How Multimodal Decision Fusion Combines Pathology and MRI Predictions

Decision fusion combines predictions made by separate pathology and MRI models at the output stage. A whole-slide-image model and a radiology model each analyze the data they are designed for, then an aggregation rule or learned combiner joins their scores into one patient-level prediction. The models remain modality-specific, while the final decision uses both streams.

This is also called late fusion or decision-level fusion. It can be easier to build and maintain than a single model that ingests raw slides and images together, but it may miss relationships that only become visible when pathology and MRI features are learned jointly. Whether it improves performance depends on the disease task, cohorts, model design, data pairing, calibration and handling of missing scans or slides.

What decision-level fusion does

Pathology and MRI describe different aspects of a patient. A pathology whole-slide image (WSI) can contain cellular and tissue architecture, while multiparametric MRI captures anatomy, water content, perfusion and other imaging signals. Their data formats, resolutions and sources are so different that one shared input pipeline is usually inappropriate.

In a decision-fusion system, each modality first passes through its own specialist model. The pathology model might output probabilities for molecular subtypes; the MRI model produces a corresponding set of scores. A decision-stage operation then combines those outputs into a joint prediction. The operation can be a fixed aggregation rule or an additional model trained to learn how much each output should influence the result. The exact operator is implementation-specific.

The patient-level flow

  1. Prepare the inputs. A digitized pathology WSI is quality-checked and divided into tiles or other model-ready regions. MRI sequences are selected, registered and normalized according to the imaging pipeline.
  2. Run modality experts. The pathology and MRI models process their own inputs independently. They may be foundation models used as fixed feature embedders, fine-tuned networks or conventional classifiers.
  3. Produce comparable outputs. Each expert returns a class score, probability distribution, embedding-derived score or another decision output for the same endpoint.
  4. Combine decisions. A fusion layer receives the outputs and generates one multimodal prediction for the patient. A separate calibration step may be applied so that the final probability reflects observed outcome frequencies.
  5. Evaluate the joint result. Performance is measured on data that were not used to fit the experts or combiner, with the cohort, split and metric stated explicitly.

“Patient-matched” means the slide and MRI belong to the same individual. Unpaired examples may share a label without supplying both modalities from one patient; that distinction affects what a fusion model can learn and how it can be evaluated.

How early, intermediate and late fusion differ

Fusion stage determines where information from pathology and MRI is allowed to interact. The three labels describe a design pattern, not one fixed neural-network architecture.

Fusion stage Where the combination occurs Potential strength Important limitation
Early (input-level) Raw inputs or very early features are joined near the model input. Can expose low-level relationships from both modalities from the beginning. Requires compatible preprocessing, alignment and often a much more complex input pipeline.
Intermediate (representation-level) Each modality is encoded first; learned representations are exchanged or combined before the prediction head. Can learn cross-modal dependencies while retaining modality-specific encoders. More parameters and training choices create additional opportunities for overfitting and modality imbalance.
Late (decision-level) Separate models produce decisions, which are joined at the output stage. Highly modular; each expert can use an architecture suited to slides or MRI and can often be replaced independently. Interactions that are not present in the final scores may be unrecoverable.

Late fusion is therefore not automatically superior or inferior. It is attractive when organizations already have validated modality-specific models, when modalities arrive through separate workflows, or when engineering simplicity matters. An intermediate or early design may be preferable when the clinical signal depends on a detailed relationship between a microscopic pattern and a spatial imaging feature.

The investigators used modality-specific foundation models as fixed feature embedders and compared three multimodal strategies: late fusion (MM-LF), early fusion (MM-EF) and a mixture-of-experts model (MM-MoE), including linear and sequence-based expert variants. The training data contained 772 pathology cases and 959 MRI scans. The independent test set contained 171 unseen, patient-matched cases.

Reported performance

The best mixture-of-experts model reached an AUC of 0.98 on validation and 0.94 on the independent test set. On held-out TCGA cases, the same general MM-MoE approach achieved 85% overall accuracy. The reported Matthews correlation coefficient (MCC) values on that test set were 0.73 ± 0.05 for MM-MoE, 0.70 ± 0.07 for MM-LF and 0.71 ± 0.02 for MM-EF.

Result Value What it describes
MM-MoE validation AUC 0.98 Ranking discrimination on the study’s validation cohort
MM-MoE independent-test AUC 0.94 Ranking discrimination on 171 unseen patient-matched cases
MM-MoE held-out TCGA accuracy 85% Share of TCGA test predictions classified correctly
MM-MoE held-out TCGA MCC 0.73 ± 0.05 Correlation between predicted and true classes, accounting for all confusion-matrix cells
MM-LF held-out TCGA MCC 0.70 ± 0.07 Late-fusion result on the same reported test comparison
MM-EF held-out TCGA MCC 0.71 ± 0.02 Early-fusion result on the same reported test comparison

AUC, accuracy and MCC are not interchangeable. AUC evaluates how well predictions rank positive and negative examples across thresholds. Accuracy depends on a chosen classification rule and class balance. MCC summarizes the full confusion matrix and is useful when classes are uneven. The 85% accuracy figure must not be presented as an AUC or MCC.

The authors reported that multimodal models consistently outperformed their unimodal counterparts in their evaluation. However, pathology-only performance exceeded MRI-only performance in the TCGA comparison. That result shows why adding a modality does not guarantee an equal contribution from both sources; the value of MRI is task- and cohort-dependent.

