Count the costs of delivery, installation, operation, consumables, maintenance, parts, downtime and end-of-life, then test how the result changes when uncertain prices or supply conditions change. There is no universal cost winner: the answer depends on the device, country, contract and hospital.
Why the quoted price is not the total cost
A device with a low purchase price can still require expensive site work, specialist training, imported consumables or costly repairs. Conversely, a higher-priced offer may include installation, warranty service or other costs that another supplier quotes separately. Comparing headline prices can therefore rank unlike packages rather than reveal which option costs less to provide the same clinical service.

The World Bank’s Medical Diagnostic Imaging Equipment Procurement Guidance defines total cost of ownership (TCO) as “an estimate of the total costs associated with a solution over the whole of the operational life including final disposal.” The guidance is specifically about medical diagnostic imaging equipment; its lifecycle approach is useful for other devices, but hospitals should adapt the cost categories to the technology being procured.

Lifecycle cost = acquisition and delivery + site preparation and installation + training + operation and supplies + maintenance and parts + expected costs of interruption + end-of-life costs − supportable residual value.
This is a planning model, not a substitute for the applicable procurement rules or accounting method. Use the same treatment of taxes, timing, inflation and any discounting for both offers. If the hospital’s policy requires present-value calculations, use the approved method and rate consistently; do not choose assumptions that favor one origin.
Set the comparison boundary first
Before asking suppliers for final prices, define what is being compared. Imported and domestically produced devices are not comparable merely because they serve a broad category such as imaging, diagnostics or monitoring. Their configuration, capacity, accessories, software, expected service output and operating environment must meet the same requirement.

Specify the service, not just the machine
- Clinical purpose: identify the service the hospital needs the device to provide, including relevant performance, safety and setting requirements.
- Configuration: list the required model configuration, accessories, software, licenses, interfaces, installation and support. Clarify whether quoted options are included or priced separately.
- Operating conditions: state the site’s utilities, space, environmental conditions, staffing and expected pattern of use. Ask suppliers to identify requirements the hospital does not currently meet.
- Service output: define the common planning basis, such as the intended operating schedule or service volume, using the hospital’s own forecast. Do not compare offers using different utilization assumptions.
- Analysis period: choose one period appropriate to the procurement and expected use, and apply it to both devices. Include the costs expected within that period and explain how replacement or residual value is treated if the period does not match the equipment’s useful life.
Have clinical, biomedical engineering, facilities, finance and procurement staff agree on these assumptions before evaluating prices. If an offer does not meet the required specification, record that as a compliance issue rather than disguising it with a lower cost estimate.
Build an auditable cost worksheet
Request prices and assumptions in a format that lets evaluators trace each figure to a quotation, contract term or documented estimate. For both offers, identify the amount, currency, when it is paid, whether it recurs, who bears it and how certain it is. Separate binding quoted prices from forecasts, allowances and unpriced risks.

