Tirzepatide acts on both GIP and GLP-1 receptors; medicines in this category do not all work identically. Treatment is prescription-based and used alongside nutrition and physical-activity measures, with eligibility, product choice, and dosing determined with a qualified health professional.
How do GLP-1 weight-loss drugs work?
GLP-1 is a hormone signal that helps regulate appetite and food intake. Semaglutide medicines such as Wegovy mimic GLP-1 signaling and target brain areas involved in those processes. Tirzepatide, sold as Zepbound for chronic weight management in the U.S., mimics both GIP and GLP-1 signals and also acts on brain areas involved in appetite and food intake. These descriptions come from the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) and the FDA’s Zepbound approval announcement.

That shared focus on appetite does not make the medicines interchangeable: their receptor actions, approved uses, formulations, warnings, and directions can differ. A clinician can explain the current U.S. label for the specific product being considered.
Who may be considered for prescription treatment?
NIDDK says a health professional may consider weight-management medication for an adult with a BMI of 30 or higher, or a BMI of 27 or higher with a weight-related health problem such as high blood pressure or type 2 diabetes. Those figures are general considerations, not a determination that a particular person qualifies; a high BMI alone does not mean medication is appropriate for everyone.

FDA describes Zepbound’s adult chronic-weight-management use as treatment for obesity, or overweight with at least one weight-related condition, alongside reduced-calorie eating and increased physical activity. Product indications and labels can change, and they are not necessarily identical across brands or formulations. Ask a clinician to assess eligibility against the current label for the medicine in question.
What should I expect during treatment?
A prescription and an individualized plan
These are prescription medicines, not stand-alone substitutes for nutrition and physical-activity measures. Your prescriber should identify the exact medicine and formulation, explain how and when to take it, and give you a plan for follow-up and questions about other medicines or health conditions. NIDDK describes semaglutide (Wegovy) and tirzepatide (Zepbound) as weekly injections; the FDA’s current Wegovy label search result also includes oral tablets. Formulations and directions may change, so confirm the instructions for your exact prescription rather than assuming that products share a schedule.

Dose changes and check-ins
Ask your prescriber how dose escalation works for your prescribed product, when to check in, and what to do if you have trouble tolerating treatment or miss a dose. Do not increase, decrease, or otherwise adjust a dose based on advice for another product or on an online account. If you use a pharmacist for counseling, have the actual package and prescription instructions available so the discussion is about the product you have.

Weight change is not predictable from a class-wide average
NIDDK reports that, on average, adults using prescription weight-management medications as part of a lifestyle program lost 3% to 12% more of their starting body weight after one year than adults in a lifestyle program without medication. This is a broad average across weight-management medicines, not a forecast for one person and not a head-to-head result comparing GLP-1 medicines. Individual results and treatment experience vary.

What side effects are common?
Digestive symptoms are among the common reported effects, but the pattern and severity vary by person and product. The FDA’s 2026 Wegovy prescribing information lists nausea, diarrhea, vomiting, constipation, abdominal pain, headache, fatigue, and dyspepsia among common adverse reactions. The FDA’s Zepbound announcement lists nausea, diarrhea, vomiting, constipation, abdominal discomfort and pain, fatigue, and reflux, among other effects.
Contact your prescriber about severe or persistent symptoms, concerns about dehydration, or another worrying change. A health professional can help assess what is happening and advise whether your treatment plan needs attention.

What safety issues should I discuss with a clinician?
Product-specific warnings and medical history
The Wegovy label carries a boxed warning that semaglutide caused thyroid C-cell tumors in rodents; whether it causes these tumors in humans is unknown. It contraindicates Wegovy for people with a personal or family history of medullary thyroid carcinoma or for people with multiple endocrine neoplasia syndrome type 2 (MEN 2). This warning and contraindication are specific to the cited Wegovy label; do not assume that every medicine has identical labeling. Review the current label and tell your prescriber about relevant personal and family medical history.
Other medicines and delayed stomach emptying
The Wegovy label warns that low blood sugar may be a concern when it is used with insulin or insulin secretagogues. It also says delayed gastric emptying may affect oral medicines. Tell your clinician and pharmacist about all medicines you take, including diabetes treatment, so they can advise you using the current label for your product.

Use the exact product and dosing instructions
The FDA has reported dosing errors with compounded injectable semaglutide, including errors involving conversions between milligrams and syringe units, different concentrations, and multidose vials. Compounded products and instructions should not be treated as interchangeable with a branded prescription. Confirm the exact product and dose with your prescriber or pharmacist, and do not try to convert units or self-adjust a dose. The FDA safety alert on compounded semaglutide dosing errors explains how to report medication errors and adverse events.
Suicidal-behavior labeling update
The FDA says its review found no increased risk of suicidal ideation or behavior with GLP-1 receptor agonists and that it requested removal of the warning from affected prescribing information, including for Saxenda, Wegovy, and Zepbound. Because label updates can take time to appear across products, check the current prescribing information and the FDA safety communication rather than relying on older descriptions of labeling.
