How Genetic Data Is Used in Personalized Medicine

Genetic data can help clinicians diagnose disease, estimate inherited risk, characterize a tumor, or choose and dose a medicine—but it is one input among many, not a stand-alone prescription. Its usefulness depends on the exact clinical question, the quality of the test, and evidence that a particular variant changes an outcome.

What personalized medicine means

Personalized medicine (also called precision medicine) combines information about an individual to prevent, diagnose, or treat disease more precisely. Genes and other molecular measurements may be part of that information, alongside the person’s symptoms, medical history, age, kidney and liver function, other medicines, environment, and lifestyle.

That distinction matters. A DNA result does not automatically establish a diagnosis, identify the best drug, or calculate a dose. The same variant can have different implications depending on the disease, the laboratory method, the patient’s other characteristics, and the strength of the clinical evidence.

Four clinical questions genetic testing can answer

1. Is an inherited condition present?

Germline testing examines DNA inherited from a parent and present in most cells. A blood, saliva, or cheek-swab sample can be used to look for variants associated with a hereditary disorder. A result may support or rule out a diagnosis, explain symptoms, or guide monitoring for a person and potentially for relatives.

Interpretation requires more than finding a difference in DNA. The laboratory and clinician must determine whether the variant is pathogenic, likely pathogenic, of uncertain significance, likely benign, or benign for the condition being considered. A variant of uncertain significance generally should not be used by itself to make major medical decisions.

2. What is a person’s inherited risk?

Some variants increase susceptibility without guaranteeing that disease will occur. Risk estimates can depend on the variant, family history, ancestry, age, and other factors. A higher inherited risk may lead to earlier or more frequent screening, preventive treatment, or a referral for genetic counseling; it is not the same as having the disease.

3. What molecular features does a tumor have?

Somatic, or tumor, testing examines genetic changes acquired by cancer cells during a person’s life. These changes are not necessarily inherited and may not affect relatives. Molecular testing is routine in care for many cancers because tumor characteristics can help clinicians select among therapies or identify people eligible for a targeted treatment or clinical trial.

A tumor result and an inherited result can overlap, but they answer different questions. A change found in a tumor may prompt a separate blood or saliva test to determine whether the change is also germline. Conversely, an inherited variant may influence cancer risk even when it is not present in every tumor cell.

4. How might a person respond to a medicine?

Pharmacogenetics and pharmacogenomics study how genetic variation contributes to differences in drug metabolism, drug effects, benefit, or adverse-event risk. Depending on the specific gene–drug association, a genotype may help a prescriber choose a treatment strategy, select a dose, avoid a preventable toxicity, or estimate the likelihood of benefit.

How genetic data moves from a sample to a treatment decision

  1. Define the clinical question. The clinician decides whether the goal is diagnosis, inherited-risk assessment, tumor characterization, or medication response. A test selected without a clear question can produce findings that are difficult to interpret.
  2. Collect and analyze a sample. DNA may come from blood, saliva, or a cheek swab. Sequencing can examine a single gene, a panel of genes, or much larger portions of the genome. The report should state what regions and variant types the method can and cannot detect.
  3. Identify variants. The laboratory compares the sequence with a reference and reports DNA differences. Detection is a technical result; it is not yet a diagnosis or a treatment recommendation.
  4. Evaluate clinical validity. Experts assess whether the variant is reliably associated with the disease, tumor feature, or drug-response question. Curated public variant databases can support this work, but database presence alone does not prove that a variant changes a patient outcome.
  5. Assess clinical utility. The care team asks whether acting on the result improves an outcome, such as survival, symptom control, prevention, or avoidance of serious toxicity. Evidence that a variant changes drug concentration is not necessarily evidence that testing improves effectiveness or safety.
  6. Integrate the result with the patient’s situation. Disease state, concurrent medicines, allergies, organ function, age, pregnancy, adherence, and patient preferences remain relevant. The final choice belongs in a clinician–patient discussion.
  7. Document and revisit when appropriate. Knowledge about variants and treatments changes. A result that was uncertain may later be reclassified, while a treatment decision may change as the disease or medication list changes.

