Dengue, West Nile and chikungunya can all begin with fever and other overlapping symptoms, but their incubation periods, warning signs and risks are different. A mosquito bite alone does not establish an infection, and symptoms cannot reliably identify the disease without a clinician’s assessment.
Contact a healthcare provider about fever or other symptoms after living in or recently traveling to an area where dengue or another mosquito-borne infection occurs. Seek urgent medical attention for dengue warning signs such as severe abdominal pain, repeated vomiting or bleeding, and for possible West Nile illness with high fever, neck stiffness, weakness, confusion or tremors. U.S. Centers for Disease Control and Prevention (CDC) guidance is written for U.S. public-health practice; people elsewhere should also check their local health authority.

How mosquito-borne diseases spread
A mosquito becomes infectious after acquiring a germ from an infected person or animal. The germ can multiply or develop inside the mosquito. During a later bite, saliva and the pathogen enter the next host. The mosquito is therefore a vector: it carries the germ between hosts rather than becoming sick in the same way a human does.
The usual route for dengue, West Nile and chikungunya is the bite of an infected mosquito. Dengue is commonly transmitted by infected Aedes mosquitoes, which may bite during the day as well as at night. West Nile and chikungunya also require an infected mosquito for their usual transmission. Most mosquitoes do not carry these pathogens, and an ordinary bite is not proof that transmission occurred.

Mosquito bites are the main route, not necessarily the only possible route for every pathogen. CDC describes uncommon pregnancy-related and healthcare or laboratory routes for dengue. West Nile virus has also been associated with rare blood-transfusion or organ-transplant transmission. These exceptions do not change the practical rule that mosquito-bite prevention is the primary way to reduce everyday exposure, but they are important when a clinician is taking a history.
What happens after an infectious bite
The incubation period is disease-specific
After exposure, a person may have no symptoms for days. The interval between infection and symptoms is not interchangeable among diseases:
| Infection | Usual symptom onset | Common clinical pattern |
|---|---|---|
| Dengue | Usually within two weeks of infection | Fever, body aches, nausea or vomiting, and sometimes a rash |
| West Nile | Usually 2–6 days; reported range 2–14 days | No symptoms in most infections; otherwise mild flu-like illness or, rarely, neurologic disease |
| Chikungunya | Commonly 3–7 days | Fever with prominent joint pain |
These ranges are guideposts rather than a personal diagnosis. The date of a bite may be unknown, mosquitoes can bite without being noticed, and a person may have had more than one exposure. A clinician interprets timing together with travel, local disease activity, symptoms, age, immune status and other medical conditions.

Symptoms can overlap
Fever, fatigue, headache, muscle aches, nausea and rash can occur in several mosquito-borne infections and in many non-mosquito illnesses. The overlap is why a “mosquito-bite fever” label is unsafe. Joint pain that is especially prominent can occur with chikungunya; dengue often combines fever with aches, nausea or vomiting, or rash; West Nile may cause no illness at all or a flu-like syndrome. Neurologic symptoms raise concern for severe West Nile disease but are not a substitute for examination or testing.

How dengue, West Nile and chikungunya differ
The following comparison is limited to the features established in current CDC material and is intended to help organize questions for a healthcare professional, not to identify an infection at home.
| Condition | Usual transmission | Typical onset | Symptoms that commonly point clinicians toward it | Important severe-illness clues |
|---|---|---|---|---|
| Dengue | Usually an infected Aedes mosquito bite; uncommon pregnancy-related and healthcare or laboratory routes are documented | Within two weeks | Fever, aches, nausea or vomiting, rash | Abdominal pain or tenderness, repeated vomiting, bleeding, blood in vomit or stool, extreme tiredness or restlessness |
| West Nile | Usually an infected mosquito bite; rare blood or organ-related routes are documented | Usually 2–6 days (range 2–14) | No symptoms in most infections; mild flu-like illness in some | High fever, neck stiffness, muscle weakness, confusion or tremors can signal neurologic disease |
| Chikungunya | Usually an infected mosquito bite | Usually 3–7 days | Fever and often marked joint pain | Newborns infected around birth, older adults and people with some chronic conditions have higher risk of severe illness |
What the published estimates mean
- For West Nile virus, CDC estimates that about 80% of infected people do not develop symptoms, about 20% develop mild illness, and less than 1% develop severe illness. Those percentages describe West Nile infections, not mosquito-borne diseases as a group.
- People age 65 and older are reported by CDC to be three times as likely to develop neurologic West Nile illness as people younger than 65.
- For dengue, CDC estimates that about one in four infected people become sick, and about one in 20 people who become sick develop severe dengue. CDC also estimates that up to 400 million people are infected with a dengue virus each year. These are population estimates published in 2025, not a prediction for an individual bite.
When to seek medical care after a bite, fever or travel
Contact a healthcare provider for possible dengue
Call or arrange medical advice if you develop a fever or other symptoms after living in or recently traveling to a place with dengue risk. Tell the provider exactly where you were, when you returned, when symptoms began and whether you noticed mosquito exposure. Travel information can change which tests and diagnoses are considered.
Dengue can worsen as the fever goes away. CDC warning signs may begin 24–48 hours after fever resolves, so improvement in temperature is not always the end of the risk period. Go immediately to a local clinic or emergency department for any of these signs:
- Abdominal pain or tenderness.
- Vomiting at least three times in 24 hours or repeated vomiting that prevents normal fluid intake.
- Nose or gum bleeding.
- Blood in vomit or stool.
- Extreme tiredness or unusual restlessness.
Do not wait for every warning sign to appear. A person with a recent dengue-risk exposure and a rapidly worsening illness needs prompt professional evaluation.