| Cost or decision area | Questions to answer for each offer |
|---|---|
| Clinical and technical fit | Does the offered configuration meet the same service need, performance, safety and operating-setting requirements? Are necessary accessories and functions included? |
| Purchase and delivery | What does the price include? Who pays shipping, insurance, handling, customs charges, applicable taxes and inland delivery? What delivery terms apply, and when do ownership and risk transfer? |
| Site preparation and installation | Are building or utility changes, installation, commissioning, acceptance testing and validation included? What work and cost remain with the hospital? |
| Training and staffing | Who trains operators and technicians, in what language and for how long? What skills, staff time or additional staffing does operation require? |
| Operation | What electricity, water, gas or other utilities and routine operating inputs are required? Are there software, service or other recurring charges? |
| Consumables and supplies | Which supplies are necessary, how much is expected to be used, what do they cost, and what are their shelf life and availability? Are there restrictions on compatible supplies? |
| Maintenance and service | What do the warranty and any service contract cover, for how long, and at what cost? Compare preventive and corrective maintenance, labor, travel, response times and access to factory engineering support. |
| Spare parts and downtime | Where are parts held, how are they sourced, what lead times and stock arrangements apply, and who can repair the device? What are the operational consequences if it is unavailable? |
| Currency and continuity | Which costs are exposed to exchange-rate movement or interrupted supply? What evidence supports the estimates, and which plausible scenarios should be tested? |
| End of life | What will deinstallation, disposal or replacement cost? Is any residual value supportable, and how was it estimated? |
Request clarifications where suppliers use different scope or terminology. For example, one quote may include delivery to the hospital while another ends at a port or other handover point. Do not add an assumed charge until the responsible party and applicable contract or local requirement are established. Mark unanswered items as unpriced and resolve them before award where they could change the comparison.
Separate one-time, recurring and uncertain costs
One-time and start-up costs
Capture purchase, transport, insurance, handling, applicable import charges, inland delivery, site work, installation, commissioning, testing and initial training. Ask the supplier to make clear whether quoted installation includes the hospital’s required acceptance tests and validation, or whether these are separate services. Include hospital-paid work even when it does not appear on the supplier’s invoice.
Recurring costs during operation
Estimate utilities and routine inputs using the same expected use assumptions for both devices. Include consumables, software or service charges, preventive maintenance, corrective repairs, parts, labor and travel where relevant. Distinguish costs that recur on a schedule from those that depend on usage or failure; multiplying a one-time figure across every year can materially distort the estimate.
For consumables, request unit prices, expected consumption and supply terms. Check shelf life, availability, compatibility restrictions and whether the item has to be obtained from a single source. If use is uncertain, calculate a range using the hospital’s own low, base and high utilization assumptions rather than treating a supplier estimate as a guaranteed quantity.
End-of-life and residual value
Include costs for deinstallation and disposal where the hospital expects to incur them, and include replacement within the analysis period if it is part of the plan. Deduct a residual value only when it is supportable and its basis is documented; do not assume that an old device will have resale value or remain serviceable. Keep the end-of-life method consistent across offers.

Account for import costs without guessing
An imported offer needs a delivered-cost breakdown, not a generic percentage added to the supplier’s price. Ask for the point of delivery, applicable delivery terms and the party responsible for transport, insurance, handling and each handover. Establish which customs duties, taxes and fees apply under the procurement’s jurisdiction and contract. Rates cannot be generalized without knowing the country, product classification and transaction details.
Identify which costs are quoted in foreign currency and when payment is due. The finance or procurement team should use an exchange-rate assumption appropriate to the procurement and its rules, record its source and date, and test a range of plausible movements. Do not present a scenario as a forecast or treat a rate as applicable to every purchase.
Trace the supply chain beyond the initial shipment. Consumables and replacement parts may also be imported, and their cost or availability can matter throughout the operating period. Ask the supplier where required items are stocked, how they are replenished, what lead times are quoted and what arrangements apply if a shipment is delayed. Model a disruption as a scenario only when the assumptions are stated; do not invent a downtime probability or repair duration.
Evaluate domestic offers by capability, not label
Apply the same clinical, technical, safety, quality, service and lifecycle requirements to domestically produced devices. A domestic origin does not by itself establish that a device is available, less expensive, easier to maintain or preferable. Nor does an imported origin by itself establish higher quality or greater risk. The evidence for each offer is what matters.
Ask the supplier to document manufacturing and quality capacity relevant to the offer, the service footprint, spare-parts sourcing, consumables, operator and technician training, warranty terms and support over the analysis period. Confirm whether service and parts are available where the hospital operates, rather than inferring coverage from a supplier’s general presence in the country.

