How Is Colorectal Cancer Diagnosed in Children and Teens?

Doctors diagnose colorectal cancer in a child or teen through a medical evaluation, usually including a review of symptoms and family history, an examination, and—when indicated—a colonoscopy with tissue sampling. Stool and blood tests can add information, while imaging is generally used to assess the cancer’s extent after diagnosis. No single symptom, stool result, blood marker, or scan by itself confirms colorectal cancer.

When symptoms need medical assessment

Possible symptoms include abdominal pain, constipation or diarrhea, blood in the stool, nausea or vomiting, unexplained weight loss, an abdominal lump, reduced appetite, and anemia. These symptoms have many possible causes and do not, on their own, mean a child has cancer. The National Cancer Institute (NCI) says: “These symptoms may be caused by problems other than colorectal cancer. The only way to know is for your child to see a doctor.” (NCI, Childhood Colorectal Cancer (PDQ®)–Patient Version.)

Colorectal cancer is rare in children. The NCI’s health professional summary reports 0.5 cases per 100,000 people younger than 20 annually in the United States and fewer than 100 U.S. childhood diagnoses each year (NCI, Childhood Colorectal Cancer Treatment (PDQ®)–Health Professional Version, 2025 figures). Rarity is context, not a way to rule cancer in or out for an individual child.

How doctors investigate possible colorectal cancer

1. Medical history and examination

The clinician asks when symptoms began, how often they occur, and about the child’s personal and family medical history. A physical examination follows. The findings help determine whether further testing is needed and which tests are appropriate; not every child has the same workup.

Except for standard USB webcams (which use USB data only), they are not compatible with HDMI/DisplayPort cables, video-capable USB-C hubs, or docking stations with video output.
  • Convert USB-A Ports to USB-C: Designed to connect USB-C earphones, cables, flash drives, card readers, and other USB-C accessories to standard USB-A ports. Plug-and-play with no drivers or software required.
  • Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
  • Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
  • 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.

2. Colonoscopy and tissue examination

A colonoscopy lets a clinician examine the lining of the rectum and colon for polyps or abnormal areas. Instruments passed through the scope can remove a polyp or take a tissue sample. A laboratory examines sampled tissue under a microscope, which can establish whether cancer is present. A colonoscopy may therefore provide both direct visual information and material for diagnosis.

3. Stool and blood tests

These tests can contribute evidence, but they do not replace tissue diagnosis when cancer is suspected.

  • Stool blood testing: A fecal occult blood test looks for blood that cannot be seen with the naked eye. NCI describes collecting stool on cards for testing. A positive result does not identify its cause or prove cancer.
  • Complete blood count (CBC): Measures blood-cell counts, including hemoglobin and hematocrit. It can help identify anemia, but anemia has causes other than colorectal cancer.
  • CEA blood test: Measures carcinoembryonic antigen, a substance released by normal as well as cancer cells. An elevated result is not specific proof of colorectal cancer.
  • Liver and kidney function tests: The NCI health professional summary includes these among studies that may be part of evaluation.

4. Molecular tests and other studies

Molecular testing can examine genes, proteins, or other molecules in tissue, blood, or bone marrow. Results may help with treatment planning, assessing response, or estimating prognosis; they are not a substitute for the clinician’s full diagnostic assessment. NCI says its Molecular Characterization Initiative offers free testing to children, adolescents, and young adults with certain newly diagnosed cancers. Eligibility and program details can change, so families should confirm them with NCI.

Depending on the clinical question, sources also list procedures and imaging such as barium enema, video-capsule endoscopy, other radiographic studies, chest CT, and bone scans. These are possibilities, not a standard checklist every child must undergo.

What each type of test can show

Assessment What it examines Information it can provide Place in evaluation
Colonoscopy with biopsy Rectum and colon lining; sampled tissue Visual findings and tissue for microscopic examination Diagnostic assessment when indicated
Stool occult blood test Stool Whether hidden blood is present Supporting evidence; does not establish cancer
CBC, CEA, and organ-function blood tests Blood Blood-cell counts, a non-specific marker, or liver and kidney function Supporting assessment; results require clinical interpretation
Molecular testing Genes, proteins, or other molecules in tissue, blood, or bone marrow Molecular features that may inform care May contribute to characterization and treatment planning
CT, MRI, PET, and other selected studies Images of body regions Information about disease extent or other clinical questions Often used for staging after diagnosis; selection varies

How staging differs from diagnosis

Diagnosis asks whether cancer is present; staging assesses its extent and helps guide treatment. NCI says colorectal cancer in children and adults is usually described using TNM staging, which considers the primary tumor, lymph nodes, and spread to other parts of the body. CT, MRI, and PET-based imaging may be used for staging, depending on the case.

NCI notes that pediatric cases have often spread to lymph nodes, beyond the bowel, or to abdominal organs by the time they are diagnosed. This describes a pattern across cases, not what will happen in any one child. The care team interprets imaging and other findings together to determine an individual child’s stage.

Specialist and inherited-risk assessment

A child diagnosed with colorectal cancer will be referred to a pediatric oncologist, according to NCI. Because some childhood cases are associated with inherited syndromes, genetic counseling may also be considered. A counselor can review the child’s and family’s history, discuss whether gene testing may be useful, and explain possible implications for relatives.

Genes that may be considered include APC, NF1, MUTYH, and NTHL1, among others. The sources do not establish one testing panel for every child; the care team individualizes the discussion and testing decisions.

What families should take away

  • Symptoms warrant a clinician’s assessment, but common gastrointestinal symptoms are not a diagnosis.
  • Colonoscopy can reveal abnormal areas and allow tissue sampling; microscopic review of tissue is central to confirming cancer.
  • Stool tests, blood tests, molecular analysis, and imaging answer different questions and are chosen according to the child’s situation.
  • Staging follows diagnosis and describes how far disease has spread; pediatric oncology and inherited-risk evaluation may be part of care.

NCI’s PDQ summaries provide patient and professional information, not a personalized diagnostic plan or formal healthcare recommendations. Decisions about tests should be made with the child’s healthcare team.
