How Oncogenic Proteins Help Cancer Cells Grow—and How Targeted Drugs Block Them

Oncogenic proteins are abnormal or overactive proteins that keep cancer cells receiving signals to grow, divide, survive, move, or recruit blood vessels. Targeted drugs are designed to interrupt a specific protein or pathway, but they work only when a tumor has a reachable target on which it depends. The same protein name does not automatically mean the same drug, indication, or result in every cancer.

The key link between a tumor’s biology and treatment is biomarker testing. A laboratory test may identify a mutation, amplification, fusion, protein overexpression, or another feature that has an approved treatment, an off-label option, or a clinical-trial match. It may also find no actionable match, and cancers can acquire resistance after an initially effective response.

What is an oncogenic protein?

An oncogenic protein is a protein that contributes to cancer when it is produced in excessive amounts, switched on continuously, changed by a mutation, fused to another protein, or placed under abnormal control. The protein may be a receptor at the cell surface, an intracellular enzyme, a transcription regulator, or another component of a signaling network.

Healthy cells use these proteins as controlled switches. Signals from outside the cell can tell a cell when to grow, divide, repair itself, move, or die. Oncogenic changes jam one or more of those switches in the “on” position. Cancer cells can then keep dividing without the normal checks, resist cell death, invade nearby tissue, spread to distant sites, and stimulate blood-vessel growth. The National Cancer Institute describes targeted therapy as treatment aimed at proteins that control how cancer cells grow, divide, and spread.

The abnormal protein is not always the original cause of the cancer. A genomic alteration in DNA may be the event that changes the protein, while the protein is the molecule that actually transmits the growth signal or performs the altered function. That distinction matters when interpreting a pathology or sequencing report.

How oncogenic proteins keep growth signals active

Receptors that signal without the usual instruction

Many growth-factor receptors sit in the cell membrane. Normally, a growth factor binds to the outside portion of the receptor, causing its internal kinase domain to switch on briefly. Mutations or extra copies of the receptor gene can make signaling excessive or continuous. The signal is passed through networks that regulate cell division, metabolism, survival, and movement.

Kinases that become permanently active

Kinases add phosphate groups to other proteins, changing their activity. A mutation or gene fusion can leave a kinase active even when no growth factor is present. The resulting chain of signals can make a cell behave as though it is constantly being told to proliferate.

Signals that bypass the receptor

Some oncogenic proteins operate farther downstream. In that situation, blocking a surface receptor may not stop the signal because the altered downstream protein continues to send growth instructions. This is one reason that a drug aimed at a familiar receptor is not automatically useful for every tumor in which that receptor is detected.

Signals that help tumors survive and spread

Oncogenic signaling can increase resistance to programmed cell death, alter how a cell uses nutrients, change its attachment to surrounding tissue, and promote migration. Other targets influence angiogenesis, the process by which tumors encourage new blood vessels to grow. Targeted treatment can therefore aim at the cancer cell itself, its blood supply, or the interaction between the tumor and the immune system.

Mutation, amplification, fusion, and protein target: what is the difference?

These terms describe different levels of the same biological story:

  • Mutation: a change in the DNA sequence of a gene. Only some mutations alter protein behavior, and only some of those are useful treatment biomarkers.
  • Amplification: extra copies of a gene, which can result in overproduction of its protein. HER2 amplification is one example of a change that can create a treatment-relevant protein excess.
  • Fusion: two genes become joined after a chromosomal rearrangement. The resulting fusion protein may contain a kinase that is active in the wrong cellular context. BCR-ABL and ROS1 fusions illustrate this pattern.
  • Protein target: the molecule a drug is intended to block, bind, degrade, or use as a delivery address. A target can be altered directly by a mutation or activated indirectly by another genomic event.
  • Biomarker: a measurable feature of a tumor, including a gene alteration, protein level, or other characteristic, used to help select treatment or estimate response.

A report can therefore show a DNA alteration without proving that a particular drug will work. The alteration must be biologically relevant, present in enough tumor cells, reachable by the drug, and covered by evidence or an applicable regulatory indication for that cancer setting.

How targeted drugs block oncogenic proteins

Small-molecule inhibitors

Small molecules can enter cells and reach intracellular enzymes such as tyrosine kinases. They may occupy the enzyme’s active site, stabilize an inactive shape, or otherwise prevent the protein from passing on its signal. Imatinib, for example, inhibits the overactive BCR-ABL tyrosine kinase created by a characteristic chromosomal rearrangement in leukemia.