The World Health Organization’s 2016 report on local production examined policy, infrastructure, capacity and feasibility in case studies in Ethiopia, Nigeria, South Africa and the United Republic of Tanzania. It supports a context-specific feasibility assessment, not a conclusion that domestic production is universally available or lower cost. See WHO’s report on improving access to medical devices through local production.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Compare serviceability and the cost of interruption
A device that cannot be repaired promptly may disrupt clinical services even if its quoted maintenance price is low. For both options, ask for warranty coverage and duration, service-contract scope, preventive-maintenance requirements, corrective-service arrangements, response times, labor and travel charges, and access to specialist or factory engineering support. Clarify what is a contractual commitment and what is only an estimate or aspiration.
For critical components and consumables, record the primary and backup source, stock location, stated delivery lead time, reorder arrangements and repair capability. Have clinical and operations teams describe the consequences of unavailability for the service being procured. Where the supplier cannot substantiate an availability or downtime estimate, label it unknown or test explicit scenarios instead of inserting a fabricated number into the cost model.
WHO’s 4 September 2026 article on lifecycle implementation says that “Improving access requires ensuring that devices and diagnostics remain available, appropriate, safe, functional, and fit for purpose throughout their lifecycle.” Its practical emphasis includes maintenance systems, spare-parts planning, service-level agreements, installation, training and functionality monitoring. Those are procurement and operational considerations, not merely post-purchase tasks. Read the WHO South-East Asia lifecycle guidance.
Test whether uncertainty changes the ranking
A single total can make weak assumptions look precise. Prepare low, base and high scenarios for material uncertain inputs, such as foreign-exchange movement, consumable use, repair costs or parts delays. Use ranges supported by supplier terms, hospital records or clearly identified planning assumptions. Keep the scenarios comparable: if utilization or service coverage changes for one offer, explain why that condition is specific to it.
Best Value
- Calculate the base case. Sum like-for-like costs over the agreed period, using the common service-output and operating assumptions.
- Mark confidence. Label each input as a binding price, contractual term, supplier estimate, hospital estimate or unresolved item.
- Vary the uncertain inputs. Change one material assumption at a time to see its effect, then test combined low and high cases where risks could occur together.
- Find the break-even point. Identify how far an uncertain input would have to move before the lower-cost offer changes. This helps show whether a small price difference is robust or depends on a fragile assumption.
- Report cost and service consequences separately. Present lifecycle cost alongside compliance, expected service availability and material risks. A low estimated cost does not make an offer acceptable if it fails a required clinical or safety criterion.
Do not force every concern into a monetary value. If a service interruption, safety requirement or supply condition cannot be credibly priced, report it as a separate decision factor with its evidence and implications. This makes the trade-off visible without pretending the estimate is more certain than it is.
Use a procurement sequence that leaves an audit trail
- Agree the clinical specification and comparison period. Document required performance, configuration, operating conditions and expected service output before reviewing offers.
- Issue a common cost schedule. Ask every supplier to price the same delivery scope, installation, training, warranty, service, consumables and spare-parts information. Request explicit statements for excluded items.
- Check local rules and tender terms. Confirm how the procurement must treat taxes, delivery terms, evaluation criteria, local-policy provisions and any required price adjustments. The World Bank’s Project Procurement Framework is relevant to World Bank-financed project procurement; it is not a substitute for the rules governing every hospital purchase.
- Resolve scope gaps. Ask written questions about unclear inclusions, delivery handover, warranty exclusions, service response, parts availability and training. Record supplier answers and update both offers on the same basis.
- Validate hospital-side costs. Facilities staff should confirm site requirements; biomedical engineering should review maintenance and parts assumptions; clinical leads should assess service fit and interruption consequences; finance should validate currency and cost treatment.
- Calculate scenarios and document the decision. Preserve the source, date, currency and confidence level for each input. Explain the compliant offer selected, the lifecycle comparison, material uncertainties and any non-cost criteria applied under the tender.
Standardized specifications, service arrangements, spare-parts planning, maintenance and functionality monitoring are also consistent with WHO’s lifecycle approach. A procurement file should make it possible for a later reviewer to see not only the final number, but why the included costs and assumptions were considered comparable.
What the final comparison should show
A useful evaluation report gives decision-makers a transparent view rather than a single unexplained total. Include a side-by-side summary of compliant configuration, delivered acquisition cost, installation and site work, expected operating and supply costs, maintenance and parts assumptions, end-of-life treatment, and total estimate over the shared period. Show the low, base and high scenarios and identify which assumptions could reverse the ranking.

Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Keep estimates distinct from commitments. Attach or reference supplier quotations, contract terms, delivery terms, applicable local tax or duty determinations, hospital utility and staffing estimates, and the basis for any residual value. Note items that remain unresolved and whether they are important enough to require clarification before award.
There is no support for a universal percentage of lifecycle cost that applies to every medical device or market. A World Bank consultation feedback summary contains an industry-participant estimate for a 20-year equipment lifecycle, but it is not an audited universal benchmark; do not use it as a general rule for a hospital’s device category. Use the actual offer and operating assumptions under consideration.
