How Fluoride Release and Strength Affect Glass Ionomer Cement Performance

Glass ionomer cement (GIC) performance is a formulation-dependent balance, not a contest to maximize one number. Conventional and resin-modified GICs can release fluoride for long periods, yet fluoride output alone does not predict compressive strength, fracture resistance, or clinical success. The meaningful comparison is between a defined fluoride test, a defined mechanical property, the cement’s formulation and intended use, and the quality of evidence behind the claim.

What glass ionomer cement performance actually depends on

GIC is a family of dental cements rather than one uniform material. Conventional, resin-modified, metal-reinforced, high-viscosity and additive-modified formulations can differ in powder chemistry, liquid composition, setting reaction and handling. A cement selected for luting, lining or base work is not automatically interchangeable with one intended for a restoration, sealant or orthodontic adhesive.

Three questions should therefore be kept separate:

  • How does fluoride behave? Consider the initial release, the later release phase, fluoride uptake and release after exposure to another fluoride source, solution pH and the measurement period.
  • Which mechanical property is being reported? Compressive, flexural, tensile strength, fracture toughness and bond strength answer different questions.
  • What is the intended clinical application? A value measured on a laboratory specimen does not, by itself, establish suitability for a particular tooth or load-bearing site.

The 2016 review A Review of Glass-Ionomer Cements for Clinical Dentistry and the American Dental Association’s Materials for Direct Restorations page both show why category and composition matter. The 2024 clinical-application review likewise places GIC below amalgam and resin composite in compressive and flexural strength, which limits broad use in high-stress situations.

Does glass ionomer cement release fluoride?

Yes. Both conventional and resin-modified GICs release fluoride for considerable periods after setting. Nicholson, Sidhu and Czarnecka describe this conclusion in their review, Fluoride exchange by glass-ionomer dental cements and its clinical effects: a review.

Release is not a single constant rate. The surrounding solution and the cement’s formulation change the pattern.

The early wash-out phase

Immediately after setting, fluoride can leave the cement relatively quickly. This early wash-out phase is distinct from the slower release that follows. A high early value therefore does not mean that the same rate will continue for months or years.

Long-term release in neutral conditions

In a neutral solution, the review describes the later phase primarily as diffusion. Fluoride moves gradually through the cement matrix and into the surrounding fluid. The amount measured depends on specimen dimensions, storage solution, temperature, surface area, renewal of the solution and the chosen time points.

What changes in acidic conditions

Acidic conditions increase the early release. Over the longer term, release involves slow dissolution as well as the initial wash-out. A result obtained in an acidic laboratory solution should not be presented as though it were the release rate in a neutral oral environment.

Fluoride uptake and later release

GIC can take up fluoride from its surroundings and release it later. This is often called recharge, but the amount taken up and subsequently released depends on the exposure conditions. A cement’s ability to exchange fluoride does not establish that a restoration will prevent caries in every patient.

What fluoride release can—and cannot—tell you clinically

Laboratory studies summarized in the 2023 review describe fluoride concentrations that can inhibit nearby bacteria and support remineralization under experimental conditions. The same review cautions that the overall contribution to oral health remains uncertain when other fluoride sources, including toothpaste, are present.

That distinction is essential:

  • Established mechanism: fluoride can be released, taken up and released again, and can reach biologically relevant concentrations near a cement specimen in some tests.
  • Unresolved clinical question: how much that exchange changes caries incidence or restoration longevity in routine care, where saliva, diet, biofilm, brushing and other fluoride exposures vary.

Do not turn an in-vitro release curve into a categorical promise that a GIC restoration prevents recurrent decay. Clinical outcomes require clinical studies, not only chemical or microbiological measurements.

Does more fluoride release mean lower strength?

No universal inverse relationship has been established. A formulation may show relatively high release and acceptable strength, while another may show lower release for reasons unrelated to its load-bearing performance. Comparing a fluoride number with a strength number from different products, test methods or time points can create a false trade-off.

Composition affects both properties, but not in one predictable direction. The 2016 clinical-dentistry review reports compressive-strength trends improving with a higher powder-to-liquid ratio and higher polyacid concentration. Those are formulation trends, not a rule that every change in powder, liquid or acid content produces the same result.

Strength also depends on specimen preparation, maturation, storage and the test protocol. A dry, freshly prepared sample and a water-stored sample tested after a longer interval may produce different values even when the cement is nominally the same.

Strength numbers: compare the property, category and test context

Compressive strength indicates resistance to crushing in a specified test. Flexural strength reflects behavior when a specimen bends. Neither is a substitute for fracture toughness, tensile strength or bond strength. The 2024 review in the Journal of Dental Sciences reports broad GIC ranges of 60–300 MPa for compressive strength and 14–50 MPa for flexural strength. Those ranges summarize a review literature set and should not be treated as a specification for every product.

The ADA table provides category examples with more specific values:

ADA category example Compressive-strength value Fluoride release over 30 days How to read the figure
Type I GIC Not stated in the cited ADA table excerpt 700 μg Thirty-day table value; not a universal product guarantee.
Type II GIC 196–251 MPa 650 μg Compressive range and fluoride value come from an ADA category table; test details and product formulation still matter.
Type III GIC Not stated in the cited ADA table excerpt 1300–1600 μg The highest listed 30-day release range in this table does not establish the highest strength.
Metal-reinforced type IV cement 176–212 MPa 300 μg Contains a metal reinforcement category; lower listed release does not by itself establish clinical superiority.
Hybrid restorative 202–306 MPa Not stated in the cited ADA table excerpt Hybrid is a category label, not a guarantee that every formulation reaches the upper value.
High-viscosity material 301 MPa Not stated in the cited ADA table excerpt A category example from the ADA table, not a universal minimum or maximum.

