How Hospitals Can Segment Networks to Contain Ransomware and Intrusions

Hospitals can make it harder for ransomware or an intruder to move from one compromised system to others by dividing networks into meaningful zones, restricting traffic between them to what operations require, and monitoring those boundaries. Segmentation limits paths; it does not guarantee containment. Its design must reflect the hospital’s actual clinical, business, vendor, and operational dependencies.

What network segmentation can—and cannot—do

Segmentation creates boundaries between groups of systems and controls how they communicate. If an attacker gets into one area, well-enforced boundaries can reduce the routes available for lateral movement and help responders isolate the affected zone. CISA’s #StopRansomware Guide, revised October 19, 2023, says segmentation can help contain an intrusion’s impact and prevent or limit malicious lateral movement.

It is not a guarantee. A rule that permits unnecessary connections, a device that connects to multiple segments, or an operational workaround can undermine the boundary. Segmentation also does not replace secure configuration, access controls, monitoring, backups, or incident response. Treat it as one part of a broader defense-in-depth program.

How to plan hospital network zones

There is no universal hospital zone map in the cited guidance. Start with local workflows and dependencies, then define boundaries around function and risk. Depending on the environment, that may mean separating user devices, business or departmental resources, production systems, critical systems, externally facing services, and operational technology (OT). These are design considerations, not a prescribed template.

Before deciding what may communicate across a boundary, determine which systems depend on one another. A separation that looks sensible on a diagram can disrupt a clinical or operational service if it blocks a required connection. Validate proposed boundaries and changes with the teams responsible for the affected workflows and systems.

Implementation sequence

  1. Inventory systems and dependencies

Record relevant IT and OT assets, software, network interfaces, owners, criticality, data handled, and dependencies. Identify systems important to health and safety, as well as the services that rely on them. Secure the inventory and keep an offline copy available for response.

  2. Map network flows and trust relationships

Document major networks, IP schemes, topologies, internal and external endpoints, and the connections among systems. Include third-party and managed service provider access, remote access, and cloud connections. Keep diagrams current, secure, and accessible to responders during an incident.

  3. Define zones around function and risk

Use the inventory and flow maps to identify boundaries that can limit unnecessary access without breaking required services. Consider business or departmental separation and IT/OT separation where applicable. The Healthcare and Public Health Sector Coordinating Council’s 405(d) practice material calls for clearly defined zones; it does not establish a standard layout for every hospital.

    • EFFECTIVE PROTECTION: Employs advanced filtering technologies to safeguard against sophisticated threats.
  4. Allow only necessary inter-zone traffic

Document required connections and enforce them with appropriate policy controls. CISA guidance identifies router access control lists (ACLs), stateful packet inspection, firewall capabilities, and demilitarized zones (DMZs) as possible tools. VLANs can group devices, but a VLAN by itself should not be treated as a complete security boundary: the design must also control and observe the traffic that crosses it.

  5. Restrict administrative and remote pathways

Apply least privilege to administrative access, control remote access and remote monitoring or management tools, and monitor privileged activity. Do not assume a connection is trustworthy merely because it comes through a VPN. Define and secure the pathways administrators need to reach systems across zones.

  6. Log and monitor movement between zones

Retain relevant network, host, and cloud logs; centralize and correlate them through a SIEM or equivalent log-management process; and establish a baseline of normal traffic. Monitor for unusual connections and lateral movement, including attempts that a policy blocks. CISA’s October 19, 2023, #StopRansomware Guide says to retain critical-system logs for at least a year if possible.

  7. Plan isolation without losing sight of clinical operations

Define who is authorized to isolate systems or zones, how that decision is coordinated with clinical and operational leaders, and how essential services will continue. Prepare an out-of-band communication method where appropriate. If a device cannot be disconnected, CISA says powering it down may be considered in the stated circumstances, but warns that doing so loses volatile-memory evidence; it is a last resort, not a routine containment step.

  8. Review and exercise the design

Reassess diagrams, access rules, and response procedures as systems and workflows change. Exercises should test whether teams can identify affected zones, coordinate containment, preserve useful evidence, and sustain essential services. CISA recommends regular assessments but does not set a hospital-specific testing schedule.

How the main segmentation mechanisms differ

Mechanism What it can contribute Design consideration
VLANs Group devices into logical network segments. A grouping alone does not establish that traffic between segments is restricted; define and enforce permitted flows.
ACLs Apply rules to permit or deny specified network traffic. Rules need to match documented dependencies and be reviewed as systems change.
Firewalls and stateful packet inspection Enforce policy at network boundaries and inspect traffic according to the configured controls. Choose boundaries and rules that support required clinical and operational communication while limiting unnecessary paths.
DMZ constructs Place selected services in a separated area between less-trusted and internal networks. Specify which connections are allowed between the DMZ and other zones; do not assume placement alone makes a service safe.

These mechanisms are options, not a ranking. Compare any proposed approach by how strongly it enforces boundaries, how finely it can separate systems, whether it fits clinical workflows and legacy equipment, what inter-zone activity it can log, how manageable its rules are, and whether responders can isolate an affected area without disabling unrelated services.

When microsegmentation may be relevant

Microsegmentation applies boundaries at a finer level than broad network zones, potentially around individual workloads or smaller groups. CISA’s 2025 announcement described microsegmentation as a component of zero trust with potential benefits that include reducing attack surface, limiting lateral movement, and improving visibility. The announcement concerned Part One, an introductory planning document for federal civilian agencies, and said a later technical guide was planned. It is not, by itself, a hospital deployment recipe or evidence of a measured reduction in hospital ransomware impact.

Whether finer-grained segmentation is suitable depends on the hospital’s systems, dependencies, staffing, and ability to maintain and troubleshoot detailed policies. Evaluate it against those operational requirements rather than assuming more boundaries automatically produce better containment.

What a hospital-specific plan must establish

General guidance can identify useful practices, but it cannot determine which connections a particular hospital must permit, how a given clinical system will behave after a change, or what isolation is safe in a specific incident. Those decisions require local asset and workflow analysis, input from system owners and vendors, and an understanding of safety and operational constraints. The cited guidance does not quantify a hospital-specific reduction in ransomware spread or outages, nor does it endorse a particular firewall, switch, or monitoring appliance.