Inherited (germline) data versus tumor (somatic) data

Aspect Germline testing Somatic tumor testing
Biological source DNA inherited from parents and usually present throughout the body DNA changes acquired by tumor cells
Typical sample Blood, saliva, or cheek swab Tumor tissue, blood-based tumor DNA, or another validated cancer sample
Main questions Inherited diagnosis, family risk, or susceptibility Tumor classification and treatment selection
Family implications May be relevant to biological relatives Usually not inherited, although a finding can trigger germline confirmation
How it can affect care Screening, prevention, diagnosis, and sometimes drug selection Choice among cancer therapies, trial eligibility, or understanding resistance

The two test types should not be treated as interchangeable. A negative tumor test does not necessarily exclude an inherited risk, and a germline result does not describe every molecular change in a tumor.

How pharmacogenomics can influence medication decisions

Metabolism and drug exposure

Variants can alter the activity of enzymes or transporters that absorb, break down, or move a drug. A slower or faster pathway may change the amount of active drug in the body. In a validated setting, a prescriber may use that information to select another medicine or adjust the starting dose.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Drug targets and likelihood of benefit

A variant in a drug target or disease pathway may be associated with a greater or smaller chance of response. In oncology, for example, a molecular feature of the tumor can help identify a therapy designed for that feature. The association must match the disease and treatment being considered; a result from one cancer or medicine cannot automatically be transferred to another.

Risk of adverse effects

Some gene–drug associations identify an increased risk of a serious reaction. When the evidence and the drug label support an action, the prescriber may avoid the medicine, choose an alternative, or use a different dose and monitoring plan.

What FDA labeling does—and does not—say

FDA drug labels may describe pharmacokinetic differences, response variability, adverse-event risk, genotype-specific dosing, mechanisms of action, relevant genes, and how a clinical trial was designed. Some labels specify a concrete action based on a biomarker; others provide information without directing a particular test or dose.

FDA’s Table of Pharmacogenetic Associations lists selected associations for which evidence suggests altered metabolism and, in some cases, different therapeutic effects. Inclusion in that table does not by itself mean FDA recommends testing everyone before a prescription. A pre-prescription test may be required when a validated companion diagnostic is specified, but many listed associations have not been evaluated for whether testing improves clinical outcomes such as effectiveness or fewer adverse events.

For that reason, clinicians should consult the current FDA-approved labeling for the exact medicine and indication. Patients should not start, stop, or change a medicine based only on a genetic report.

Why a pharmacogenetic result cannot predict every medication response

Genes are only one source of variability. The following can change drug exposure or effect even when genotype is known:

  • Kidney or liver impairment
  • Age, body size, pregnancy, and hormonal state
  • Other prescription medicines, over-the-counter products, and supplements
  • Diet, alcohol, smoking, and adherence
  • The disease’s severity and biological subtype
  • Interactions between several genes rather than one variant

A study may show that a genotype changes blood concentration (a pharmacokinetic difference) without proving that patients have different symptom relief, survival, or rates of serious harm. The strength of evidence must therefore be judged for the exact gene, variant, drug, dose, and clinical outcome.

Clinical tests and consumer genetic reports are not the same

Clinical testing is ordered and interpreted for a defined medical question, with laboratory requirements and follow-up suited to patient care. Direct-to-consumer (DTC) reports are marketed for specified purposes and have defined limitations. A DTC report may be useful for discussion, but it may not test all relevant variants, and its findings may not be sufficient for diagnosis or prescribing.

FDA advises consumers to read the test’s intended use and limitations and discuss health-related results with a healthcare professional. A genetic counselor or board-certified medical geneticist may be appropriate when a result could affect screening, relatives, or treatment.

In an October 31, 2018 announcement about authorization of one specific DTC pharmacogenetic report, FDA described a test detecting 33 variants across multiple genes. FDA said the report was intended to provide information about variants potentially associated with metabolism of some medicines and to inform a discussion with a healthcare provider. It did not determine whether a medication was appropriate or predict response to any specific medication. The announcement quoted Tim Stenzel, then director of FDA’s Office of In Vitro Diagnostics and Radiological Health: “Consumers should not use this test to make treatment decisions on their own.” That historical authorization does not establish the product’s current status or validate every pharmacogenetic claim made by consumer tests.