The ADA values were accessed in 2026. They are published table values, while the 2024 review ranges aggregate studies that may use different categories, specimen preparation and protocols. Do not combine the two sources as though they were one standardized experiment.

How GIC types change the fluoride–strength balance

Conventional GIC

Conventional GICs use the traditional acid–base setting system. They release fluoride for considerable periods, with an early wash-out followed by a slower phase. Their mechanical performance depends on the particular powder, liquid, powder-to-liquid ratio, acid concentration, maturation and storage conditions.

Resin-modified GIC

Resin-modified GICs incorporate methacrylate-based monomers. The ADA describes this category as offering improved setting and polishability properties. They still release fluoride after setting, so the presence of a resin component should not be interpreted as eliminating fluoride exchange. Comparisons must identify the exact formulation and test conditions rather than assuming every resin-modified cement outranks every conventional cement.

Metal-reinforced GIC

The ADA describes metal-reinforced GICs as containing silver alloy or silver sintered to glass, intended to increase stress-load capacity and fracture toughness. The category’s example in the ADA table has a 176–212 MPa compressive-strength range and 300 μg of fluoride release over 30 days. Those two values describe one table category; they do not prove a universal strength or release ranking across all metal-reinforced products.

High-viscosity and hybrid restorative materials

High-viscosity and hybrid restorative categories can show higher compressive-strength examples in the ADA table, including 301 MPa for the high-viscosity example and 202–306 MPa for the hybrid restorative example. A higher compressive value does not automatically answer questions about flexural failure, wear, fracture toughness, adhesion or fluoride recharge.

Is glass ionomer cement strong enough for a filling?

That depends on the cement category, the restoration’s design and the stresses expected in the intended application. The 2024 clinical-application review reports lower compressive and flexural strength for GIC than for amalgam and resin composite, limiting broad use in stress-bearing areas. This is a material-level limitation, not a statement that every GIC restoration fails in every posterior situation.

  1. Identify the application: determine whether the material is intended for luting, lining or base work, a restorative indication, a sealant or an orthodontic adhesive.
  2. Identify the dominant failure mode: crushing, bending, cracking, loss of bond or surface wear require different evidence.
  3. Read the formulation: conventional, resin-modified, metal-reinforced, high-viscosity and additive-modified labels describe different material systems.
  4. Match the test: use compressive data for compressive questions and flexural or fracture data for bending and cracking questions. Do not substitute one metric for another.
  5. Check the evidence level: separate an in-vitro result from a clinical endpoint such as restoration survival or caries incidence.

  • the measurement period and sampling intervals;
  • the analytical method and whether the result is cumulative or interval release;
  • whether the specimen was exposed to a fluoride-recharge cycle; and
  • the mechanical property, conditioning period and loading protocol used in the strength test.

A 30-day cumulative value cannot be compared directly with a one-day rate, and a result in acid cannot be treated as a neutral-solution result. Likewise, a compressive-strength value cannot be used as a proxy for fracture toughness or clinical longevity.

What additive research says about fluoride performance

A 2026 systematic review, Effects of Additive Incorporation on Fluoride Release of Orthodontic Glass Ionomer Cements and Adhesives: A Systematic Review, included 23 in-vitro studies. It found that additive effects varied with additive type and concentration, base-material composition, storage conditions, pH and evaluation period.

The review did not perform a meta-analysis because the outcomes and methods were too heterogeneous. It calls for standardized long-term and clinical studies before recommendations are made. This matters when a modified cement is marketed as having more fluoride release: an increase observed under one protocol may not persist under another, and it does not establish improved patient outcomes.

A practical framework for selecting and reporting GIC performance

For clinicians and material purchasers

  • Start with the indication and expected stresses, not the largest fluoride number.
  • Ask for the relevant strength property and test condition, including specimen age and storage.
  • Check whether fluoride data are cumulative, whether the solution was acidic or neutral, and whether recharge was tested.
  • Use category values as orientation, then consult the specific product’s instructions and independently generated evidence.
  • Treat claims about bacterial inhibition or remineralization as mechanistic or laboratory evidence unless clinical outcomes are supplied.

For researchers and technical writers

  • Report fluoride release and mechanical results separately before discussing a possible relationship.
  • Name the material category and formulation instead of calling all GICs equivalent.
  • Keep pH, storage duration, specimen preparation and measurement period beside every numerical result.
  • Do not infer a universal fluoride–strength trade-off from a small set of products.
  • Distinguish laboratory release, bacterial assays and remineralization tests from clinical caries or restoration-survival outcomes.

The bottom line for interpreting GIC data

Fluoride release is a useful property of glass ionomer cement, but it is not a stand-alone score for strength or clinical effectiveness. Conventional and resin-modified materials can release fluoride for long periods and can exchange fluoride with their environment. Mechanical performance varies with composition and processing; published category examples span substantially different compressive values, and broad review ranges cover both compressive and flexural behavior. The defensible comparison is always formulation plus fluoride conditions plus the named mechanical property plus intended use and evidence level.
