How Heart Transplant Candidates Are Evaluated and Matched With Donors in the U.S.

In the United States, a transplant hospital decides whether to list someone for a heart transplant; after listing, OPTN policy ranks candidates for each donated heart using medical urgency, compatibility and geography. The transplant team then reviews the specific donor and organ information before accepting or declining an offer. Being evaluated, listed or assigned a high urgency status does not guarantee a transplant or predict an individual waiting time.

How evaluation and listing work

A heart specialist can refer a patient to a transplant hospital, but referral does not automatically put someone on the waiting list. The hospital’s transplant team gathers medical information, conducts tests and discusses the patient’s current treatment. A coordinator and often a social worker help guide the process. The team uses the overall medical picture to decide whether and when to list the candidate; no single test or universal public checklist determines the outcome. See HRSA/OPTN’s Transplant Process.

Evaluation and listing are decisions made by the transplant hospital. The matching system later applies allocation policy to candidates and a particular donor; it does not make the original listing decision.

How adult urgency status works

Adults listed for a heart transplant are assigned one of six medical urgency statuses: Status 1 is the highest urgency and Status 6 the lowest. The status reflects detailed medical criteria and the candidate’s current support or treatment. Advanced support, severe or difficult-to-control conditions, treatment complications and the need to remain hospitalized are among the factors described in HRSA/OPTN’s Adult Heart Allocation System FAQs.

Status can change as health or treatment changes. Candidates and caregivers should tell the transplant team about major changes; the team can submit updated information, which may affect status. A higher status can give a candidate greater priority, but status alone does not guarantee an offer: the candidate must also be considered for a specific donor under compatibility and allocation rules.

Children younger than 18 use a different system, with statuses 1A, 1B and 2 and separate medical criteria. Adult status categories should not be applied to pediatric candidates.

  • View Chambers, Valves and Major Vessels – Removing the front section provides a closer look at important internal anatomy, including the heart chambers, valves and major vessels, helping students understand the relationship between external and internal cardiac structures.
  • Removable from the Stable Display Base – The heart can be lifted from the included stand for closer inspection from multiple angles, then returned to the base for hands-free study, classroom demonstrations and desktop display.
  • Made for Anatomy Teaching and Cardiology Study – Suitable for medical and nursing students, teachers, science classrooms, patient education and anyone studying human heart anatomy. The compact life-size format is easy to use for individual learning or guided instruction.

How a donor heart is matched to an adult candidate

When a donor heart becomes available, the OPTN matching system identifies candidates who may match and orders them under heart-allocation policy. It is not a simple first-come, first-served queue. For adult heart allocation, HRSA highlights medical urgency, blood-type match and distance from the donor hospital to the transplant hospital. Other general matching considerations can include donor and candidate size, age category and waiting time; which details matter, and how they are applied, depend on the applicable policy and offer.

Blood type and size help determine whether a donor heart is medically suitable for a candidate. Geography matters because the organ must be transported and transplanted promptly. Candidates closer to the donor hospital receive priority over similarly situated candidates farther away; if nearer candidates do not match or their programs decline, offers may move to more distant hospitals. HRSA says hearts and lungs are usually transplanted within six hours after removal. That is general guidance, not a guaranteed deadline for every individual offer. HRSA’s general donor-matching explainer says the system does not consider ethnicity, sex, religion, financial status, celebrity status or citizenship. Separately, the adult heart FAQ says insurance type and the cause of heart disease do not determine an adult candidate’s urgency status. These are distinct points: factors excluded from general matching are not the same as factors that do not set urgency status.

What happens when the team receives an offer

After the matching system orders candidates, the organ procurement organization (OPO) contacts the transplant team for a candidate near the top of the match run. The team receives detailed information about the donor and organ, then decides whether to accept or decline. It may decline if the candidate is too sick at that moment or if medical details indicate the heart is not a suitable match. An offer is therefore not a guarantee that transplantation will proceed.

Why waiting times vary

There is no universal waiting-time estimate in the HRSA/OPTN patient resources. How long someone waits can depend on blood type, tissue type, candidate height or weight, donor-organ size, medical urgency and time on the list, as well as whether a suitable donor becomes available under the matching rules. A waiting-time estimate for one person should come from that person’s transplant team, which knows their medical situation.

Waiting time by itself also cannot show how well a transplant hospital or OPO is performing: candidates differ in medical need and in the availability of a suitable match. For current operational rules, consult the HRSA/OPTN policy index, updated October 1, 2026. Individual questions about eligibility, status or an offer belong with the transplant team.
