IVF incubators protect embryos by keeping temperature, carbon dioxide, oxygen and related culture conditions within validated limits while reducing the disturbances caused by door openings, handling, evaporation, contaminants and equipment failure. These controls preserve a stable laboratory environment; they cannot guarantee embryo development, pregnancy or live birth.
What an IVF incubator controls
An embryo is cultured in a small volume of nutrient medium. Conditions in that medium can change when temperature, gas levels, evaporation or handling changes. The incubator is therefore more than a warm box: it is part of a monitored culture system that includes the dish, medium, oil, work surfaces, laboratory air, sensors, alarms and written procedures.

Temperature
Embryology laboratories set a target temperature in their validated standard operating procedure (SOP). The important point is stability. Opening a door, moving a dish to a cold work surface or leaving a culture plate outside the chamber can create a rapid change even when the incubator display later returns to its set point. Heated stages and blocks are used during handling so that the dish and medium remain close to the laboratory’s validated target.
Carbon dioxide and pH
Carbon dioxide supplied by the incubator interacts with the culture medium’s buffering system. If the gas environment changes, the medium’s pH can move outside its intended range. Recovery after a door opening depends on the equipment, the load inside it and the laboratory’s procedure, which is why laboratories monitor recovery rather than assuming that a displayed set point proves that the medium is stable.

Low oxygen
Embryo culture commonly uses a low-oxygen atmosphere, produced with premixed gas or nitrogen input. The required oxygen concentration is specific to the laboratory’s validated system. Oxygen should be measured when the SOP calls for it, with alarms and corrective procedures for readings outside established tolerances.
Humidity, osmolality and medium chemistry
Humidity affects evaporation from culture drops. Evaporation concentrates salts and nutrients, changing osmolality and potentially altering the culture environment. Temperature, carbon dioxide, oxygen, humidity, pH and osmolality are interdependent; checking only one number on the incubator front panel is not enough. The Cairo culture consensus advises laboratories to validate their complete system and compare readings with pre-established targets and tolerances using independent calibrated instruments, not the built-in display alone (Cairo Consensus Guidelines on IVF Culture Conditions).
Which disturbances laboratories try to prevent
| Potential disturbance | What can change | Typical control |
|---|---|---|
| Frequent door or lid openings | Temperature and gas concentration can fluctuate; recovery takes time. | Enough incubator capacity, fewer openings, rapid recovery and documented opening procedures. |
| Removing dishes for assessment | Embryos leave the stable chamber while they are viewed or handled. | Fast assessment, heated work surfaces, compartmentalized equipment or time-lapse imaging. |
| Long transfers between rooms or benches | Cooling and exposure to a different atmosphere can occur. | Short travel routes, pre-warmed equipment and coordinated workflow. |
| Evaporation | Medium volume and osmolality can change; pH and temperature may also be affected. | Appropriate dish design, validated oil overlay and controlled humidity. |
| Laboratory contaminants | Particles, microorganisms and volatile organic compounds (VOCs) may affect the culture environment. | HVAC design, cleaning controls, material selection and air-quality monitoring. |
| Power loss or equipment failure | Temperature and gas conditions may drift or stop being regulated. | Alarms, backup power, preventive maintenance and an incident-response SOP. |
How laboratories keep the chamber stable
Capacity and door-opening discipline
The American Society for Reproductive Medicine (ASRM) recommends providing enough incubator capacity to limit door openings and to maintain culture capacity during maintenance or an equipment outage. Staff group tasks, prepare materials before opening and close the door promptly. A laboratory may use separate units for different stages or patient groups so that one opening does not disturb every culture dish.

Independent measurement and daily checks
The Cairo culture consensus calls for daily assessment of temperature and carbon dioxide. Oxygen and humidity are assessed when appropriate to the laboratory SOP. Measurements are compared with pre-set limits, recorded and reviewed. Independent calibrated thermometers and gas analysers provide a check against sensor drift or a faulty display. A reading outside tolerance should trigger investigation, documentation and a decision about whether cultures must be moved to a validated backup unit.
Calibration, maintenance and alarms
Calibration should use certified standards and follow the equipment manufacturer’s instructions. The Cairo guidance recommends preventive maintenance at least annually, with more frequent checks when the manufacturer or the laboratory’s risk assessment requires them. Alarms should be tested regularly, set so that staff can respond before conditions become unacceptable, and connected to an escalation plan. Backup electricity and, where applicable, backup gas supplies reduce the time cultures remain exposed during a failure. ASPIRE’s 2023 laboratory-practice guidance likewise treats monitoring, maintenance and contingency planning as routine laboratory responsibilities.
What happens when embryos leave the incubator
Heated work surfaces
Assessment, media changes and other procedures occur on heated stages or blocks. The surface must be validated for the dishes and temperatures used; a household heating device is not an equivalent substitute. Staff also limit the time a dish is open and avoid placing it on an unheated bench.

Short, organized transfers
Embryology laboratories arrange incubators, microscopes and workstations so that dishes travel the shortest practical distance. Staff prepare labels, pipettes and media first, then remove the dish for the minimum time needed. This reduces exposure to room temperature and room air, and helps keep the workflow repeatable.

