You may notice clearer vision after the first night of orthokeratology (Ortho-K), but the effect usually takes several nights to build and may need about a week—or sometimes longer—to settle. The reshaping is temporary: you need to keep wearing the lenses on the schedule your eye-care professional prescribes to maintain it. How long vision stays clear during the day, and how quickly it changes if you stop, varies by person.
When does Ortho-K start improving vision?
Some people notice a change the morning after their first night wearing Ortho-K lenses. That early improvement is not necessarily the finished result. In a small 2003 study of 10 people using specific reverse-geometry lens designs, visual and corneal measurements reached their maximum by the end of one week. A 2006 review describes most change as occurring after the first night, with refractive stability often reached within seven to ten days. These are study-based expectations, not a timetable guaranteed for every wearer.

Your response depends in part on your starting prescription, corneal characteristics and lens fit. The FDA notes that results can differ substantially from study averages, and that clinical studies have not established reliable methods for predicting who will achieve the greatest corneal flattening. An individual fitting and follow-up are therefore more useful than assuming a particular number of nights will be enough.

How long does the effect last during the day?
Ortho-K temporarily changes the shape of the cornea while you sleep. The resulting daytime vision does not have a universal duration: the effect can change over the course of the day, and the amount of clear vision varies among wearers. FDA study documentation provides study-specific estimates of regression as guidance, not as a promise of a set number of hours for an individual.

Use your own vision and your clinician’s advice to judge whether the correction is meeting your needs. Do not assume that every wearer will have clear vision for exactly 24 hours or that a particular daytime schedule applies to you.
Do you have to keep wearing the lenses?
Yes. The corneal reshaping is temporary, so maintenance requires continued overnight wear according to your prescribed schedule. The FDA’s EUCLID system approval record says overnight lens wear must continue on a prescribed schedule to maintain the myopia-reduction effect. That labeling applies to the named system; device indications and instructions can differ, so follow the directions for your own lenses.

What happens if you stop wearing Ortho-K?
The correction regresses after lens wear stops, but the available evidence does not establish a universal number of days for vision to return fully to baseline. In one 2004 study of low myopes, central corneal thickness recovered after one night without lens wear following one month of treatment. That narrow finding concerns corneal thickness in a particular group; it is not a general estimate of how long vision takes to return to its original state, especially after longer-term wear.

Ask your eye-care professional how to manage a planned pause or a missed night. A short-term study result should not be used to predict your own recovery timeline.

Why can results differ, and what should you consider about safety?
Ortho-K uses specially designed rigid contact lenses worn during sleep. The FDA’s device-specific documentation describes possible blurry distance vision, flare or ghosting, corneal swelling or staining, irritation, abrasion and vision distortion. It also warns that rare permanent corneal scarring and decreased vision can occur; overnight lens wear carries a greater risk of serious problems such as corneal ulcers and vision loss. These warnings and study details relate to specific device documentation, not necessarily every lens system in identical terms.
Lens fit, starting myopia and individual eye response all affect the outcome. The FDA says these lenses should be fitted by a contact lens fitter trained and certified in the relevant fitting methods. Consult an appropriately trained eye-care professional about candidacy, fitting, hygiene and follow-up, and seek their advice promptly if you develop concerning symptoms or a sudden change in vision.
