Nanoparticles may help some medicines reach the brain by carrying a drug payload and engaging transport processes in the brain’s blood vessels. They do not all cross the blood–brain barrier (BBB), and detecting a particle near or within brain tissue does not by itself prove that an active drug reached its target or helped a patient. The field includes multiple experimental strategies, with no universally best carrier or routine nanoparticle treatment for neurological disease established by the reviews discussed here.
Why the blood–brain barrier makes brain delivery difficult
The BBB is a selective interface formed largely by the cells lining brain blood vessels, together with supporting cells and surrounding structures. Its protective function is essential: it helps regulate what moves from blood into the central nervous system. That selectivity also limits access for many medicines, particularly large biologic molecules. A drug that works elsewhere in the body may therefore fail to reach an effective concentration in the brain.

Nanoparticle delivery is an attempt to solve part of this access problem. A carrier can package or associate with a therapeutic payload, and its surface can be designed to interact with features of the brain endothelium—the cells that line the vessels. The intended result might be transport through those cells, release of the payload on the brain side, or a combination of these steps. Each must work well enough for the relevant disease and target.

“Nanoparticle” describes a broad family of carrier systems, not a single material or biological behavior. A particle’s composition, surface, payload and release properties all matter. The fact that a substance is nanoscale, or belongs to a carrier class used in brain-delivery studies, does not establish that it crosses an intact BBB.

How receptor-mediated transport is intended to work
One prominent strategy is receptor-mediated transcytosis. In this process, a ligand—a molecule chosen to bind a cell-surface receptor—is attached to or displayed by a nanoparticle. If the ligand binds a suitable receptor on a brain endothelial cell, the cell may internalize the particle into a vesicle. The intended pathway carries that vesicle across the cell, followed by release on the brain-facing side.
- Associate a payload with a carrier. The therapeutic may be packaged within the particle, attached to it, or otherwise carried by the formulation. The design must account for whether the payload remains active and how it will become available at the destination.
- Choose a surface ligand and receptor. Reviews discuss the transferrin receptor, low-density lipoprotein receptor-related protein 1 (LRP-1), and insulin receptor as candidate targets on brain endothelial cells.
- Bind and enter the endothelial cell. Receptor binding and cellular uptake are early steps, not proof that the particle has crossed the cell. A formulation can be taken up yet fail to be transported to the other side.
- Move through the cell and release appropriately. The carrier or its payload must reach the brain-facing side in a useful form. Depending on the design, the particle may remain intact during transit or the therapeutic may be released at a particular stage.
- Reach the relevant brain compartment and target. Delivery to an endothelial cell or a brain-associated sample is not equivalent to delivery into the brain parenchyma—the tissue beyond the vessel wall—or to uptake by the disease-relevant cells.
Receptor targeting is therefore a proposed route, not a guarantee. A useful formulation must balance transport with suitable payload release, distribution to the intended site, and tolerable exposure in other tissues. Strong binding alone is not the final objective; the desired outcome is active drug at the right target, with acceptable safety.
Other carrier designs and transport concepts
Receptor-mediated transcytosis is one option among several. Reviews also discuss adsorptive transport and transporter-related approaches. In broad terms, these strategies attempt to use interactions with the endothelial surface or transport biology rather than relying on a particle to passively slip through the barrier. The exact design and evidence differ between systems, so these labels should not be treated as interchangeable mechanisms.

Carrier composition is another design axis. Reviews cover liposomes, polymeric particles, lipid-based systems, micelles, dendrimers and inorganic carriers. Some categories overlap in how a formulation is built or modified. None is intrinsically a “brain-crossing” material: the material alone does not establish successful transport, meaningful brain-parenchyma distribution, clinical benefit or safety.
Surface engineering can alter how a carrier interacts with cells, while particle size, charge, ligand density, payload chemistry and release behavior are among the design choices that may affect performance. These features are not independent: changing one can affect other steps in delivery. The available reviews do not establish a head-to-head winner across these choices, and a formulation useful for one payload or disease context may not translate to another.
Do not confuse crossing the BBB with opening or bypassing it
Not every brain-delivery method works by a nanoparticle traversing an intact barrier. Some approaches physically increase barrier permeability; others use routes that bypass the BBB. Those methods may be discussed alongside nanocarriers because they address the same delivery problem, but the mechanism is different.