Compartmentalized and rapid-recovery designs
Some incubators divide cultures into smaller compartments. Opening one compartment then exposes fewer dishes than opening a single large chamber. Rapid-recovery systems are designed to restore temperature and gas conditions quickly after access. These features reduce the magnitude or duration of a disturbance, but a laboratory still needs validated operating procedures and independent checks.
How time-lapse incubators change assessment
In conventional culture, staff remove a dish to view embryo development under a microscope. A time-lapse system places a camera inside or alongside the incubator and records images at programmed intervals. Staff can review a sequence without taking the dish out for every observation. This can reduce handling and door openings, particularly when the system uses separate compartments.

The reason for using time-lapse is environmental stability and additional observation data, not a guaranteed treatment benefit. The cited consensus guidance does not establish that avoiding removal by itself improves implantation, pregnancy or live-birth rates. A clinic should explain how its system fits the validated SOP, how images are reviewed and what happens if the camera, software or compartment fails.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Why the laboratory room matters
Incubator control cannot compensate for a poorly controlled room. Air-handling design addresses particles, microorganisms, cleaning residues and VOCs that can enter the work area or incubator. The Cairo consensus on the IVF laboratory environment gives aspirational design benchmarks of total VOCs below 500 micrograms per cubic metre and aldehydes below 5 micrograms per cubic metre. These are expert-consensus targets, not universal legal limits or a promise that embryos are protected when a number is met (Cairo consensus on the IVF laboratory environment and air quality).
The same 2018 consensus recommends 15 total air changes per hour, including three fresh-air changes per hour. It describes an ideal laboratory overpressure of plus 38 to plus 50 pascals, with a recommended minimum of plus 30 pascals. Actual airflow, pressure and filtration must be designed by qualified specialists and interpreted with local building, occupational-health and clinical requirements. Cleaning products, construction materials, cosmetics and maintenance chemicals are controlled because VOC exposure can originate outside the incubator itself.

Oil overlay: useful, but not an airtight barrier
Many culture systems place a layer of oil over medium droplets. The oil slows evaporation and can reduce fluctuations in temperature, pH and osmolality. It does not stop those changes completely. The size of the drop, dish design, incubator atmosphere, oil properties and time in culture all influence the result.
ESHRE’s 2026 recommendations on good practice in the IVF laboratory state that oil intended for IVF culture must be tested for embryo toxicity. They also report no solid evidence that changing oil viscosity improves laboratory performance indicators or clinical outcomes. Laboratories therefore choose a tested product through their validation and quality-control process rather than treating viscosity as a stand-alone predictor of success.
Comparing incubator systems without relying on marketing
If a laboratory is evaluating equipment, the useful comparison is operational rather than promotional. A feature matters only if the laboratory can validate, monitor and maintain it in its own workflow.
| Comparison question | What to examine |
|---|---|
| Disturbance during assessment | How often dishes must be removed, whether imaging is internal, and how much of the chamber is exposed during access. |
| Recovery | Measured temperature and gas recovery after a defined opening, not merely the manufacturer’s set-point claim. |
| Compartmentalization and capacity | Whether one access event affects all cultures, and whether spare capacity exists during cleaning or downtime. |
| Monitoring and alarms | Independent sensor compatibility, alarm escalation, data logging, calibration intervals and test procedures. |
| Maintenance and contingency | Access for servicing, backup power and gas arrangements, validated backup units and documented failure response. |
| Fit with the SOP | Whether staff can reproduce the validated temperature, gas, humidity and handling conditions for every dish type used. |
No single incubator architecture has been shown by these operational criteria alone to guarantee a clinical outcome. A less feature-rich unit that is correctly validated, monitored and maintained may be safer in practice than a more complex system that the laboratory cannot support reliably.
What quality control looks like in practice
- Define targets. The laboratory sets acceptable ranges for temperature, carbon dioxide and, where relevant, oxygen, humidity and recovery time.
- Measure independently. Staff use calibrated instruments and compare them with the incubator display and the established limits.
- Record and review. Daily readings, alarm tests, maintenance and corrective actions are documented so that trends can be identified.
- Investigate exceptions. An out-of-limit result is not ignored or averaged away. Staff assess duration, affected cultures, sensor performance and whether transfer to a backup unit is required.
- Maintain the system. Preventive servicing, calibration and cleaning follow the manufacturer’s schedule and the laboratory’s risk assessment.
- Test emergencies. Backup power, alarms, gas supplies and communication chains are exercised before an actual outage occurs.
Questions patients can reasonably ask their clinic
- How are temperature and carbon dioxide measured independently of the incubator display?
- How often are oxygen and humidity checked in the system used for my embryos?
- What happens if an alarm sounds or the incubator loses power?
- How are dishes kept warm during assessment and transport between workstations?
- Does the laboratory use time-lapse imaging, and what is its purpose in the validated workflow?
- How are air quality, cleaning chemicals and construction-related VOC risks managed?
- How often are incubators calibrated and serviced, and how is maintenance documented?
These questions assess the laboratory’s process. They cannot predict how an individual embryo will develop, because embryo biology and patient factors remain important and are not controlled by the incubator.
The practical conclusion
Incubators protect IVF embryos by reducing avoidable variation: they stabilize temperature and gas chemistry, limit exposure during access, control evaporation, support warm and rapid handling, and provide alarms and backup plans when equipment or power fails. Effective protection is a quality system spanning the chamber, the workbench and the room. Stable conditions lower environmental risk, but no incubator feature, oil type, air benchmark or time-lapse system can promise embryo quality, pregnancy or live birth.
