NFL players rebuild from an Achilles rupture through staged protection, calf-strength work, balance and movement training, then impact, running and football-specific drills. There is no publicly established, universal NFL team protocol in the sources cited here, and a calendar date alone does not determine readiness to run. NFL studies show that a return to a game is possible, but they do not promise that an individual player will return or regain preinjury performance.
What NFL return-to-play studies actually show
Two retrospective NFL studies report different return rates because they examined different groups and used different definitions. Their figures are useful context, not interchangeable predictions for a player recovering today.

| Study and group | Return finding | What the result measures |
|---|---|---|
| Jack et al., 2017: 95 NFL players who had 98 Achilles tendon repairs | 71 of 95 players returned to NFL sport (72.4%); mean time from surgery to return was 339.8 ± 84.8 days. | The broader cohort’s return to NFL sport. This is a group average, not an individual timetable. Study abstract. |
| 2021 study: NFL skill-position players from the 2009–10 through 2015–16 seasons | 57% returned to play. Tight ends had the highest position-specific rate (71%) and wide receivers the lowest (38%). | Return required playing in at least one game after repair. The skill-position cohort and definition differ from the 2017 study. Study abstract. |
Returning for at least one game is not the same outcome as recovering a former role, performance level, career length or full movement capacity. In the 2017 cohort, matched controls had longer NFL careers than the repair group (3.6 ± 2.1 years versus 2.7 ± 2.1 years), and postoperative performance scores were significantly worse than preoperative scores for running backs and linebackers. The 2021 study also found performance decreases in some positions among players who returned. These are associations from retrospective studies; they do not establish that the repair itself caused the differences.
How rehabilitation builds from protection to football demands
Rehabilitation generally moves from protecting the healing tendon to restoring controlled motion, calf capacity and balance, then tolerating impact and sport-specific loading. A 2024 clinical review describes these phases as a framework, not an NFL schedule. The treating surgeon and rehabilitation team adjust the plan to the injury, procedure, symptoms and progress. 1. Protect the repair and regain controlled motion
Early recovery limits stress on the repaired tendon while movement is reintroduced in a controlled way. The American Orthopaedic Society for Sports Medicine (AOSSM) describes use of a boot or brace for about 6 to 8 weeks after surgical repair or during non-surgical treatment, followed by gradual restoration of motion, strength and function. A boot is a protection tool, not a rehabilitation plan on its own; fit, heel-lift configuration and timing belong to the treating clinician.
The 2024 review supports early protected mobilization and progressive loading, while emphasizing control of tendon elongation and avoidance of premature stress. In the approach it describes, passive calf stretching is generally avoided for up to 12 weeks after surgery. Those details are not universal instructions: the surgeon’s protocol takes precedence. AOSSM patient guidance, reviewed November 2025.

2. Rebuild calf strength, endurance and movement quality
The calf muscles produce plantar flexion—the action used to push off the ground. Rehabilitation therefore works progressively toward stronger and more controlled calf loading, including double-leg and eventually single-leg heel raises. Balance, proximal leg strength and movement mechanics matter too; being able to perform one heel raise is not, by itself, proof that the tendon and leg are ready for running.
The review describes assessing strength and movement quality before impact training. For field and court athletes, its authors recommend prerequisites that include seated isometric plantar-flexion strength of 1.5–2.0 times bodyweight, ten full-range single-leg heel rises and good single-leg squat control. These are the review authors’ clinical recommendations, not validated NFL clearance thresholds or a universal standard. Review of rehabilitation and return-to-sport approaches.

3. Introduce impact and running when function allows
Once strength, symptoms and mechanics support progression, training can move through low-amplitude impact and plyometrics toward running. The 2024 review says its authors typically begin running no sooner than 12–16 weeks after surgery, while stressing that timing varies. It describes a late rehabilitation phase at 12–24 weeks, including gradual loading, strength above 70–80% limb symmetry and initiation of plyometrics or return to running; its return-to-sport phase begins at 24 weeks or later and emphasizes increasing training volume and meeting strength and power criteria. These are proposed clinical phase markers, not dates on which an NFL player is automatically cleared.
There is no well-established single return-to-sport test. Clinicians may consider plantar-flexor strength, heel-rise performance, jumping and movement quality together, interpreting the results against the demands of the athlete’s sport. Football adds high-speed acceleration, deceleration, cutting, jumping and repeated exposure to contact, so a first run is only one step in a broader progression.

4. Progress from running to football practice
Running strength must be built into sport-specific capacity: tolerating greater speed and volume, changing direction and completing football drills without symptoms or deteriorating mechanics. The available public sources do not describe a single NFL-wide sequence of drills, practice restrictions or team clearance criteria. A player’s medical and performance staff can use individualized milestones, but those private plans should not be inferred from general rehabilitation reviews.

A modified Delphi study published online on July 26, 2026, asked international experts to assess return-to-running criteria after surgical repair of an acute Achilles rupture. Thirty-five experts were invited, and the study used a 75% agreement or disagreement threshold. Seven criteria reached consensus for inclusion:

Calf-circumference symmetry was the one criterion that reached consensus for exclusion. The authors say the agreed criteria may inform clinical decisions, but require prospective validation before they can be used as decision rules. They should therefore be read as expert consensus—not a proven checklist that independently clears an NFL player to run. Consensus study abstract.
Why “six months” is not the same as an NFL return date
AOSSM says return to athletic activity typically occurs after six months in its general patient guidance. That is a broad estimate for athletic activity, not a forecast for professional football or a benchmark that applies to every repair. The 2017 NFL cohort’s mean was about 340 days from surgery to return to NFL sport, a different endpoint in a selected group of professional athletes. Neither number sets an individual player’s clearance date.
Does every player need surgery?
No universal treatment choice follows from these NFL outcome studies. AOSSM says operative and non-operative treatment can both be considered and that the patient and treating physician should weigh factors such as age, activity level and other medical issues. Injury details, goals and sport demands also inform the clinician’s assessment. The rehabilitation framework discussed here applies to recovery after repair; it should not be used to decide whether a particular rupture requires surgery. - It does not establish one standardized NFL Achilles rehabilitation pathway or reveal the private care plan of a particular team or player.
- The NFL studies are historical retrospective cohorts, not individualized prognostic tools; differences in sample, era and outcome definition matter.
- General clinical rehabilitation phases and expert consensus criteria can help explain progression, but they are not proof of a universal professional-football protocol.
For an individual rupture, the surgeon and rehabilitation team must guide loading, running progression and return to sport. This article describes general evidence and historical NFL outcomes; it is not a personal rehabilitation prescription.