A useful distinction is whether a strategy uses an endogenous transport route, changes barrier permeability, or bypasses the barrier. In the first case, the carrier is intended to engage cellular transport. In the second, delivery depends on an induced change in the barrier. In the third, the therapeutic reaches the central nervous system by another route. Calling all of these “nanoparticles crossing the BBB” obscures what was actually done and what safety questions apply.
How to judge a claimed brain-delivery result
“Reached the brain” can refer to different measurements, and they do not all establish the same thing. A reported signal associated with brain tissue may reflect material in brain vessels or endothelial cells rather than in the parenchyma. Even parenchymal presence does not prove that enough intact, active payload reached the relevant cells to produce an effect.
When evaluating a study or a proposed therapy, ask what was measured and at what level:

- In vitro transport: Did a formulation move across a cell-based barrier model? This can test a mechanism, but a model is not a person or a complete brain vasculature.
- Animal biodistribution: Where was the carrier or payload detected in an animal? The species, disease model, barrier state, measurement method and timing affect how the result should be interpreted.
- Parenchymal distribution and target-cell access: Was the signal distinguished from material remaining in blood vessels or endothelial cells? Did it reach the intended brain region or cell type?
- Functional efficacy: Did the delivered therapeutic produce a meaningful effect in the relevant disease model, rather than only a transport signal?
- Human evidence and regulatory status: Has the formulation been tested in people for the intended indication, and what has a regulator authorized in the relevant jurisdiction? Preclinical promise is not the same as a clinical treatment.
- Safety and selectivity: What is known about effects on the endothelium, distribution outside the brain, immune responses and repeated dosing?
This framework matters because the delivery chain has several possible failure points. A particle might bind a receptor but be retained in the endothelial cell; cross a model barrier but not distribute through brain tissue; carry a payload that is released too early or remains unavailable; or reach the intended site without producing enough therapeutic effect. Evidence for one link should not be presented as proof of the whole chain.

| Carrier category | What the category identifies | What it does not establish |
|---|---|---|
| Liposomes | A lipid-based carrier class reviewed for drug delivery. | That a particular liposome crosses the BBB, reaches brain parenchyma or benefits patients. |
| Polymeric particles | Carriers built around polymer materials; included among traditional systems in reviews. | That polymer composition alone determines transport, release or therapeutic success. |
| Other lipid systems and micelles | Additional formulation families surveyed in reviews of CNS delivery. | That a formulation in either family has proven efficacy or a universal advantage over other carriers. |
| Dendrimers and inorganic carriers | Other broad carrier categories discussed in the literature. | That the category is suitable for a particular payload, disease or patient, or has established clinical value. |
This comparison is intentionally about what can be inferred from a category name. The reviews do not provide a single, standardized head-to-head dataset that would support ranking these classes for brain delivery. A meaningful comparison needs to specify formulation, ligand and surface design, therapeutic payload, disease context, model, measured brain compartment, efficacy evidence and safety.
Why translation into treatment remains difficult
Getting a carrier into or through endothelial cells is only one part of developing a useful therapy. The payload must arrive in an adequate active amount, remain effective, reach the relevant site, and produce a meaningful benefit. CNS physiology and disease can affect delivery, while results in preclinical systems do not automatically generalize to people. Safety and manufacturing requirements also matter alongside transport performance.
Best Value
A 2016 review provides historical context: at that time, it described relatively few preclinical in vivo studies assessing efficacy against brain disease with nanocarrier delivery. That observation is not a current count, but it illustrates why evidence of a transport mechanism should not be mistaken for an established treatment.
A 2026 review in the Journal of Controlled Release, titled “Clinical translation of nanomedicines for brain diseases: Current challenges and future directions,” reports that “over 50 nanomedicines have received regulatory approval for the treatment of various diseases, with more than 100 currently undergoing clinical trials.” Those figures concern nanomedicines overall; they are not counts of products that cross the BBB or treat brain disease. The review’s graphical abstract states that NanoTherm was the only nanomedicine with European Medicines Agency (EMA) approval for brain tumors among the systems it discusses. That is a scoped, secondary-source statement, not a verified exhaustive inventory of current regulatory authorizations.
Accordingly, these figures should not be used to imply that nanoparticle-based BBB delivery is routine clinical care. A complete, current list of authorized or trial-stage nanoparticle systems by indication and jurisdiction is not established here, and a regulator-specific claim should be checked against the relevant primary records before being treated as definitive.

Nanoparticles can be designed to carry drugs and interact with brain endothelial transport processes, with receptor-mediated transcytosis among the best-known strategies in reviews. Other carrier materials and transport concepts are being explored, but no carrier class guarantees passage through an intact BBB. The decisive question is not simply whether a particle was detected near the brain: it is whether an active payload reached the intended brain compartment, showed useful efficacy, and did so with acceptable safety. The reviews describe an active field with substantial challenges between laboratory delivery results and dependable treatment.
