How Long Do Fever and Other Common Vaccine Side Effects Last?

Most vaccine reactions are mild and short-lived. Fever or a general “unwell” feeling usually improves within a day or two, while soreness, redness, or swelling where the needle went in commonly settles within several days. The exact pattern depends on the vaccine, the person’s age, and the dose.

Some vaccines can cause symptoms later than expected. For example, fever after the measles component of MMRV may begin six to 10 days after vaccination in some children. This guide explains normal time courses, how to monitor symptoms, and when to seek medical help. It is general information, not a diagnosis.

Typical timelines for common reactions

The following ranges are general expectations rather than guarantees. A symptom that lasts longer, gets worse, or appears with other concerning signs deserves medical advice.

Reaction Typical timing What the guidance means
Fever or feeling unwell Often 1–2 days The NHS gives this as a general estimate; individual responses and vaccines vary. NHS guidance
Sore, red, or swollen injection site Often 2–3 days This is the general NHS estimate and is also used in England guidance for babies and children up to age five. England guidance
COVID-19 vaccine symptoms Usually start 1–3 days after vaccination and resolve within 1–3 days The CDC notes that frequency and severity vary by age, manufacturer, and dose. CDC COVID-19 vaccine safety guidance
Fever after MMRV in some children May begin 6–10 days after vaccination England’s Department of Health and Social Care says the measles component can cause this delayed fever in about one in 10 children. England guidance

The CDC’s broader guidance for vaccines licensed in the United States similarly describes most reactions as minor and resolving within a few days. CDC, Possible Side Effects from Vaccines

How fever and local soreness differ

Fever and feeling unwell

A raised temperature, tiredness, headache, or a generally unwell feeling is a whole-body response. For routine vaccines, these symptoms usually follow the short course shown in the table. Watch the person’s overall condition, not only the number on the thermometer: alertness, drinking, breathing, and whether symptoms are improving are all relevant to deciding whether to call a clinician.

Soreness, redness, and swelling

Pain or tenderness around the injection site can make an arm or leg uncomfortable when it is moved. Mild redness or swelling can accompany the soreness. These local effects usually fade over a few days. If the area is rapidly worsening, unusually severe, or remains a concern after the expected period, contact a healthcare professional rather than assuming it is a normal vaccine reaction.

Why some vaccines cause delayed symptoms

COVID-19 vaccines

For COVID-19 vaccines, the CDC reports that most symptoms begin one to three days after a dose and resolve one to three days later. The pattern is not identical for everyone: age, the vaccine manufacturer, and whether the dose is part of a series can affect how often reactions occur and how strongly they are felt. Read the CDC safety considerations.

MMRV

MMRV (measles, mumps, rubella, and varicella) is a documented example of a vaccine with a later fever window. England’s child-vaccination guidance says the measles component may trigger fever beginning six to 10 days after immunisation in about one in 10 children. A fever in that window can therefore be compatible with a vaccine reaction, but timing alone cannot prove that the vaccine caused it.

MenB given with infant vaccines

Fever is common when meningococcal B (MenB) vaccine is given with routine vaccines at two and three months in England. The Department of Health and Social Care specifically states that the height of the fever depends on the individual child and does not show how well the vaccine worked. A child who does not develop a fever has not therefore had an ineffective dose. See the England post-vaccination guidance.

How age, dose, and vaccine context affect expectations

There is no single “vaccine side-effect clock.” The same symptom can be more or less noticeable depending on age, the product used, and whether it is a first dose, a later dose, or part of a series. CDC COVID-19 guidance specifically notes differences by age, manufacturer, and dose. A clinic’s product information is therefore more useful than comparing your experience directly with another person’s.

Local reactions and whole-body symptoms should also be considered separately. A tender injection site that is improving on day three follows a different pattern from a new high fever on day three. Recording each symptom independently prevents one lingering sore spot from making an otherwise improving reaction seem more serious than it is.

How to monitor a possible vaccine reaction

  1. Record the timing. Note when the vaccine was given, when each symptom began, and whether it is getting better, stable, or worse. Delayed onset matters because some vaccines have a later expected window.
  2. Measure temperature accurately. For a child, use a thermometer rather than estimating by touch. England’s guidance specifically advises caregivers to check a child’s temperature with a thermometer. England guidance
  3. Assess the whole person. A temperature reading should be considered alongside hydration, alertness, breathing, pain, and behavior. A mild symptom that is steadily improving is different from one that is intensifying or accompanied by a marked change in condition.
  4. Use the vaccine information supplied at the appointment. Product-specific leaflets and local health services can provide age- and dose-specific instructions. Advice from the vaccinating clinic or a clinician takes priority over a general timeline.
  5. Keep the context. Exposure to a respiratory infection, stomach bug, or another illness can coincide with vaccination. Do not label every symptom after a shot as a vaccine side effect without considering other explanations.

A practical decision path

Start by asking when the symptom began and which vaccine was given. A mild fever or tiredness during the first one or two days, or a sore injection site during the first two or three days, is commonly monitored if the person otherwise seems well. Continue checking whether the symptom is trending down rather than focusing on one isolated reading.

If onset is delayed, compare it with the known schedule for that product. MMRV can produce fever six to 10 days after vaccination, whereas that timing should not automatically be applied to other vaccines. If the product is unknown, contact the vaccinating service or review the record rather than guessing.