A practical checklist for using a genetic result safely

  • Ask what sample was tested: inherited DNA, tumor tissue, or another source.
  • Read the intended use: diagnosis, risk, tumor treatment selection, or drug response.
  • Check the exact variant and evidence category: a gene name alone is not enough.
  • Find out whether an action is specified: look for current drug labeling or a companion-diagnostic requirement.
  • Confirm unexpected findings clinically: a consumer or screening result may need a validated laboratory test.
  • Review the whole medication list and organ function: genotype does not replace routine prescribing checks.
  • Discuss family implications and privacy: inherited findings may matter to relatives, while data storage and sharing policies vary by testing service.
  • Do not alter treatment independently: bring the report to the prescriber who manages the condition or medicine.

How much pharmacogenomics is already relevant?

The National Human Genome Research Institute reported that, in 2022, 14% of FDA-approved medications had a pharmacogenomic testing recommendation. NHGRI estimated that those medicines affected 6.7 billion outpatient prescriptions in the United States that year. These figures describe 2022, not a current 2026 estimate, and they do not mean that 14% of patients need a genetic test before taking a medicine.

The statistic also includes medicines and recommendations with different evidence levels and clinical contexts. Whether testing is useful for one person depends on the specific drug, the indication, the available assay, and whether the result would change management.

Equity, ancestry, privacy, and access

Ancestry representation

Many pharmacogenomic datasets have been drawn predominantly from people of European ancestry. As a result, tests may miss variants more common in other populations or perform less well for some patients. Broader inclusion of ancestries and transparent reporting of performance are necessary to reduce this gap.

Access and interpretation

Testing may be unavailable, unaffordable, or difficult to interpret in under-resourced settings. A result without a clinician who can connect it to a diagnosis or prescription may not improve care. Health systems need laboratory capacity, decision support, counseling, and follow-up—not just sequencing.

Privacy and family considerations

Genetic data can reveal information about biological relatives as well as the person tested. Before testing, ask who stores the data, whether it may be used for research, how long it is retained, and whether it can be deleted. The practical and legal protections differ by jurisdiction and by service, so the provider’s current policy should be reviewed directly.

Questions to ask before ordering or acting on a test

  1. What medical decision will this test help make?
  2. Which genes and variant types does it cover, and what will it miss?
  3. Is the test clinically validated for this disease, tumor, or medicine?
  4. What result would change treatment, screening, or monitoring?
  5. Would a negative result rule out the condition or only reduce its likelihood?
  6. How will uncertain or unexpected findings be handled?
  7. Will the result have implications for relatives?
  8. Who will explain the report and coordinate follow-up?

Bottom line for patients

Genetic data is most useful when it answers a specific question and is interpreted with strong evidence, current drug labeling, and the rest of the patient’s clinical information. Tumor profiling can guide cancer treatment, inherited testing can clarify diagnosis or risk, and pharmacogenomics can sometimes inform drug choice, dose, benefit, or toxicity. None of these uses makes genetics a substitute for a clinician’s assessment. Treat a genetic report as decision support to discuss with a qualified healthcare professional, not as an automatic diagnosis or prescription.

Frequently Asked Questions

Does a genetic test tell me which medicine will work?

Sometimes a well-validated gene–drug result can support a choice, dose, or avoidance decision, but most responses also depend on the disease, other medicines, organ function, and other patient factors.

Is a tumor mutation inherited by my children?

Usually not. Somatic changes arise in tumor cells, but a clinician may recommend separate germline testing when a tumor finding could indicate an inherited variant.

What should I do with an unexpected consumer-test result?

Do not change treatment yourself. Take the complete report to a healthcare professional and ask whether a clinically validated test and genetic counseling are needed.

Does a negative pharmacogenetic result prove a medicine is safe or effective?

No. It only means the tested variants did not provide the reported signal; it does not remove non-genetic risks or prove that the medicine will work.