Get immediate attention for possible neurologic West Nile disease
Seek immediate medical attention for high fever, neck stiffness, muscle weakness, confusion or tremors after a possible mosquito exposure. These symptoms can indicate neurologic involvement, which requires urgent assessment. Tell the clinician where and when exposure may have occurred; testing and evaluation depend on the symptoms and the exposure history.
When the situation is unclear
If symptoms are mild but persistent, or if you are unsure whether a trip or residence involved dengue or West Nile risk, contact a healthcare provider rather than trying to match symptoms to an online checklist. A bite can be unnoticed, and other infections can resemble mosquito-borne disease. If you become severely ill or cannot safely reach a clinic, use your local emergency-service system.
What information to prepare for the clinician
A concise exposure history helps a healthcare professional decide whether testing or monitoring is appropriate. Write down:

How to prevent mosquito-borne infections
Use repellent correctly
Choose an EPA-registered mosquito repellent and follow its product label, including directions for reapplication and use on children. Repellent lowers the chance of a bite; it does not treat an infection, remove a pathogen already transmitted, or guarantee complete protection.
Cover exposed skin
Wear loose-fitting long-sleeved shirts and long pants when mosquitoes are active. Clothing that covers more skin reduces the number of places a mosquito can bite. This is useful both outdoors and in areas where mosquitoes can enter living spaces.
Reduce mosquitoes around the home
Use practical mosquito-control measures around the home, following local public-health advice. Preventing bites is particularly important for dengue because the mosquitoes that commonly transmit it can bite in daylight as well as at night. Continue protection even when no one has a visible bite or when local activity seems low.
Protect others when illness is suspected
Ask a healthcare provider or local health authority whether additional precautions are appropriate for your illness and location. Local mosquito species, disease activity, travel advisories and available prevention measures can change. CDC guidance is U.S.-specific; readers in other countries should consult their own public-health authority for current recommendations.

Who may need extra caution
Risk is not uniform across diseases. Older age, immune status and chronic medical conditions influence the chance of severe West Nile disease. CDC identifies people age 65 and older as having a substantially higher risk of neurologic West Nile illness than younger adults. For chikungunya, newborns infected around birth, older adults and people with some chronic conditions are at higher risk of severe illness. These risk groups should not be automatically applied to dengue or to every mosquito-borne infection.
Pregnancy can change the significance of a suspected infection and is one reason to report travel and symptoms promptly. Dengue also has documented uncommon pregnancy-related transmission routes. A pregnant person, a newborn, an older adult or anyone with immune or chronic health conditions should seek individualized advice rather than relying on general symptom timelines.
Other mosquito-borne diseases and the limits of this overview
CDC lists malaria and Zika among mosquito-borne diseases, along with dengue, West Nile and chikungunya. The disease-specific symptom timelines and care instructions above address only the three conditions for which detailed guidance is presented here. Malaria and Zika require their own current clinical and travel guidance; fever after travel should be reported with the full itinerary instead of being assumed to be dengue, West Nile or chikungunya.
Local transmission patterns, travel advisories, testing practices and vaccine availability can change. This overview cannot diagnose an individual or replace a clinician’s evaluation. The safest approach is to prevent bites, record travel and symptom timing, and obtain medical advice promptly when fever follows exposure. Use urgent care for the listed dengue and neurologic warning signs, even if the fever has recently improved.
A practical decision checklist
- Record the exposure: Note where you were, the dates, and whether mosquito bites were possible. Include travel outside your usual area.
- Track the timeline: Write down the first day of fever or other symptoms. Remember that West Nile commonly begins 2–6 days after exposure, chikungunya 3–7 days, and dengue within two weeks, but these ranges do not diagnose a case.
- Check for urgent signs: Look for dengue warning signs, including abdominal pain, repeated vomiting, bleeding, blood in vomit or stool, or extreme tiredness/restlessness. For possible West Nile, watch for high fever, neck stiffness, weakness, confusion or tremors.
- Call the right level of care: Contact a healthcare provider about fever after residence or travel in a dengue-risk area. Go immediately to a clinic or emergency department for the warning signs above.
- Prevent additional bites: Use labeled repellent, cover skin with loose clothing and reduce mosquitoes around the home while you follow professional advice.
Mosquito-borne disease is a group of infections, not one illness with one timetable. Knowing the usual transmission route, keeping disease-specific incubation periods separate and recognizing the warning signs that require urgent evaluation are the most useful steps a reader can take.
