What differs is how rarely the disease occurs in young people, the limited pediatric evidence, the tendency in reported cases to be diagnosed at an advanced stage, and the importance of pediatric cancer expertise. The individual plan depends on the tumor’s stage and location, its pathology and molecular features, inherited risk, overall health, and prior treatment.
Why treatment can differ from adult colorectal cancer care
Colorectal cancer is rare in children and teens, so the evidence base for their care is much smaller than it is for adults. The National Cancer Institute (NCI) says pediatric and adolescent cases are more likely to have mucinous or signet-ring features and to be diagnosed at an advanced stage. These are observations about groups, not predictions about an individual child. Adult treatment regimens and outcome statistics should not be applied directly to a child without guidance from a pediatric oncology team.

In most reports summarized by the NCI’s 2024 clinician PDQ, 80%–90% of pediatric patients presented with Dukes stage C/D or TNM stage III/IV disease. The same summary reports mucinous adenocarcinoma in 40%–50% of pediatric and adolescent cases, compared with about 15% of adult lesions. These figures describe the populations and reports cited by the NCI; they do not establish a particular child’s stage, tumor type, or outlook. See the NCI childhood colorectal cancer clinician summary and its adult colon cancer clinician summary.

| Issue | What the NCI describes for children and teens | What that means for families |
|---|---|---|
| Evidence and frequency | The disease is rare in young people, and pediatric evidence is more limited than adult evidence. | A plan should be considered by clinicians experienced in childhood and adolescent cancer, rather than assumed from adult practice. |
| Stage at diagnosis | Most reports summarized by the NCI found 80%–90% of pediatric patients had Dukes stage C/D or TNM stage III/IV disease. | The extent of spread is a key factor in whether a tumor can be removed and which additional treatments may be considered. |
| Tumor features | Pediatric and adolescent cases are more likely to have mucinous or signet-ring features; inherited syndromes and specific molecular findings can also affect options. | Pathology, tumor testing, and family history may inform the discussion; age alone does not establish eligibility for a treatment. |
| Tumor location | The NCI clinician summary describes radiation with chemotherapy for rectal and lower-colon tumors. | Whether radiation is considered depends in part on where the tumor is located, alongside other individual findings. |
| Specialist team and trials | The NCI recommends multidisciplinary care with childhood cancer experience and says a clinical trial may be an option for some children. | The team can weigh pediatric-specific expertise and check whether a study fits the child’s diagnosis and circumstances. |
The comparisons above reflect NCI summaries, not a single treatment rule. The NCI’s patient and clinician PDQs provide evidence information; they do not make individualized treatment recommendations.

What types of treatment may be used?
The options below are broad categories and examples described by the NCI, not a recommended regimen. A child may receive one or more approaches depending on the cancer and the treatment team’s assessment.
Surgery
When the cancer can be removed, complete surgical excision is a central goal. The NCI clinician summary identifies complete removal as an important prognostic factor, but says it may not be possible when disease has spread extensively. The surgeon and oncology team can explain what the tumor’s location and extent mean for the proposed operation.

Chemotherapy
Chemotherapy may be used in different combinations, including for advanced disease. The NCI patient summary lists capecitabine, fluorouracil (5-FU), irinotecan, and oxaliplatin among drugs used alone or in combination for children. The clinician summary describes fluorouracil and leucovorin with radiation for rectal and lower-colon tumors and notes that other agents, including irinotecan, may be useful. The drug names are examples, not a prescription or a complete plan.

Radiation is more location-specific in the NCI’s description: it is discussed with chemotherapy for rectal and lower-colon tumors. Whether it is appropriate for a particular child depends on the full clinical picture and should be addressed by the treating team.

Immunotherapy and tumor testing
The NCI patient summary lists nivolumab and pembrolizumab as immunotherapy medicines that may be used in selected cases, such as certain inherited cancer syndromes or tumors with specific gene changes. The clinician summary reports FDA accelerated approval for nivolumab in patients aged 12 years or older with MSI-H or mismatch-repair-deficient metastatic colorectal cancer that progressed after combination fluoropyrimidine, oxaliplatin, and irinotecan. That is a specific eligibility description reported in the NCI clinician summary, not a general option for every child; families should ask the oncologist to confirm current eligibility and labeling.
Inherited-risk management
For children with certain inherited syndromes, the NCI patient summary describes approaches that may include risk-reducing colon surgery before cancer forms or medicines to reduce colon polyps. These are prevention or risk-management considerations for selected inherited conditions, not routine treatment for every child diagnosed with colorectal cancer. A genetics professional and specialist team can explain whether an inherited risk is relevant.

Who should lead the care?
“A pediatric oncologist, a doctor who specializes in treating children with cancer, oversees treatment of colorectal cancer,” says the NCI’s patient summary. NCI describes a multidisciplinary team that may also include pediatricians, pediatric gastroenterologists and surgeons, radiation oncologists, pathologists, genetic counselors, pediatric oncology nurses, social workers, rehabilitation specialists, psychologists, and child-life specialists. Its clinician summary recommends referral to centers with multidisciplinary teams experienced in childhood and adolescent cancer.

When to ask about clinical trials
Because colorectal cancer is rare in children and teens, families can ask the oncology team whether a clinical trial is suitable. The NCI patient summary points to its childhood colorectal cancer information and trial-search resources and to ClinicalTrials.gov for studies sponsored by other organizations. A possible match depends on factors such as age, diagnosis, prior treatment, tumor features, location, and the study’s current status; a search result alone does not establish eligibility.
Questions to take to the oncology team
- What is the exact tumor location, stage, and pathology, and has it spread?
- Can the tumor be completely removed, and what would surgery involve?
- Would chemotherapy or radiation be considered for this tumor and why?
- Have molecular or inherited-risk findings changed the options?
- Is a clinical trial appropriate, and who can check current eligibility?
- Which specialists with childhood and adolescent cancer experience are involved in the plan?
