ditropan XL as an oxybutynin OROS product designed to deliver medicine for up to 24 hours and support once-daily dosing. That describes this specific product; it should not be generalized to every osmotic pump or every oxybutynin formulation. FDA review and product context
OSMOLEX ER
OSMOLEX ER uses a different configuration: amantadine is in an extended-release core, with an outer immediate-release layer. Its labeling explains that water crosses a semipermeable polymer membrane, and a laser-drilled opening lets the osmotic gradient between the core and gastrointestinal fluid drive release. The label also says the mechanism by which amantadine produces its therapeutic effect is unknown; the delivery mechanism explains how the dosage form releases medicine, not why the medicine works. What osmotic delivery can—and cannot—offer
For particular products, an osmotic design can extend delivery and allow less frequent dosing. The FDA safety review says the OROS/GITS systems it discusses may be less dependent on gastrointestinal acidity, alkalinity, or food content. These are system-specific observations, not proof that all osmotic products behave identically or outperform other modified-release designs. FDA safety review of OROS/GITS products
Recommended Free Tools
When comparing modified-release medicines, the useful questions are how the drug is released, what release profile the specific formulation is designed to provide, how the gastrointestinal environment may affect it, and how often the medicine is taken. Also check whether the tablet must be swallowed whole, whether remnants may appear in stool, and what warnings apply to that product. The FDA sources cited here do not establish a head-to-head performance ranking of osmotic pumps versus matrix or other release systems.

Tablet remnants and product-specific safety
Some osmotic tablets leave inert material that passes through the digestive tract. OSMOLEX ER labeling says its biologically inert tablet components remain intact during gastrointestinal transit and pass in stool as a tablet shell. That description applies to that product, not automatically to every extended-release tablet.
The FDA has reviewed intestinal obstruction reports involving specific nondeformable OROS/GITS products. Those reports make dosage-form design and individual patient circumstances relevant, but they do not establish that obstruction is common or that every osmotic tablet carries the same risk. Follow the current labeling for the prescribed medicine and ask a clinician or pharmacist about concerns relevant to your health. FDA safety review

Safe use depends on the exact medicine
- Do not crush, chew, split, or otherwise alter an extended-release tablet unless that medicine’s current labeling explicitly says it is permitted.
- Do not infer that a medicine is suitable for you because its release system is osmotic; delivery technology does not determine which treatment is appropriate.
- For instructions about swallowing, missed doses, tablet remnants, or warnings, use the current product labeling and consult your clinician or pharmacist.
