Bubonic plague usually begins 2–8 days after exposure. Pneumonic plague usually begins 1–3 days after inhaling infectious droplets, sometimes as soon as one day; a CDC fact sheet gives a reported range of 1–6 days. Septicemic plague is less precisely defined but is thought to develop within days. If you had close, sustained contact with a person who had pneumonic plague while they were coughing, or direct contact with infected body fluids or tissues, call a healthcare provider and your local or state health department promptly. They can determine whether seven days of preventive antibiotics are indicated. While you feel well, check your temperature twice daily for seven days and obtain immediate medical care if fever or other concerning symptoms appear.
How long after exposure do plague symptoms start?
The timing depends on the form of plague and how the bacteria entered the body. The three clinical forms are bubonic, pneumonic and septicemic plague. Flea bites and contact with infected animals are more commonly associated with bubonic or septicemic disease, while inhaling respiratory droplets from an infected person or animal can cause pneumonic disease.

| Form of plague | Typical time to symptoms | Important qualification | Early symptoms to know |
|---|---|---|---|
| Bubonic | Usually 2–8 days | CDC provider guidance, 2026; timing varies with the exposure route and dose. | Fever, headache, chills, weakness and one or more swollen, painful lymph nodes (buboes). |
| Pneumonic | Usually 1–3 days after inhalation | CDC describes illness beginning as soon as one day; its April 2026 fact sheet reports a typical range of 1–6 days. | Fever, weakness, headache, rapidly developing pneumonia, shortness of breath, chest pain, cough and sometimes bloody or watery mucus. |
| Septicemic | Probably within days, but the incubation period is poorly defined | It may occur as the first form of disease or after untreated bubonic plague. | Fever, chills, extreme weakness, abdominal pain, shock and bleeding into the skin or organs. |
These are clinical ranges, not a guarantee that a person is safe after a specific number of days. A clinician or public health official must interpret the timing alongside the source of exposure, whether respiratory protection was used, how long contact lasted and whether symptoms are present.

Why pneumonic plague requires the fastest response
Pneumonic plague affects the lungs and is the form that can spread from person to person through infectious respiratory droplets. The CDC describes a close contact as being within six feet of a patient with pneumonic plague while that patient was coughing. Risk is assessed more carefully than distance alone: sustained exposure, repeated contact, lack of personal protective equipment and contact with respiratory secretions can all matter.

Pneumonic illness can worsen quickly. A person with a compatible illness should not wait for a laboratory result, a longer symptom history or a public health callback before seeking treatment. CDC clinical guidance says suspected cases should be treated promptly, isolated and placed on droplet precautions, with immediate notification of local and state health departments.
Who may need preventive antibiotics?
Post-exposure prophylaxis is not automatically required for every person who was in the same building, town or event as a plague patient. CDC clinical-care guidance identifies known exposures for which prophylaxis may be indicated, including:
- Close, sustained contact (less than six feet) with a person or animal with pneumonic plague, particularly while the patient was coughing.
- Direct contact with infected body fluids or tissues.
For these known exposures, CDC recommends a seven-day course of preventive antibiotics. The specific medicine, dose and suitability depend on the individual’s age, pregnancy status, allergies, kidney function, other medicines and the local public health assessment. Do not use leftover antibiotics, borrow someone else’s prescription or wait to see whether symptoms develop before asking for advice. Preventive treatment is a clinical decision made with a healthcare professional and public health authorities.
Contact a healthcare provider and the local or state health department as soon as possible after a qualifying exposure. Tell them the date and location, the type of contact, whether the source was coughing, whether fluids or tissues were involved, what protective equipment was used and whether you have any symptoms. That information helps officials classify the exposure and decide whether prophylaxis and additional monitoring are needed.

What close contacts should do, step by step
1. Call for an exposure assessment
Use a healthcare provider, urgent-care service or emergency department as appropriate, and notify the local or state health department. Explain that the concern is possible plague exposure rather than simply saying you have a fever or cough. Public health officials can coordinate testing, prophylaxis, monitoring and instructions for avoiding further transmission.
2. Follow the seven-day monitoring period
CDC’s person-to-person transmission guidance advises close contacts who remain well to measure their temperature twice a day for seven days. Record the time and reading so that a clinician can see a trend. A digital thermometer is sufficient; use it consistently according to the manufacturer’s instructions. Monitoring does not replace a medical assessment or preventive antibiotics when those are indicated.
3. Watch for specific symptoms
During the monitoring period, pay attention to fever, chills, unusual weakness, headache, cough, shortness of breath, chest pain, swollen or painful lymph nodes, abdominal pain or unexplained bleeding. Pneumonic symptoms can progress rapidly, so breathing difficulty or a rapidly worsening cough is an emergency warning sign.
4. Seek immediate care if symptoms begin
Anyone with symptoms that could represent plague should obtain immediate medical attention. Tell the medical facility before arrival that plague exposure is possible, so staff can arrange appropriate infection-control precautions. Do not wait for symptoms to become severe, for a test result or for the seven-day observation period to end.

