The American Diabetes Association (ADA) reports that lean body mass (LBM) represented about 25% to 30% of weight lost in meta-analyses of GLP-1 receptor agonist obesity medicines. That is a pooled estimate, not a forecast for every person or medication dose.
The practical response is not to avoid medically indicated treatment. It is to protect function while losing weight: eat enough protein and micronutrient-rich food, perform progressive muscle-strengthening exercise, and have strength and physical function checked when risk is high.

What “muscle loss” means on a GLP-1 drug
Body-composition reports use several terms that are often treated as interchangeable even though they are not. Lean body mass (LBM) and fat-free mass (FFM) include skeletal muscle but also water, organs, connective tissue, bone minerals and other non-fat components. A scan or impedance device that reports less LBM has not necessarily demonstrated an equivalent percentage loss of contractile muscle.

The measurement method, hydration, timing, amount and speed of weight loss, and the person’s baseline condition all affect the result. A decline in LBM therefore needs to be interpreted alongside strength, mobility and health outcomes.

Weight reduction can still improve physical function and quality of life even when some lean tissue is lost. Conversely, an older adult or someone with severe obesity-related disease may be vulnerable to sarcopenic obesity, in which low skeletal-muscle mass and poor function occur together. That is a clinical issue, not a diagnosis that can be made from a consumer scale.
How much lean mass is typically lost?
In its 2025 obesity-pharmacotherapy standards, the ADA summarizes meta-analyses in which LBM loss accounted for 25% to 30% of total weight reduction with GLP-1 receptor agonist obesity medications. For example, a 10-kg reduction would correspond to roughly 2.5 to 3 kg of measured LBM if that pooled proportion applied; an individual result can be substantially different.
The same standards note that many nutrition-intervention studies find LBM contributes about 25% of total weight reduction in general. Keep that broader observation separate from the GLP-1-specific meta-analysis estimate: neither is a guarantee for a particular patient.

A 2025 joint professional-society advisory, discussing the STEP 1 trial, reports an average 13.6-kg body-weight reduction consisting of 8.3 kg of fat mass and 5.3 kg of lean body mass. Those figures are the advisory’s account of that trial population, not a universal outcome for people taking semaglutide or another GLP-1 medicine. The report does not mean that 5.3 kg of skeletal muscle was lost.

Why people respond differently
The proportion of fat and lean mass that changes is influenced by protein intake, exercise, age, sex and starting adiposity, according to the ADA. Other practical variables include how rapidly weight is lost, baseline muscle and physical activity, illness, gastrointestinal symptoms and whether a person can continue training.
Medication name alone does not answer the muscle question. The available sources do not establish a reliable head-to-head ranking of individual GLP-1 drugs or doses for preserving muscle. Comparisons should specify what was measured (LBM, FFM or skeletal muscle), how much weight was lost and over what period, the population studied, dietary and exercise support, and whether strength or function changed.
Nutrition when appetite and portions shrink
Set a protein target with a clinician
The ADA gives a general minimum of 60 grams of protein per day and notes that higher intake during weight reduction is often advised at 1.2–1.6 g per kilogram of body weight per day. These are starting points for individualized counseling, not universal prescriptions. Chronic kidney disease, heart failure, frailty and other conditions can require a different target. A prescribing clinician or registered dietitian nutritionist should adapt the goal to medical history, nutritional status, preferences, social circumstances and values.

Protein by itself is unlikely to preserve muscle without a structured resistance stimulus. Conversely, lifting while routinely under-eating can leave recovery and total nutrient intake inadequate. The useful plan combines an appropriate protein target with enough overall food and progressive strengthening.