What the pairing analysis means

In this particular training setup, randomly pairing labels was nearly as effective as patient-matched training, and fully unpaired training was broadly comparable for early fusion and mixture-of-experts models. Late fusion was an exception because its joint output head could not be trained in the same unpaired manner. These findings are evidence from one glioma study, not a general rule that paired records are unnecessary. For a new clinical application, the pairing strategy must match the intended inference workflow and be tested separately.

The authors used attention analyses and SHAP attributions to inspect which patterns influenced predictions. Such maps can reveal model behavior and possible artifacts, but they are not causal explanations and do not prove that the highlighted tissue or imaging region is biologically responsible for the diagnosis. The study also notes that one commonly confused subtype pairing was absent from its dataset, making external validation and coverage of difficult populations essential.

A second example: breast-cancer treatment response

A separate retrospective, cross-institutional study evaluated an attention-based multimodal fusion transformer called MuFi for predicting response to neoadjuvant therapy in breast cancer. It included 567 biopsy-confirmed patients from two institutions and combined pathology slides, MRI and clinical data.

The abstract reports AUCs of 81.9% in discovery, 78.5% in validation and 79.3% in external testing. In that analysis, the multimodal model outperformed clinical-only, single-modality and late-fusion comparisons. These numbers belong to a different endpoint, disease and study design from the glioma work; they should not be compared as if one percentage ranked the two models directly.

How to design a pathology–MRI decision-fusion system

1. Define the decision before choosing the fusion layer

Specify the endpoint, prediction time and action that the output is meant to support. Molecular subtype, treatment response, recurrence risk and survival are different targets with different labels, censoring issues and acceptable errors. A model that changes treatment requires stronger evidence than one used for exploratory case review.

2. Build and audit each modality expert

Validate the pathology and MRI models independently before combining them. Document slide scanners, staining protocols, MRI vendors, sequences, preprocessing, resolution and exclusion rules. Check whether one expert is effectively making all predictions because the other stream is noisy or missing. Preserve patient-level separation when creating training, validation and test sets so tiles or repeated scans from one person cannot leak across splits.

3. Choose the fusion stage to match the data

Use late fusion when modularity, independent architectures or staged deployment are priorities. Consider intermediate fusion when useful cross-modal relationships are expected but raw-input alignment is impractical. Early fusion demands the strongest harmonization and is sensitive to missing channels and registration errors.

4. Decide how outputs will be combined

A simple aggregation rule is transparent and can be a useful baseline. A learned combiner can adapt to evidence strength, but it needs its own training data and regularization. Train and calibrate the combiner without allowing test patients to influence its parameters. Report the operator, input type (scores, probabilities or representations), threshold policy and handling of ties or low-confidence outputs.

5. Plan for missing modalities

Real workflows may have a pathology slide without usable MRI, or MRI without tissue. Decide whether inference is allowed with one modality, whether a fallback unimodal model is used, or whether the case is rejected. Evaluate each pathway separately. A fusion model trained only on complete, perfectly paired cases may fail when scans are delayed, sequences are absent or slides are technically inadequate.

6. Test generalization, not just an internal benchmark

Use an independent or external cohort with different patients and, where possible, different institutions, scanners, staining processes and referral patterns. Report confidence intervals or variability where available, class-specific errors and calibration. A high AUC on a curated split does not establish that predictions remain reliable in routine practice.

7. Measure clinical usefulness

Ask whether the joint prediction changes a real decision safely. Decision-curve analysis, prospective silent deployment, reader studies and assessment of treatment-relevant errors may be more informative than a small benchmark gain. Examine whether false positives delay or intensify treatment, and whether false negatives withhold a useful option.

Common failure modes and safeguards

  • Data leakage: tiles from the same WSI or serial MRI studies appear in different splits. Split by patient before any tiling or augmentation.
  • Site shortcuts: the model learns scanner, stain or institution signatures instead of disease biology. Use site-diverse validation and inspect errors by site.
  • Uncalibrated probabilities: a score of 0.9 is treated as a 90% risk without calibration testing. Assess reliability and recalibrate on an appropriate held-out set.
  • Hidden modality dominance: one expert overwhelms the other. Compare unimodal and multimodal results and perform ablations that remove each stream.
  • Incomplete coverage: a subtype or difficult confusion pair is missing from development data. Seek representative external cases before making broad claims.
  • Overinterpreted explanations: attention or SHAP maps are presented as proof of mechanism. Treat them as inspection aids and corroborate findings with pathology and radiology expertise.
  • Metric confusion: AUC, accuracy and MCC are reported without cohort or threshold context. Name the metric, split, population and calculation whenever quoting a result.

What the evidence supports

Pathology–MRI decision fusion is a practical way to combine modality-specific predictions: experts analyze their own inputs, and a final layer joins their outputs. The 2026 glioma study provides encouraging evidence that multimodal systems can outperform single-modality models in a retrospective evaluation, while its comparisons also show that early, late and mixture-of-experts designs have different trade-offs. The breast-cancer example demonstrates the same broad idea in another endpoint.

None of these results establishes prospective clinical benefit, regulatory approval or a universally best fusion architecture. A credible deployment claim requires independent validation, robust missing-data behavior, calibrated probabilities and proof that the prediction improves a clinical decision rather than merely raising a benchmark score.
