How Hospital Pricing Works in India: From Room Rent to Procedures

There is no single standard price for a hospital stay in India. The bill depends on the hospital’s tariff, the treatment and room category, what is bundled into any package, and—if you use a government scheme or insurance—the rules that apply to that arrangement.

What makes up a hospital bill?

A hospital bill can combine accommodation, professional fees, diagnostics, medicines, procedures, nursing and other services. The amount depends on the treatment and length of stay, but also on which tariff applies: a hospital’s own self-pay rates, a government scheme schedule, or the terms agreed by a hospital and insurer.

These are not interchangeable price lists. CGHS package rates apply in the context of CGHS beneficiaries and empanelled care; Central Services (Medical Attendance) guidance has its own beneficiary rules. Neither sets a general price for private patients paying out of pocket.

Room or ward rent

The room category and number of days can materially affect the bill. A ward rate may cover more than the physical room, but the inclusions depend on the applicable tariff. Under the 2025 CGHS notification, ward charges are consolidated and include accommodation and medical-record charges, duty medical officer, nursing care, registration or admission, and other items listed in that notification. That definition is specific to the CGHS rules; do not assume it describes every private hospital’s room charge.

Procedures and package rates

A package rate is a set price for a defined treatment or service bundle under a particular scheme or contract. Its scope may specify which professional fees, diagnostics, medicines, consumables or days of care are included. Exclusions, add-ons and the circumstances that change the price depend on the package terms. The 2025 CGHS notification sets package and billing rules for CGHS beneficiaries; it does not establish a universal private-hospital price.

Diagnostics, medicines and consumables

Whether these appear as separate line items depends on the treatment setting and package. Under CGHS rules, common consumables are included in a surgical procedure package; reasonable consumables may be billed in certain non-surgical cases where there is no package rate. For insured treatment, IRDAI guidance identifies items that must be included in room, procedure or treatment costs, subject to the policy contract and applicable arrangements.

Equipment and ICU care

The 2025 CGHS notification says equipment used during inpatient or surgical procedures is included in room rent or procedural package rates and should not be billed separately under those rules. It also describes ICU charges as inclusive of specified services. These are CGHS-specific billing rules, not a basis for assuming that every private bill uses the same inclusions.

How much does a hospital room cost in India?

The 2025 CGHS rate notification lists the following daily room and ICU charges. They are rates in the CGHS scheme context, not national private-hospital prices. Check the applicable current CGHS document and the patient’s ward entitlement before relying on them.

Category in the 2025 CGHS notification Listed charge Scope
General ward ₹1,500 per day CGHS rate; subject to applicable scheme rules and entitlement.
Semi-private ward ₹3,000 per day CGHS rate; subject to applicable scheme rules and entitlement.
Private ward ₹4,500 per day CGHS rate; subject to applicable scheme rules and entitlement.
Listed ICU or critical-care categories ₹5,400 per day CGHS rate; the notification describes specified inclusive services and accommodation for the ward entitlements it covers.

The Ministry of Health and Family Welfare also announced these CGHS room rates and the ₹5,400 ICU charge in 2023. The 2025 notification lists the same figures. They should not be mistaken for an average or standard private-hospital charge.

The official material cited here does not establish a reliable national average cost for a private hospital stay in India. A single average would also conceal differences in treatment, location, room, duration and what is bundled into the bill.

Why can the insurance approval be lower than the hospital bill?

The hospital’s submitted bill and the insurer’s payable amount are not automatically the same. The insurer assesses a claim under the policy wording and schedule, the applicable network or hospital arrangement, and any pre-authorization or claim conditions. Room limits, exclusions and co-payments can affect what is payable.

IRDAI’s standardization guidance says insurers should put measures in place so hospitals do not separately bill policyholders for items that belong in room, surgical procedure or treatment costs, including diagnostics, as identified in the guidance. It also makes clear that settlement remains subject to the policy’s terms and conditions. The guidance is not a promise that every charge on a hospital bill will be covered.

Can a hospital charge separately for consumables?

There is no single answer for every hospital bill. Check the applicable package, tariff, scheme or insurer arrangement. Under CGHS, common consumables are part of a surgical procedure package; reasonable consumables can be billed in certain non-surgical cases without a package rate. IRDAI guidance separately addresses items that insurers should ensure are included in room, procedure or treatment costs for insurance claims, subject to policy terms.

Are hospital rates regulated uniformly across India?

No single rate-display rule or rate cap can be assumed to apply identically throughout the country. Government material on the Clinical Establishments Act and Central Government Rules describes requirements to display rates in local language and English and to charge within applicable prescribed ranges in states and Union Territories that have adopted the framework. Adoption and enforcement are state-dependent. The Ministry of Health and Family Welfare’s Clinical Establishments Division provides rate-display and procedure-costing templates; patients should check local rules rather than assume the framework applies everywhere.

What to ask before admission

  1. Ask for a written estimate. Confirm the room category and expected number of days it assumes.
  2. Ask whether the treatment is priced as a package. Request the inclusions, duration, exclusions and circumstances that could change the package price.
  3. Clarify separate charges. Ask which items are bundled into room, procedure or treatment charges and which may be billed separately under the hospital’s applicable tariff.
  4. Check insurance details before admission. Review the policy’s room limits, co-payment and exclusions; confirm network status and whether cashless pre-authorization has been requested.
  5. Request the rate schedule and an itemized bill. Where the Clinical Establishments framework applies, compare the estimate with displayed rates and local rules.

These questions help clarify an estimate; they do not guarantee that a hospital uses one national bill format or that an insurer will approve every charge.

How to compare two hospital estimates

Compare the same treatment and clinical scope, not just the headline package price. Ask both hospitals for written estimates and check the following items side by side:

  • Room category and assumed number of days.
  • Package inclusions, duration and exclusions.
  • Surgeon, anaesthesia, operating theatre and diagnostic charges.
  • Medicines, implants and consumables.
  • ICU or day-care assumptions and possible add-ons.
  • What the insurer or scheme says it will reimburse.

For CGHS beneficiaries, also check the applicable city or hospital category and ward entitlement in the current rate document. For private self-pay, compare the hospitals’ own estimates; CGHS rates are not a universal benchmark.