Choose protein-dense, nutrient-rich foods
Small meals can make each eating opportunity count. The joint advisory lists beans, peas, lentils and whole grains alongside dairy, seafood, eggs and lean poultry. It recommends limiting or minimizing red and processed meat in light of broader health associations. Pair protein with vegetables, fruit, fiber-rich carbohydrates and unsaturated fats so that a reduced appetite does not produce a diet deficient in vitamins, minerals or energy.
- Distribute protein across meals and snacks that you can tolerate rather than waiting for one large meal.
- Use soft or liquid foods temporarily if chewing or early fullness makes normal portions difficult, while still prioritizing nutrient density.
- Track fluid intake and symptoms such as persistent nausea, vomiting, diarrhea or constipation; dehydration and poor intake can worsen weakness.
- Ask a clinician about a protein supplement only when food is insufficient or supplementation is otherwise appropriate. It is optional convenience, not a substitute for resistance exercise.
Know when intake is too low
Appetite suppression, early satiety, skipped meals and gastrointestinal adverse effects can make adequate protein and micronutrients difficult. Tell the prescribing clinician if you are repeatedly unable to meet nutrition needs, losing weight very rapidly, becoming dizzy or weak, or unable to train. The ADA notes that a clinician may consider dose reduction when medication effects leave nutritional intake inadequate. Do not independently change, stop or escalate a dose.
Why strength training matters
Resistance exercise gives muscle a reason to remain during an energy deficit. ADA guidance supports muscle-strengthening activity during caloric restriction, and the joint advisory emphasizes structured resistance training because protein alone is probably insufficient. Exercise cannot promise that every gram of lean tissue will be preserved, but it improves the chance that weight loss is accompanied by maintained strength and useful function.
Build a safe, progressive routine
- Get a baseline. Record what you can do comfortably: sit-to-stand repetitions, a short walk, stair climbing or light pushing and pulling. If you have cardiovascular disease, severe obesity-related complications, substantial weakness or recent inactivity, obtain medical advice before starting.
- Train the major movement patterns. Include a squat or sit-to-stand, hinge, push, pull, carry and calf raise, using body weight, machines, free weights or resistance bands. Equipment is optional; the muscle-strengthening stimulus is what matters.
- Start below your limit. One or two sessions per week with manageable resistance and controlled technique is more sustainable than an exhausting first workout. Leave recovery time between sessions for the same muscle groups.
- Progress gradually. Add a small amount of resistance, a repetition or a set only when form is stable and recovery is acceptable. Pain, marked breathlessness, dizziness or symptoms that persist after exercise warrant a pause and clinical review.
- Keep some aerobic movement. Walking, cycling or water exercise supports cardiovascular health and mobility. It complements rather than replaces resistance work.
A physical therapist or qualified exercise professional can modify movements for arthritis, balance problems, limited mobility or prior injury. The goal is consistent, tolerable progression—not a promise that exercise completely prevents lean-tissue loss.

Monitor function, not just the scale
Scale weight and a body-composition percentage cannot show whether you are becoming weaker. The ADA suggests regular checks such as chair-to-stand performance, handgrip strength, knee-extensor strength, walking tests or a functional screening tool such as SARC-F. A clinician can select an appropriate test and repeat it under similar conditions.
- Notice whether rising from a chair, carrying groceries, climbing steps or walking your usual route is becoming harder.
- Record training loads and repetitions so a trend in performance is visible even when body weight fluctuates.
- Interpret home impedance readings cautiously; hydration and device algorithms can move the result without a corresponding change in skeletal muscle.
- Seek assessment for falls, repeated near-falls, new difficulty with daily activities or persistent weakness.
Older adults and people with severe obesity-related disease or complications may need closer monitoring for sarcopenic obesity. A clinician may combine functional tests, examination, nutrition review and a validated body-composition method rather than relying on one reading.
A practical plan for someone starting treatment
Before or early in treatment
- Discuss your medical history, kidney and heart conditions, current diet, gastrointestinal symptoms and exercise experience with the prescriber.
- Agree on an individualized protein and calorie strategy; ask whether a dietitian referral is appropriate.
- Document baseline weight, relevant body-composition data if clinically useful, strength and daily-function measures.
- Plan two or more weekly opportunities for muscle-strengthening activity at a level you can recover from.
During active weight reduction
- Prioritize protein-containing foods at meals while maintaining micronutrient variety.
- Adjust portion size and meal frequency to appetite without allowing persistent skipped meals to go unaddressed.
- Progress resistance training slowly and keep notes on symptoms, recovery and performance.
- Review nutrition, strength and function at follow-up visits, not only the number on the scale.
If progress or intake becomes concerning
- Contact the prescriber for ongoing vomiting, diarrhea, inability to eat, dehydration, severe weakness or a sudden decline in function.
- Do not self-prescribe very high protein, supplements or a medication change, especially with kidney, heart or other chronic disease.
- Ask whether the rate of weight loss, dose or exercise plan should be modified after a clinical assessment.
Questions readers commonly ask
Do GLP-1 drugs cause muscle loss?
They can be associated with a reduction in measured LBM during weight loss. That does not prove an equal loss of skeletal muscle or establish that the medication directly destroys muscle. Strength, function, diet, exercise and the measurement method determine the clinical meaning.
How do I keep muscle while losing weight on a GLP-1?
Follow an individualized protein plan, perform progressive resistance training, maintain adequate micronutrient intake and have strength or function monitored when risk is elevated. These steps reduce risk; they cannot guarantee zero lean-mass change.

How much protein should I eat?
The ADA cites at least 60 g daily as general guidance and 1.2–1.6 g/kg/day as a range often used during weight reduction, with modification for conditions such as chronic kidney disease or heart failure. Your clinician or dietitian should set the target.
Is lean mass the same as muscle?
No. LBM and FFM include non-muscle tissues and body water. A body-composition report must be interpreted separately from evidence about skeletal-muscle size, strength and function.
Does strength training prevent all muscle loss?
No. It supplies an important preservation signal and supports function, but some LBM change may still occur during substantial weight reduction. Adequate nutrition and medical monitoring remain necessary.