Monoclonal antibodies

Monoclonal antibodies circulate outside cells and bind a defined target on a cancer cell or in its surrounding environment. A receptor-directed antibody can prevent ligand binding, reduce receptor signaling, mark the cell for immune attack, or trigger receptor removal from the cell surface. Trastuzumab is used against HER2-driven disease in defined settings.

Antibody-drug conjugates

An antibody-drug conjugate combines a targeting antibody with a potent toxic payload. The antibody binds a selected surface protein, the complex is taken into the cell, and the payload is released to damage the cancer cell. This approach still depends on sufficient target expression and on the tumor’s ability to internalize and process the drug.

Anti-angiogenic and immune-directed strategies

Some targeted treatments block signals that tumors use to recruit blood vessels. Others alter how the immune system recognizes cancer cells. These strategies are targeted because they act on a defined molecular interaction, but their effects extend beyond a single mutated protein and their use depends on the cancer type, biomarker, prior treatment, and approved indication.

How biomarker matching connects a tumor to a drug

Biomarker testing is the practical bridge between molecular biology and treatment selection. The National Cancer Institute gives EGFR testing as an example: a result may lead to an FDA-approved therapy, an off-label therapy, or a clinical trial. A result can also identify a biomarker for which no treatment is currently available, or produce a finding that is not clinically useful.

  1. Obtain a tumor sample. Testing may use tissue from a biopsy or, when appropriate, DNA shed into blood. A blood result can be useful when a tumor sample is unavailable, but a negative liquid-biopsy result does not necessarily prove that the tumor lacks the alteration.
  2. Choose the test scope. A focused assay may look for one alteration, while a broader panel can examine many genes and alteration types. The laboratory report should state what the test can and cannot detect.
  3. Confirm the alteration and its meaning. The report distinguishes pathogenic or likely actionable findings from variants of uncertain significance and findings that have no established treatment relevance.
  4. Match the result to the exact disease context. The same protein can be relevant in different cancers but have different evidence, dosing, sequencing, or regulatory language. Histology, stage, prior therapy, organ function, and coexisting conditions also matter.
  5. Check the treatment category. The match may be an approved therapy for that indication, an off-label use, or a clinical trial. FDA approvals and labels change, so the current label and approval date must be checked before relying on an indication.
  6. Reassess if the disease changes. A tumor that progresses may have acquired a new alteration or bypassed the original target. The treating team may consider repeat tissue or liquid testing when it could change management.

The FDA has authorized liquid-biopsy tests including Guardant360 CDx and FoundationOne Liquid CDx. Their availability does not mean that every alteration detected by every assay has a matched drug.

Examples of oncogenic proteins and targeted treatments

Target and alteration Where the protein acts Examples of targeted drugs Illustrative cancer context Key qualification
BCR-ABL fusion Intracellular tyrosine kinase created by a chromosomal rearrangement Imatinib Leukemia caused by the characteristic BCR-ABL rearrangement The treatment is tied to the fusion-driven disease, not to any unrelated BCR or ABL finding.
HER2/ERBB2 amplification, overexpression, or activating alteration Cell-surface receptor tyrosine-kinase pathway Trastuzumab and other HER2-directed treatments; trastuzumab deruxtecan is an antibody-drug conjugate used in defined settings Molecular subtypes of breast cancer, biomarker-defined non-small-cell lung cancer (NSCLC), and other cancers HER2 status, the exact alteration, tumor type, and current label determine eligibility.
EGFR mutation Cell-surface receptor tyrosine kinase Erlotinib and gefitinib A subset of lung cancers with activating EGFR mutations Not every EGFR alteration predicts the same sensitivity; the mutation must be interpreted in context.
KRAS G12C mutation Intracellular RAS signaling protein Sotorasib and adagrasib Biomarker-selected NSCLC The G12C substitution is required; other KRAS mutations are not interchangeable with it.
ROS1 rearrangement Fusion-driven intracellular kinase Crizotinib, entrectinib, and repotrectinib ROS1-positive NSCLC ROS1 rearrangements occur in approximately 1% to 2% of NSCLC patients, according to the National Cancer Institute.