5. Follow isolation instructions if pneumonic plague is suspected
If a clinician suspects pneumonic plague, the patient should be isolated and placed on droplet precautions while evaluation and treatment proceed. Avoid close contact with household members and visitors until healthcare and public health professionals provide specific instructions. They will determine which contacts need monitoring or prophylaxis.
What if the contact was not close?
Some contact with a person who had plague does not necessarily meet the definition of a known high-risk exposure. For example, being in the same general area without close, sustained proximity to a coughing pneumonic patient may carry a different risk than sitting within six feet during repeated coughing. However, only a clinician or public health official can classify an individual event.
If you had some contact but not the close contact described above, monitor yourself for fever or cough. Notify a healthcare provider and your state or county health department if symptoms develop. Include the date of the possible exposure and any details you know about the source patient or animal. Do not assume that the absence of immediate symptoms rules out disease, because incubation varies by form.

Bubonic plague
Bubonic plague is characterized by fever and one or more enlarged, painful lymph nodes called buboes. The nodes may occur near the site where bacteria entered through a flea bite or break in the skin. Headache, chills and marked weakness commonly accompany the fever. Untreated bubonic infection can spread into the bloodstream and become septicemic.

Pneumonic plague
Pneumonic plague involves the lungs. Early features can include fever, headache and weakness followed by rapidly developing pneumonia, shortness of breath, chest pain and cough. Mucus may be watery or bloody. Because respiratory droplets can transmit the infection, suspected pneumonic cases require urgent clinical evaluation and infection-control measures.
Septicemic plague
Septicemic plague is infection of the bloodstream. It can occur first or follow untreated bubonic disease. Fever, chills, extreme weakness, abdominal pain, shock and bleeding into the skin or organs are possible. The exact incubation period is not well established, but it is thought to be within days of exposure. These symptoms require emergency care.

Plague is serious but treatable. CDC clinical guidance states that when plague is suspected, treatment should begin without waiting for laboratory confirmation. Testing can take time and may require specialized specimens, while untreated illness can progress quickly. Healthcare teams may collect samples and notify public health authorities while starting appropriate therapy and precautions.
The same principle applies to contacts: do not self-diagnose from an online symptom list, but do not dismiss a plausible exposure because symptoms are mild at first. Early communication gives clinicians the best chance to prevent illness or treat it before complications develop.

How exposure details change the recommendation
| Exposure situation | Recommended next action while well | What to do if symptoms appear |
|---|---|---|
| Within six feet of a person with pneumonic plague while they were coughing, with sustained contact | Contact a healthcare provider and local or state health department promptly; ask whether seven days of post-exposure antibiotics are indicated. Check temperature twice daily for seven days. | Seek immediate medical care and disclose the exposure before arrival. |
| Direct contact with infected body fluids or tissues | Promptly obtain a clinician and public health assessment; prophylaxis may be indicated. Monitor temperature as instructed. | Seek immediate care and report the fluid or tissue exposure. |
| Brief or distant contact without known respiratory or fluid exposure | Ask public health officials how the event should be classified. Watch for fever or cough. | Notify a healthcare provider and state or county health department immediately. |
| Flea or potentially infected animal exposure without person-to-person contact | Tell a clinician about the animal, bite, handling or outdoor exposure. Follow individualized monitoring advice. | Seek urgent evaluation for fever, painful swollen nodes, severe weakness or other compatible symptoms. |
The table is a practical guide, not a substitute for an exposure investigation. Timing, duration, protective equipment and the confirmed diagnosis of the source all affect the decision.
Reducing the chance of future exposure
For people who live, work or recreate where plague occurs naturally, prevention focuses on rodents, fleas and infected animals. CDC prevention guidance recommends reducing rodent habitat around homes and workplaces, avoiding contact with sick or dead wildlife, and wearing gloves when handling or skinning animals that could be infected. During camping, hiking or outdoor work where flea exposure is possible, use an insect repellent according to its label directions and wear clothing that limits exposed skin.
These measures are for future risk reduction. They do not replace a medical assessment, temperature monitoring or urgent care after a suspected exposure.
When to call emergency services
Call emergency services for severe shortness of breath, chest pain, confusion, collapse, signs of shock, uncontrolled bleeding or rapidly worsening illness. If you are stable but have fever, cough, painful swollen lymph nodes or profound weakness after a plausible exposure, seek same-day medical attention and identify the possible plague contact immediately.
Key dates and guidance behind the timing
- CDC provider diagnosis guidance dated October 7, 2026, describes the usual 2–8-day bubonic incubation and 1–3-day pneumonic incubation.
- CDC’s public symptom information, dated May 15, 2024, notes that pneumonic illness can begin as soon as one day after inhalation and advises immediate medical attention for possible symptoms.
- CDC clinical-care guidance dated May 15, 2024, describes seven days of prophylaxis for known exposures.
- CDC’s person-to-person transmission fact sheet, dated April 1, 2026, defines the close-contact example and recommends twice-daily temperature checks for seven days.
- CDC prevention guidance dated March 5, 2025, covers rodent control, protective gloves and repellent use for outdoor flea exposure.
If plague is suspected, local and state health departments should be notified immediately. That public health coordination is essential for arranging testing, treatment, contact tracing and protection of other people.