Escalate when the pattern changes: symptoms that intensify, fail to improve, or occur with breathing difficulty, a seizure, marked drowsiness, dehydration, or another serious change need prompt professional assessment. Bring the vaccination date, product name if available, temperature readings, onset times, and a list of any medicines already taken.

When to seek medical advice

Get urgent help for severe or worsening symptoms

Contact a healthcare professional promptly for symptoms that are severe, worsening, or otherwise concerning, even if they began soon after vaccination. The expected ranges above are not a substitute for an assessment when someone looks seriously ill or is deteriorating.

England-specific advice for young children

The England Department of Health and Social Care guide applies to babies and children up to age five. It advises urgent medical help if a child has a fit (seizure) and says to call a doctor immediately when the child’s temperature reaches 39–40°C or above. These are instructions from that England guide, not universal thresholds for every country, age group, vaccine, or clinical situation. Check the full England guidance.

Possible serious allergic reaction

The NHS says serious allergic reactions to vaccination usually happen within minutes and that vaccinators are trained to treat them. That is one reason people are observed according to local vaccination procedures after an injection. If a serious allergic reaction is suspected at any time, seek emergency medical care immediately. NHS vaccination information

Does a fever prove that the vaccine worked?

No. Fever is an immune-system response, not a test of protection. England’s MenB information says explicitly: “The level of fever depends on the individual child and does not indicate how well the vaccine has worked.” The same principle means that no fever does not show that a vaccine failed. Protection is assessed from the vaccine schedule and evidence about the product, not from whether a person feels ill afterward.

Could the illness be unrelated to vaccination?

Yes. A health problem that occurs after a vaccine may have another cause, including an infection acquired around the same time. The CDC cautions that reports following vaccination and the timing of an event by themselves do not establish that the vaccine caused it. Clinicians consider the symptom pattern, timing, background illnesses, exposures, and examination findings when assessing causality. CDC, Preventing and Managing Adverse Reactions

This is particularly important for delayed fever. A later symptom can fall within a known vaccine window, as with MMRV, while still requiring attention to the child’s overall condition and other possible causes.

How to interpret the timeline in practice

Symptoms that start soon after the dose

A sore arm or a short-lived fever beginning soon after vaccination fits the common pattern described by public-health agencies. Track whether the symptom is easing over the following days. Persistence beyond the usual course, a sudden escalation, or a new serious symptom changes the decision from routine monitoring to medical advice.

Symptoms that start several days later

Do not dismiss a delayed symptom automatically, but do not assume it is vaccine-caused either. Check which vaccine was given, whether delayed reactions are documented for it, and whether anyone else in the household is ill. MMRV’s six-to-10-day fever window is a specific example; it should not be generalized to every vaccine.

What the official guidance covers

These timelines combine guidance from different health systems, so their scope matters. CDC pages cited here address vaccines licensed in the United States and draw on Vaccine Information Statements and Advisory Committee on Immunization Practices recommendations. NHS estimates are general UK guidance. The fever thresholds and child-specific advice come from a 2025 Department of Health and Social Care guide for England and children up to age five; they should not be presented as rules for every jurisdiction.

Across the sources, the consistent message is that most ordinary reactions are temporary, vaccine-specific schedules can create different onset windows, and severity or deterioration matters more than a calendar alone.

Frequently Asked Questions

How long does a fever last after a vaccine?

For many routine vaccinations, fever or feeling unwell improves within one to two days. COVID-19 vaccine symptoms commonly resolve within one to three days. Duration varies, so seek advice if fever is severe, worsening, or not trending better.

How many days is a sore arm normal after a shot?

Soreness, redness, or swelling at the injection site commonly lasts two to three days. A rapidly worsening area, unusually severe pain, or persistent concern should be assessed by a healthcare professional.

Does having a fever mean the vaccine worked?

No. Fever does not measure protection, and the absence of fever does not mean a dose failed. The MenB guidance states that fever height depends on the individual child and does not indicate how well the vaccine worked.

When is a child’s temperature after vaccination an emergency?

The England guide for babies and children up to age five advises calling a doctor immediately at 39–40°C or above and seeking urgent help if the child has a fit. These thresholds are England-specific; follow local medical advice elsewhere.

How can I tell a vaccine reaction from an unrelated illness?

Timing alone cannot establish cause. Consider the vaccine, symptom pattern, exposures, and whether other household members are ill. A clinician can assess persistent or concerning symptoms and decide whether another illness is more likely.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When do serious allergic reactions happen?

The NHS says serious allergic reactions usually occur within minutes of vaccination, and vaccinators are trained to treat them. Breathing difficulty, collapse, or other signs of a severe allergy require emergency care immediately.

What information should I give a clinician?

Provide the vaccination date and product if known, when each symptom started, temperature readings, how symptoms have changed, relevant exposures, and any medicines already taken. This helps distinguish an expected reaction from another problem.

The Bottom Line

Most fever and general symptoms after vaccination settle within one to two days; a sore, red, or swollen injection site commonly lasts two to three days. COVID-19 reactions usually begin and end within a few days, while MMRV can cause fever six to 10 days later. Monitor the whole person, use a thermometer for children, and seek prompt care for severe, worsening, allergic, seizure-related, or otherwise concerning symptoms.