KRAS G12C evidence in NSCLC

In the phase II sotorasib study cited on the National Cancer Institute’s current NSCLC PDQ page accessed in 2026, the overall response rate was 37.1% among 124 evaluable patients. Median progression-free survival was 6.8 months, and median overall survival was 12.5 months. These are study-level outcomes for the specified population and treatment setting; they are not a prediction for an individual patient or for every KRAS-mutated cancer.

HER2-variant NSCLC evidence

In a National Cancer Institute-cited study of trastuzumab deruxtecan for HER2-variant NSCLC, 55% of 91 patients had a confirmed objective response. Median progression-free survival was 8.2 months and median overall survival was 17.8 months. The figures apply to that study population and its defined treatment context, not to all HER2-positive tumors.

Why the same target does not produce the same result in every cancer

A target must meet several conditions before a drug can help:

  • The alteration must activate or sustain a dependency that the cancer actually uses.
  • The drug must reach the target at an effective concentration, including in sites such as the brain when relevant.
  • The tumor cells must retain enough target expression or altered protein for the drug to matter.
  • The cancer’s lineage, coexisting alterations, and previous treatments can change sensitivity.
  • The drug must be authorized or supported by appropriate evidence for the specific disease setting.

Consequently, “HER2-positive,” “EGFR-mutated,” or “KRAS-mutated” is not a complete treatment plan. The exact assay result, cancer type, stage, prior therapy, and current regulatory indication are essential.

Why targeted therapy can stop working

Resistance means that cancer continues to grow despite a drug that previously controlled it, or never responds in the first place. Molecular resistance can arise because the target changes so the drug binds less effectively, because the cancer activates a bypass pathway, because target expression is lost, or because different subclones expand under treatment pressure. A tumor can also progress in one site while remaining controlled elsewhere.

Resistance is not proof that the original biomarker was “wrong.” Cancer cells are genetically diverse, and treatment changes which subpopulations survive. When progression is suspected, clinicians interpret scans, symptoms, treatment exposure, and sometimes a new tissue or blood test together. A new result may identify a resistance alteration, reveal a different actionable target, or show that no established match remains.

Limits, adverse effects, and changing approvals

Targeted therapy is more selective than nonspecific chemotherapy, but “targeted” does not mean harmless. The target may also be present in healthy tissues, and pathway blockade can affect organs, blood counts, heart function, skin, digestion, or other systems depending on the drug. Antibody-drug conjugates add payload-related toxicities, while immune-directed treatments can cause inflammatory effects. Clinicians weigh these risks against the expected benefit and monitor patients according to the drug’s label.

Approvals are not static. FDA notices in 2026 include molecularly defined oncology approvals or accelerated approvals involving RAS-family inhibition, HER2 alterations, and ESR1-mutated disease. An indication can be expanded, restricted, converted from accelerated to regular approval, or withdrawn as evidence develops. Any treatment claim should therefore be checked against the current FDA label for the exact cancer, biomarker, line of therapy, and date.

How to read a molecular report without overinterpreting it

  • Identify the alteration type: mutation, amplification, fusion, copy-number change, or protein-expression result.
  • Record the exact variant: for example, KRAS G12C is more specific than “KRAS mutation.”
  • Check the evidence category: approved match, clinical-trial match, off-label possibility, resistance marker, prognostic finding, or uncertain significance.
  • Look for test limitations: sample quality, tumor fraction, genes or alteration classes not covered, and whether the assay was tissue-based or liquid-based.
  • Separate biology from eligibility: a biologically interesting target does not by itself establish that a person should receive a particular drug.

Precision medicine means using genetic changes in a patient’s tumor to help determine treatment. It is a decision framework, not a guarantee that testing will find an effective drug.

The practical takeaway

Oncogenic proteins are the working machinery through which cancer-causing genomic changes keep growth and survival signals active. Targeted drugs interrupt that machinery with small molecules, antibodies, antibody-drug conjugates, angiogenesis blockers, or immune-directed approaches. Their value depends on a verified biomarker, a drug that can reach and inhibit the relevant target, evidence in the exact cancer setting, and continued control of resistance and toxicity. BCR-ABL, HER2, EGFR, KRAS G12C, and ROS1 show why molecular matching must be precise rather than based on a protein name alone.
